Employee was putting air dry in rock truck and hose came off letting the air dry hit EE in the eyes. Employee started missing work on 1/18/2016.

Lining up track shoe to put track back together on a drill - the track pad slipped and mashed left index finger causing a laceration.

Fall from machine

Employee was cleaning mirrors and dropped cleaner on the ground. When he was climbing back up the end-loader his foot missed the top step and he slipped falling to the ground. Started missing work on September 11, 2015.

Struck against stationary object

Victim was climbing up ladder on 785B truck when he hit his knee on button of step. Made a small incision across knee cap about 1/2 to 3/4 long. Because it was in bend of knee, would not close. Taken to hospital for stitches.

While trying to get in a better position to install a wiring harness on a transmission he fell on his back straining it

Was on D11R fixing a light. His foot slipped as he was holding on causing him to stretch out pulling muscles in stomach. Taken to Hazard ARH on 10/30/2014 and released to return to work. He worked through to 11/10/2014, returned to doctor who stated a possible hernia and has been off work since. Scheduled to see doctor on 10/21/2014 for possible surgery.

Fall from machine

EE was operating a 785 Caterpillar when a fire erupted. Flames were blocking the escape ladder so the employee jumped from the catwalk a distance of about 14 feet.

Employee was operating an 988 Cat Loader when he backed into a 16G Cat Grader that had entered the pit area. EE continued to work and went to the DR. on 06/18/2012 after receiving a jar which resulted in reoccurring back pain and was placed off work pending an evaluation by a specialist. First day off work was 06/19/2012.

Employee was climbing into the back of his mechanics truck when he slipped and fell twisting his right knee. EE was placed on restricted work until 09/09/2011

Employee was using a pry bar to remove a pin from a lift jack on a D-11 Caterpillar Dozer when the bar slipped catching his finger between the bar and the lift jack. Employee was placed on restricted duty for approximately 4 weeks on 03/04/2011.

EE was installing connecting block pin in D11 dozer blade and pin slipped, striking finger, cutting end of finger off.

EE had attended a safety meeting and was getting ready to board the man trip to go to his assigned work area, when he slipped on some ice and fell. EE continued to work his shift and went to his family Dr. the next day.

EE was helping change a deck bushing and as he was guiding the bushing in place, it slipped, pinching his middle finger on his right hand.

He was changing the left rear inside tire on a 1500 Komatsu truck. When removing the lock ring the gap of the lock ring caught finger and crushed finger.

EE stated that as he was dismounting a D-11 Cat Dozer his knee locked up and made a popping sound. EE finished shift then went to his family DR where X-Rays were Negative. EE returned to Dr. and an MRI revealed some torn cartlidge. EE's first day off work will be 07/05/10 as the mines were shut down from 06/26/10 thru 07/05/10 due to vacation.

Employee was scaling rocks off highwall when a rock came through the windshield of the excavator he was operating causing cuts and a possible fracture.

EE was cleaning out a hose with a welding rod and received a puncture wound to the left hand. First aid was applied on site. EE who is diabetic finished shift and went to Dr next day when hand swelled up. EE was placed in hospital for treatment of infection.

Employee was using a air grinder on a bolt ont he pivot shaft of a d-10 caterpillar dozer. The grinder disc caught his glove and jerked his hand into the disc resulting in a laceration to the top of his hand. 5 stitches were required return to work the same day.

Employee was operating a 777C cat truck and was in the process of putting seat belt on. When he reached behind the seat to obtain buckle his right index finger went through heater blower guard and came in contact with blower.

Employee was in process of changing the y-pin on 992G cat loader when the tool that presses the seal in place fell and trapped his thumb between the loader tire and the press tool. This resulted in a mashed thumb and employee had outpatient surgery for damage to thumb on right hand. (Worked two days then took off).

EMPLOYEE WAS SHUTTING ENGINE COMPARTMENT DOOR ON LOADER WHEN HIS FOOT SLIPPED, CAUSING PAIN TO HIS LOWER BACK.

Employee was working in shotrock. A rock rolled into the loader bucket causing a sudden jar. He complained of back pain but finished shift. He went to the doctor on Saturday, Jan. 8, 2005.

EE LEFT THE JOB & WENT HOME, THEN HE DECIDED HE WAS HURT SO HE CALLED HIS FOREMAN TO INFORM HIM THAT HE WAS GOING TO THE DR WITH BACK PAIN. NO ACCIDENT REPORT WAS FILLED OUT BECAUSE HE LEFT TH E JOB WITHOUT INFORMING ANYONE & THIS ACCIDENT IS STILL UNDER INVESTIGATION. HE TOOK A VACATION DAY ON THE 18TH AND RETURNED TO WORK ON THE 19TH.

FOREMAN DROVE HIS PICK UP INTO THE COAL PIT WHERE HIS MEN WERE WORKING. HE STATED THAT THE EQUPMENT LIGHTS SHINING IN HIS EYES CAUSED HIM NOT TO SEE THE HOLE. CONTACT WITH THE HOLE CAUSED A SP RAINED MUSCLE TO HIS LOWER BACK.
