Fall from machine

EE WAS OPERATING A DM600 MACK FUEL TRUCK WHEN HE APPARENTLY LOST CONTROL OF THE TRUCK AND JUMPED OUT OF THE TRUCK. IN DOING SO, HE SUSTAINED SEVERE HEAD INJURIES AND DIED ON 10-4-01. THE ACCID ENT HAPPENED ON A DESCENDING ROAD. CAUSE IS STILL UNDETERMINED.

The Caterpillar 992K caught on fire in the Winifrede pit at approximately 1:15 AM and was engulfed in seconds. The operator jumped from the loader and received no injuries.

Struck by falling object

Installing a starter on a 785C CAT truck, threads pulled out and fell onto left thumb causing a laceration and broken bone in thumb only.

EE was boarding the 336 E Excavator when EE's hand slipped off the handrail causing EE to fall backwards to the ground.

Victim was twisting the belly pan on a dozer to put on right way and chain slipped and unhooked and caught finger against the track. Pinched and cut left pointing finger. X-rayed with no broken bones and had to have stitches.

Victim was using a shovel to fill blast hole. When he stood up his lower back began hurting. Worked rest of shift. When he got up next morning it was still hurting so he went to Doctor. Gave him a steroid injection. He returned to work on 4-1-11.

Victim stepped from lube truck to ground and slipped on muddy ground. His back began hurting. He drove himself to ER. Given a steroid injection and told to stay off work for a week. Returned to work on 3-14-11.

Fall down stairs

Victim is an employee of the KY DOT. They were hauling shot rock from the job site to use in road repair. EE was exiting the office after receiving Hazard Training. He slipped and fell on the steps which resulted in a broken left leg.

MECHANIC WAS TRYING TO TAKE A TRACK APART AT THE MASTER LINK. THE MACHINE HAD A DEAD BATTERY AND WOULD NOT START. INSTEAD OF MOVING MACHINE UNTIL MASTER LINK WAS IN POSITION RECOMMENDED IN SERVICE MANUAL, THE MECHANIC STOOD ON TOP OF THE TRACK AND REMOVED THE BOLTS. WHEN HE REMOVED THE LAST BOLT THE TRACK CAME APART AND LAUNCHED HIM OFF OF THE MACHINE.

Employee was unloading parts from his truck to the company storage unit. The surface where the truck and ramp met was uneven and employee mis-stepped and broke his leg.

Victim was tightening segment bolts on a D-10 dozer. The wrench slipped striking him in the head resulting in a laceration. Five (5) stitches to close wound.

Employee was using a hammer to strike a chisel for separating head end of cylinder from barrel in order to reseal the cylinder. Upon striking the chisel a piece broke off and entered his left upper arm.

Accident type, without injuries

A fire occurred at this operation taking more than thirty minutes to extinguish. The fire involved a D11R Cat Dozer, Serial No. 9TR00200, and began at approximately 2:30 A.M. Indications from the mine site point toward the fire began around the engine area.

Employee was stepping down off of a prell truck when his right hand caught the corner of a sign and cut it receiving stitches.

Struck against a moving object

Victim was driving the ANFO truck back to the parking area at shift change. While going down a grade, the truck came out of gear and the brakes failed to slow the truck and the truck overturned. Victim has a broken jaw and 12 stitches on inside of his mouth. On light duty until Doctor gives complete releast to return to work.

Struck by falling object

A socket for an air wrench fell from the top of a loader tire and struck him in the forehead. Required five stitches to close the wound.

Victim was performing maintenance on brakes of truck. The rigging slipped slightly and a plate fell and pinched the outside of his little finger. The pinch did not involve the bone, only the ligaments and flesh on the finger. Surgery was done to repair damage and a full recovery is predicted.

EE INJURED BACK WHILE MOVING 60# BOXES FROM STORAGE INTO TRUCK. EE DID NOT NOTIFY SUPERVISOR OR COMPLETE AN ACCIDENT REPORT. EE HAD BACK SURGERY ON 6/30. CO. WAS NOT AWARE THAT THE INJURY WAS WORK RELATED UNTIL 7-14. EE CONTINUED TO WORK ATHIS REGULAR JOB UNTIL 6-30-03.

EE WAS REPAIRING THE CHAIN DRIVE CASING OF THE GRADER. HE HAD FINISHED WELDING THE OUTSIDE OF THE CASE AND WAS PREPARING THE INSIDE FOR WELDING. HE USED STARTING FLUID TO CLEAN THE INSIDE ALLO WING 20 MIN FOR THE FUMES TO CLEAR AND ALSO USING COMPRESSED AIR TO BLOW OUT CASE. HE STRUCK AN ARC AND IGNITED FUMES CAUSING BURNS TO HIS ARM 2 ND DEGREE.

EMPLOYEE WAS STACKING CRIB BLOCKS UNDER 992G ENDLOADER TO BLOCK THE MACHINE UP IN PREPARATION OF MAKING REPAIRS. A CRIB BLOCK FELL & HIT HIM ON THE ARM CAUSING A LACERATION (6 STITCHES).

EE WAS INSTALLING A STEERING PUMP ON A 777 CAT. HAUL TRUCK. HE BUMPED HIS KNEE ON THE PUMP. HE CONTINUED TO WORK UNTIL 4-8-02 WHEN HE SOUGHT MEDICAL ATTENTION.

EMPLOYEE WAS FUELING A ROCK TRUCK WHEN THE FUEL HOSE CAME OUT OF THE FUEL TANK NECK. IN DOING SO HE ATTEMPTED TO JUMP OUT OF THE WAY AND STUMBLED. HE FELL ON HIS WRIST CAUSING A STRESS FRACTUR E AND IS WEARING A SOFT CAST.

EMPLOYEE WAS CHECKING THE ANTI-FREEZE FLUID LEVEL ON A BULL DOZER. HE OPENED THE CAP TO RELIEVE PRESSURE. THE RADIATOR THEN SPURTED OUT HOT LIQUID AND SCALDED HIS HAND. HE WAS WEARING GLOVES.

EMPLOYEE HAD FINISHED SERVICING ON END LOADER. AS HE WAS WALKING HE TWISTED HIS KNEE. HE CONTINUED TO WORK UNTIL JUNE 14, 2001 WHEN HE BEGAN LOSING TIME.

EE WAS GETTING OUT OF THE FUEL TRUCK CAB. HIS FOOT SLIPPED & HE FELL AGAINST THE FUEL TANK BRUISING HIS BACK.

EE WAS CLIMBING UP ON A D8N DOZER TO FUEL IT. HE FOOT SLIPPED AND HE LANDED STANDING UP. HE ALLEGES PAIN IN HIS LOW BACK. HE SLIPPED FROM 1 ST STEP WHICH IS 18"" FROM GROUND LEVEL.

EE WAS GETTING ON A ROCK TRUCK. HIS FOOT WAS ON THE FIRST STEP AND HIS LEFT HAND WAS ON THE HAND RAIL. HE ALLEGES HIS FOOT SLIPPED AND CAUSED A GROIN STRAIN.
