Struck against a moving object

Scale clerk was operating the 980G loader. While navigating a decline, the loader began to pick up speed. The operator panicked & could not locate brake pedal. While attempting to steer thru a curve, the operator steered too far to the right which deflected the loader to the left. Left front tire hit a soft low spot overturning loader into a settlement pond.

Over-exertion in pulling or pushing objects

While pulling on water hose underneath the wash plant and the hose got caught on support beam; as the employee attempted to pull on the hose to free it he felt a sharp pain and burning sensation to the left elbow.

While walking up the catwalk alongside the conveyor to remove spillage, employee slipped and fell. He sustained injuries to his neck and back.

Unbolting flange to flex coupling from Dredge to pipeline. Flex coupling when disconnected, swung to side, bruising hand of EE between workbarge ""A"" frame and flex coupling.

Over-exertion in lifting objects

EE was assisting in rebuild of wet end of dredge pump. He was attempting to remove two bolts holding pump shell to base using an end wrench. The ee lifted on the wrench with his back, straining lower back. He failed to use proper tool for this procedure and while using the wrong tool the ee lifted with his back and should have lifted with his legs.

Over-exertion in pulling or pushing objects

Employee was working with dredge 16"" discharge pipeline. He was on the work barge helping to push the pipe from the barge. He lost his balance and fell into the water. He didn't feel the effects of the injury until the next morning. He was taken to doctor and diagnosed with a right shoulder strain.

Employee was traveling from dredge to bank in transport boat with 3 other employees. The outboard motor stalled. As he pulled the cord to try to restart the motor, the motor locked and jerked the cord back. He had forgot to take motor out of gear. He was diagnosed with a strain to his left wrist.

Employee was climbing the ladder of a 980-F loader to enter the cab, upon reaching the top rung he reached over and down to open the loader door. He felt a pain in his back after the door opened. Upon his reporting the incident he was taken to the emergency room and diagnosed with a back strain.

Absorption of radiations, caustics, toxic and noxious substances

Employee entered portable toilet at desanding plant and noticed a spider residing in a web in the lower corner. He used his right hand to brush the spider nest away and felt as if he may have been biten by the spider when a pain began radiating up his right arm.

Employee & co-worker were splicing a conveyor belt back together and when EE stepped onto catwalk his foot slipped causing him twist his knee.

Employee was using impact wrench to remove door on engine compartment on Front-end Loader. The rotation of the wrench was opposite as expected and pulled finger in, and hammered against a piece of metal causing a laceration requiring 5 stitches.

Employee was changing return idler with another employee. His side was in position and as the other side was being position, his side fell out of place and against his left middle finger, causing bruising at the knuckle.

Struck by falling object

Employee was working in material recovery tunnel, trying to unclog one of the recovery doors. The door was not secure properly and fell, cutting his right hand between the thumb and index finger, requiring 11 stitches.

Employee tried to lift outboard motor from boat without assistance and pulled his back.

Employee was traveling down a stacker belt. The walkway was slippery from dew/mist. He slipped and to catch himself from falling, he grabbed the handrail with his left hand, and his arm twisted.

Employee was carrying a piece of broken pump impeller up the boat landing. As he intended to release it into the scrap pile, a part of the piece was caught on his clothing (overalls), and fell on the top of his left foot. X-rays showed no obvious fracture or dislocation. The area is swollen and tender. Employee was placed on restrictive duty.

Employee was pulling down on wrench to loosen bolt. The wrench broke loose from the bolt and struck him in face just below his right eye causing some slight bruising and headache.

Struck against stationary object

Employee was changing the v-belts on the single deck shaker. When breaking the locknum free on the tensioner, he smashed his left pinky finger on the catwalk beneath the v-belt drive.

Employee was standing by the 500 hp electric motor. He turned to walk in the opposite direction and tripped over the push bolt that was attached to the v-belts on right angle drive for underwater pump.

Weld on corner post of hand rail around dredge broke and cable pulled and threw employee back down.

Fall down stairs

Employee slipped and fell on slippery step. He fell approximately 4 feet to the next level landing on his feet but struck his right elbow on the hand rail. His elbow was bruised and began to swell some.

Struck by falling object

Employee needed to open the top hatch of the cherry picker to see his signal man. The hatch wouldn't stay open and kept falling back down. He propped the hatch open with a mirror he found laying behind the seat. When he applied the swing brake, the mirror fell, struck his hand, broke, and cut his left index and middle fingers requiring stitches.

EE WAS REMOVING THE FRONT LINER FROM PUMP. THE BOTTOM OF THE LINER SLID FORWARD CAUSING THE TOP OF THE LINER TO SLIDE BACK. HIS LEFT MIDDLE FINGER GOT CAUGHT BETWEEN THE LINER AND THE PUMP SHE LL. HIS FINGER TIP IS CRUSHED AND BROKEN AND HIS FINGERNAIL HAD TO BE SEWN BACK ON REQUIRING 8 STITCHES.

EMPLOYEE WAS CRAWLING OUT OF GRAVITY SCREEEN BOX AND FELL APPROXIMATELY 4' LANDING ON HIS CHIN. HE BUSTED HIS CHIN OPEN REQUIRING 5 STITCHES.
