Employee stated they were pulling electrical wire and slipped and fell on a rock landing on right shoulder. Employee was pulling wire for a new construction crushing plant. Employee said they were not injured. Employee worked the rest of the day. Employee works half a day on Wednesday July 9th and complains of shoulder pain.

EE stepped onto a concrete support approximately 6 inches tall, EE's foot shifted, and EE stumbled. Causing EE to fall. EE chose to not go to the Dr that day then the next day worked full day and felt should get it checked. Dr took x-ray and it came back negative. EE was seen on 10/24 and diagnosed with a triangle crack.

On 11/10, EE told supervisor EE was having abdominal pain, thought it was a personal health issue. On 11/15, doctor diagnosed with a groin strain and gave restrictions. EE claims the injury occurred while handling worn shoes on the VSI crusher on 11/09. Worn shoes weigh approx. 50 lbs.
Employee was changing out a punch screen on a screen deck with a pry bar. The pry bar slipped causing the screen to fall, pinching the right middle finger between the screen and screen deck structure. The finger suffered a fracture on the tip. Employee was released to full duty.

EE was drill steel on a drill. He was using his boom to hold the steel with a nylon strap. He was holding the strap and his hand was between the steel and strap. The steel caught on the striker bar and started turning the strap which wrapped around the steel while it turned it grabbed the tip of left little finger and jerked it off.
