While removing tape from waterline coupling, the knife used to cut the tape had slipped causing laceration to left hand.

Employee was working on 270 excavator changing bush hog attachment back to bucket. Employee picked up pin to install in back of bucket when he felt pain in his left shoulder. Employee became a lost time accident on 9/12/2014 due to his injury.

Fall from ladders

He said he was sandblasting steel and began to fall from a ladder that he was using. He said he went to grab a piece of the siding that was rusted and not bolted. He said he cut his left wrist when he reached for the steel and fell on his left shoulder on the ground. He said he did not want to see a doctor initially. He said he thought it would work itself out.

Employee was cutting skirt rubber on refuse belt with pocket knife when knife slipped cutting top of left hand, sutures required, employee has returned to regular duty on 02/04/2010 with no restrictions.

Employee was primining pond pump on 06-10-07, when finished employee stepped back and foot slipped in mud causing employee to fall spraining left knee. Employee had continued to work until 3-24-08, when employee had surgery on left knee. This is an injury that was filed on 6-10-07, it became lost time on 03-24-08.

The employee was working on the dozer and struck the muffler with his arm. He had sutures placed in the cut. He returned to work his next scheduled shift to his regular job.

He was using a 10 lb hammer when he felt a pain in his back. He stated he thought he had pulled a muscle in his back. He went to the doctor on January 31, 2006 and was placed on restricted duty. He taken off restricted duty Feb. 3. 2006.

Employee was carrying gear oil up to a stacker drive; he slipped on loose coal. He twisted his back and is having back pain. He is on restricted duty for 10 days. He returned the next scheduled shift.

EMPLOYEE WAS INSTALLING A CLEAN-UP SIEVE IN THE PLANT WHEN HE FELT PAIN IN THE LOWER PART OF HIS BACK. THEN ON 9/9/03 HE WAS LIFTING A 55 LB BAG OF MATERIAL WHEN THE PAIN GOT WORSE. HE WENT TO THE DOCTOR ON 9/10 AND MISSED TWO DAYS OF WORK. IT BECAME REPORTABLE 9/10/03.

EE WAS HOLDING A DOOR OPEN AND IT SHUT CATCHING HIS LEFT RING AND MIDDLE FINGER. SUTURES WERE REQUIRED (5) IN HIS MIDDLE FINGER. HE RETURNED TO WORK THE DAY OF THE ACCIDENT.

EE WAS USING AN AIR WRENCH ON A SCREEN IN THE PLANT, WHEN DEBRIS GOT IN HIS EYE. HE FLUSHED IT BUT WENT TO THE DOCTOR ON 8/7/02. METAL WAS REMOVED. THIS WAS NOT A LOST TIME ACCIDENT.

EE WAS BURNING TRASH AND WAS TRYING TO START THE FIRE USING DIESEL FUEL, WHEN IT FLAMED UP BURNING HIS LEFT HAND AND BOTH FOREARMS. HE RECIEVE A BURN ON HIS LEFT HAND BETWEEN A 2ND AND THIRD D EGREE BURN.
