Victim was in operation of Genie S-85 aerial lift when the lift became caught on exterior wall girt of building under construction. In an attempt to free the caught basket the machine's boom reacted after the basket came free, resulting in the victim's head receiving injuries which were fatal.

Employee was working under the turn table of the clean coal stacker when the wind blew causing material to get into left eye. The material was embedded in the employee's left eye and was removed at the Dr's office.

Contractor was hauling soil from the back side of gob hill. As EE was returning for another load, EE heard a pop in the engine and started smelling and seeing smoke in the cab. As EE stopped to make a check, the truck was on fire. EE tried to put the fire out with a fire extinguisher EE had along with another one from another truck. Fire Dept was called. No injuries.

Employee was putting bolts in a piece of rubber on the vibrator lip when the walk board broke causing EE to fall into the chute. EE hit head shoulder and hip on the metal sides. EE had 6 staples in head.

Employee was operating a dozer while loading a train; a chute stopped up and was not feeding coal. After about 20 minutes of it not feeding the operator's were told to stay off of the feeder. The employee drove his dozer to close as he was feeding the main feeder and the dozer fell in bridge area of the feeder. The employee was not entrapped and he got out on his own.

EE was in the process of cutting a steel cable that was tangled on the D8N dozer at the trash dump. As ee lit the torch the first time it went out. As he was relighting it a buzzing nose and a ignition happened. This caused burns to the left elbow and low arm.
Magnet on raw coal B VIB, The hook broke and struck ee in the back of the head, also in the middle of his back.
This contractor was lifting a pipe up about 4ft to put it into place. The pipe weight was 140. The flange end had a sharp edge that wasn't noticed. Another EE was helping do this job.

After employee had locked & tagged the belt out for repairs he decided to stay and help change the roller. This was a 54 in. troughing roller. While putting the roller in position to stand it up the roller slipped and mashed his left thumb between the belt frame and roller. 10 stitches to injury.
Employee was trying to break loose a bolt with a wrench, in doing so the wrench slipped and hit him in the mouth, cutting his bottom lip and chipping his tooth.

While attempting to stop a leak on the refuse box, two workers were going to apply a patch by using a piece of belting and metal. Co-worker climbed up a permanent ladder and injured employee grabbed the closest ladder, the upper part of an extension ladder. As he climbed up the ladder, it slipped causing him to fall.
EE was trying to dislodge a screwdriver from a shaker in the prep plant. EE missed hitting the screwdriver and hit his hand. This caused him to have 5 stitches.

EE was replacing 200 amp fuse for the deep well Pump ""D"". While installing a fuse his right thumb made contact with the voltage barrier causing a laceration to the pad of right thumb.

EE was cutting a piece of bucker rubber for repair on the vibrator. While sitting down, he was holding the rubber in his left hand and in his right hand he was using a belt knife to cut the rubber with. After cutting through the hard surface, he entered the soft spot part with extra force, causing the knife to slip and cut his upper leg.
Pressure washer had quit. Worker climbed into back of truck, fixed the problem by securing a wire, turned around & fell off the truck. Initially, this was not a reportable accident because worker did not seek med. treatment & no time was lost. Worker did not seek med attn until 9/14/10 & was placed on restricted duty. Effective 9/20/10, he was off work completely.

Employee was in the coal loadout tunnel watching the belts while the train was being loaded. He noticed a piece of coal was stuck in the skirt rubber on the south side of the belt (main). He removed the piece of coal and the conveyor belt caught his right hand and arm
EE was grinding on a 988 loader bucket, as he was grinding down a vertical position, sparks were going down and to his right. Foreign material entered his left eye. Safety glasses with side shields were worn at the time of the injury.
Employee was grinding on a dozer blade. As he was grinding he felt like something was in his right eye. He finished the shift then he went to the nurses station & had it washed out. He then had to to to the Dr. to have a foreign object removed from his eye. Safety glasses with side shields were worn at the time of the accident.

