Struck by... (Not Elsewhere Classified)

EE was watching the electric cable on a 2800 electric shovel which was moving. The cable had slack in it and as that slack was pulled out the EE was not far enough away from the cable when the slack came out striking him on his right knee dislocating it. As cable was pulled, the shovel door opened striking the cable jerking it into the EE.

Accident type, without injuries

Fuel Truck # K9079 caught on fire on top of hill at the Halls Creek mine on 2nd shift, operator was o.k. and no one was injured. Calls were made to MSHA and State Agencies informing them of the situation. (See Attached Sheet)

Struck by falling object

Injured party was pressure washing a hyd. cylinder with a pressure washer. He was using a 2' wand to wash with instead of the 4' wand. The wand slipped out of his left hand & caught his finger. He required 4 sutures to close up the wound. He came back to work & did not miss any work days.

Over-exertion (Not Elsewhere Classified)

Employee climbed down on a step ladder and when he got to the last step he stepped down onto uneven ground under the last step and hyper extended his right knee. Tore a tendon behind the knee cap.

Employee had just finished his shift and had parked the cat scraper he was operating. As he stepped down off the last step his foot got hung up in the last strap causing him to stagger back twisting his right knee.

Party was climbing down his haul truck. When he got to the last step to step down, his foot got caught causing him to fall backward on the ground with his foot still caught.

Accident type, without injuries

A Cat D11R Bulldozer was found on fire after work hours by an EE who was coming to work on the next shift. 2nd shift had parked the Cat D11R Bulldozer at 2:00AM at the end of the shift in the parking lot. EE coming to work on day shift at 3:30AM came upon the fire as he came in to work. No one was injured. Cat D11R Dozer was totaled.

Party was standing on the racks of a 475 Komatsu bull dozer replacing cylinder blade when he slips and fell off the dozer striking his right lower leg against the dozer requiring 11 stitches.

Party had just put water in water truck and was walking to get on water truck when he turned and heard his right knee pop. Started feeling pain soon after this.

Party was climbing the Cat 657E scraper on the rear. He was in the process of starting the rear engine when he fell off the Cat 657E scraper.

Injured party was disconnecting a pull rope off a piece of eq. He bent over and was pushing and pulling on the pull rope when he felt a pop in his chest.

Party was doing a pre-op check on his Cat 777F back dump truck. As he grabbed a hold of a handrail to bend over and look at his truck engine a piece of rust fell behind his eye wear into his right eye. He tried to wash it out but had no luck.

The injured party had climbed on a loader to use it. The loader had just been pressure washed. Injured party was cleaning the windshield, when he slipped and fell.

Injured party needed to measure something on the DrillTech drill. He got his step ladder out of his truck and climbed about 2 steps when he jammed his head into a cross beam jamming his neck.

Injured party had step off the his equipment was standing in mud. He turned to go pick something up when his left knee stuck in the mud causing him to twist his knee.

Accident type, without injuries

A fuel truck pulled up to the fuel tanks. when the driver got out he noticed smoke & fire coming out from behind cab. Called for help. Fire Dept arrived about 15 minutes later & extinguished the fire. No one was injured. There was only about 150 gal of fuel in the tank at the time of the fire.

Struck against a moving object

Injured party was driving a 777C end dump truck hauling rock out of the pit. The shovel excavator that was loading him swung around to put a load in the truck bed when it hit the back of the 777C end dump truck jarring the operator.

Injured had just got through greasing a D9N dozer and as he was climbing down his foot slipped causing him to land on his left ankle spraining it.

Injured party was getting off his bulldozer at quitting time, when he slipped as his foot touched the ground, causing him to strain his lower back. It had rained that afternoon and everything was muddy.

Injured party was holding a pin while someone was hitting it. A piece of metal flew off and struck him on his right wrist. *Didn't miss any work.

Injured party was climbing the ladder to get on his back dump truck when he struck his left knee against the ladder.

Injured party was welding on a stabilizer bar on a bolt when a piece of metal popped or blew up under his shield and lodged in his right eye. He worked the rest of his shift & did not go to the doctor until 5-19-07.

Inj EE was washing down his service truck when the steam jenny handle busted in his hand. Steam want was a ST-1500. Do not beleive the steam jenny had anything to do with this. The handles are abused and this probably caused the want or handle to bust. EE did not miss any work.

The dozer was down and several mechanics were working trying to get the track off. As the track was coming over the sprocket, one of the bushings got in a bind and a piece of metal popped off the tip of the sprocket, striking injured party under his right eye on his face.

Injured party was walking on a belt when he sliped and fell backwards striking his left shoulder. Did not miss any work until 1/24/07. This accident happened on 10/23/06. The dr did not do anything to him and he did not miss any work. Missed his 1st day of work on 1/24/07.

Fall onto or against objects

EE was G.P.S. the coal pit in north end of pit when a rock came off wall and hit him. He fell back onto other rocks hitting his head, right leg and left side.

Airing up a tire of a Mack truck, when the tire blew out and blew dust and small rocks into his face and left eye.

GOING DOWN THE RAMP IN THE WATER TRUCK & CAME OUT OF GEAR, TURNED RIGHT TO MISS HIGHWALL & ROLLED OVER ONE TIME. THE TRUCK WAS FULLY LOADED WITH WATER. THE SEAT BELT WAS ON. THE INJURED WENT TO THE E.R. & HAD SEVERAL STITCHES PUT IN HIS LEFT HAND, HE NICKED THE TENDON IN HIS HAND.

GETTING OFF OF DOZER, HE LOST HIS BALANCE AND TOE OF BOOT WAS CAUGHT ON TRACK PAD CAUSING HIM TO FALL OFF THE DOZER. HE STRAINED MUSCLES IN HIS SHOULDER AND BACK.

PARKING BRAKE ON GREASE TRUCK FAILED TO HOLD WHILE GREASER WAS SERVICING DOZER. IT TOOK OFF ROLLING BACKWARD TOWARDS FUEL TRUCK; GREASER JUMPED OFF OF DOZER TO STOP IT, HE HURT HIS BACK.

EE WAS FUELING THE 1800 SHOVEL. WHEN COMING BACK DOWN THE LADDER HIS FOOT SLIPPED OFF THE SECOND STEP AND FELL BACK ON HIS BACK ON THE ROCK FLOOR. LANDED ON SMALL ROCK.

HAD REPLACED VALVE (APPROX WT 5#) ON D-11. TURNED TO GET OFF OF THE D-11 AND TWISTED BACK (PULLED MUSCLE).

Absorption of radiations, caustics, toxic and noxious substances

WASHING ENGINE ON 1800 SHOVEL. FILLING UP A SPRAY BOTTLE FROM A 5 GALLON BUCKET (WITHOUT A HANDLE) OF WASHING SOLUTION AND DROPPED THE BUCKET. WASHING SOLUTION SPLASHED INTO EYES.
