Employee was going around a corner on a shuttle car. Their hand was on the handle on the canopy. They rubbed the corner of the rib with the canopy. They caught a finger between the hand hold and the rib causing a laceration that required stitches.
Employee was sitting on a bucket at the end of the boom loading a shuttle car with a miner. A rock fell off the boom and landed on both of EE's feet. This caused a laceration on EE's left foot that required 4 stitches and a bruise on EE's right foot.

Employee was loading plates on the bolter. They turned around and their elbow came in contact with a bundle of plates. This resulted in a laceration that required 8 stitches.
Employee was trying to cut straps on cable bolts. EE was cutting away from self and couldn't cut the bands. EE turned the knife around and was using it like a saw. when the band finally cut, the blade contacted EE's gloved hand causing a laceration in the palm of EE's thumb. This required 6 stitches.
Employee was down on 1 knee cleaning EE's dust box on the roof bolter. A piece of rock fell from between the rib and outside bolt. The rock landed on the back of the foot at the heal. This caused a bruise to the foot.
A non injury roof fall was found on 1 belt break 5 in the travel way. The mine examiner had finished their examination and was on the way to the surface when the fall was found.
Employee was straightening bolts that were laying on the bolter in order to make room for plates that were being loaded. Another employee threw a bundle of plates on the bolter. The plates struck employee's right hand. This caused the middle finger to become smashed which resulted in a laceration to the finger and a fracture to the tip of the finger.
Employee was on top of the bolter on knees sliding plates across the bolter. They twisted and felt a pain in their back.
Employee was using a multiplier to remove lug bolts from a scoop. The socket shattered causing EE's hand to slip and hit EE's arm. This caused a laceration that required stitches.
A roof fall was found during the weekly exam. The location was panel SC-2 #10 entry, 1 break up and 1 break to the left of spad 370. All entries in the panel are common return.
Employee had moved miner from #9 to #6. They had the ropes hooked to the head and pushed the cable into the heading. They went to the head and unhooked the ropes, when they turned around there was a rock sticking off the side of the miner. They struck the rock causing a laceration to the bridge of their nose which required stitches.
Employee was helping remove a flight from a miner chain. Employee was over the chain. Another employee was using a hammer to drive the pin out. A piece of metal broke off the pin, went under the safety glasses and embedded in the Employee's eye.
A non injury roof fall was found during the weekly airway run. It was on the NW parallel, panel P5 in the #4 entry. This was a continuation of an existing fall.
Employee had drill a hole in the roof to install a bolt. The drill steel got stuck, EE was using both hands to pull the steel out of the hole. When the steel came out of the hole, it dislodged a piece of rock that struck the employee on the arm causing a laceration that required stitches.
Employee was running through a flypad, a bolter was parked about 3' from the flypad with the power off. EE hit EE's head on the ATRS causing a laceration the required 4 stitches.
Employee was starting to drill the outside hole. When the steel touched the roof, the bit broke and the up pressure caused the drill steel to bend in 2 places. EE hit the rotation causing the steel to spin around and strike EE's right thumb causing a fracture.

