Richland No 9Coal

The employee was in an area where conveyor covers were being removed. A cover that was not secured by the workers removing it slid down the conveyor striking the employee.

While pulling feeder to outside FM xc2 making the turn at xc1 slop[e bottom. The feeder slid to corner of rib causing the chain to break striking EE on forehead toward right eye. A laceration of about 1.5 inches was treated by MET and was transported to RMC.

A rock fall occurred on No. 1 unit on 5th East Main at spad No. 15 + 50 in No. 3 Entry Intersection. 18 ft wide, 22ft in length and 8 ft high. The area is dangered off and cribs installed and barricaded off. Additional cribs are also installed across face and all three entries & crosscuts 15 + 50.

A roof fall occurred on No. 1 unit on 5th East Main at spad No. 15 + 50 in No. 1 Entry last open intersection. 18 ft wide by 35 ft long by 8 ft high. the area is dangered off and cribs installed and barricaded off. No equipment involved.

Employee was ""squaring up"" pinner into No 3 right x-cut with the side control tram levers when pinner shocked him and lost power. Investigators discovered a grounded cable on the reel and ineffective insulated tram lever control covers due to age and wear contributed to employee receiving shock.

An inundation of water occurred on the No. 1 unit in the No. 2 entry at x-cut left of MMU-005-0 section at spad No. 11 + 09. The working section is presently under a 103-K order issued by MSHA on 5/14/14 at 0036 hour.

Employee stated he was struck across bridge of his nose by roof bolter's trailing cable, causing fracture to his nose requiring surgery to his nasal system on 9-16-13 --- Employee started missing work on 9-16-13.

EE stated he was building a concrete block ventilation stopping. Cut a steel band with hammer that was holding block onto a pallet. When the band was cut one end came up and struck EE's right hand causing a laceration requiring six stitches.

Employee stated he was loading 8 foot cable bolts onto the double boom roof bolter...struck his left forearm against an overhead roof bolt plate causing a laceration requiring 15 stitches...employee did not miss any work.

Employee stated he was operating opposite double boom roof bolter drilling a one inch into the roof, piece of draw rock fell from roof. Reposition drill steel when up pressure was applied the drill steel snapped in two striking employee over his right eye brow.

Employee stated while building a concrete stopping he was in the process of splitting a block when he struck his right forearm against a steel band...causing small laceration requiring nine stitches...no lost time

EE stated he was unwinding a spool of elec. cable. Reel was suspended from the roof cable became fouled causing reel to swing back towards EE's face he threw up his right arm to catch reel when his ring finger was caught by the chain holding the cable reel and severing the end of his ring finger back to the first joint.

Employee stated he was changing miner bits. Struck bit wrench with hammer, small chip of steel off hammer struck employee's right upper arm, penetrating the biceps muscle. Piece of steel was removed by Dr.

EE stated he was installing belt key hold hangers into mine roof. Pinner steel struck EE across left forehead above eye brow requiring 7 stitches EE RTW on same shift.

A roof fall occurred in the main west at xcut 39 spad 4 & 80 in the #2 return entry. Length 19' x Width 18' x Height 8'. Area developed 12/23/07.

A roof fall occurred on 6/2/11 at spad # 13 & 30 in the #2 return entry. Fall was 18'W. X 30' L. X 6' H. Did not affect flow of air and will not be cleaned up. No equipment involved.

EE stated he was operating battery scoop hauling mud-muck and loose rock that had been piled up in XC between supply road and belt entries. While backing the scoop a large piece of rock came in operators compartment struck the EE on his left shoulder causing contusion. EE started missing work on 5/27/11.

EE stated he was replacing conveyor chain on a joy 21sc shuttle car using a com-a-long. Chain broke on the com-a-long causing ee to fall backward, causing a possible strain to ee back and neck. EE started missing work on 4/14/11.

Employee stated he was changing insert on drill steel. Roof bolter operator moved the roof bolting machine forward. Right front wheel (tire) ran upon employee's right foot causing a sprain. Started missing work on 3/22/11.

Occurred during the week of 2/10/11 thru 2/17/11. Roof fall was found on the weekly examination by EE and MSHA inspector. High 12ft X Width 20ft X Long 45ft at spad No.4t80 in the main west No. 1 entry. Return air course.

A roof fall occurred in East sub main at spad No. 13+00 in the No.3 entry. 35' long x 20' wide x 6' high. Roof fall won't be rehabilitated. All available entrances to roof fall will be supported according to our roof control plan. No equipment involved. Did not impair ventilation, nor impede travelway.

Employee stated he stepped off roof bolting machine and felt pain in left knee. Started missing work on 4/29/10. Returned to work on 5/3/10.

Employee stated he was changing transmission in a Dapco three wheel rib runner. While lifting up on the transmission, employee stated he felt a sharp pain in his back. Started missing work on 12/16/09.

