Struck against stationary object

Employee was injured while working on a header. EE was using a pipe wrench on a steamboat jack, when the force made the pipe wrench break loose from the jack and caused EE's arms to extend abruptly and hit EE's left arm and left shoulder on the header framing. Started missing work on 04/14/2026.

Struck against a moving object

While using a tractor to pull a header to bottom, the operator drove over a head that had been dragged into the roadway, resulting in a jarring motion that caused low back pain. Started missing work on 2/26/2026.

Fall to the walkway or working surface

While watering the roadways they stopped the diesel tractor to check the sprays on the water cart. As they were walking to the back of the water cart, they were tripped up by a nylon band laying in the road causing them to fall and twist their right knee. They worked until a week before surgery. Started missing work on 03/30/2026.

Caught in, under or between running or meshing objects

While trying to remove the outside roof bolt that had bent entering the roof strata during the bolting process, the operator used the boom to remove the roof bolt and start over. In doing so the operator swung in pulling the pin and a piece of roof strata down with the boom, catching their left hand between the tray and the roof strata. Started missing work on 2/6/2026

Caught in, under or between a moving and a stationary object

The Mechanic was assisting in changing the cats on a miner. While the mechanic was picking the cats up to pull slack towards the back of the miner it pinched the left pinky finger in the hinge point of the cats causing a laceration and receiving stitches. NO work was missed.

Struck against stationary object

The operator dozed off while driving a mantrip, causing the trip to hit the rib. The operator hit EE's left knee on the dash of the ride resulting in sutures to close a cut. No work was missed.

Fall from machine

A passenger of a mantrip that hit the corner of a rib was dislodged from the seat and landed on the mine floor. EE was diagnosed with two stable vertebral compression fractures in the spine. Started missing work on 12/02/2025.

Over-exertion in lifting objects

When hanging a cable in the belt line during a power move, the employee felt a pop in left shoulder causing pain in an area that had been repaired before. They continued to work. Started missing work on 3/3/2026.

Over-exertion in lifting objects

While lifting the framing at a header with right arm the belt foreman felt a burning sensation in right shoulder. Worked until surgery date. Started missing on 03/25/2026.

The mechanic was standing on the shuttle car fender that was laying on the ground. While they positioned themselves to fix the problem, they slipped on the side of the fender causing pain in left knee. Started missing work on 11/14/2025.

A roof fall occurred on the 4th southwest BL seal area. in #11 entry from spad 93+75 through spad 93+10. The fall measured approximately 85'x 19' x 8'. This fall will not be loaded out, and the area has additional support added.

Roof bolter was reaching into pin tray to retrieve a steel when a small head came from the roof smashing the tip of ring finger on right hand. Resulted in a fracture and started missing work on 10/29/25.

Struck by falling object

The roof bolter was drilling a hole for EE's rib pin when a rock fell hitting the canopy swinging in and striking EE's right shoulder, lower back and EE's right knee. The rock pushed EE's knee into EE's boom causing a contusion and swelling to the knee. Started missing work on 10/17/2025.

The miner operator used EE's index finger to rub EE's right eye causing dirt from EE's glove to enter and scratch the eyeball. Seen the doctor on 10/15/2025 where medication eye drops were prescribed and no work was missed.

While crossing over a supply trailer the tractor operator slipped and fell onto the ground. The operator worked until needing surgery to repair the left knee. Started missing work on 11/26/2025.

Over-exertion in lifting objects

while hanging the miner cable and water line that goes to the miner, the shuttle car operator strained EE's left shoulder. Started missing work on 10/03/2025 and returned to work on 10/08/2025.

Struck by falling object

The mechanic was setting on a golf cart when a thin piece of draw from the mine roof fell striking the mechanic in the scalp. Resulting in staples to close the wound. No work was missed.

Struck against stationary object

While drilling a hole for the roof bolting process, the operator's steels became clogged up causing the steels to bend under pressure. While looking back at the tray and reaching for the steels at the same time the bent steels struck the operator in the base of the thumb on the left hand. Started missing work on 09/24/23025.

Walking around the tailpiece EE stepped in a hole, slipped causing pain in EE's left knee. Started missing work on 09/24/2025 and returned on 09/26/2025.

Employee was helping load some belt framing on the 3/60N beltline. As EE was dragging a 60 inch top roller under some water line, EE felt pain in EE's lower middle back and down EE's left leg. Employee started physical therapy on 9/22/25.

The mechanic was pulling an old piece of high voltage into a crosscut and pulled EE's right bicep. The mechanic continued to work until surgery was needed to repair. Started missing work on 10/22/2025.

