A non-injury roof fall occurred in the #4 right crosscut and extened to #6 entry of the last open crosscut on 001 mmu for approximately 100 feet long and 5 feet high. Survey station 5128 located in #2 entry of the last open crosscut.
Found Roof Fall on weekly bleeder examination that was blocking the bleeder. One break inby spad number 5053, Fall was 175 feet long, 20 feet wide, 9 feet high.
Belts had stop running started to investigate why belts were off and found Rock Fall on #1 belt 75 feet underground from surface. Fall was 22 feet long 18 feet wide and 15 feet hight.
Placing beams against mine roof with jacks, when one of the jacks kicked out, throwing the beam sideways and striking EE. He had to have 6 stitches for a laceration above his left eye.
He turned to tram the roof bolter forward, twisted back. He felt something pop in the middle of his back. Also had pain in neck area.
Employee was traveling from #3 belt line to #4 head drive on a Johnson Industries 2 man carrier, when a piece of rock fell from in-between the roof bolts hitting on the Johnson carrier. The employee had his right hand on the steering wheel and the rock struck his right hand and left leg. The rock measured 48"" long 24"" wide and 5"" thick.
Employee was operating the left side of a dual boom bolter when he got his left hand entangled with the drill steel and a wrench. This resulted in an amputation of his left hand and arm near his elbow. He was treated on site and then taken by ambulance to a local hospital. He had surgery and his arm was reattached. He was released from the hospital on 4-4-08.
Employee was trimming a belt splice, using a utility knife, when he accidentally cut his index finger. The incision required 4 stitches to close.
Employee was operating the continuous miner, when a piece of draw rock measuring 2 feet by 4 feet by 2 inches fell, strking him on his back and hips. Employee has no broken bones but is loosing time.
A potential inundation of water occurred as the miner cut into a bleeder area of the mine to establish additional bleeder entries. Water came on to the section and after a while created a possible inundation.
Employee who was operating a roof bolter stated, that when he knelt down to get on his knees after positioning the bolter, he felt a burning sensation in his right knee.
Non EE received injuries that proved to be fatal after he wrecked an ATV on a section of road that is not a part of the mine. ICG Knott County does not believe submission of this report is required by the Mine Act or regulations and is submitting it under protest.
On May 5, 2007 at 10:45 am a fire occurred involving the mine office, employee change room and a supply trailer. All buildings were located on the surface at the mine site. All three buildings were destroyed and there were no injuries.
Employee was removing a metal rub rail guard from the continuous miner when he accidentally dropped it on his right foot. This resulted in a bruise and sprain to his foot.
Employee was pulling on a piece of conveyor belt loading it to take underground when he slipped and fell. As he fell he twisted his right leg resulting in a fracture to his lower leg.
Employee was operating one side of a dual boom bolter when a piece of draw slate measuring 2 inches by 2 feet by 3 feet fell striking him on the left arm and back. He continued to work until 08/21/06 at which time he was taken off work due to the ongoing pain in his back.
Employee was using a hammer and a punch to knock a pin out of a cutter chain when the punch broke striking his left hand. This resulted in a laceration that required 4 stitches to close.
Employee was assisting fellow employees build cribs when he got struck on his left hand with a crib block. This resulted in a laceration on his index finger that required 6 stitches to close.
Employee bent over to pick up a wooden crib block that weighed approximately 20 pounds when he felt a sharp pain in his lower back. Employee continued to work until he decided to seek medical attention a couple of days later on 02/21/05 and is now missing time due to the injury.
Mechanic was installing a metal cover on the continuous miner when he mashed his index finger on his right hand. The finger had a laceration that required stitches to close.
EE WAS USING A SLATE BAR TO REMOVE A PIECE OF ROCK FROM OFF OF THE CONTINUOUS MINER WHEN HE INJURED HIS BACK. THE EE HAS CONTINUED TO WORK UNTIL 4/23/04 AT WHICH TIME HE BEGAN LOSING TIME.LAST DAY WORKED, 4/23/04.
EMPLOYEE WAS CHANGING A STABILIZING JACK ON A STAMLER BELT FEEDER WHEN HE GOT HIS FINGER CAUGHT BETWEEN THE JACK AND THE FRAME OF THE FEEDER. THIS RESULTED IN A LACERATION THAT REQUIRED STITCH ES.
EE WAS USING A UTILITY KNIFE TO CUT A PIECE OF WATER LINE WHEN HE LET THE KNIFE SLIP CUTTING AN INCISION IN HIS LEFT LEG. THE INCISION REQUIRED 3 STITCHES.
A ROOF FALL MEASURING 18' X 30' X 5' FELL IN THE BELT ENTRY APPROXIMATELY 120' FROM THE MINE PORTAL.
A ROCK APPROX. 5"" X 44"" X 5 1/2"" UNDETECTED BY SIGHT FELL WHEN STARTING DRILL, PINNING HIM DOWN HURTING LOWER BACK AND LEGS.
AS THE EMPLOYEE WAS PREPARING TO DRILL A HOLE IN THE ROOF WITH THE ROOF BOLTER, A SMALL PIECE OF ROCK FELL FROM AROUND THE DRILL STEEL & HIT HIS LEFT HAND WHICH HE WAS USING TO GUIDE THE DRILL STEEL. HIS HAND WAS EXAMINED BY TWO INDIVIDUALS & SHOWED NO EXTERNAL SIGNS OF SWELLING, SCRAPES OR ABRASIONS AT THE TIME OF THE EXAMINATION.

EE WAS LOADING ROOF BOLT SUPPLIES AND A PIECE OFDRAW ROCK FELL AND HIT HIS LEFT SHOULDER. MINOR INJURY (BRUISES AND SCRAPES).

EE WAS UN HOOKING PULL CHAIN WHEN SCOOP JUMPED INTO POINT, PINNING HIM BETWEEN TWO PIECES OF EQUIPMENT INJURING BOTH LEGS.
EE SUSTAINED BRUISES WHILE EXITING THE MINE. WEDGED BETWEEN ROOF AND MANTRIP.
EE MASHED TWO FINGERS ON LEFT HAND.
EE WAS CLEANING FEEDER AND HURT HIS BACK REPLACING LIDS.
NAILING FLYPAD UP AND SOME LOOSE MATERIAL FELL FROM THE BOARD AND GOT IN HIS EYE.
EE STATED HE HURT HIS LEG ON THE POT ON THE ROOF BOLTER.
EE WAS DRIVING S/C AND WENT BY A ROOF BOLT HANGING DOWN & CAUGHT THE RIGHT INDEX FINGER AGAINST HAND REST ON UNDER SIDE OF CANOPY.
EE BENT OVER TO PICK UP 2 BUCKETS OF GREASE. FELT PAIN IN LOWR STOMACH
EE BENT OVER TO PICK UP A 6"" CINDER BLOCK & FELTPAIN IN LOWER LEFT SIDE STOMACH
EE WAS HOOKING A CHAIN UP TO PULL A PIECE OF DRAW ROCK OFF OF A JOY 14.10 MINER. MINER OPERATOR SWUNG MINER AROUND CATCHING HIM ACROSS CHEST WITH BOOM.
