EMPLOYEE WAS ASSITING ON WORKING ON WASHBOX IN PLANT, WHILE MOVING PARTS FOR THE WASH BOX EMPLOYEE STRAINED THEIR BACK. THEY BEGAN MISSING DUE TO THIS INJURY ON 01/21/2026.

EMPLOYEE WAS CHANGING A ROLLER ON BELT STRUCTURE AND GOT UP ON BELT LINE TO GO TO OTHER SIDE TO PIN ROLLER IN PLACE, WHILE ATTEMPTING TO GET TO OTHER SIDE STRUCK A PIECE OF METAL HANGING FROM DRAWOFF TUNNEL TOP AND STRUCK HEAD AGAINST METAL RESULTING IN LACREATION TO HEAD

EMPLOYEE WAS USING SLATE BAR TO ROLL A TOP IDLER INTO PLACE, IN THE LOAD OUT TUNNEL. WHILE PUSHING SLATE BAR WITH EE's HANDS SOMETHING LET LOOSE IN LEFT CALF. EMPLOYEE STARTING MISSING WORK ON 4/3/2024 AS A RESULT OF THIS INJURY.

EMPLOYEE WAS ON PLATFORM AT #1 FEEDER, THEY GAVE THE HOSE THAT WAS BURIED UNDER MATERIAL A HARD PULL AND FELT PAIN IN THEIR BACK. THEY STARTING MISSING AS A RESULT OF INJURY ON 10/09/2023.

EMPLOYEE WAS CHANGING SCREEN'S IN MID'S REF, HAD TO CUT RUBBER ON SCREEN WITH RAZOR KNIFE. THE KNIFE SLIPPED AND CUT THE PALM OF EE'S HAND NEAR THE THUMB.

EMPLOYEE WAS HELPING ADJUST LINKAGE IN A BREAKER AND EMPLOYEE WAS ASKED TO RELEASE BUTTON DOWN BUT INSTEAD THEY PUT A FINGER IN THE MECHANISM AND THE PRESSURE OF THE MECHANISM RELEASED CAUSING LACERATION AND BROKEN BONE IN FINGER

EE and co-worker had just finished up their work assignment and was cleaning work area up, co-worker stated co-worker took some tools back to tool room and came back to work site area and found EE laying on floor unresponsive.

Truck Driver was attempting to dump load of coal when the bed of the truck broke loose from frame and during this occurrence it caused the cab of the truck to bounce pretty hard resulting in driver fracturing three ribs on left side

Employee was closing the knife valve on Midd's pump so they could take apart and repair pump, when EE's lower back started hurting

Employee was lifting a beam when EE felt a sharp pain in lower back

Employee was changing rollers when EE went to go underneath belt struck EE's head against a protruding object.

Truck had a blow out tire on inside wheel. Driver dumped load short started back down hill to get spare truck. Stopped to get out and check tire, while in front it blew off spoke striking driver.

Employee was removing center strip from raw coal screen. EE went to reposition self and felt pain in the left knee. Shift started at 6:00 pm on 12/27/16 accident occurred at 02:00 am 12/28/16.

Chute plugged on #1 cc belt head chute and employee was digging it out with a long spade and was leaning over into the chute and slipped/fell approximately 6 to 8 feet.

Employee was helping remove drive pulley on effluent pump. They had a chain hoist pulling on the pulley and using pusher bolts when it let loose from taper lock and hit employee in back and right leg.

Employees were changing hangers on roller. Employee was holding wrench while co-worker was loosening bolt. The wrench slipped causing employee's finger to strike structure.

Employee was getting out of cab of truck when he lost his footing and fell from fuel tank. He grabbed the hand rail on the cab of the truck and strained his left shoulder.

Employee was climbing up to service dozer, not sure what he hit his right eye on causing laceration

Employee was getting out of the mechanics truck on the passengers side and slipped and fell onto the ground. He said his legs were going numb at the time on him.

Coal truck driver parked on railroad crossing, then came into guard shack to pick up tickets. Train backed up on track and hit truck pushing it into guard's car and guard shack. Guard was knocked out of the door and was injured in his lower back.

EMPLOYEE WAS HELPING LOADING EQUIPMENT( A VULCANIZING PRESS) ONTO OUR TRUCK. THE PRESS WAS 3'OFF THE GROUND WHEN THE EMPLOYEE WAS GUIDING THE PRESS AWAY FROM THE RAILING TO PREVENT FROM HITTING WHEN THE STRAP BROKE .THE PRESS CAME DOWN THE Employer's LEGS THEN LANDING ON BOTH ANKLES AND FEET.

EE went to close gate slipped while pulling on handle, fell 15 feet to belt then 5 feet to floor.

Fell off Fuel Tank.

Employee was carring a roll of skirt rubber down into the clean coal tunnel. When he got to his work station, he threw the roll down and a piece of metal that was holding material together cut the palm of his left hand.