Employee was plastering a brattice when some plaster got in EE's right eye. Employee was seen by an Eye doctor and was given a prescription eye drop for a scratch on EE's right eye.

Employee jumped off a ledge at the dust hole onto a dust trailer. EE then jumped from the dust trailer onto the top of a diesel scoop and grabbed a trussbolt to catch EE's balance causing EE to strain EE's left shoulder. Started missing work on 09/08/2025.

The examiner was traveling in a golf cart when they stopped for traffic. A piece of slate fell, striking their hard hat and both arms. The slate broke, causing a laceration to left forearm that required sutures. No work time was missed.

The mine foreman was on a diesel golf cart traveling out on the supply road when a piece of slate dropped on right shoulder, right arm and right leg fracturing two ribs. No work was missed.

The miner helper was cutting a crosscut when a piece of slate dropped striking EE on top of the hard hat and then hitting the back of EE's neck causing a laceration on the base of the neck then hitting the right leg below the knee. Staples were used to close the laceration. No work was missed.

The operator of the bolter was tramming past the supply trailer and pinched EE's 4th and 5th fingers on EE's left hand between the trailer and the roof bolter. Resulting in 3 stitches in the pinky finger and a fracture to the ring finger. The employee started missing work on 7/28/2025.

Shuttle car driver was entering EE's car when a piece of coal rib struck EE in the back and the right ankle. Started missing work on 7/25/2025 for inflammation.

Shuttle car operator was turning a tight muddy corner, hitting the rib with the bumper of the car it jarred EE's hand loose from the handle on the canopy, causing EE's left arm to get pinched between the car and the rib. Operator received two stitches.

Roof bolter operator was swinging out to install the rib pin when two rocks fell hitting the operator in the back and the left leg. Roof bolter started missing work on 7/22/2025 for inflammation.

The rock duster was dusting the belt line when EE had carried the dust hose back to the duster, EE felt sharp pain in EE's back. Started missing work on 07/22/2025.

The employee was loading miner cable into the bed of a miner when EE felt pain in EE's right shoulder. No work was missed, continued to have pain, started physical therapy on 09/04/2025.

The mechanic and miner operator picked up the rub rail for the miner to access the panel. The mechanic felt a sharp pain in abdomen. The mechanic started missing work on 7/30/2025 for hernia repair.

While raising the lids on a battery scoop the operator strained their groin. The employee started missing work on 7/22/2025 for a hernia.

While ratcheting a come along to pull the miner cat together the mechanic felt pain in right shoulder. No work was missed.

Reclaiming belt rollers, the outby worker was stacking rollers onto the ground when they caught their right ring finger between two causing a small cut. Two stitches in the end of the finger and no work missed.

While drilling a hole for the outside pin a head dislodged from the roof hitting the roof bolter operator in the right leg just above the ankle causing a fracture. Started missing work on 6/12/2025.

While installing a chain guard on a belt drive. The left middle finger of the belt mechanic was caught between the two chain guard handles. This was a two person install, when positioning the guard, it slid into the guard handle next to it. Antibiotics where given No work was missed.

Employee was replacing some hoist cable pads for the slope hoist rope. When EE went to turn and walk EE's left foot stayed planted on the ground causing EE to twist EE's left knee. EE started missing work on 05/22/2025

Employee was dropping EE's canopy with EE's right hand while placing EE's left hand on some boards near the front of the tray. The canopy pinched EE's left pinky finger and when EE reacted with a tug, EE pulled the nail off.

While the mechanic was cutting a pin out of a lift jack on a diesel scoop with a torch, the grease port was exposed letting the grease get hot and shoot out hitting the mechanic in the nose, cheek, and bottom lip causing a burn. Employee started missing work on 5/12/2025

Roof bolter was moving when the reel got tight pulling the cable down hitting the roof bolter in the side of the hard hat causing a laceration that required 4 sutures. No work was missed.

While picking up a trailer hitch to install on a tractor the employee strained their lower back. Started missing work on 04/29/2025.

Air compressor was down on the P3 shaft. This was not an accident. A 7000-1 form is being filled out to terminate citation 9760488.

Employee was installing a bit into the miner head. Driving the bit in with a hammer a piece of carbide came off the bit embedding in the right arm below the bicep. No work was missed.

Struck against stationary object

Employee was cleaning out around a clamp on a waterline and cut EE's right index finger on a sharp rock. Glued the finger and no work was missed.

