During the preshift/weekly examination a roof fall was found in the number 5 entry at spad 64. Roof fall 20'w X 20'l X 6' H.

During the weekly examination of the idle mine a roof fall was discovered at crosscut #4 spad 39 in the #4 entry. The roof fall was apx. 20'X 20'X 5-6 'high. An investigation was conducted local MSHA was contacted and on 7-27-16 MSHA determined that the roof fall was reportable.

While making a preshift exam at this idle coal mines the mine foreman found a roof fall in #5 entry just in by spad 64 measuring approx. 5' in height 20' in width and 20' in length. Said roof fall did not impede travel or ventilation.

A ROOF FALL WAS DISCOVERED WILL MAKING AN WEEKLY EXAM OF THE RETURN OF THE 2ND SW MAINS. THE FALL IS IN THE #8 ENTRY. IT IS APX. 40'LONG X 20'WIDE X 8'HIGH. THE FALL DID NOT AFFECT MINERS, VENTILATION NOR EQUIPMENT. THE AREA WILL NOT BE CLEANED UP AT THIS TIME. THE AREA HAS BEEN TIMBERED AND DANGERED OFF TO PREVENT ENTRY.

During an investigation of a roof fall in the abandoned 1st SW mains another roof fall was discovered in the #4 entry 1 crosscut inby spad 284. The fall is appx. 20'L X 20'W X 10-14'high. The fall is in an entry where the air is common to other entries in the area. No persons, ventilation nor equipment were affected by the fall.

During an investigation of a roof fall in the abandoned 1st southwest mains another roof fall was discovered in the #2 entry at spad 316. The fall is appx. 60'L X 20'W X 10-14'H. The fall is in an entry where the air is common to other entries in the area. No persons, ventilation nor equipment were affected by the fall. The fall will not be cleaned up at this time.

A roof fall was discovered in the abandoned 1st south west mains in the #1 entry just outby spad 297. The fall was approximately 40'L X 18'W X 10-14'thick. The fall will not be cleaned up at this time. The fall did not affect miners, ventilation or equipment.

Accident type, without injuries

A roof fall was discovered in the 5 entry 1 xcut inby spad 608. The fall is sitting on cribs and not broken up about 3'above the mine floor. The area is apx.40'Wx20'Lx7'H.The fall has no effect on miners, vent. or equipment. Supplemental roof support consisting of cribs and beams has been installed. An investigation is underway to determine if cleanup of the fall will be required.

Accident type, without injuries

A roof Fall was discovered in the #1 entry less than 1 cross cut outby spad 615. The fall was approximately 40'long X 20'wide X 7'high. The fall was cleaned up with the continuous miner and re-supported with cribs, beams and roof bolts. The fall did not have any affect on miners, equipment nor ventilation.

Struck by rolling or sliding object

Employee was roof bolting when a piece of rock rolled out from the rib approximately 47"" long X 7.5"" thick X 14"" wide and struck him on the left leg. He was transported to MARH hospital where he received 15 stitches. Employee had a follow up visit with the dr. on 5-1-12 and returned to work on restricted duty on 5-2-12.

Struck against a moving object

Employee stated that he was hauling a load of belt structure with a scoop when he ran over a large rock and hurt his neck and back. He continued to work until 4-9-13 when his Dr. removed him from work and scheduled surgery for 5-1-13.

Struck by falling object

EE stated that he had hung a curtain 40 ft. inby spad 639 in #2 entry when a piece of rib apx. 4"" thick X 18"" wide X 24"" long fell and struck him on the right leg. Employee was transported to MARH hospital where he was diagnosed with both bones fractured in his lower leg.

Struck by falling object

Employee stated that he had a rib channel lying against the roof bolter when it fell over and struck him in the mouth requiring 2 stitches to his lip. Employee returned to work on the next shift. A safety meeting was conducted with employees to prevent similar occurrence.

Fall onto or against objects

EE stated that he was walking back toward the miner after conducting an air reading when he slipped on a rock and fell into the gear case of the miner. He was treated at MAR Hospital for a laceration to the forehead requiring 7 stitches. EE returned to work on the next shift at full duty.

A roof fall was discovered on the old abandoned 002 section. The fall was apx. 24'long X 18'wide X 15-18'high located in the #6 entry 1 xcut inby spad 274. The fall did not affect persons, ventilation nor equipment. The fall will be left in place as it would be safer than to clean up.

A roof fall was discovered in the #2 entry on the track of abandoned 001 section. The fall did not affect ventilation, miners nor equipment. The fall was apx. 30 feet long X 25 feet wide X 10 feet high. The fall will not be cleaned up and will be left.

A roof fall was discovered in the #1 entry outby the 002-0 section between spads 560 and 549. The fall was apx. 85ft long X 19'wide X 12-14' high.

A roof fall was discovered in the mains near the split of 001 and 002 sections in the #5 entry at spad 107. The fall was apx. 25'L x 25'W x 10-12' high. No persons nor equipment were involved in the fall of roof. Action plans have been submitted and the fall will be cleaned up and resupported with an arch.

A roof fall was discovered in the number 2 entry. at crosscut 37 out by the 001-0 section. The fall is approximately 20'long X 30'wide X 10'high. The area has been dangered off and will be cleaned up and an arch will be installed. The fall did not affect persons, equipment nor ventilation.

A rain storm in excess of 5.74 inches of rain occurred at the mine causing an inundation of water into the mine resulting in 3 3rd shift employees being trapped underground. The employees were rescued at 8:20 pm.

