While directing activities to re-support ribs in an intersection, a rib roll struck a standing roof support which in turn struck the foreman in the head causing fatal injuries.

Struck against stationary object

Employee struck head against the mine roof which caused stiffness and pain in employee's neck while operating a Lo Trac Machine. Employee did not want to go to the hospital/doctor at the time of the accident but went the next day.

A roof fall was discovered on this date while conducting other work in this area. The fall measured 20' by 20' and was 6-8' in thickness. The fall was located in the #1 entry of the 3-North-2 Mains at crosscut #11. The area was originally supported with 5' Hi-Tech bolts. The area was dangered off. No persons or ventilation was affected.

While dismounting the mantrip EE said slipped when getting out, put hand down to catch self & felt a pop in left shoulder.

Over-exertion in lifting objects

Employee was putting a split header on a roof bolt while standing on a ladder. Another employee handed a header and as employee raised the header ee felt something pop in left shoulder. EE began missing work on 8/16/2017.

Over-exertion in lifting objects

While handling a bundle of 8 foot roof bolts the employee felt sharp pain in right shoulder. EE began missing work on 8/15/2017 to receive treatment on the shoulder.

Struck by falling object

While worker was repairing a water valve, a piece of draw rock fell striking EE on the right shoulder and head. EE continued working and began missing work on 3/27/2017 when taken off work by a physician.

Struck against a moving object

Employee was driving a man trip down the roadway and hit a pothole causing employee to be jolted straining lower back.

Struck by falling object

While the roof bolter operator was starting to drill a hole for a rib bolt, eight channel straps slid forward into the operator station striking right leg above the ankle causing a fracture.

Over-exertion in lifting objects

Employee was lifting two five gallon oil cans when EE felt something pop in left elbow. EE continued working and later sought medical attention. A medical procedure was performed on the elbow on 1/17/2017 making this incident reportable.

A roof fall was discovered on this date. The fall was located on the 1-West Main at crosscut #12 in the #1 entry. The fall measured 20' by 25' and was 10' in thickness. The area was previously supported with 5' Hy-tech bolts and 12' cable bolts. Affected miners were removed from inby the location and the primary escapeway was re-routed.

A roof fall was discovered on this date. The fall was located on the 3-North-1 Mains at crosscut #9 in the number 4 entry. The fall measured 16' by 18' and was 5-7' in thickness. The fall was originally supported with 5' hi-tech bolts and 8' cable bolts. The primary escape way was rerouted and all employees were notified of these changes.

Struck by falling object

While employee was pulling a piece of top belt structure out of the belt, the structure became stuck. EE pulled harder and when the structure released, EE fell backwards with the structure hitting right leg below the knee causing a laceration requiring staples. EE returned to normal duties the next working shift.

Over-exertion in lifting objects

While employee was hanging a shuttle car cable, he missed the hanger causing he cable to pull his arms forward straining his right shoulder. He continued working but began missing work on 1/18/2016 when he had surgery to repair a tear in the shoulder.

Struck against a moving object

Employee was pushing a ram car up a grade with a diesel scoop. The ram car gained traction allowing the bucket of the scoop to lose contact with the ram car. The front wheels of the scoop were raised approximately 10 inches off the mine floor. The scoop fell back to the floor causing the operator to hit his head on the canopy of the scoop causing a neck strain.

When employee lowered the boom on the roof bolter after drilling a hole, the steel came out of chuck and struck him on the left thumb. He later sought medical treatment and had surgery on the thumb to repair a torn ligament on 10/22/2015.

A roof fall occurred on 3-North-1 Mains, #2 entry, crosscut #9. The fall measured 20' by 20' and was 9' in thickness. The area was originally supported with 5' Hi-Tech bolts, truss bolts, cribs and beams. There were no injuries and the escapeway was re-routed. The fall will be cleaned up and re-supported.

A roof fall was discovered on this date. The fall was located on 3-North-2 Mains in the #4 entry between the #14 & #15 crosscuts. The fall measured 30' by 19' and was 58"" in thickness. The area was previously supported by 5' Hy-Tech bolts and beams. The fall will be cleaned up and the escape way was re-routed.

Rubbed or abraded

On this date, employee reported swelling and pain in his right knee at the beginning of his shift. He sought treatment on 3/10/2015 and was diagnosed with prepatellar bursitis (Miner's Knee)in the knee which required surgery. He began missing work on 3/10/2015.