An EE raised up and got the metal handle into the bottom return roller on #1 clean coal belt and it whipped around and hit injured EE who was standing about 6 feet from him, he had lacerations over both eyes, which required stitches.

Employee was using a chain ratchet to raise a piece of metal and the chain ratchet came loose striking employee in head requiring 5 stitches.

Deck screen fell off and onto the left foot pinning it to the floor.

Climbing across belt structure slipped hit right leg

Replacing bearings on belt line, he hit his head on a beam. Hard hat fell off.

Employee was watching another employee excavate a footer, to watch for a ground cable and conduit, when the backhoe bucket struck him knocking him down. He struck his chin on something causing a laceration.

While putting a lid on a dryer, the lid fell causing a laceration to the right index finger.

Prying on dozer track with slate bar, bar slipped & he fell back on his right foot.

Employee was getting off Dozer and his feet slipped out from under him. He hit the lower part of his back and the right hip on the dozer blade arm.

A Komatsu dozer 375A was pushing coal in the clean coal stock pile at the plant, and the operater noticed that smoke was coming from the back of the dozer. We could not extinguish the fire in 30 minutes, but it was contained.

Employee was cutting a plug out of a piece of wood when his pocket knife slipped cutting his left hand requiring stitches.

Employee was unloading materials with fork-lift and got off and twisted right knee. Started missing 12/01/2005.

Door of crusher fell on right hand smashing his ring finger & middle finger fracturing middle finger.

EMPLOYEE FELL BACKWARDS STRIKING HEAD ON I-BEAM REQUIRING STITCHES.

EMPLOYEE WAS REPAIRING PUMP WHEN A PIECE OF METAL BECAME EMBEDDED IN HIS RT. HAND & HAD TO BE REMOVED, REQUIRING STITCHES.

EMPLOYEE REPORTED SEVERE BACK PAIN WHILE OPERATING A BULLDOZER.

EE was traveling under a low clearance area when he struck his head on a low beam causing him to jam his neck.

EE SLIPPED & FELL ON ICE AND SNOW, INJURING HIS RIGHT KNEE.

EE WAS CLOSING RAILROAD CAR DOORS WITH A SLATE BAR. THE BAR SLIPPED, CAUSING HIM TO FALL BACKWARDS, TWISTING HIS ANKLE.

DELIVERY DRIVER EE BY SUPERIOR CARRIERS WAS DELIVERING A LOAD OF NALCO 9853 CAT-FLOC WHEN HE SUFFERED A FATAL HEART ATTACK WHILE SITTING IN HIS TRUCK.

EMPLOYEE WAS OPERATING A CHAIN HOIST AT PALLETIZER. A CO-WORKER WAS DRIVING WHEELS IN PLACE ON SHAFT WITH A HAMMER WHEN A CHIP OF METAL FLEW OFF ROLLERS AND STRUCK EMPLOYEE IN CHIN. METAL LODG ED IN EMPLOYEE'S CHIN.

EE SLIPPED ON ICE AND SNOW WHILE WALKING, INJURING HIS LEFT KNEE. ORIGINAL INJURY WAS NOT REPORTABLE UNDER PART 50 AT THE TIME OF ACCIDENT. HIS CONDITION HAS NOW DETERIORATED AND HIS PHYSICIAN HAS RECOMMENDED SURGICAL REPAIR. SURGERY IS SCHEDULED FOR AUGUST 20, 2003.

EMPLOYEE SLIPPED ON GRATE AND TWISTED HIS KNEE.

EMPLOYEE WAS ATTEMPTING TO DISLODGE A PIECE OF BELT LINE FROM THE COARSE REFUSE CHUTE WHEN A ROCK FELL OUT AND HIT HIS RIGHT HAND. A CUT TO HIS RIGHT HAND REQUIRED THREE SUTURES.

WHILE UNLOADING A 30 HP MOTOR FROM F250 INTO BOBCAT BUCKET, THE MOTOR SLIPPED AND CAUGHT THE FOREMAN'S FINGER BETWEEN THE MOTOR AND THE BACK OF THE BUCKET.

EE WAS USING A RATCHET WRENCH TO TIGHTEN A TIRE.THE RATCHET SLIPPED CAUSING THE EE TO FALL ONTO THE CONCRETE BERM. EE STRUCK LEG AGAINS BERM RESULTING IN A FRACTURED BONE

EMPLOYEE WAS LOWERING PIPE DOWN ON 5TH FLOOR (TO USE FOR REPAIR WORK) WHEN HE RELEASED BUTTON TO STOP HOIST, IT DIDN'T STOP AND FELL ON HIS LEFTFOOT.

INJURED WAS DUMPING WHEELBARROW DURING CLEAN UP, AND IT TURNED OVER IN THE FLOOR WITH MUD IN IT AND WHEN INJURED ATTEMPTED TO LIFT IT BACK UP HE ALLEGED A SPRAIN TO HIS LOWER BACK.