A roof fall was discovered in the primary escape way #5 entry at crosscut 33 outby 001-0 at 6:50am. The fall was approximately 14 feet high x 19 feet long and 80 feet wide. The fall did not affect persons, nor ventilation. It was cribbed, cleaned up and an arch installed.

Caught in, under or between a moving and a stationary object

EE stated that he was replacing the CLA on the conveyor pot of the miner. As the mechanic was raising the pan on the miner EE said that he stumbled and fell onto the pan. His left hand was pinned between the stop and the pan resulting in broken bones in the hand. EE was treated at the University of Tennessee Hospital and had pins and plates put in his hand.

EE stated that he was using a hammer to break the keeper pin on a miner bit when he missed and struck his left index finger. The results were his finger was dislocated and a laceration requiring 5 stitches. EE stated that his light cord may have deflected the hammer. EE returned to work on the next scheduled shift with modified or restricted duty.

EE stated that he was getting into the railrunner when he fell and struck his left chin requiring 6 stitches. EE returned to work on the next shift.

Caught in, under or between a moving and a stationary object

Employee stated that he had his hand on the leveling arm of the roof bolt machine when he moved the head his finger was pinched. He received 6 stitches. He returned to work on the next shift.

A roof fall was discovered on the track outby 001 section @ spad 166 in the #2 entry. The fall did not affect persons, equipment nor ventilation. The fall is approximately 30'L x 30'W x 8'H. The fall will be cleaned up and an arch installed (upon approval of MSHA)for support.

A roof fall occurred on 002 section in #2 entry in the intersection just inby spad 316. The fall did not affect persons, ventilation nor equipment. The fall was appx. 30'L x 20'W x 9'H. The fall will be cleaned up and resupported with roof bolts.

Inhalation of radiations, caustics, toxic and noxious substances

On 11-29-10 we recieved an ILO radiograph report which stated that EE has Pneumoconioses with a profusion 1/0. The reading was conducted on 11-26-10 by Dr. of Harlan, Kentucky.

A roof fall was discovered at spad 310 in the #3 entry at 9:55 pm during a preshift examination. The fall was appx. 20 feet long x 16 feet wide x 8 feet high. The fall did not affect miners,ventilation nor equipment.

EE was cutting conduit to repair a light on the shuttle, when the utility knife that he was using slipped and cut his left index finger, requiring 3 stitches.

Fall to the walkway or working surface

Employee stated that he was walking down the #2 entry,when he fell onto his back. He was transported to the hospital with complaints of numbness in his left arm. He was examined with no fractures or abnormality. He returned to work on light duty 11-10-10 and was on call for Dr. apt. to give full release. He missed work on 11-09-10 while at hospital.

Struck by falling object

EE stated that he was roof bolting when a piece of draw rock fell striking his right forearm. EE was treated at the ER at Middlesboro ARH where xrays showed no fractures. EE went to the Dr. on 11-3-10 and was admitted to the hospital with cellulitis in the area of the injury.

EE was working constructing cribs on top of a canopy. When he jumped down to get off the canopy he thought he had turned his ankle however when he was xrayed at the ER it showed to be fractured.

Struck by falling object

EE stated that he was standing at the back of the bolt machine as the foreman was checking a test hole in the roof, when a piece of draw rock fell and struck him on the right index finger. EE was taken to the hospital and recieved 13 stitches to the tip of the finger. X-rays showed no fractures. EE returned to work on the next scheduled shift.

A roof fall was discovered in the number 6 entry just by the 002 section. The fall is appx. 60' long X 20' wide X 14' high. The fall was in the intersection outby spad 232 and inby spad 220. The fall did not affect persons, ventilation nor equipment. The area has been dangered, cribbed and supplemental roof support installed.

Accident type, without injuries

During the pre-shift examination a roof fall was discovered 2 cross cuts outby the face between #5 and #6 heading. No persons, equipment, nor ventilation was affected. The fall was app. 20' long X 20' wide X 6' high. The fall was just outby spad 232. The fall will not be cleaned up at this time.

A rock fall was discovered outby the 001-0 section in the #4 entry. The fall did not affect persons, ventilation nor equipment. The fall was approximately 12'high X 30'long X 20'wide. The escapeway was rerouted and all miners informed of the new location. The fall was dangered and cribbed off.

A roof fall occurred in the 4 right crosscut into the #5 entry. The fall was approximately 8' high x 25' long x 18' wide. The fall did not affect workers, ventilation nor equipment. As a continuation the fall also fell into the #5 heading on 4-22-10 appx. 8' high x 18' wide x 10' in length. The area has been cleaned up and resupported.

Injury was using a drill in the installation of a mechanical belt guard. The drill bit hung and broke causing lacerations to the fingers and fractured his right index finger.

EE was in the process of hanging the conveyor belts system by using a chain ratchet. He was driving the hook of the ratchet under a roof bolt plate when he struck his left index finger. He was examined at Middlesboro Hospital where it was determined that he had fractured the tip of the finger. He returned to work that day and did not miss any time

The roof fall fell in the intersection of #5 heading. It was approximately 20'wide X 30'long X 8' high. No persons nor equipment were involved. The area has been dangered off and cross beams installed.

EE was roof bolting when the drill steel broke and struck him in the mouth. He was transported to the hospital and dentist where he recieved 5 stitches to the inside of his mouth. He also lost a tooth. EE returned to work on the next scheduled shift.