Employee has been having pain in his back. On this date be began missing work due to pain in his lower back. An MRI examination shows degenerative disc disease at the L5/S1. He remains off work at this time.

A roof fall occurred on this date. The fall is located on 3-North-1 in the #2 entry at crosscut #7. All miners located inby the fall were brought outby the fall area and the secondary escapeway was rerouted. The fall area was previously supported with 5' Hy-tech bolts and supplemented with cable bolts. The fall will be cleaned up and the area resupported.

Caught in, under or between a moving and a stationary object

While the ride was backing out of the motor barn, the employee got his foot caught between the ride and a barrier post that was located outside the motor barn. He suffered lacerations on three toes on his left foot that required sutures. The employee returned to work his next regular shift on 12/1/2014 after the Thanksgiving holidays.

Fall from machine

While employee was getting out of a scoop, his boot hung up on the door causing him to fall. He tried to catch himself he suffered a fracture of his right wrist. He returned to work under restricted duty after missing two days for treatment.

A roof fall was discovered on this date while examiner was conducting his weekly exams. The fall was located on 4-North Mains, #5 entry between crosscuts #10 and #11. The fall was 20' wide by 30' long and was 8' in thickness. Ventilation or persons was affected. The fall is cribbed off.

While employee was tramming a roof bolter from #3 to #2 entry, he engaged the cable reel take up on the roof bolter which caused the cable to pull a rope hanger attached to the cable from a rib bolt. The hook on the hanger struck the employee in the right eye causing an eye injury that required medical treatment and lost days. He was wearing eye protection.

After the ATRS had been set worker was walking up the drill station when a piece of rock measuring 48"" by 26"" by 1 1/2"" fell and slid down the T-bar and struck him on the head and back. He suffered a laceration on his head that was 2 centimeters in length that required three staples.

The fall measured 20'X 20' and was 8 feet in thickness. The fall will be cleaned up and resupported. the areas was previously supported with 5' hy-tech bolts and 8' cable bolts. Caught in, under or between a moving and a stationary object

While installing a cable bolt, the employee reached up to align the plate when he hit the joystick on the roof causing the tip of his middle right finger to get caught between the plate and the rock deflector causing an avulsion.

Struck by rolling or sliding object

While employee was trying to search for a broken tie rod end on the car he was operating, he was struck by a piece of rib rock that rolled down the rib striking his left ankle causing a fracture and dislocation.

Struck by falling object

While employee was installing a permanent stopping, a piece of draw rock fell striking him on the left foot resulting in a fractured big toe requiring treatment.

While employee was bolting, the strap he was installing got caught on the mast of the roof bolter as it was being swung out. When the strap came free, it struck the employee on the forehead over the right eye causing a laceration requiring sutures.

Fall from ladders

As the employee was standing on a ladder to hang a ventilation curtain, the ladder fell over resulting in the employee falling to the mine floor. He suffered a dislocated left ankle.

Struck by rolling or sliding object

While the foreman was walking to the continuous miner, a piece of rib rock slid down the rib. When the foreman put his hand out, the rocks struck his right hand causing a laceration to his right index finger requiring sutures.

While dumping a load of coal at the feeder, the employee placed his right forearm on the spill board. A piece of rock traveling down the car conveyor struck his forearm causing a laceration requiring sutures.

EE has recently had arthroscopy surgery to his right knee. He now alleges that he fell on or around Feb. 6, 2012 while hanging curtains. He states that his knee became increasingly painful and he sought medical attention. He had not reported any injury to me and I assumed this was a chronic problem that was unrelated to his employment with our company.

Over-exertion in pulling or pushing objects

While pushing a skirt at a tail piece, the employee experienced pain in his right arm. It was later determined that he had suffered a torn tendon in his elbow. He had surgery on the elbow on 1/30/2012 and missed two shifts of work.

Fall from ladders

Employee was standing on a ladder hanging a high voltage line when the ladder twisted causing him to fall approximately 7 feet to the mine floor injuring his right knee. He began missing work on 3/27/2012 to have surgery for a meniscal tear.

Employee was cutting a rope hanger to lower the high line to the mine floor during a power move. When he cut a hanger near a coupling, the coupling fell causing employee to cut his right leg with the knife he was holding. The cut required sutures.