Employee was pre-shifting and found a fall on 1A travelway from break 16 to 17 #4 entry
Employee was swinging in after installing the outside bolt. A piece of rock fell from the top hitting the employee in the right arm causing a laceration that required stitches.
Employee was drilling a test hole when employee put the steel in the chuck, a piece of rock fell and hit employee's left hand between the thumb and index finger.
Employee was bending 2 roof bolts over the tire of the bolter. EE felt a pain in EE's groin area. EE was diagnosed with a hernia. EE started missing work on 9/24/2024.
That crew was lifting beam up and it kicked out falling on EE's leg. It was 5 and 1/2 feet tall in that area, bottom was clean.
Employee was on EE's knees pushing the battery lids up to do maintenance. As EE was pushing, EE felt a pain in EE's stomach. EE was diagnosed with a hernia and started missing work on 10/7/2024.
Fall was found while make weekly exam. Top appears to be okay in #6 entry, inby and outby the fall.
Employee was operating the boom on the maintenance scoop. A piece of rock fell from the bolt to the rib and struck employee in the back. This caused a bruised muscle.
Employee was on EE's knees cutting out inserts on a miner. As EE was cutting, another employee was using a hammer to drive the others in. They were about 3 feet apart. A piece of insert broke, went above EE's glasses and into EE's eye.
Employee was stacking plate on the bolter. After they stacked the plates, they turned around to see where they were going to step and caught their wrist on the edge of the plates causing a laceration that required 3 stitches.
Employee was using a slate bar to pull a rock off of the miner (the continuous miner was located in a heading). When the rock slid off the side of the miner, they did not allow enough room to move out of the way, the rock landed on their left leg, this resulted in a fracture.
Employee was bolting in #7 heading, EE swung the pot in to put up the inside bolt. A rock slid off the top of the canopy and hit EE's foot. EE did not miss any work until 6/10/2024. Because of the swelling, EE's boots caused a blister that became infected.
Employee had swung the pot out to put up a corner bolt in the first cut out of a break. As EE was getting on knees, a piece of rock from the first row of bolts outby the cut from the outside bolt to the rib striking the side of EE's leg and ankle causing a fracture.
Employee was walking from a power center to the head drive and felt a burning sensation in their left knee.

Employee found a roof fall in the off neutral after making a preshift. The fall was located on 2 belt at break 18 in the off neutral in the intersection.
Contract employee was pulling a bolt off of the roof bolter that was under a rock. They felt a pain in their lower back.
EE was loading the roof bolter. EE had loaded the operator's side with plates. As EE was throwing the plates to the other side, EE came in contact with the plates and caused a laceration on EE's forearm that required stitches.
An alliance coal employee from central office was helping straighten up the supply yard. Employee was helping move a canopy post from 1 pallet to another. The end slipped and mashed a finger between the post and a miner drum causing a laceration. This required 3 stitches.
Employee got a Rope Bolt stuck in the hole as EE pulled it out of the hole by hand, the end of the bolt hit EE's lip causing a puncture that required 2 stitches. This also resulted in a broken tooth.
Contract Employee was wedging a brattice with a pole ax. They had both hands on the handle driving the wedge. The handle of the ax came in contact with the block, mashing their index finger. This caused a fracture to the end of the finger.
EE was cutting a zip tie with the utility knife. EE had left hand resting on the machine. When EE cut the zip tie the blade slipped and cut the palm of the left hand.
Employee was watering the scoop batteries that was sitting on the ground. EE had the lids raised with EE's hand on the batteries. The lid fell and caught EE's fingers causing a fracture to EE's left ring finger.
A non injury roof fall was found when the mine examiner was making their airways. The fall was located out by 7 section on their right return.
Employee had arrived at the bolter. EE set a bucket of bits on the bolter and felt a pain in the shoulder. EE was removed from work pending a MRI.
Employee was installing a bolt. The bolt was bent to get it in the hole. EE started to straighten the bolt to put it in the pot. EE felt a pop in EE's elbow. This resulted in a strain to EE's elbow.
Contract employee had installed first bolt. EE was swinging in to put the inside bolt up. As EE was swinging the boom, a piece of rock hit the canopy, slid off and struck EE's foot. This resulted in a fracture to the foot.
Employee was picking up a 4 wheeler motor off of ride. EE picked it up and fell backwards to the ground holding stomach saying EE felt something tear in stomach. EE ended up having a hernia in stomach.
Employee had inserted glue into a hole that EE had drilled to install a bolt. While holding the glue in place, EE reached down with EE's other hand to grab the bolt. EE's arm caught the corner of the plate causing a laceration that required stitches.
Contract employee was in the bolter training program under the supervision of a foreman. When the employee was trying to put the put the bolt in the pot, EE hit the up pressure instead of the down. The bottom of EE's middle finger got caught between the bolt and the pot. A skin graft was required to repair the finger.
Employee was loading the bolter. Employee was throwing bundles of plates across the bolter. They started to throw a bundles of plates and hit the head of a bolt with the top of their hand causing a fracture.
Employee was bending a bolt over the tire of a bolter. The bolt rolled over causing EE to lose EE's balance. EE fell to the ground causing a sprain to EE's right thumb.
Employee was picking up trash and when employee went to get back in the scoop a piece of rib rolled off and struck employee's arm around the elbow. X-rays was negative. Employee was taken off work pending an MRI.

