EE WAS UNHOOKING A CHAIN FROM THE TAILPIECE THAT WAS ATTACHED TO A SCOOP AND DID NOT GIVE THE SCOOP ENOUGH TIME TO COME TO A COMPLETE STOP AND GOT HIS FINGER CAUGHT IN THE CHAIN, RESULTING IN A CUT TO THE END OF HIS FINGER.

EE WAS MAKING A BELT SPLICE. AS HE STRUCK THE CRIMPING TOOL WITH THE HAMMER, A SMALL PIECE OF THE CRIMPING TOOL CAME OFF AND LODGED IN HIS LEFT ARM.

EE was moving the continuous miner cable out of the haul road and the blew up and burned his right arm and right side. The cable had been ran over by a shuttle car.

EE was bending a roof bolt while bolting top and felt a sharp pain in his groin area.

Was lifting miner cable to hang it and pulled groin.

EE was cutting a dust hose for the roof bolter and the knife slipped and cut his left leg requiring stitches.

EE was in the No. 3 entry of the 003-004 MMU bolting top. He was positioning his drill steel against the mine roof and a piece of draw rock fell and struck him on the left hand causing a cut on the left ring finger.

EE was jacking a mantrip back onto track and the jack slipped out and the bar caught his foot and fractured it.

A ROOF FALL OCCURRED IN THE NO. 1 ENTRY TWO BREAKS INBY SURVEY STATION NO. 26118 OF THE 8TH NORTHWEST PANEL BLEEDER SYSTEM. THE FALL IS APPROXIMATELY 18 FEET LONG X 18 FEET WIDE X 10 FEET THICK. ROOF FALL WAS FOUND AND REPORTED BY MINE FOREMAN AND OMSL INSPECTOR WHILE TRAVELING THE BLEEDER SYSTEM.

The continuous miner was cutting in the #8 heading of the 7A Northeast Mains when it breached old works in the right hand corner of the face. The old works had been mined in 2004. Water entered the active section through the hole. The power was disconnected and the men immediately evacuated the section to the surface. MSHA was notified at 4:07 pm.

A roof fall occurred one break inby survey station 25895 in the #6 entry of the 6A Northeast Mains. The fall was approximately 60' long x 20' wide x 8' thick. The fall is approximately 1600' from the working section.

#5 entry loading supplies and cut (R) forearm, 10 stitches.

Dislocated (L) shoulder.

A roof fall occurred approximately 280' outby the working section in the #3 entry. The fall was 1 break inby survey station # 25975. The fall was approximately 25' long x 18' wide x 10' thick.

Caught in, under or between a moving and a stationary object

EE had put rock dust in a scoop bucket and laid his hand between the ejector blade and the back of the scoop bucket. He told the operator he was done and the operator started the scoop and pulled on the ejector blade lever to release the hydraulic brake, resulting in his fingers(2) on the left hand being broken.

EE was using a cutting torch to cut a piece of metal. There was grease on the back side of the metal, and the grease popped out onto his hand, causing burn.

While doing an E01 inspection of the bleeder system on the 11a southwest panel, a roof fall was found. The fall was located 3 breaks inby survey station # 24977 beginning in the X-cut between #5 & #6 and ending between #7 & #8. The fall was approx. 140' long x 19' wide x 8' high.

While doing and E01 inspection of the bleeder system on the 11a southeast panel, a roof fall was found. The fall was located 3 breaks inby survey station # 2535 in the #8 entry. The fall was in the intersection and measured approx. 30' long x 19' wide x 4' high.

A NON-INJURY ROOF FALL OCCURRED ALONG THE NO. 10 CONVEYOR BELT ENTRY. THE FALL MEASURED 40' LONG, 20' WIDE. AND 5' THICK. THE FALL WAS THREE BREAKS INBY SURVEY STATION NO. 25120 AND WAS 280' OUTBY THE COAL FEEDER.

A roof fall occurred in the #4 entry approximately 560 feet from the working section. The fall is located 2 breaks outby survey station #25119. The fall measured 30' long, 09' wide, and 6.5' thick.

EE was helping to change bits on the continuous miner. Another employee had been tapping a bit with a hammer and EE thought that he was finished hitting it. As he grabbed hold of the bit, the other employee struck him on the right thumb, resulting in his thumb being broken.

A roof fall occured in the #5 entry (beltline) one (1) break outby survey station #24897. The fall measured approximately 30 ft. long, 20 ft. wide, and 7 ft. thick. The fall was approximately 2400 ft. from the active 003-0/004-0 mmu's. No airway or emergency escapeways were affected by the fall.

EE was walking in the #3 entry of the 003 mmu and stepped in a hole in the haul road and strained his back. He worked regular job on 11/3/06. Did not seek medical attention until 11/6/06.

EE was operating a continuous miner in the #6 entry of the 004 section and a coal rib rolled off and struck him on the right side of his body, injuring his back, hips, and leg.

Smashed his finger while changing rollers. He dropped roller on his finger while changing out belt rollers.

EE was operating a rail runner and entered a dip in the track where another motor was located. He struck the mantrip motor, causing injury to lower right leg. EE did not see medical attention until 9/17/05. He was diagnosed with a hairline fracture. He is presently working light duty. Warning lights were on, which indicateds that there was another motor in the dip.

MAN TRIP LOST TRACTION ON AN INCLINE AND STRUCK BELT RIDE. THIS HAPPENED ON TRACK UNDERGROUND.

Struck by falling object

EE was drilling a hole in the mine roof with a roof bolting machine and the drill steel got fouled. After removing the first piece of steel, the second piece of steel fell from hole and struck him on the right index finger.

Over-exertion in lifting objects

Two employees were working on a belthead, preparing to take the slack out of the drive chain. When one lifted the top cover that guards the drive chain, he felt a sharp pain in his lower back.

EE WAS TRAMMING IN HIS SHUTTLE CAR TO THE MINER WHEN A RIB ROLLED OFF STRIKING A PILE OF TIMBERS LYING IN BREAKER LINE NEXT TO THE HAUL ROAD. A TIMBER FLIPPED UP & INTO DECK OF THE SHUTTLE CAR HITTING HIM IN THE RIBS. HE WORKED THE REST OF HIS SHIFT & WENT HOME & HAD HIS WIFE TAKE HIM TO THE E.R. THAT EVENING. EXAMINED & RELEASED.