While employee was bolting a piece of draw rock fell striking the canopy of the machine. When the rock fell he turned and placed his hand on the bolter fan. The rock fell off the canopy and struck his left hand causing a fractured little finger and lacerations.

While changing out a scrapper, the scrapper fell catching employee's right little finger between the scrapper and crossunder guard. The finger was lacerated and fractured just behind the fingernail. The employee returned to work his next scheduled shift.

While moving an oxygen cylinder the employee tripped over a rock and twisted his right knee. This injury became reportable when he received medical treatment on 12/03/2010. The employee has not misssed any work.

As employee was trying to lower the boom on the roof bolter, he hit the wrong lever and closed the steel guide catching his finger between the guide and steel. He suffered a fractured left middle finger. He began missing work due to the injury on 3/15/2010.

Employee has been diagnosed with carpal tunnel illness in both wrists. There is no particular injury date involved. It has been determined that the illness was work related.

Employee was installing a rib bolt, when a piece of drawrock fell from the top measuring 15""X13""X3.5"" and struck his hand, causing a laceration requiring 10 stitches on the left index finger.

While removing a panel from a Kennedy Stopping a piece of draw rock fell striking the employee on the right forearm causing a laceration requiring sutures. The rock measured 6' by 3' and was 2"" in thickness. The employee returned to regular duties the next shift.

Inhalation of radiations, caustics, toxic and noxious substances

On this date, we were notified that this miner had been diagnosed with Coal Miner's Pneumoconosis. The employee is still working.

A roof fall was discovered on this date. Fall was located on 3 North #2, the fall started in 3 right and extended over into the intersection in #4 entry in the 4th crosscut. The fall measured 8 Feet in thickness. The fall was originally supported with 5 foot High Tech bolts with supplemental cable bolts in inetrsections.

EE reported that he bent over to pick up one end of a jack to set it for roof support. When he raised up, holding the end of the jack, he said he felt a ""pop"" in his mid lower back area. He finished his shift and sought medical treatment the following day, due to continued pain to his lower back.

A roof fall occurred on the 2-East Return in the #1 Entry between the #27 and #29 crosscuts. The fall started inby X-cut #27 and ended outby x-cut #29. The fall was around 8' in thickness. The area was previously supported with 5' Hy-Tech Bolts supplemented with 8' cable bolts.

A roof fall was discovered on 003 section while a preshift examination was being performed. The fall measured 40' by 18' and was 8' in thickness. The area was supported with 5' Hy-Tech bolts and 8' cable bolts. The fall covered up the continuous miner that was not in operation.

While the employee was on a ladder hanging a ventilation curtain, a piece of draw rock fell striking him and the ladder causing him to fall from the ladder. He suffered a laceration on his left arm and a contusion to his left hip.

While checking the suction on the drill head, the roof bolter steel came out of the drilled hole and struck the employee on the left middle finger causing a laceration requiring sutures.

While descending from the tip of the continuous miner, the employee slipped and fell, striking his back on the mine floor. He began missing work on 4/01/2009 and the injury was confirmed as work related on 4/6/2009.

While shoveling a tailpiece, the employee was struck by a piece of rib rock on the head and neck. The rock measured 30"" by 8"" and was 1"" to 4"" in thickness. The employee continued working and started missing work on 4/13/2009.

As employee dropped the head on the roof bolter a hose on the boom hit a piece of drill steel that was sticking out of the tray. The drill steel came up and hit the employee in the mouth chipping two teeth.

While bolting, the drill wrench broke. While the employee was retrieving the broken piece of the wrench out of the drill pot, the drill steel fell out of the hole striking his left index finger resulting in a fracture to the finger.

The roof fell in the #12 lift after the lift had been mined and extended out into the entry in front of the #2 MRS. The fall pulled four bolts. A retriever was used to recover the MRS. The roof was previously supported with 5' Hy-Tech bolts. The fall was on 003 section 4-Right Panel between the #11 and #12 crosscuts.

A roof fall was discovered in the roadway during a preshift exam. The fall measures 12-14' wide by 20' long and was 5-6' in thickness. The fall was in the 4-North Mains at crosscut #38 in the #2 entry. The area was previously supported with 5' HY-Tech bolts and truss bolts. The area will be cleaned up and resupported. An MSHA inspector was present at the minesite.

