LOADED COAL TRUCK WAS LEAVING PIT. FOR SOME REASON STEEP HAULWAY ROAD TRUCK ROLLED BACK. APPARENTLY DRIVER TRIED TO JUMP OUT. HE WAS CAUGHT BY TRUCK AND KILLED.

NOTIFIED OFFICE ON 11/30/12 OF ACCIDENT THAT WAS CLOSING GATE AT SITE ON 11/15/12 AND STEPPED IN A RUT AND KRILLED ANKLE AND KNEE HIT GATE AND HURT LEG

Employee lost his footing on the bottom step while attempting to mount a Cat. 992G loader at the beginning of the shift. He fell to the ground, landing on his left knee/shoulder. Diagnosis from Doctor was no broken bones, just bruised.

While sitting in his rock truck waiting to be loaded, a rock rolled out of the shot, striking the rock truck in the rear of the truck, causing a quick jerk to the operator.

As employee was servicing the D11R Dozer, (Co. Unit No. 98), he lost his footing while standing on the push arm and fell backward, landing on his back. This is a distance of approximately 3 feet. Diagnosis is pending.

Employee was pushing in a shot with a D11 Cat. Dozer, as he backed up, he ran over a rock, causing a quick jerk to his lower back. He was diagnosed as having a strained lower back muscle.

EE stated that while being loaded, the loader man dropped a rock into the bed of his rock truck, causing back pain and tingling in his legs.

While pulling at a hose, EE stated that he had hurt his back. The preliminary diagnosis was a muscle strain.

Employee stated that while fueling equipment, he was dragging the fuel hose in the mud when he felt a sharp pain in his lower abdomen. He did not go to the doctor until 3/30/2011. He stated that he had a Hernia but did not and has not had it repaired. This is a Partial Permanent Disability.

While employee was cutting a tree with a chain saw, a dead limb fell from the tree, striking him in the back. He lost control of the chain saw and it struck him in the left leg, causing 52 stitches. He was transported to the Hazard ARH where he was treated and released.

MECHANIC WAS CHECKING SQUEALING NOISE ON JOB FOREMAN TOYOTA PICKUP. HE WAS SPINNING THE FRONT RIGHT WHEEL WITH HIS LEFT HAND. WITH HIS THUMB ON THE RIM HIS GLOVE CAUGHT SOMETHING ON THE BACK AND PULLED HIS THUMB INTO ONE OF THE HOLES ON THE RIM. HIS THUMB STOPPED ON THE BRAKE CALIPER AND THE WHEEL KEPT TURNING. HIS THUMB WAS MASHED AS A RESULT.

Employee was pushing a rock with the corner of his blade of his Cat. D10 Dozer when the blade slipped off the rock, causing a quick jerk to employee's back. He was transported to the Hazard ARH where the preliminary diagnosis was a muscle strain.

EE said that the truck jumped out of gear and he could not get it to go back into gear. EE got the truck out of gear himself. (Shifting)

EE was preparing to fill tank on water truck. While walking around the truck pulling a chain EE stepped up onto a rock and slipped. Falling to the ground his leg became hung causing him to twist his leg on the way down. At the time ee felt like he could complete his shift and did. EE did go see his doctor a few days later.

Rock truck driver left work at 11:15 AM to go see his doctor. He said he wasn't feeling well and he felt his face going numb. He mentioned that the bed on the RT23 truck was coming down hard, but didn't think this had anything to do with the way he was feeling. ***Days later he said the Dr said the bed coming down hard could have been a factor.***

Employee was removing the water pump from a D11 dozer and the pump slipped out of his hand, catching his hand between the water pump and frame.

EE was taking fan shroud off 777D when ladder he was on shifted. Felt like he pulled something in the lower left part of his back.

When getting into service truck, EE reached for the grab rail to pull himself into the truck. When he did he felt a sharp tear like pain in his right shoulder. EE did not mention this incident until a few days later when he sought medical attention.

When climbing back on dozer after filling up with hydraulic oil, operator slipped on the tracks and fell onto the dozer/ground. Conditions were wet and muddy. Leg and ankle were hurt during the fall. Was treated and sent to hospital for X-rays.

