Struck by flying object

Employee was standing near head pulley on C-5 and radioed the operator to start the belt. When started the torque arm holding the gear box assembly broke free allowing the assembly to spin forward. The assembly struck the employee in the left leg.

Accident type, without injuries

A contracted mechanic was conducting hot work activities on a drill rig, in close proximity to three freshly drilled bore holes. As EE was using a cutting torch, EE alleges to have heard a noise come from one of the holes. There were no open flames or any material ejected from the hole.

Non occ. non work related heart attack.On 2/3/12 a haul truck operator at the Quarry experienced chest and arm pain. EE's at the site called 911. The haul truck operator was transported to the local hospital via ambulance.

Employee was installing a striker bar in a Universal Crusher Model 130/150. The bar shifted when it was being inserted into the crusher causing the employee's left middle finger to be pinched between the bar and drive sheave pulley resulting in a fracture to that finger.