Struck by falling object

Employee was drilling a hole for a roof bolt. EE was reaching for a second steel and when EE turned back around the first steel fell out of the mine roof and bounced off the drill pot knocking EE's glasses off causing the steel to hit EE on the left eye. EE was seen by a doctor and was given eye drops and a protective contact lens.

Employee was scattering rock dust out of a scoop bucket with a shovel. As EE went to sling the shovel of dust, EE hit the scoop bucket with the shovel causing the shovel to abruptly stop causing a torn bicep on the left arm. Started missing work on 4/3/2025

The roof bolter was attempting to cut a suction hose with a belt knife. Holding the hose in EE's left hand and cutting with the right, the knife came out of the hose and cut the left hand. Received 6 sutures, no work was missed.

While preparing the belt for a belt lay the belt mechanic was cutting the belt, when the knife slipped out of the belt and cut right leg just below the knee. Causing the belt mechanic to receive 10 staples, no work was missed.

Miner operator was starting to load a car when the face fell in leaving a pile of rock just inby the last row of bolts. A rock then fell out just inby the last row of bolts landing on the pile of rock causing the rock to slide in on the operator's left ankle. Resulting in a fracture the miner operator started missing work 1/24/2025.

While setting in between the air doors the outby set was closing, forcing air/debris up under the inby set causing irritation to the right eye. The employee did not miss any work.

While the opposite operator had EE's hand on the T-bar, the operator was spotting a pin outby, dropped EE's boom hitting the ATRS pinching the opposite operator's hand between the pad and T-bar. This caused a laceration that required 12 stitches and started missing work on 1/16/2025.

Opposite operator roof bolter was drilling a hole for a roof bolt when the operator pinched right index finger between a board and the drill pot. Started missing work on 12/20/2024.

Mechanic was cutting the outer jacket on the miner cable to make a splice, slipped and cut the index finger on EE's left hand. The cut required 6 stitches. No work was missed.

While walking to the opposite operator boom to install a roof bolt, the employee slipped and fell on a rock. The employee caught their body with their left hand spraining wrist. Started missing work on 12/12/2024.

Employee was riding in the passenger seat of a diesel jake. At idle speed the driver swerved to miss something in the road and the passenger fell off of the ride, causing pain in right shoulder. Started missing work on 11/11/2024

A grinder was dropped by a belt mechanic while the blade was engaged, hitting a two man diesel ride, it bounced off of the jake and hit the index finger on the right hand of a spectator causing three sutures. No work was missed.

Operator dropped the boom when hitting the pin that was made up to put in the roof, the board that was on the premade roof bolt slid over and the boom caught the board which hit the operator in the right jaw causing a decade tooth to break on the upper left side. While trimming a piece of metal off of a scoop with a cutting torch a piece of hot metal bounced up getting behind the cutting glasses burning the left eye.

Employee was helping put a spool of leaky feeder cable on a spool cart and pinched right index finger between the spool bar and cradle. Employee received 4 sutures.

Employee was a passenger in a mantrip when the operator of the mantrip hit a rough spot in the road causing the injured employee to hit their lower back on the edge of the seat. Started missing work on 09/26/2024.

Employee was climbing a bank out of the dust hole and slipped falling onto the side of a dust wagon landing on EE's right side under EE's right arm resulting in two fractured ribs.

The employee was standing on the opposite side of the bolter and the operator repositioned the bolter running over the toes on the right foot of the opposite operator causing a fracture to the big toe and the third toe. Started missing work on 09/25/2024.

While setting in the shuttle car at the feeder the corner of the rib rolled off and some of the coal fell into the cab pinching the skin on the back of the right arm causing two small lacerations. 3 sutures in one and 5 sutures in the other.

Accident type, without injuries

A roof fall occurred in the 4th south west sub mains, number 1 entry between xc 159 and xc 160 at spad 108+80 and 109+55. The fall measured approximately 65'x19'x8'. The fall will not be loaded out and the area has additional support added.

Struck by falling object

The employee knelt beside the tool slide doing inventory, when a rock fell out and struck EE in the top of the head and then the right knee. 3 staples were required in the cut on the head. no work missed.

Struck by falling object

While installing the outside roof bolt, a rock fell out hitting the canopy of the bolter. The rock then fell off of the canopy hitting the operator in the right ankle. Taken off work on 09/05/2024 with a facture to the ankle.

Fall onto or against objects

The employee exited the shuttle car and tripped over a piece of coal. Falling to the ground employee's rib cage on the left side landed on a chunk of coal that had fallen out of the rib, resulting in a fractured rib. The employee did not miss any work.