Employee finished refueling equipment spread & was rolling up the fuel hose in the rear of grease truck. A wind gust caught the rear door of the truck slamming it into employee's head knocking him down. The door was fastened at the time of accident/injury, per accident witnes. Weather was very windy, with gusts ranging upwards up to 40-50+ MPH.

EE was working on radiator using a pry bar. The pry bar slipped hitting the ee in the nose.

Employee was climbing off service truck at side entrance when he lost his hold and fell approx. 5 feet to the ground landing on his rear side-lower back. Was helped up but was able to walk uner his own power.

Employee was returning from break. He was remounting the Dozer when his back gave out which caused him to fall back twisting his left leg and left arm.

Employee was fueling up the coal truck he was driving and all employees are told not to fuel up truck after dark. He lit his cigarette lighter to see if the tank was full and he got burnt. They are always told in safety meetings not to smoke or use anything that could cause a fire.

Rock Truck Operator was climbing up and onto his rock truck when he lost his grip and fell approx. 12 feet to the ground landing on his left leg and ankle. Weather conditions were rainy, causing the ground to be muddy.

When walking across the parking lot a gust of wind blew a foreign object into employees eye. After several attempts to wash the object out failed, Injury went to the hospital to have the object removed.

In the rain, employee was putting Hyd Oil in a D-11-R Dozer when he slipped on the dozer's tracks causing him to twist his ankle. He completed the remaining 15 minutes of his shift. Later that day employee went to the Doctor and X-ray revealed a chipped bone in left ankle.

Injury was attempting to replace a bad roller on a dozer when the roller slipped. Trying to catch the roller, he pulled his back muscle.

Waiting on repairs being done to his dozer, employee started inspecting a ladder rung that had come off another loader. When he attempted to pick it up he felt a muscle pulling his neck.

EE was operating a D11R dozer, pushing rock with the left corner bit of the blade. The section he was pushing broke off of the rock, causing the dozer to jerk forward, at the same time throwing EE forward and then back into the seat.

Returning from break, employee was attempting to climb back into his coal loader when his right foot slipped on a ladder rung causing him to sprain his right ankle/foot.

D11R Dozer #30 was in shot ripping overburden when ripper came in contact with live shot booster and cap causing an explosion. Damages to the Dozer included a cracked door glass on left side. A nearby 992G Loader #1 received a cracked left side window. NO INJURIES TO EITHER OPERATOR WERE REPORTED.

Employee was cutting teeth from loader bucket with torch and chipping hammer. When he struck the tooth with the hammer, a piece of metal chipped off striking him in the groin.

Employee was pushing a big rock out of the road, The rock broke and came back hard against the blade. He felt the seat go back causing him to hurt his shoulder. It started burning real bad. He finished his shift. He came back the next shift but was to sore to work. I took him to the ARH hospital in Hazard, KY.

EE was grading road in 2nd gear pulling the ditch back across road. The blade hit a rock causing him to slam forward & hurting his back. He called his foreman on CB radio to come to him. When foreman arrived he asked EE if he needed an ambulance. EE said, ""No, I can drive myself to Dr's office."" Foreman asked him if he was wearing seatbelt. He said yes.

Pushing forward over rock, dozer spun around with a jerk causing him to hurt his back and leg. Refused medical attention (ambulance), saying he did not want an ambulance. Stated he would drive himself.

Employee was attempting to put an air conditioner belt on an L-1 loader. Pry bar slipped hitting him in the mouth/upper lip. He said he was holding the pry bar down with his elbow while he was trying to get the belt onto the machine.

Employee was pushing a big rock down his run. The rock rolled over pushing his dozer blade back up. When it slammed back down it caused him to pitch his body forward and hurt his back.

Employee was digging shot rock and hit a hard spot next to highwall with loader. He had been working in shot since 6:00 PM. He said it hurt his back when he hit the hard spot.

Mechanic was lifting belly pan back up on dozer and hood on chain became unhooked. Belly pan swung down and hit mechanic helper in right foot. Top of foot was blue in color.

Non-injury accident. Employee was using tourches to cut bolts from old fuel tank after he was told not to. Hot slag fell into old fuel dike starting fire.
