Caught in, under or between a moving and a stationary object

EE was attempting to align a pin up on the counterweight for a Cat D-10 dozer when EE's left wrist got caught between the frame of the machine and the counterweight.

Employee was using a hose to clean beneath a tailpiece and when EE raised up EE struck left shoulder on a cross brace for the DSS-7 belt.

Employee was unloading metal from metal rack and caught middle finger of left hand between 2 pieces of metal.

Employee was removing a Belt sequencing switch using a knife to cut the wires. The knife cut through the wires and struck EE on the left arm causing a laceration above elbow.

Employee was changing a light in the shop area of the Prep Plant when they tripped over a pipe lying in the floor and fell to the floor landing on left knee causing pain.

Employee was walking through door of warehouse carrying a razor scraper and dropped it on the floor. When they reached down to pick it up their index and middle fingers of left hand made contact with the blade cutting fingers.

Employee was pulling on skirt rubber to replace belt conveyor when employee felt pain in upper and lower back. Employee started losing time on 8/30/2017

Employee was assisting in cleaning a screen. When handing a water hose to his co-worker, his feet slipped out from underneath him, resulting in a fall.

Employee was assisting w/ the installation of a new sieve & strained his groin area. He hoped it would improve over time & when it did not, he sought medical treatment for a hernia on 5/20/15.

Fall down stairs

Employee was descending a flight of stairs when he fell, sliding all the way to the bottom and resulting in injury to his lower back.

Employee fell stepping out of the Raw Coal Control Room, injuring his Right knee.

Struck by falling object

A rock became lodged in the auger tube. When the employee pried the rock loose, bolts securing the auger also loosened, causing the auger to fall from the sample tube. The employee suffered crushing injuries to his right arm as a result of this incident.

Employee previously had rotator cuff surgery on his left shoulder. On February 4th, 2010, he slipped walking down a flight of stairs and attempted to prevent falling by grabbing onto a handrail. This motion re-injured his surgically-repaired shoulder.

Employee was shoveling material onto a conveyor belt in the raw coal tunnel when he experienced pain in his lower back. He did not report this accident until February 27th, 2009.

Coal truck driver was traveling in 4th gear at approximately 10-12 mph when he heard a noise. After hearing the noise, he let off the fuel, causing the load to shift & the truck to overturn.

Employee was removing garbage from a railroad car when he stepped on some loose coal, causing him to fall backwards. The employee began missing work as a result of this accident on 5/26/2008.

EE was fabricating a chute in the welding shop (located in the basement of the Prep Plant) in front of his tool box. The witness saw the tool box falling & yelled to warn the victim. The victim attempted to move out of the way, but was still struck by the tool box, knocking him to the ground.

Injured employee and a co-worker were sliding a raw coal screen into the bed of a truck when one of the victim's fingers became caught in between the pinch-point created by the two surfaces. The incident resulted in alaceration that required stitches.

EE was peeling insulation from a conduit when his razor knife slipped, resulting in a laceration to his left ring finger, which required stitches.

EE was shoveling coal onto the belt in the raw coal tunnel when he experienced pain in his neck and back.

Over-exertion in pulling or pushing objects

Employees were pulling wood and rocks from the raw coal crusher in an attempt to unclog it. The injured ee later experienced back pain which did not subside. He sought medical treatment for this on 2-11-05.

EE WAS RIGGING STEEL BEAMS UP ATTHE FIRST FLOOR LEVEL OF THE PREP PLANT DURING CONSTRUCTION OF AN ELEVATOR. ONE OF THE BEAMS SWUNG AND CAUGHT HIS LEFT LEG BETWEEN A 16"" HIGH CONCRETE BERM AND THE STEEL.

WHILE LOADING BELT STRUCTURE, FELT SHARP PAIN IN LOWER BACK. NOTE: INCIDENT HAPPENED ON THURSDAY EVEING, EE CAME OUT TO WORK ON FRIDAY AND INFORMED HIS SUPERVISOR BUT DIDN'T WANT TO GO TO THE DR. EMPLOYEE WAS REMOVING FEED PIPE FROM #2 SMALL COAL DRYER. HE HAD A CHAIN RATCHET ON FEED PIPE, WHEN HE GOT ALL THE BOLTS OUT, ONE END OF THE PIPE SWUNG AROUND TOWARD HIM AND CAUSED HIM TO FALL OFF THE DRYER. HIS FACE HIT #1 SCREEN BOWL CAUSIING A CUT NEAR HIS RIGHT EYE.

EE WAS TRYING TO ROLL V BELTS ON OVER PULLEY OF MOTOR AND CAUGHT RIGHT RING FINGER UNDER BELT. LACERATION AND STITCHES TO END OF FINGER.

EE WAS ATTEMPTING TO FREE A STUCK COUPLING HALF ON THE R-5 DRIVE UNIT. THE COUPLING EE FELL OFF THE SHAFT AND MASHED THE EE S FINGER CAUSING A LACERATION TO THE FINGERS.

EE WAS ASSISTING WITH THE INSTALLATION OF BELT HEAD ON CONSTRUCTION OF THE NO.4 REFUSE BELT. THE BELT HEAD HAD BEEN MOVED INTO PLACE BY A CRANE, BUT HAD TO BE SHIFTED SLIGHTLY TO MOVE INTO COR RECT POSITION. THE EE GOT HIS HAND PINCHED AGAINST AN ADJACENT CHUTE, CAUSING A CUT REQUIRING 3 STITCHES.

Rubbed or abraded

EE WAS ON HIS KNEES CLEANING UP CONCRETE THAT HAD BEEN BUSTED UP. HE EITHER TWISTED OR BRUISED HIS KNEE, WHICH CAUSED IT TO SWELL UP.