Employee stated he was working with a crew in the process of moving the mining equipment power cables up two crosscuts. His feet became entangled in a roof bolter machine cable loop causing the employee to fall causing pain to his right side. Employee started missing work on 11-6-09.

EE stated shuttle car anchor came loose that was attached to a bolt anchor to the mine roof. Cable struck EE on his right side which caused a contusion. EE started missing work on 11/05/09 RTW 11/09/09.

Employee stated he was using a belt knife. Knife slipped while cutting curtain causing laceration to right leg. Nine stitches - no lost work days.

EE stated cable broke that was holding up hood on a 10-person alpha mantrip. Struck EE on top of head (left side) (wearing hardhat), causing a possible concussion. EE started missing work on 10/2/09.

EE stated he was connecting a pull rope (wire rope) to cat.245 back hoe. Cut ring finger on right hand. Required 13 stitches. EE returned to work on 8/26/09, did not miss any scheduled work days.

EE stated he was riding in Wallace 14 person man trip. Ran over pot hole in supply road. Causing pain to employee's back. Started missing work on 8/27/09. Scheduled to return to work on 8/31/09.

EE stated he was walking down slope. Slipped and fell, causing pain in lower back. Started missing work on 7/31/09

EE stated he was crossing a belt, hung his right foot on belt rope, causing him to fall off belt. Struck 3"" water line, causing pain to his lower back. Started missing time on 7/28/09

Employee stated he was removing a belt ""A"" frame from mine roof. One end of belt ""A"" frame dropped down. Other end caught left hand ring finger between roof and ""A"" frame causing laceration to finger requiring 17 stitches.

Employee stated he was walking under a back-up ventilation curtain, struck his left knee against shuttle car bumper causing his left leg to hyper-extend causing pain to left leg.

A roof fall occurred at spad #4+80 in the no. 3 entry in the first main west supply road....width 20 ft x length 30 ft x high 12 ft.

Employee was using electrical hand held drill, drilling hole into steel lid. Drill bit hung causing drill to rotate, caught employee's right thumb in cord requiring 12 stitches to laceration on thumb. Did not miss any work days.

A roof fall occurred in the Main West No. 6 entry (intake) at spad No. 5+40. Length 30 ft x 20 ft wide x 12 ft high. No equip or ventilation affected.

EE stated he was drilling roof bolt hole in mine roof, piece of draw rock fell from roof, struck EE on his left hand and caused contusion to left hand.

Employee stated he was involved in changing out a pot on a continuous miner, cleaning area where new pot would go. His left hand was caught between pot and frame of continuous miner, causing fracture to middle finger and laceration to ring finger on left hand.

EE stated he removed belt framing from belt trailer. He dropped a piece of belt framing on top of his right foot causing swelling and appearance of a contusion to employee's entire foot. Started missing work on 3/20/08, returned to work on 3/25/08.

Employee stated he stepped off the Cat. 910 forklift onto the diesel scoop. Footing slipped. Fell off top of diesel scoop, approximately 3 1/2 feet high. Dislocated right shoulder.

Employee stated he was operating a shuttle car, struck bump in haul road, felt pain in lower back. Started missing work on 11-5-07.

EE stated he was using a pry bar to change out belt rollers, prying down on belt and felt pain in his lower back, started missing work on 11/3/07.

Employee stated he was aligning the shuttle car cable into the cable reel. Cable jerked employee's right hand into cable reel causing a laceration and fracture to his right hand forefinger. Employee did not miss any time.

EE stated he was helping the mechanic change a hyd. hose on a double boom roof bolter. He raised the boom, struck the bit tray, tray fell, struck employee's left hand between tray and joy stick knob causing laceration to the left forefinger requiring 14 stitches. Did not miss any time.

Employee stated he was hanging the continuous miner cable. felt pain in his back started missing work on 9/8/07

Employee stated he was reaching for a roof bolt and felt pain in his leg leg and knee-He was working on his knee's intalling permanent roof support-missed three work days returned to work on 9/13/07 started missing work on 9/10/07.

Employee stated he was pulling continuous miner cable. Strap being used to pull cable pulled in two. Piece of strap came off cable & struck employee on right hand, near his right thumb causing a laceration requiring seven stitches. Employee returned to work on 8/24/07 - missed one day.

Employee stated he was riding in mantrip & a piece of small black slate fell from mine roof & struck him on his chin causing a small laceration requiring five stitches. Employee did not miss any work.

A roof fall occurred in the 7th south panel at spad No. 2 + 00. Approximately 750 feet long x 20 feet wide x 6 feet high. We moved the unit out of the 7th south panel to the new 6th south panel.

Employee stated he lifted the continuous miner cable & felt pain in his right groin. Operated (surgery) on 8-7-07. Started missing work on 8-7-07.