The employee was changing a 72"" bottom roller, when the employee picked the roller up to put it in the stand employee felt a burning pain in lower back. The employee was taken off work on 8/21/2024.

The employee was walking through a curtain while the mine roof was lower on the other side hitting EE's head on a low hanging damaged roof bolt. Jamming neck causing pain in EE's neck and shoulders. Started missing work on 8/9/2024.

Employee was in the front passenger seat of a mantrip that was traveling the supply road to bottom when it drove through a clear curtain and struck the bucket of a diesel scoop that was backing into a crosscut. The injured employee hit their head on the mantrip resulting in a laceration on EE's forehead. Employee was taken to the ER and received glue on the wound.

Employee was stripping the wires for a light on a roof bolter when the knife EE was using slipped causing EE to cut left pinky finger resulting in the employee receiving 4 sutures.

Employee was pulling a trailer off the cage with a diesel tractor, the employee had hand resting on the bump stop in the pivot point of the diesel tractor. As EE made a turn, the ends of the tractor came together, EE's right pinky finger was caught in between the bump stops. This resulted in a distal fracture in the right pinky finger and also required 14 sutures.

The employee was drilling a hole in the mine roof and a donut shaped rock slid down the drill steel and flew off striking the employee just above employee's top lip causing a laceration resulting in the employee getting 5 sutures.

While setting bits on the miner, hitting a bit with a hammer a piece of metal 1/4inch long from the hammer imbedded in EE's right forearm causing infection.

The employee was loading pin plates onto the bolter. Employee swung around to place some more plates on the bolter and hit right elbow on the plates causing a laceration receiving eight sutures.

Employee was pulling a yield prop across the belt when it dropped of the belt and onto the belt framing causing a laceration to EE's left middle finger requiring sutures.

Employee was loading gob with a continuous miner and a rock that was on top of the miner slid down the tail of the miner and onto the shuttle car and then slid off the car onto the employee's left foot. Started missing work on 6/21/2024 due to a fracture in left foot.

While driving a golf cart, the front tire hit a rock causing the steering wheel to spin out of control and stopping the cart. The steering wheel caught EE's right hand dislocating EE's ring finger.

Employee was picking up crib block out of a scoop bucket and strained lower back.

The employee was eating a sandwich when a thin rock 12 inch circle fell out hitting the employee's right middle finger causing a cut that required 6 sutures. The finger became infected. The employee started missing work on 6/21/2024

The roof bolter operator was cleaning the dust box on the opposite side of the bolter. While squatting down a rock struck the roof bolter operator on top of the head causing a headache. The rock fell between the rib and the rib pin. The roof bolter operator was taken off work starting 6/17/2024.

With very little space between the grader and tractor the grader went to back up as the employee proceeded to move between the two pieces of equipment. The grader turned around to look behind and accelerated in forward instead of reverse, catching the employees' leg between the two pieces of equipment. Resulting in a fracture to the right leg. Started missing work 6/13/2024

Employee was loading cable bolt plates in the tray of EE's roof bolter. Reaching to put the plates into the tray EE slips and smashed index finger between the plates and the steel tray. Resulting in a fracture.

Employee was helping chain up a foot jack and boom on a roof bolter that was being supported by a scoop bucket when EE's right hand was smashed between the boom and scoop bucket. EE had surgery on 5/13/24.

Employee was operating a continuous miner cutting a crosscut when a piece of metal pierced employee's left bicep. The piece of metal was removed and required 1 butterfly strip. Employee returned to work on employee's next shift.

Employee was using a porta power trying to break the cab canopy loose on a roof bolter. The electric porta power was loading up when it shot out of the cab contacting the employee in the mouth. Resulting in a laceration to the upper lip with 3 sutures and 3 teeth fractured.

Employee was loading a belt roller into a scoop bucket when employee turned around to get another roller and struck cheek on the roof causing a small cut resulting in one stitch. Employee returned to work the next shift.

Employee was straightening supplies on the idle bolter just inby the last open. While bending over a rock fell hitting the employee in the head and neck area, causing a strain to the neck. Started missing work on 4/9/2024.

Employee was walking across the last open crosscut when EE hit head on a low hanging brow causing EE to jam neck. Started physical therapy on 10/22/2024.

Miner lifted the back of the mantrip off the ground with a jack, stepped between the ride and the rib to place boards under the rear tire. The jack fell over catching the miner's leg between ride and the rib, resulting a fracture to the right leg near the ankle. The Miner was taken off work starting 2/16/24.
