Fall to the walkway or working surface

EE got out of the rock truck and was walking to the control station to open the gate and fill EE's truck. EE stepped on a piece of tin laying in the walkway and slipped and caught self with right forearm causing pain to EE's right shoulder. EE asked to go to the hospital to be checked, said EE had a previous injury.

Employee had walked up and down stairs 3 times in about a 20 min span to the 3rd floor on the time EE started complaining with chest and left arm. Employee was removed from the plant and we called an ambulance to take EE to be checked.

Fall onto or against objects

Employee was helping change top deck screen on the raw coal screen in the plant on the top floor. EE stepped on the stainless and lost footing and fell flat on back on the side of screen and then to the deck.

Fall from machine

Employee was exiting dozer and stepped on the track and lost his footing. He fell on his butt and bruised his lower back and butt. Employee was using hand rail.

Absorption of radiations, caustics, toxic and noxious substances

On 7/21/14 called in stating he was going to the Doctor because he had blister on the top of his feet, on 7/14/14 Employee had helped repair a caustic soda line and reported he had spilt the chemical on his boots, over the next few days he developed blisters on the top of his feet (Employee is a diabetic)

Employee was using a chain hoist and a 3/8 chain to lift a side of a screen to change the springs on the right side the chain around a beam holding the hoist broke and the screen fell forcing a spring out hitting him in the right knee causing him to fall.

Employee was using a pry bar to help remove a pitman arm from a steering box on a Mack truck. The bar slipped causing the employee to trip and fall twisting his right ankle. Employee was taken to E.R. X-rays showed his ankle was cracked in 2 places.

Employee was using hoist to lower a section of pump to the ground. He was using his hands to guide the section to the resting place when it turned & mashed the end of his left pinky finger between his section and another section of the pump

Employee said he was back dragging a road and turned his head around to look back and said his back went out.

Struck by falling object

Loading refuse, feed was shut off. EE got out of the trk to clean windows, rock fell off trk and hit him on the lip.

Struck against stationary object

The EE was working the loadout sampler skirt boards. He had dropped one side and had loosened all the bolts on the other side, except for one. He had a socket and racket on the last bolt, when it broke. His right hand was propelled backwards and struck another bolt, puncturing his hand. He received 5 stitches.

Struck by powered moving object

Contr EE of Twin Ridge Development, cntr#b294. Acc investigated by MSHA. Refuse trk driver was struck by csx train while crossing track. Driver was fatally injured. MSHA has not determined whether it has jurisdiction/reportable since the accident location is owned by the railroad.Statement given at MSHA office 5/22/07.

Employee was connecting pipe and suffered lacerations to his head after falling from being hit in head with pipe. Employee lost consciousness for a short period of time but was well enough to return to work the next day.

Employee slipped in the mud and fell, bruising his tailbone. He worked entire shift & sought medical treatment 1-24-07 and has not returned.

Employee laid torch on its side on corner of washer vessel. Torch fell off washer and hit employee on the head cutting him near his eyebrow.

THE EMPLOYEE WAS DISMOUNTING FROM A BULLDOZER WHEN HIS FOOT SLIPPED AND HE WAS HOLDING ON THO THE GRAB RAIL. THE WEIGHT OF HIS BODY ON HIS ARM CAUSED HIS SHOULDER TO DISLOCATE.
