Employee was walking and twisted his right knee and experienced some pain and swelling after getting down on knees to hang a cable. Started missing work on 10/12/2015.

Struck by falling object

Employee was loading roof bolter with supplies and a particle of dirt got into his eye. Antibiotic drops were prescribed for his eye.

Employee was taking belt structure apart and caught his right index finger between the belt rail and a belt stand and cut his finger. Sutures were required. He returned back to work the next shift.

Over-exertion in pulling or pushing objects

Employee was installing a roof bolt and pushed the bolt forward and felt a ""pop"" in his left shoulder. He started missing work due to the pain getting worse over time. He started missing work on 8/19/2015.

Struck by falling object

The employee was struck by a falling rock brow and suffered a broken leg. Surgery was required to repair the leg. He started missing work on 8/10/2015

Employee was installing a bearing onto the #7 tailpiece roller and turned the bearing and caught his right middle and ring finger between the bearing and tailpiece. He cut his fingers and damaged a tendon. Sutures and repairing of the tendon were required.

Employee was standing beside the flat bed truck when another employee was attempting to unload a ram car boom jack. The jack spun around on truck and fell off truck and one end of the jack struck the employees left foot. Fractured foot.

Employee was observing the continuous miner and a piece of draw rock fell and struck the back of his leg and ankle. Fractured ankle.

Employee was getting a stuck rock out of a belt-drive chute, as he had a hold of the rock, it dropped down and caught his left 4th and 5th digit fingers between the rock and the belt scraper bracket and cut both fingers. Sutures were required.

Found an unintentional roof fall at Break #61, #6 Heading, parallel with the #6 Beltline. The fall did not affect ventilation or affect the escapeways. The other entries through the area were common to this fall area and were travelable.

Employee was picking up a main pump for a roof bolter and the motor shaft cut his left hand between the thumb and index finger. 5 sutures were required.

A portion of a coal rib was being removed with a jack hammer for the purpose of correcting an issued close clearance citation. The injured employee was removing the loose coal away from the rib when a rock brow fell onto him.

Employee was getting a rock off of a curtain and hurt his right arm. Tendon torn and surgery was required. Employee had surgery and started missing work on 7/12/2013.
