Accident type, without injuries

Roof Fall inby spad #2727 (60'L x 20'W x 4'H). The section was moved back 5 crosscuts.

Fall to the walkway or working surface

EE was trying to dislodge mantrip that was stuck in mud, when he fell injuring his back and left leg. He then missed work on the following day due to related back pain.

Inhalation of radiations, caustics, toxic and noxious substances

Pneumoconiosis - Premier Elkhorn Safety Dept. personnel were notified that the former employee had filed a claim.

Fall onto or against objects

Employee left foot became lodged in mud. When he tried to dislodge it by jerking his leg, he struck his left knee against a belt rail.

Over-exertion in lifting objects

Employee was lifting a continuous miner cable while on his knees and strained his back while attempting to stand.

Employee dropped rock duster on his right hand, crushing & lacerating his middle finger.

EE was tramming shuttle car when the miner operator pulled his cable too tightly, pulling it down from hangers in front of the shuttle car. The shuttle car operator then went around his car to re-hang the cable when his right knee popped.

Employee was cleaning off roof bolt machine when he pulled on some loose wire that moved underneath his safety glasses causing lacerations to his right eye.

Employee was setting timbers when one fell over, striking the middle finger of his left hand resulting in multiple fractures.

An unintentional roof fall occurred in the #11 entry 30 feet outby spad #3690. The fall measured approximately 25'-30' long x 17' wide x 4'-6' high. The affected area was dangered off.

Over-exertion in lifting objects

Employee was loading belt structure onto a 4-wheeler when he experienced pain in his left shoulder. The accident wasn't deemed reportable until the employee began taking off work due to complications from this injury on Novemember 10, 2008

Struck by falling object

On 6/24/2008, a piece of draw rock struck the employee's head, neck & back. Immediately following the accident, the employee was treated & released from a local hospital, but has since sought medical treatment. Then, he began taking off work on 5/18/2009 as a result of this accident/injury.

Employee was in the operator's deck backing the Fletcher Roof Bolter out of the #8 heading, when he got his Left ring finger caught between a trailing cable and the frame of the roof bolter, causing a laceration that required stitches.

Employee was loading oil cans onto the RamCar when he dropped one can & reached to retrieve it. The can had an upturned tab on the lid, lacerating the employee's left pinky finger and requiring stitches.

An unintentional roof fall was discovered at 5:30 am on Monday, September 17, 2007. The fall occurred at Spad # 2865, 2500 feet outby the working section. The fall measured approximately 20 feet wide x 70 feet long x 5 feet high. Additional support has been added to this area to prevent future roof falls.

The Storm Security Guard was sitting in a chair at the mine office when he suddenly fell into the floor. EMTs & METs at the site tried unsuccessfully to revive the victim until an ambulance arrived, which transported him to Jenkins Community Hospital.
