Struck against a moving object

Employee was pushing material (shot rock) from a break down shot, when his dozer went over edge of highwall. At this time accident is under investigation by MSHA and OMSL.

Struck against stationary object

Employee was removing a bolt from the bottom of a loader bucket wear plate. As they were reaching under the bucket to get the bolt, the sharp edge of the bolt cut the palm of left hand. Eight (8) sutures were required to repair the wound. The employee returned back to work on the same shift and finished the shift. Employee was returned back to work full duty.

Employee was walking from EE's private vehicle to EE's rock truck. As EE was walking across the parking at the truck ditch, EE slipped and fell on the snow and ice. EE was diagnosed with a fractured femur and started missing work on 1/11/2025

The miner was changing a tire on a Cat 789 rock truck when EE's finger was caught between the locking ring and bead ring. The tip of EE's right ring finger was smashed causing the tissue and skin to be removed from it.

Employee was trying to break up some lodged anfo in the bed of a bulk anfo truck while employee was using a shovel employee had a sharp pain travel through shoulder and neck area. Employee started missing work on 8/12/20.

Employee was starting equipment prior to other employees running the equipment. EE had started a D11 dozer and when EE stepped off the platform onto the track, EE's foot slipped off the wet track causing left ankle to roll to the side causing a severe strain.

Fall from machine

Employee was coming down hill and knocked the truck out of gear and could not get it back in gear. Employee jumped out of the truck breaking left ankle and causing a laceration to the back of the head.

Installing a hydraulic hose on main frame of 16 grader when lower back popped and EE felt a stabbing pain

Employee was backing under the shovel and hit the bucket of the shovel with the bed of the truck causing pain to neck.

Employee was running a front end loader and pushed the declutch and felt pain in left knee. Re injury of old injury.

Employee was standing on steps of 16 M grader cleaning windows and the door came back on EE knocking EE off the steps and hitting EE's head on corner of grader blade. Required 12 staples.

Employee was exiting a 4400 rock truck and slipped coming down the steps and fell breaking a bone in left hand.

While installing an engine ee began to fill a tightness and numbness in upper left arm and shoulder.

Struck by powered moving object

Employee was removing lug nuts from a 789 rock truck wheel using an impact wrench. Wrench kicked back and struck employee in the mouth causing damage to several of their teeth.

While oiling equipment a piece of rock from a blast hit the employee in lower back. Employee started missing work 12/11/2018.

Employee was driving rock truck and a rock was dumped and slid across bed of rock truck and employee stated felt pain in lower back.

Employee was installing an Equalizer E-Bar pin on a D11R, Unit #3121, at location. EE was installing the left side using a hammer when a piece of the pin broke off striking them in the left leg below the knee. Went to ER for foreign body removal. Given antibiotics for infection prevention.

Knocking a pin out of ebar and ebar fell pinning finger against the belly pan of the machine lacerating finger

Employee stated was backing up with the grader and did not see the rock truck due to the standard inside the cab of the grader. The front of the rock truck caught the ripper arm of the grader spinning the grader around and hitting the back tire of the rock truck. This caused EE to fracture 3 ribs.

EE haulage truck was transporting coal to typo tipple on haul road at east long mine when ee truck was struck in the side from behind by another ee truck driven. The other ee jumped from the truck making it a runaway because it was said it would not stop

Driver lost control, jumped from moving, loaded vehicle. Broke both feet.

Employee was tightening lug nut on the front wheel of a rock truck. EE was holding the wheel cleat with one hand and tightening nut with an impact wrench using the other hand. The wheel cleat slipped and crushed EE's finger.

Changing a tire on an articulate truck while breaking tire down from rim with a hydraulic ram the ram kicked out under pressure and struck employee in the face.

After working on an engine, crawled out from under truck raised up and hit water cannon mounted on back of water tank with head, causing a laceration

Employee stated they were climbing upon the rock truck and felt pain. No one was notified until 10/4/2016.

EE came down off of a 6ft ladder and walked towards tool box on the service truck. EE slipped on hard packed snow falling and hit right knee on a rock. EE was inspecting the Fire Suppression System on a dozer. Temp. at the time of injuring was 17 degrees with 30 mph winds.

Employee was starting pump and lost his balance and fell causing pain in left hip and leg.

Putting hose from bulk tank back up into the door of the lube truck, pushing it up with his left arm up to the right. Felt shoulder pop. Victim stated he had both rotator cups repaired in the past because of previous work related injuries for other companies.

Employee was putting oil onto truck. Ground had been frozen and was thawing. He slipped and fell onto his left knee and right hand. Said he heard knee pop and right thumb was swollen. Taken to Hazard ARH ER. X-ray's were negative. Released to work. Went back to doctor 01/20/2015. Removed from work until MRI.

Turned ankle while stepping down. Refused to go to ER. The incident occurred under previous ownership Arch Coal. Pine Branch took over the properties on March 4th. On April 16th a citation was issued to Pine Branch for failing to report. This report is being completed to avoid further action being taken against Pine Branch after discussions with MSHA.

Installing tire chains on a 992G loader tire. Using a sledge hammer and pry bars. Employee thought he had pulled a muscle in his chess/shoulder.

EE was getting off his dozer at the end of his shift when he slipped on a rock that had became wedged in the step that was located on the push arm of the dozer.

Employee had just finshed routine maintenance on a drill and as he was walking away from drill he slipped on a berm of dirt. When he reacted and tried to catch himself he devloped a pain in the left side of his ribs.EE finished his shift and went to the Dr. on 09/09/2010 and was placed on light duty work for five days.

EE was operating a 240 Ton Terex truck getting loaded when a rock slid off the bucket of the shovel causing a sudden jar which resulted in a pain in Employee's neck.

Struck by flying object

While changing rock truck tires the bead breaker fitting (when under pressure) threads stripped out and fitting struck ee in mouth & chin. Resulting to 5 stitches in the chin and a chipped tooth.

EE stated that as he was dismounting his equipment at quit time, his shoulder felt as if it popped out of socket. EE was taken to Emergency room and results were negative. EE went to his personal DR. on 06/24/09 and we are waiting results from test. EE had surgery on this shoulder in 2008.

Employee was mounting his drill when he developed a pain in his lower abdomen.

Employee stated the dozer he was operting got on a rock and slid sideways when the tracks got off the rock it came to a sudden stop causing a jar to his shoulder. He continued to work until 4/16/09 when he went to dr and was referred to farily dr for an MRI. He was placed off work on 4/21/09.

EE was operating a 1500 Komatsu Haul truck being loaded by an O&K 340 shovel and stated he received a jar from a rock that was being loaded. EE continued to work until 01/16/09 when he went to Dr. and was placed off work with a strain to the back.

Employee was cleaning floor of 777 Cat truck and developed pain in his back. EE went to Dr. on 9-12-07 and had an MRI performed on 9-21-07. MRI results were read on 9-27-07. Employee is off work.

Employe was changing oil in 988 F loader when he hit his left hand between thumb & forefinger causing laceration which required 6 stitches. EE ret'd to work & finished shift.

Employee was burning empty parts boxes after repair when an empty ether can in one of the boxes blew up burning employee's face. (Local MSHA office at Hazard KY was notified immediately.)

Employee was dismounting a d-10 after completing maintance work and stepped on a rock and sprained his ankle. He finished shift and went to dr. following day and was refered to a bone specilist who discovered a hairline fracture in ankle. first day of work was 1/5/07.