Employee was putting up EE's first bolt in a place. A piece of rock fell between ATRS and the rib hitting EE in the head causing a laceration. this required 5 staples.
Employee was riding in the back of a man trip, hit a bump in the travel way causing EE to hit EE's head on the mine roof, which caused EE's hard hat to be pushed down against EE's nose. This resulted in a fracture and a deviated septum. EE had surgery on 3/2/2023 to repair the septum, which was EE's first day of missing work.
Employee was hitting 2 hammers together to remove a stuck bit on a miner. A piece of the hammer chipped and embedded itself into the employee's wrist. The piece of metal was not removed until 2/24/2023 on which date it became reportable.
Employee had the top lid raised on the pinner working on the suction. They needed to get under the other lid. Another employee moved the second lid the first lid fell and smashed a thumb causing a fracture.
Employee was riding in the back of the mantrip on the way out of the mines. There was a low spot just in by 5C head. EE's cap light caught a plate that was against the mine roof causing EE's head to be pushed back into the back of the mantrip causing a laceration to the back of the head. This required 5 staples.

Employee was loading dust into the rock duster with the forklift. The bag dust was over the pod. The forks slipped and hit employee in the side causing a cracked rib. This is a contracted employee.
Employee was hanging the miner cable so the scoop could go under it. the scoop tire caught the side of EE's foot causing a slight fracture.
Employee was helping install structure. EE's finger got pinched between 2 pieces of rail causing a laceration and fracture.
EE was in the process of installing a roof bolt on a corner. EE could not get the bolt started in the hole because of gob. The bolt was positioned on the right side of EE's body. EE switched sides and as the bolt went in the hole, the plate caught EE's cheek and caused a laceration which caused stitches.
A non injury roof fall was found during the preshift just inby the #1 head drive.
Employee was carrying a box of glue beside the scoop. EE lost balance and fell hitting head on the scoop bucket causing a laceration above eye which required stitches.
Employee was in the process of installing a curtain board. Employee was between the canopy and board standing up. When they turned around their knee hit the joystick which caused the arm of the bolter to move up and catch their pinky between the boom and canopy post causing a laceration.
Employee was unloading supplies from a flat car with a scoop. They had the bucket bowed on the flat car with the front tires off the ground. The bucket slid off the car which caused the scoop to hit the floor and caused employee to hit their head on canopy resulting in a slight fracture to their spine in their neck. No surgery is required.
Employee was hanging a pump cable. There was a bare spot in the cable which caused employee to receive a slight shock. Employee lost footing and fell. Employee tried to catch self with hand, hurting wrist. Employee finished shift and returned to work the following day. Started missing work on 7/14/2022.
A non injury roof fall occurred in the #7 entry in the intake out by the #5 section.
Employee had got out of the deck of the bolter, walked around the off side, grabbed a crosser to cross the miner cable. After the employee took the crosser to the miner operator to put on the cable. EE was beside the bolter when the rib rolled off catching the lower part of EE's leg resulting in a fracture between the knee and ankle.
Employee was bolting on an overcast. EE had put a bolt in the hole. When EE straightened the bolt to put it in the pot, EE felt a pop in their shoulder. EE returned to work the next shift and then began missing thereafter.
Employee was bolting and a rock fell from the top, down the pin steel, and hit employee on the wrist causing injury.
Employee was in the process of setting a tailpiece. The chain slipped causing the tailpiece to set down on a crib causing the crib to roll over. The crib landed on the employee's foot causing a fracture on the end of one of employee's toes.