EE stated he was operating roof bolter machine. EE pushed up-down boom lift lever, boom came down and struck EE on his left leg and knee, causing pain. EE started missing work on 6/17/07, RTW on 7/2/07.

A roof fall occurred in the 1st West sub-panel at spad No. 7 + 00. 110 feet long X 19 feet wide X 5 feet high. Additional roof support: 12 foot cable bolts. Also changed from 5' grouted bolts to 6 foot 3/4"" tension re-bar roof bolts.

EE stated a roof bolt broke while being installed into the roof, causing roof bolt plate to come off. Struck EE on his right lower chin, causing laceration requiring 16 stitches. Started missing time 5/31/07-ret'd 6/4/07.

Employee stated he was walking underground, struck his right shoulder against a roof bolt causing pain & numbness. Started missing work on 5-23-07.

Employee stated he was using a sledge hammer to remove a pin holding boom onto roof bolting machine. A piece of steel from pin struck him in upper left arm. He missed one shift. Returned to work on 5-16-07.

Employee stated he was plastering a concrete block stopping with minova plaster---some plaster splattered striking employee in his left eye causing irriation to his eye.

Roof fall occurred in the east sub panel at spad #8 & 50 in the #6 room....40' long & 19' wide x 5 ft high...no equipment or ventilation affected.

Employee stated while getting out of a 486 S&R scoop operator's compartment, he possibly sprained his right ankle and lower leg. Started missing work on 8/29/06.

Employee stated he was retrieving panel bolts for shuttle car panel lid. The shuttle car panel lid fell on employees right foot, causing a fracture to the employees big toe. EE started missing work on 8/15/06.

Employee stated he was bending six foot roof bolts. Felt pain in his right shoulder. Started missing work on 8-8-06.

Employee stated he was lifting 50# bags of rock dust when he felt pain in his back and left leg. Missed two days away from regular scheduled work.

Employee stated he was using a hammer to drive a nail into a curtain board, missed nail, struck left thumb with hammer. Mashed finger. Started missing work on 7-13-06.

A roof fall occurred on the no. 1 unit at spad no. 6+20 in the no. 6 entry, 50 ft long x 18 ft wide x 8 ft high. Rock fall will not be cleaned up. In process of being sealed.

Employee stated he was replacing a lid cover on junction box when his ring finger on left hand was caught between lid and frame of the continuous miner machine. Mashed finger. Started missing work on 7-8-06.

A roof fall occured in the 1st south main #5 entry at spad no. 4 + 90 third x-cut outby working face 60 feet long x 20 feet wide x 8 feet high.

Employee stated he was walking underground, slipped in a wet area and hyperextended his right leg. He felt pain in his right leg and knee. Started missing time on 5/25/06.

A roof fall occurred in the 11th north panel in No. 5 entry (supply road) at spad # 3 + 70. 50 ft. long X 18 ft. wide X 6 ft. high. Did not fall on any equipment or affect ventilation. Working section moved on 5/11/2006.

A roof fall occurred in the main east main at x-cut 155 Spad station 109 + 63 in the # 4 belt entry. 55 ft. long X 29 ft. wide X 6 ft. high. No equipment involved, did not affect ventilation. Working section moved on 5-11-2006.

A roof fall occurred in the main east main at x-cut # 150 at spad station # 106 + 13 in the # 1 entry. 35 ft. long X 18 ft. wide X 6 ft. high. No equipment involved or ventilation affected.

A roof fall occured on 4/25/06. At spad # 106 + 13 in the pillar line 18 feet wide x 25 feet long x 6 feet high outby working section won't be cleaned up. cribbed off

EE stated he was pulling on continuous miner cable when he felt pain in his left knee, possibly twisted or sprained. Required surgery on 2/24/06, started missing work on 2/24/06.

Employee was disconnecting belt trailer from hitch - dropped trailer on his left foot causing a contusion. Started missing work on 12/3/05.

EMPLOYEE STATED BECAUSE HIS TASK AS A ROOF BOLTER OPERATOR IS BENDING 6'X 3/4"" TENSION RE-BAR ROOF BOLTS, BOTH HIS WRISTS, HANDS & KNEES HURT. HE HAS BEEN A ROOF BOLTER OPERATOR SINCE APRIL OF 2005, HE STARTED MISSING WORK ON 11/14/05.

He was lifting roof bolts onto the roof bolting machine when he felt pain in his back. Started missing time on 11-4-05.

EE stated he was lifting and pulling continuou miner cable---felt a sharp pain in his lower left abdomen---a possible strain muscle or hernia---Employee started missing work on 9-29-05 be off work approx. two weeks.

