A roof fall was located on shift exam @ 4:25AM. Roof fall occurred one break inby SPAD 29, Entry #5. The roof fall was approximately 20'W, 25'L and 3'W.

Employee had turned off the feeder, pulled rock up on the side of feeder, then turned the feeder back on. Employee then reached up to pull the rock over to bust it up and a rock flipped up and hit a finger.

He was operating personal carrier and hit rock, jarring steering wheel out of hand hitting his finger.
EE was helping repair a broken feeder chain. He was using a hammer to replace the straps on the links & accidently struck his finger with the hammer. He finished his shift & sought medical attention at hospital. He continued to work until 11-4-08 when he went to Pikeville Medical Center due to pain in his finger. X-rays revealed a fracture to his left middle finger.
Roof fall in return 500' from surface was found when traveling return with msha EE on e01 inspector.
Browe fell off high wall mashing from House/Size 3' thick x 10' wide x 12' long. Destroyed fan house apron blocking fan/repared to msha ee 8:10 am 3-7-08.

Employee was helping install a ripper tooth on miner. He got in scoop bucket to help push tooth in place when he may have re-injured himself from a previous injury.
A roof fall was found by Pre-Shift foreman. He notified me. Re-routed escapeway, timbered and dangered off, did not clean up. Had safety talk with all employees. Notified MSHA and OMSL Agencies
Employee was installing roof bolts. He bent a bolt & tried to straighten it. Slipped & fell hitting his left shoulder & neck on T-bar.

He was operating shuttle car, and hit his head on the mine roof.
Roof fall anticipated due to visual observation of area, occurred in the area of the last open crosscuts and intersecting entries no. 1 through 5. The fall occurred as a result of the coal rider seam dipping toward the seam being mined in a low cover area.

Pre-shift examiner discovered a roof fall which had fallen sometime over the week-end. The roof fell in the face area of no.2 entry, across the intersection to the No.3 entry and was 5' thick.
EE WAS HAULING COAL WITH A SHUTTLE CAR AND A PIECE OF DRAW ROCK FELL OUT IN BETWEEN THE ROOF BOLTS, HITTING HIM IN THE LEFT SHOULDER AND SIDE.

EMPLOYEE WAS CUTTING THE CONVEYOR BELT IN PREPARATION OF MAKING A SPLICE. THE BELT KNIFE SLIPPED AND CUT HIS KNEE. STITCHES WERE REQUIRED TO CLOSE THE WOUND.
A ROOF FALL OCCURED ON THE 001 SECTION IN THE FACE OF #2 ENTRY THE FALL MEASURED 30'L X 18'WX 8'THICK. THE SECTION HAD ONLY 120FT LEFT FOR ADVANCE IT WAS DECIDED TO START RETREAT MINING
EMPLOYEE HAD COMPLETED WORK IN THE BELT ENTRY. AS HE WAS PASSING THROUGH A MANDOOR IT CLOSED ON HIS LEFT HAND AND FINGERS. HIS FINGERS WERE BRUISED AND A FINGERNAIL WAS REMOVED.
EE ALLEGES A STRAINED BACK MUSCLE. HE DOESN'T KNOW IF HE INJURED HIMSELF WHILE LOADING ROOF BOLTING SUPPLIES OR HANGING THE CONTINUOUS MINER TRAILING CABLE.
EE WAS HELPING MAKE A BELT CONVEYOR SPLICE. WHEN HE WAS RELEASING THE TENSION ON THE RED DEVIL THE JACK BAR SLIPPED AND STRUCK HIM ON THE LEFT DCHEEK.
A ROOF FALL OCCURRED AT SPAD 2026 IN THE CONVEYER BELT ENTRY. THE FALL MEASURED 20 FT WIDE, 20 FT LONG, 7 FY HIGH. A PLAN WAS DEVELOPED TO CLEAN UP THE FALL AND BOLT THE AREA.
EE WAS MOVING A CONTINUOUS MINER TRAILING CABLE OUT OF THE ROADWAY. HE EXPERIENCED A BURNING SENSATION IN HIS GROIN. HE CONTINUED TO WORK UNTIL 3/12/01.

EMPLOYEE WAS LOADING A BARREL OF OIL ONTO A SCOOP WHEN HE FELT PAIN IN HIS RIGHT SHOULDER.
EE WAS SLIDING AN ELECTRICAL PANEL IN PLACE WHEN HE FELT PAIN IN HIS LOW BACK.
EE WAS IN THE PROCESS OF SHOVELING COAL SPILLAGE FROM THE #5 MAIN BEL. HE STATED THAT WHEN HE SHIFTED HIS BODY TO PLACE THE COAL ONTO THE BELT, HE OVERLY TWISTED HIS BACK AND A SHORT TIME LATE R HE BEGAN TO EXPERIENCE SORENESS IN THE LEFT SIDE OF HIS BACK.
EE WAS LIFTING CONCRETE BLOCKS AND STACKING THEM TO BUILD A BRATTICE. HE BEGAN COMPLAINING THAT HE FELT PAIN IN HIS BACK AND LEG.
