While pulling feeder to outside FM xc2 making the turn at xc1 slop[e bottom. The feeder slid to corner of rib causing the chain to break striking EE on forehead toward right eye. A laceration of about 1.5 inches was treated by MET and was transported to RMC.
A rock fall occurred on No. 1 unit on 5th East Main at spad No. 15 + 50 in No. 3 Entry Intersection. 18 ft wide, 22ft in length and 8 ft high. The area is dangered off and cribs installed and barricaded off. Additional cribs are also installed across face and all three entries & crosscuts 15 + 50.
A roof fall occurred on No. 1 unit on 5th East Main at spad No. 15 + 50 in No. 1 Entry last open intersection. 18 ft wide by 35 ft long by 8 ft high. the area is dangered off and cribs installed and barricaded off. No equipment involved.
Employee was ""squaring up"" pinner into No 3 right x-cut with the side control tram levers when pinner shocked him and lost power. Investigators discovered a grounded cable on the reel and ineffective insulated tram lever control covers due to age and wear contributed to employee receiving shock.
An inundation of water occurred on the No. 1 unit in the No. 2 entry at x-cut left of MMU-005-0 section at spad No. 11 + 09. The working section is presently under a 103-K order issued by MSHA on 5/14/14 at 0036 hour.

Employee stated he was struck across bridge of his nose by roof bolter's trailing cable, causing fracture to his nose requiring surgery to his nasal system on 9-16-13 --- Employee started missing work on 9-16-13.
EE stated he was building a concrete block ventilation stopping. Cut a steel band with hammer that was holding block onto a pallet. When the band was cut one end came up and struck EE's right hand causing a laceration requiring six stitches.
Employee stated he was loading 8 foot cable bolts onto the double boom roof bolter...struck his left forearm against an overhead roof bolt plate causing a laceration requiring 15 stitches...employee did not miss any work.
Employee stated he was operating opposite double boom roof bolter drilling a one inch into the roof, piece of draw rock fell from roof. Reposition drill steel when up pressure was applied the drill steel snapped in two striking employee over his right eye brow.

Employee stated while building a concrete stopping he was in the process of splitting a block when he struck his right forearm against a steel band...causing small laceration requiring nine stitches...no lost time
EE stated he was unwinding a spool of elec. cable. Reel was suspended from the roof cable became fouled causing reel to swing back towards EE's face he threw up his right arm to catch reel when his ring finger was caught by the chain holding the cable reel and severing the end of his ring finger back to the first joint.
Employee stated he was changing miner bits. Struck bit wrench with hammer, small chip of steel off hammer struck employee's right upper arm, penetrating the biceps muscle. Piece of steel was removed by Dr.
EE stated he was installing belt key hold hangers into mine roof. Pinner steel struck EE across left forehead above eye brow requiring 7 stitches EE RTW on same shift.

A roof fall occurred in the main west at xcut 39 spad 4 & 80 in the #2 return entry. Length 19' x Width 18' x Height 8'. Area developed 12/23/07.
A roof fall occurred on 6/2/11 at spad # 13 & 30 in the #2 return entry. Fall was 18'W. X 30' L. X 6' H. Did not affect flow of air and will not be cleaned up. No equipment involved.
EE stated he was operating battery scoop hauling mud-muck and loose rock that had been piled up in XC between supply road and belt entries. While backing the scoop a large piece of rock came in operators compartment struck the EE on his left shoulder causing contusion. EE started missing work on 5/27/11.
EE stated he was replacing conveyor chain on a joy 21sc shuttle car using a com-a-long. Chain broke on the com-a-long causing ee to fall backward, causing a possible strain to ee back and neck. EE started missing work on 4/14/11.
Employee stated he was changing insert on drill steel. Roof bolter operator moved the roof bolting machine forward. Right front wheel (tire) ran upon employee's right foot causing a sprain. Started missing work on 3/22/11.

A roof fall occurred in East sub main at spad No. 13+00 in the No.3 entry. 35' long x 20' wide x 6' high. Roof fall won't be rehabilitated. All available entrances to roof fall will be supported according to our roof control plan. No equipment involved. Did not impair ventilation, nor impede travelway.

A HIGHWALL FAILURE OCCURRED IN THE UNDERGROUND PIT AREA ON THE WEST SIDE OF THE HIGHWALL. VERY LITTLE ROCK WAS DISLODGED FROM THE HIGHWALL. APPROX. 14 CRACKS APPEARED IN THE SERVICE ROAD NEAR THE TOP OF THE HIGHWALL, RANGING FROM 1"" TO 21"" WIDE, FOR A DISTANCE OF 381 FEET.
773 BOBCAT MACHINE ACCIDENTLY STRUCK 6"" PLASTIC WATER LINE CAUSING 2"" PIECE OF STEEL PIPE PARTIALLY DRIVEN INTO THE GROUND TO FALL OVER STRIKING EMPLOYEE IN MIDDLE OF FOREHEAD CAUSING AN OPEN LACERATION REQUIRING SEVEN STITCHES.