EE stated he was installing roof support---drilling a one inch hole into mine roof with a one inch pinner steel---pinner steel bent---struck ee on his right wrist---a possible fracture to employees right wrist---started missing work on 8-7-05.

EMPLOYEE STATED WHILE REMOVING BELT CONVEYOR TAIL PIECE - HE SLIPPED IN A MUDDY AREA AROUND TAIL PIECE AND FELT PAIN IN HIS BACK AND RIGHT KNEE. HE STARTED MISSING WORK ON 8/2/05.

Employee was drilling a 1"" hole into mine roof. Drill steel bent, striking employee's right thumb. Jammed thumb causing pain. Employee started missing work on 6/21/05.

EMPLOYEE STATED HE WAS MOVING BELT FRAMING & STEPPED ON A PIECE OF CONVEYOR BELT, FOOT SLIPPED CAUSING PAIN TO HIS BACK. EE STARTED MISSING WORK ON 6/10/05.

Employee stated he was changing bits on the continuous miner drum, when employee struck bit wrench with a hammer a small piece of steel off the bit wrench struck employee just above his knee, possibly lodging in his femur bone, started missing work on 7-15-05.

EE STATED WAS BUILDING A CONCRETE BLOCK STOPPING, PICKED UP A CONCRETE BLOCK, TURNED AND TWISTED RIGHT KNEE, POSSIBLE STRAINED KNEE. STARTED MISSING WORK ON 04-18-05

EE stated he bent down on his knees, struck his right knee cap on a lump of coal, causing a cracked knee cap. Started missing work on 4-16-05.

EE stated he was supporting the mine roof---installing roof bolts on his knees---reached for a roof bolt felt pain in his upper right leg---a possible pulled strain hamstring.

EE stated he was walking underground, struck his head against the mine roof causing pain to his head, neck and back. Started missing work 2-26-05.

Employee stated he was operating the continuous miner with the remote control, turned and twisted his right knee causing pain to knee. He started missing work 2-5-05.

Employee stated he was shoveling loose coal onto the belt line, as he turned to throw another shovel of coal onto the belt he twisted his left knee, causing pain to left knee. Started missing work on 1-21-05.

Employee stated he was walking underground & struck his head against a roof bolt, jamming his neck. This caused pain to his cervical area of the neck & back.

Employee stated he was walking underground, started to crossover shuttle car cable, tripped over shuttle car cable, causing employee to fall onto his left arm, causing fracture to lower left arm.

Employee stated he was using a belt knife to cut a piece of 42 inch conveyor belt---knife slipped of belt---struck employee's right thigh causing a laceration requiring five stitches.

Employee was dismounting 773 ir bobcat...slipped and fell on his right wrist causing fracture to wrist...fracture required surgical attention by placing three steel pins in ee's wrist

A HIGHWALL FAILURE OCCURRED IN THE UNDERGROUND PIT AREA ON THE WEST SIDE OF THE HIGHWALL. VERY LITTLE ROCK WAS DISLODGED FROM THE HIGHWALL. APPROX. 14 CRACKS APPEARED IN THE SERVICE ROAD NEAR THE TOP OF THE HIGHWALL, RANGING FROM 1"" TO 21"" WIDE, FOR A DISTANCE OF 381 FEET.

773 BOBCAT MACHINE ACCIDENTLY STRUCK 6"" PLASTIC WATER LINE CAUSING 2"" PIECE OF STEEL PIPE PARTIALLY DRIVEN INTO THE GROUND TO FALL OVER STRIKING EMPLOYEE IN MIDDLE OF FOREHEAD CAUSING AN OPEN LACERATION REQUIRING SEVEN STITCHES.

HIGHWALL FAILURE OCCURRED IN THE UNDERGROUND PIT AREA ON THE NORTHWEST SIDE OF THE HIGHWALL ~75' LONG X 25' HIGH TO 10' WIDE, CAUSING SEVERAL TONS OF ROCK TO ACCUMULATE IN THE PIT AREA.

EMPLOYEE STATED HE WAS WALKING DOWN THE #3 RETURN SLOPE ENTRY SLIPPED AND TWISTED LEFT KNEE CAUSING A POSSIBLE STRAIN TO HIS KNEE.

EE STATED HE WAS WALKING DOWN MINE SLOP, SLIPPED AND FELL, CAUSING PAIN TO RIGHT GROIN, POSSIBLE STRAIN.

EMPLOYEE STATED HE WAS CUTTING A BELT PIN WITH A PAIR OF SIDE CUTTERS. HAND SLIPPED OFF SIDE CUTTERS STRIKING SIDE OF BELT FRAMING AND ROLLER CAUSING LACERATION TO INDEX & MIDDLE FINGERS LEFT HAND, REQUIRING THREE STITCHES.

EMPLOYEE WAS ASSISTING IN RUNNING SITES ON THE #3 RETURN ENTRY.
