Struck against stationary object

Employeee was leaving a lube trailer through a low door at Hanson Heidelburg AA Quarry in Grayson KY when employee raised up before clearing the door and struck head on the door frame. Employee was wearing PPE (Hard Hat) .

Employee was working on repair of a loader when EE fell off, hitting the ground and breaking the wrist.

Struck against a moving object

Employee was operating a CAT 740B Haul Truck and hit a rut in the road, causing back pain. The injury was not reported to management until 9/9/19.

Fall to the walkway or working surface

Employee was walking across a parking lot towards his haul truck when he slipped on a patch of ice and fell, resulting in sprains to his left knee and shoulder and abrasion to his left knee. Employee was placed on three days restricted duty, operator accommodated restrictions - no lost workdays were incurred as a result of this injury.

Injured employee was kneeling in area between the cab and engine compartment on a Cat 988B Loader while performing maintenance to the loader. When employee stood up he heard a snapping noise in his left knee and felt that his left knee had popped out of socket. Initial medical review of this injury resulted in restricted duty with no lost time.

Employee was on scales. He got out of his truck and stepped on the guardrail they have for them to step on to get his load ticket. He turned to get back in his truck when his foot slipped on guardrail and he fell hitting his left side on guardrail and fracturing a rib.

Employee was using 12 volt tester. The tester slipped off temp switch and came in contact with fan blade. The fan blade struck tester and drove blunt end into palm of right hand requiring 16 stitches.

Employee was operating a 2003 model 226 cat skid steer cleaning out between beltlines. He backed skidsteer upon existing stockpile with bucket raised cousin skid steer for overturn. He substained scretches and lacerations requires medical attention.

LOADED DOZER TRACKS ON FLATBED TRACK FOR TRANSPORT. TRACKS WERE IN SPIRAL ROLLS, ONE END OF ROLL OPENED. EE WENT TO PULL OPEN END BACK TO ROLL, LOADER MOVED TRACK TO ADJUST HOLD. EE PINCHED TH UMB CAUSING LACERATION.

EE WAS ASSISTING IN INSTALLING A RETURN ROLLER GUARD. WHILE HE WAS HOLDING THE GUARD IN PLACE, HIS FOOTING SLIPPED. RESULTED IN A STRAINED BACK. HAD RESTRICTED WORK DUTIES.

EMPLOYEE WAS USING A SLEDGE HAMMER TO DRIVE A PIN INTO THE TRACK OF A 235 CAT EXCAVATOR WHEN A PIECE OF THE PIN BROKE OFF STRIKING AND LODGING IN THE EMPLOYEE'S LEFT LEG.

Over-exertion in lifting objects

EMPLOYEE WAS HELPING TO INSTALL A SET OF ROLLERS ON THE IMPACTOR CONVEYER. HE TURNED TO GET A PRY BAR, AFTER DOING SO HE BEGAN TO COMPLAIN OF PAIN IN HIS BACK. HE CONTINUED TO WORK HIS SHIFT T HAT DAY. WENT TO THE DOCTOR 1-7-2000, AND WAS DIAGNOSED WITH A STRAINED BACK MUSCLE. TOLD TO STAY OFF WORK A FEW DAYS AND GIVEN PAIN MEDICATION.
