Employee was on the operator side of roof drill installing roof strap. It was the first row of bolts in #4 entry, and employee was installing outside bolt and was trying to install strap to hold loose rock. When the T-Bar was lowered a piece of rock measuring 7' long 3' wide & 3 inches thick slid down the T-Bar and struck EE in the left lower leg and ankle.
Employee was struck by a piece of draw rock 3' long by 2' wide 4 to 6 inches thick. The rock glanced off the employees hard hat and struck right hand. Employee felt pain in neck and right hand.

Rub Rail of Continuous Miner Fell on Employees Right Foot
A rockfall was found by second shift at break 67 on 1 West Mains.
Employee was in a scoop bucket going to the batteries first and the breaker knocked on the scoop which in turn locked up the park brake striking EE up against the ejector blade.
Employee was helping wash slope when employee fell on foot hurting ankle.
Employee bent down to check water line coupling when EE stood up the left knee popped and EE felt pain in knee. However EE came back to work the following day and worked 5 days before starting to miss work on May 1st. EE did not notify us of absence nor did the mines.
Employee was cutting a hole in a bit bucket. Knife slipped and cut employees left hand.
Employee taking vent pipe down from old charger barn and when employee went to pitch vent pipe the edge cut through employee's glove and cut the palm of the hand.