EE was removing a chute from the #2 belt drive stand. On his last attempt to adjust the chain rigging, he stepped in a spot where a roller bearing had been removed, stepped in grease and his left foot slipped backwards and he fell through an opening in the floor, 12 ft to the ground. He started missing work 5-22-08.
Employee was standing on the North winch table hooking up ground for the welder. After hooking the ground up, he went to step off of the table and lost his footing, landing on uneven ground on his right leg. He started missing work 3/16/08. He will be off 4 months (ACL).

EE was in the process of taking out the old belt scales at the loadout tunnel. As he was cutting the last leg, which was tied off by the winch cable to the boom truck, the metal kicked back, causing his finger to get pinched between the I beam.
As ee was drilling a borehole into the old works, he stepped off the drill platform and twisted his right knee. 7/13/07 was his first day to miss work.

EE was in the man lift on the west side of the gob belt bottom counter weight roller. He put his left hand on the roller bearing to see if it was heating up and an eye welded to the other rollers came around the inside of the bearing and caught his index finger on the left hand.
Employee was trying to open the flop doors at the syn-fuel discharge belt. As he was pushing the handle over, something gave way and broke. This caused the handle to go forward and away rapidly. He held onto the handle causing him to spin around and fall backwards approx 12 feet to the concrete pad.
While moving a 1"" thick piece of plate metal app 8""x33""x1"" the plate was being pulled off the bed of the welding truck, it slipped out of ee's hand creating a whoosing effect stirring up rust. dust and possibly weld slag. He felt something in his right eye immediately.

Ee was standing by the core drill assisting the e-logger on the report sheet when the wind started blowing causing debris to go into the eye.
BANDING A TEMPORARY PATCH ON A PIPE. EE STARTED TO LOAD THE BANDING TOOL WHEN HE DISCOVERED AN OLD PIECE OF BAND IN THE TOOL. WHEN HE WAS HOLDING THE TOOL WITH HIS LEFT HAND & USING CHANNELLOCK PLIERS WITH HIS RIGHT HAND, THE OLD PIECE CAME OUT FASTER THAN EXPECTED, CUTTING HIS KNUCKLE ON LEFT THUMB - 3"" IN LENGTH.

EMPLOYEE WAS IN THE PROCESS OF CHANGING THE DRILL BIT ON THE DK40 DRILL, AS HE WAS LIFTING THE BIT HE SLIPPED & TWISTED HIS BACK.
EE WAS OPERATING A D9L DOZER ON THE RAW COAL STOCK PILE, WEATHER FOGGY. WHEN HE STARTED TO PUSH THE COAL, HE TRIED TO ADJUST THE C.B. RADIO HE NOTED HE HAD GONE TOO FAR ON THE PILE. HE TRIED TO PUT THE DOZER IN REVERSE GEAR, BUT COULD NOT, THE SEAT BELT WAS IN BACK OF THE SHIFTER. HE WAS NOT WEARING THE SEAT BELT.
WHILE TRYING TO TURN THE NEW GOB CHUTE THAT HE WAS FABRICATING, EE WAS USING A STEERING LOADER TO PICK IT UP & USE A PRY BAR TO TURN IT OVER. AS HE WAS TURNING IT OVER, THE CHUTE SLIPPED CAUSING THE BAR TO PIN AGAINST THE FLOOR, WITH HIS FOOT IN BETWEEN.
EMPLOYEE WAS OPENING ACCESS DOOR TO THE #5 COUPLING AT THE SLOPE HEND DRIVE, THE DOOR BECAME OVER BALANCED & STARTED TO FALL AWAY FROM EE. HE REACHED OUT WITH HIS LEFT HAND TO CATCH THE DOOR & HIS THIRD FINGER GOT MASHED BETWEEN DOOR & THE SPEED REDUCER.

EE WAS HELPING WORK IN THE WASH PLANT TAKING DOWN A CHUTE. THE CHUTE WAS RESTING ON THE FLOOR AND TOP OF NEXT FLOOR. A LADDER WAS NEXT TO THE CHUTE WHEN EE WAS MOVING THE LADDER IT CAME OVER HITTING EE IN THE RIGHT SHOULDER.
EE WAS GETTING READY TO OPERATE THE FRONT END LOADER, HE STARTED TO GET ON THE MACHINE, HE WAS ON THE 3RD STEP AND HE SLIPPED OFF, FALLING TO THE GROUND.
