On December 12, 2009 at approximately 6:38 am, a pre-dawn, low temperature accident occurred on a haulage road at PE Southern Pike Co. The driver of DT 178 exited the loaded coal truck, resulting in his death.

As employee was walking, his feet slipped out from underneath him, causing him to fall and land with his right leg pinned underneath him, resulting in a fracture to his fibula.

EMPLOYEE WAS CHECKING THE DOZER FOR A LEAK. THE PUSH ARM WAS OILY & SLIPPERY. HE SLIPPED ON THE PUSH ARM & FELL OFF ONTO THE GROUND.
Got out of tractor/truck to clean windshield. Lifted hood on tractor. Climbed up on diver side tire to clean the windshield, foot slipped. Fell between the hood & bumper, stuck left rib cage.

EE WAS PRYING WITH A BAR ON THE EXAVATOR TRACK WHEN THE BAR SLIPPED CAUSING EE TO FALL AND FEEL PAIN IN HIS BACK.
SLIPPED WHILE MOUNTING EQUIPMENT, FELL ON LEFT ANKLE AND HAND. FRACTURED BOTH.