On this date the employee stated that his doctor had diagnosed him with carpel tunnel in both wrists. Surgury may be performed on a later date.

Struck by falling object

A piece of draw rock measuring 24"" by 12"" and 1-3"" in thickness fell hitting the canopy of the roof bolter. A piece of the rock measuring 8"" by 12"" and 3"" in thickness broke off the larger rock and struck the roof bolter pusher plate mounted on the drill boom, and then bounced back striking the employee in the nose resulting in a laceration requiring 12 sutures.

Caught in, under or between a moving and a stationary object

Employee was near the end of a power center, watching a bridge cable being pulled through a sheave wheel attached to the power center. The cable hung up, causing the power center to move onto the employees hand. He suffered a dislocated little finger and a laceration, requiring sutures on his ring finger on his right hand.

A roof fall occurred in the #4 entry at the #11 crosscut on the 3-North-1 Mains in the intake entry. The fall was 8' to 9' in thickness and measured 20' by 20'. The area was previously supported with 5' HY-Tech Bolts. The fall was cleaned up and re-supported.

As the employee was drilling a hole to install a roof bolt, a piece of draw rock fell lacerating his right arm requiring sutures.

A roof occured in the 3-North Intake Entry. The fall was located in the #12 crosscut in the #4 entry. The fall was 20' by 20' and 8' to 9' in thickness. The area was originally supported with 5' Hytech Bolts. The fall will be cleaned up and re-supported. The fall was immediately reported to MSHA.

As the employee was walking down a ramp in the #6 entry, the gob under his foot gave way causing him to twist his left knee. He started missing work on 6-3-08 for surgery to repair a torn meniscus.

The EE was taking down a kennedy stopping panel next to the coal rib. As the panel was removed, a piece of draw rock fell, striking the EE and caused him to fall to mine floor. The panel then fell, striking him on the wrist, causing a laceration that required sutured. The rock measured 17"" X 14"" and was 2"" in thickness.

The roof fell in the #31 lift after the lift had been mined putting extra pressure on the #3 MRS. No roof bolts were pulled in front of the MRS. A retrelver was used to pull the MRS free. There were no injuries.

On this date we were notified by MSHA that the employee was diagnosed with pneumoconiosis.

Struck by falling object

As EE was checking the suction on the head of roof bolter, a piece of drill steel fell out of hole being drilled & struck left ring finger.

Struck by falling object

As EE was operating the continuous miner, a piece of rock hit him on the front of his hard hat, pushing it down into his face. Either the bill of the hard hat or the rock hit his nose, causing his nose to be fractured. The EE does not know if the rock came from the roof or rib. He is scheduled to have surgery on 10/3/07.

The roof fell prematurely in front of the #3 and #4 MRS units pulling 4 rows of roof bolts. The miner breaker had knocked while in the #32 lift and the miner had to be retrieved. This delay allowed the roof to work and load up the MRS. When the MRS units were released to move, the roof fell. The roof was previously supported with five foot Hy-Tech bolts.

As EE was walking through a water hole, he slipped & strained back. The EE did not fall. EE started missing work 7/10/07.

A roof fall occurred during retreat mining, pulling two rows of bolts in front of the #1 MRS Unit. The area was previously supported with 5' Hi-Tech Bolts. There were no injuries to persons. The fall was located at 1-Right North-2 at spad 1470, at break #39.

Ee presented a letter to Lone Mountain reporting the results of a chest x ray finding from the National Institute for occupational safety and helath. The result noted evidence of pneumoconiosis. He continues to work.

After taking the second lift (#42 on the pillar cut plan) a piece of draw rock fell cutting the #2 MRS cable. This delay allowed the roof to work, then fall on the #1 and #2 mobile roof supports before the cable could be repaired. The fall measured 18' by 28' long and was 4' to 5' in thickness.

A roof fall has occurred on 1-South, 19th crosscut in the no. 7 entry intersection extending toward the no. 6 entry. The fall was 40' x 18' x 5-6 feet in thickness. The area was originally supported with 5' High Tech Bolts. The fall did not affect ventilation.

EE had just finished working on reel chain on a shuttle car. After repairs were complete, the car operator began moving the car. After the car left the feeder & started to the face, the EE was tripped by a loop of cable he did not notice in a mud hole. He fell onto his right shoulder resulting in the shoulder being dislocated.