Employee was helping hang miner cable to back the miner around into 6L to begin cutting 6R. Employee felt a pain in lower back.
Employee was walking to the scoop. Employee slipped in some mud. When they raised up, they hit their head on a piece of waterline causing a laceration just above right eye, in which required 2 stitches.
Employee was using a sledge hammer to drive a head jack pin back in on the miner that had worked out. As employee was striking the pin, a piece of metal broke from the edge of the pin, flew under employee's safety glasses piercing lower eye lid and embedding in eye.
Employee was pulling rocks off the miner. EE was pulling a small rock for the scrubber inlet. When it slid off the miner it cut the tip of EE's finger. EE did not miss the first day of work until 2/1/2022.
Employee was cleaning out the pre-dump on the bolter. While employee had hand in the pre-dump employee's partner closed it causing a laceration on employee's thumb.
Employee was turning 4R. EE had loaded approx. 5' out of the cut. EE walked up and placed their hand on the miner to look at the cut. A piece of rock fell from between the bolts and smashed EE's left hand causing a hair line fracture to the hand.

Employee had turned out of belt entry into 5 left. Employee said a piece of rock hit canopy, fell off and hit shoulder and neck while employee was tramming the shuttle car.
Employee walked through a fly pad and came in contact with a roof bolt plate, which caused a scratch on the eye. Employee worked 3rd shift and EE's time started on 12/03/2021 at 10:30 pm.
Employee was loading a S/C with the miner. Employee had seen a rock come down the boom. Employee thought it was loaded on the S/C. After the S/C had left employee looked back at the face to continue cutting. The rock that was on the boom fell off and struck employee on left ankle. Employee continued to work until 11/18/2021.
Employee was pulling slack for a CM cable and felt pain in a knee Employee did not miss work until 11/17/2021 when they were diagnosed with a tear in the knee that will require surgery.
EE is in the process of training on the roof bolter. EE was trying to get a hole started to install a bolt. EE dislodged a piece of rock and it mashed the tip of EE's pinky finger causing a laceration and fracture.
EE was sitting on bucket operating the continuous miner. As EE was getting off of the bucket to move the miner cable, foot slipped, causing EE to overextend groin muscle.
EE was picking up the miner cable to lay across the head to back across the section. EE felt a pain in lower back. EE continued to work until 9/07/2021.

Employee was on knees operating a continuous miner. A piece of draw rock measuring 48""x36""x3 1/2"" fell from the outside bolt to the rib landing on the bottom of employee's feet, resulting in fractures to both feet.
EE was riding in the front seat of the mantrip going to the section. There was pogo sticks on the front of the man trip. The trip passed through a flypad and the flypad caught the pogo stick and forced them into EE's chest causing a laceration and a broken rib.

Employee was dragging a belt roller. As EE was dragging the roller they bumped head into the mine roof causing a laceration.

Employee got off of the scoop to roll up the curtain to finish cleaning the face area. EE stepped off of some gob and heard a pop and felt pain in the right leg below the knee. The X-ray was negative for a broken bone and EE was referred to a specialist.
EE was helping set cribs, EE had picked up a crib out of the scoop bucket, someone else picked up a crib and smashed EE's finger between 2 crib blocks
EE had put outside bolt up, EE leaned over bolter pot to move bolt out of way to swing in to put inside bolt up. Rock fell from top and cut tri-cep of left arm which required 4 stitches
Another employee was drilling with Jackleg drill. Drill got in a bind and kicked back hitting injured employee in head causing IE to fall backwards. Laceration on head behind left ear - 7 stitches.
