Struck against stationary object

Employee was in the surface shop and bumped a hand against a circular saw blade causing a cut on right hand. The cut was on the knuckle between the index and middle finger. Employee was taken to a medical clinic in Hazard, KY and received 4 stitches.

Struck by falling object

Employee was on the operator side of roof drill installing roof strap. It was the first row of bolts in #4 entry, and employee was installing outside bolt and was trying to install strap to hold loose rock. When the T-Bar was lowered a piece of rock measuring 7' long 3' wide & 3 inches thick slid down the T-Bar and struck EE in the left lower leg and ankle.

Employee was struck by a piece of draw rock 3' long by 2' wide 4 to 6 inches thick. The rock glanced off the employees hard hat and struck right hand. Employee felt pain in neck and right hand.

Employee was in a scoop bucket going to the batteries first and the breaker knocked on the scoop which in turn locked up the park brake striking EE up against the ejector blade.

Employee was helping wash slope when employee fell on foot hurting ankle.

Employee bent down to check water line coupling when EE stood up the left knee popped and EE felt pain in knee. However EE came back to work the following day and worked 5 days before starting to miss work on May 1st. EE did not notify us of absence nor did the mines.

Employee was lifting a drill pot out of a mantrip to load onto another mantrip, with assistance from another employee. When the assisting employee picked up one side of the drill pot shifting all the weight to the injured employees side and EE stated that felt a tearing and ripping sensation in left arm, shoulder, neck and back.

Employee was at the bottom of slope uncoupling the supply cars from the slope car. When he pulled the lever to uncouple the cars he felt a sharp pain in his lower/mid back and he had numbness in his left leg.

Employee was installing a draw head on his RRD motor before coupling to the rock duster. A second supply motor was coming up the track and the operator did not notice that the switch was thrown toward the side track where the employee was working. The second motor hit the RRD motor and knocked it into employee.