He was standing outside of the shuttle car when a rib roll occurred and hit his leg against the deck of the car, resulting in a strain to the right knee. This occurred on the 003-0 section at the section feeder.

Fall from ladders

While employee was on ladder hanging a line curtain, the ladder slipped causing him to fall off the ladder onto some gob. He twisted his right ankle.

He was working on the head motor leads of the continuous miner and was cutting the shielding from the leads. His knife slipped and caused a laceration to his left hand, requiring stitches.

Employee was unhooking a chain hoist that was being used to position a run through the tail piece when he slipped & fell off the conveyor belt onto the mine floor. He suffered contusions to his left hip & lower back.

Employee was drilling hole to install roof bolt when piece of draw rock fell striking his right elbow causing a laceration. The rock measured 12"" x 6"" x 1/2"" in thickness.

A roof fall occurred on the 001-0 section in the 5 Left off of 1 East Panel; at the #8 crosscut, between # 2 and # 3 entries. The fall caught the 2 MRS's, falling in front of them and through the crosscut. The fall was approximately 4 to 7 feet thick and 20 feet wide. It ran from the # 2 intersection over through the crosscut toward # 3.

A gob pillar fall occured inby on the 002-0 section that allowed some top to set down on 2 MRS's. As they prepared to advance one of the MRS's up, additional rock fell in front of the MRS. Both MRS's were recovered safely. This occured in the 1st left panel off of 1 south, in the #4 entry, at spad #970 at the #43 lift.

He was moving a floor jack and threw it down. The jack handle bounced up and struck him in the face below the eye.

EMPLOYEE WAS TURNING AROUND ON A HILL WITH A BAG OF ROCK DUST IN HIS HANDS & TWISTED HIS KNEE. HE CONTINUED TO WORK & HAD KNEE SURGERY TO REPAIR TORN CARTILAGE ON 4/14/06. HE RETURNED TO WORK ON 4/17/06 ON RESTRICTED DUTY.

HE HAD REPAIRED A CABLE ON SHUTTLE CAR & RESTORED THE POWER. HE INSTRUCTED THE OPERATOR TO START THE PUMP MOTOR TO REEL UP THE CABLE, WHEN THE CAR STARTED, A LOOP OF THE CABLE PULLED HIS FEET FROM UNDER HIM, CAUSING HIM TO FALL.

HE WAS OPERATING THE RIGHT DRILL HEAD ON THE 001 SECTION DRILL. HE HAD DRILLED THE HOLE AND WAS INSTALLING A CABLE BOLT. HE HAD PUSHED THE BOLT INTO THE HOLE AND WAS ATTEMPTING TO PLACE THE BOLT HEAD INTO THE TIGHTENING WRENCH WHEN HE CLOSED THE STEEL GUIDE ON HIS THUMB.

Struck by falling object

He was cleaning the scrubber as part of normal maintenance. He placed his hand on the frame of the miner to pull slack to the water hose and the scrubber lid fell on his hand.

EE WAS ASSISTING 2 CO-WORKERS IN MOVING A CABLE OUT OF THE ROADWAY BY HAND. AS THEY LIFTED THE CABLE, HE FELT PAIN IN HIS BACK. HE BEGAN MISSING WORK 9/17/04

EE ALLEGES THAT HE ASKED A COWORKER TO SCOOP THE FEEDER BECAUSE COAL HAD BEEN SPILLED IN FRONT OF IT. EE ALSO ALLEGES THAT THE COWORKER GOT MAD AND STRUCK HIM IN THE HEAD WITH A BLOCK HAMMER. THE COWORKER ALLEGES THAT HE WAS ATTACKED BY EE. NO OTHER WITNESSES. BOTH EMPLOYEES WERE TERMINATED. (LONE MOUNTAIN DISAGREES THAT THIS IS REPORTABLE, REPORTING PER MSHA'S INSTRUCTIONS.)

HE WAS OPERATING THE RIGHT DRILL HEAD ON THE 001 SECTION DRILL. HE HAD DRILLED THE HOLE AND WAS INSTALLING THE RESING CARTRIDGE AND BOLT. THE BOLT HUNG IN THE LIP OF THE HOLE, CAUSING THE BOLT TO BEND, CATCHING HIS LEFT RING FINGER BETWEEN THE BOLT AND THE MAST.
