Fall from machine

The miner was operating a 16M Caterpillar Road Grader that developed a oil leak. The miner was leaning out of the stopped Grader looking for the leak when EE fell to the ground injuring EE's ankle

Fall down stairs

Employee was walking down steps in Prep Plant when ee slipped and tried to catch self and bent left wrist backwards. Employee did not miss a scheduled work day until 5/16/2019

Employee was climbing up in the cab of Excavator when EE twisted knee stepping on the track.

Struck by flying object

Employee was replacing a roller on a beltline outside. Conditions were windy at the time. Employee was wearing safety glasses and at some point debris went behind the glasses into their eye.

Over-exertion in lifting objects

Unloading flumes out of crane basket and pulled left calve muscle. Injury happened on the 24th, went to the Doctor on the 26th and was put off work on the 27th.

Struck against a moving object

It had been raining that morning and haul road was wet and slick. The grader had not graded that section of haul road prior to the accident. As EE went around curve, the truck started to slide in the mud. Truck slid causing it to overturn.

Driver went into a curve and turned tractor/trailer over. Driver says does not know what happened.

Employee was replacing grizzley flume screen when their hand slipped catching their right forearm on the rib on the grizzley causing a puncture to forearm. They received stitches and returned back work.

Struck by flying object

A piece of metal was being thrown into a scrap metal bin and was deflected causing it to hit the employee in the mouth, damaging a tooth.

Over-exertion in pulling or pushing objects

Employee was in the plant thickner tank helping clean out mud when he pulled on a pump hose to reposition it. The hose was stuck in mud and was hard to move. He started having lower back pain after he went home. Employee missed his first scheduled day of work on 7/6/2015.

EE was working on bottom floor pump in Prep Plant when he left to go see if the pump was pumping he stepped into an area where they had removed a section of the floor drain grating to clean for pump. Causing over extension of his knee. EE finished his shift and missed his first scheduled work day on 5/25/2015

EE was operating Komatsu 65 dozer in Gob refuse when he backed over a small pile of coarse refuse at a angle causing the seat to bottom out and have lower back pain. EE finished his shift and did not miss a scheduled work day until 4/30/2015.

Employee was welding on chute in plant when a piece of slag fell and entered his boot burning a place on top of his foot. He finished his shift and did not miss a scheduled work day till 4/1/2015.

Employee was welding (all day) and got a piece of slag in his right eye. He was wearing safety glasses anytime he was welding. He washed his eye out at home that night but next morning still has something in eye so he went to doctor to have it removed. Missed one day of work.

Absorption of radiations, caustics, toxic and noxious substances

Employee had shut a manual ball valve off on a liquid lime tank and started to uncouple the line at fitting due to line being blocked. When hose came loose the line blew liquid lime in his eyes and mouth due to the handle turning on shaft of ball valve and not closing.

Employee was stripping insulation off of wire on substation when belt knife slipped cutting left arm. Employee had three stitches and returned back to work.

Employee had crawled under belt line to cut piece of belt structure. After he finished, he attempted to turn from laying on his stomach to his back. He heard something ""snap"" in his chest as he tried to turn over.

Victim was cleaning a coal spill inside Prep Plant using a shovel and wheelbarrow when his right shoulder and elbow started to have some pain in them. Victim did not miss a scheduled work day until 03/13/2014

Victim was walking across snow/ice covered ground when he slipped and fell injuring his back. Victim did not miss a scheduled work day until 02/13/2014.

Employee was working to clear a blocked discharged line. When the obstruction broke loose, he was hit with water causing him to fall backwards. His foot became lodged between a wall and pipe as he fell resulting in a fracture to his leg.

The employee had opened the feed chute door to make sure the chute was clear. When he closed the door his hand was in the pinch point by the hinged side of the door causing a pinch/laceration to his hand below the pinky finger.
