EE and co-worker had just finished up their work assignment and was cleaning work area up, co-worker stated co-worker took some tools back to tool room and came back to work site area and found EE laying on floor unresponsive.

Truck Driver was attempting to dump load of coal when the bed of the truck broke loose from frame and during this occurrence it caused the cab of the truck to bounce pretty hard resulting in driver fracturing three ribs on left side

Truck had a blow out tire on inside wheel. Driver dumped load short started back down hill to get spare truck. Stopped to get out and check tire, while in front it blew off spoke striking driver.

Employee was removing center strip from raw coal screen. EE went to reposition self and felt pain in the left knee. Shift started at 6:00 pm on 12/27/16 accident occurred at 02:00 am 12/28/16.

Chute plugged on #1 cc belt head chute and employee was digging it out with a long spade and was leaning over into the chute and slipped/fell approximately 6 to 8 feet.

Employee was helping remove drive pulley on effluent pump. They had a chain hoist pulling on the pulley and using pusher bolts when it let loose from taper lock and hit employee in back and right leg.

Employee was getting out of cab of truck when he lost his footing and fell from fuel tank. He grabbed the hand rail on the cab of the truck and strained his left shoulder.

Employee was getting out of the mechanics truck on the passengers side and slipped and fell onto the ground. He said his legs were going numb at the time on him.

Coal truck driver parked on railroad crossing, then came into guard shack to pick up tickets. Train backed up on track and hit truck pushing it into guard's car and guard shack. Guard was knocked out of the door and was injured in his lower back.

EMPLOYEE WAS HELPING LOADING EQUIPMENT( A VULCANIZING PRESS) ONTO OUR TRUCK. THE PRESS WAS 3'OFF THE GROUND WHEN THE EMPLOYEE WAS GUIDING THE PRESS AWAY FROM THE RAILING TO PREVENT FROM HITTING WHEN THE STRAP BROKE .THE PRESS CAME DOWN THE Employer's LEGS THEN LANDING ON BOTH ANKLES AND FEET.

Employee was carring a roll of skirt rubber down into the clean coal tunnel. When he got to his work station, he threw the roll down and a piece of metal that was holding material together cut the palm of his left hand.

An EE raised up and got the metal handle into the bottom return roller on #1 clean coal belt and it whipped around and hit injured EE who was standing about 6 feet from him, he had lacerations over both eyes, which required stitches.

Employee was getting off Dozer and his feet slipped out from under him. He hit the lower part of his back and the right hip on the dozer blade arm.

A Komatsu dozer 375A was pushing coal in the clean coal stock pile at the plant, and the operater noticed that smoke was coming from the back of the dozer. We could not extinguish the fire in 30 minutes, but it was contained.
