Fall to the walkway or working surface

Traveling down a snow/ice covered road, truck that he was driving spun around, got out to look at the road, slipped on ice, lost footing, fell on back.

Struck by falling object

Victim was removing the cutting edge from a 980 loader bucket. He was on next to the last bolt when the cutting edge swung down and struck him on the top of his left foot. He went to his family doctor and was sent for an X-ray which showed no broken bones, bruises only. Missed 3 days work to allow swelling to subside.

Struck against a moving object

Victim was driving a powder truck up an incline. There was loose material on the roadway, which caused the truck to lose traction. The truck rolled back onto the berm and overturned onto the passenger side. Victim's safety glasses caused a laceration above right eye. Required 4 stitches to close.

Deck of shovel was muddy and his feet slipped from under him and he grabbed for the handrail and strained his left shoulder, elbow, wrist and hand.

Victim was checking for a leak on a rock truck. To determine location of the leak the bed had to be raised & lowered. He had another employee raise & lower the bed. He got his finger between the hoist & mast and when the bed was lowered it pinched the tip of his finger off. No bone involved. Cleaned & wrapped at hospital.

Struck against stationary object

Victim was checking the differential oil on a 992-G loader. When loosening the fill plug, he slipped and hit his left elbow on the rim of the loader. Sustained a fracture to his left elbow. Missed 4 days work: returned 8-2-08

Victim was entering job at the start of shift, another car went around a water truck that had stopped & struck his vehicle coming onto the job. Originally reported as no injuries in this vehicle until 6-13-08. Victim went to the doctor and was told he possibly had whiplash. Missed 4 days work:returned 6-19-08.

Victim was leaving job at the end of shift, he went around the water truck that had stopped along the haul road and struck another vehicle that was coming onto the job. No injuries in other vehicle. Victim was taken to the hospital and x-rayed : no broken bones just bruises. Missed one days work because of being sore.

EE was lying on the ground on his back installing cutting edges on the blade of a 16-G grader when he says he had a sharp pain in his lower back going down his right leg.

Struck against a moving object

EE states that as truck was being loaded with rock, he got shook up and a little while later his back and left leg started hurting. Was taken to hospital, was released and told to rest 4 days.

Victim cut the index finer on left hand with a utility knife while cutting out a wooden plug. Six stitches to close.

Struck by falling object

Employee was installing new corner bits bolts in dozer blade. He had all old bolts removed and put a bar in bolt hole to hold bit in place. The bar slipped and the corner bit fell striking his left foot at the base of his toes. Three toes were surgically removed, the big toe and the next two.

Victim was struck in the back of the head by a hose that blew loose on the Anfo truck. He complained of being dizzy for a little while, then felt better and returned to work. On 6/18/04 he again complained of being dizzy and was sent to the doctor. He has been off since that date is in physical therapy.

VICTIM WAS PERFORMING REPAIR WORK ON DOZER WHEN A CHAIN SLIPPED. THIS CAUSED HIM TO SLIP & FALL. COMPLAINS OF PAIN IN LOWER BACK AND LOWER RIGHT SIDE.

EE WAS UNDER DOZER DOING MAINTENANCE, WHILE CRAWLING OUT HE HIT LEG ON A ROCK. NOTIFIED SUPERVISOR ON 8/20, WENT TO ER ON 8/21.

EE WAS LOADING A CASE OF PRIMERS FOR BLASTING. HE PICKED UP THE BOX OF PRIMERS AND PULLED A MUSCLE IN HIS LOWER BACK.

THE EE WAS OPERATING A ROCK TRUCK BEING LOADED BY 2 DOZERS. THEY WERE PUSHING MATERIAL OUT ABOVE THE PIT HE WAS WORKING IN. A ROCK ROLLED INTO THE TIRE OF THE TRUCK JARRING THE TRUCK. EE STRU CK HIS HEAD ON THE WINDOW GLASS.

EE WAS COMING DOWN THE LADDER ON HIS ROCK TRUCK, HE SLIPPED ON THE LOWER STEP AND FELL BACKWARD, LANDING ON HIS LEFT HAND AND HIP.

EE WAS CLIMBING DOWN THE LADDER TO GET OFF THE 785 ROCK TRUCK, HIS FOOT MISSED THE BOTTOM SET OF STEPS HE LOST HIS BALANCE AND FELL OFF HITTING THE GROUND.

EMPLOYEE STARTED DOWN HILL IN THIRD GEAR, DID NOT LIKE THE WAY HIS BRAKES FELT, TRIED TO GO BACK TO SECOND GEAR, COULD NOT GET INTO GEAR. HE JUMPED OUT OF THE TRUCK BEFORE IT TURNED OVER. IT WENT 200-300' BEFORE TURNING OVER.
