As the middle bridge was being added in, the back carrier trammed up and came in contact with the cribs that had the bridge cribbed up and caused the bridge to slide over striking EE.

Caught in, under or between a moving and a stationary object

EE and the scoop man were going to get the maintenance insert and some oil to take to the miner to service it. EE got in the bucket of the scoop between the insert and the ram. The scoop man backed up the heading and pulled up into 4 right to turn. As they pulled forward the insert contacted the rib, pushing it into EE.

Over-exertion in lifting objects

Injured and another employee was putting a set of scoop batteries on charge. Injured bent over to pick up the female end of the jumper cable when upon doing so, they felt a pain in their back going down right leg. We were not informed of the results from the specialist until 08/28/2025.

Struck by falling object

EE was scattering structure when EE tripped over a roller. As EE fell, the stand that EE had landed on EE's legs.

Employee was removing some curtain in 3 left cross cut. EE slung the curtain and as EE did EE's left hand struck a roof bolt plate.

a non-injury unplanned fall occurred on the parallel mains in the track entry beginning at the in by corner of break 36 and extending approximately 70'.

Employee was gathering parts for a head drive. EE sat down to eat lunch and took off EE's gloves. When EE stood up and was putting on EE's gloves, EE slipped, and EE's hand hit a piece of metal used for a back board on the drive. EE received three stitches in EE's right hand.

Over-exertion in pulling or pushing objects

Buggy became stuck while crossing the rails. Employee got a jack and a bar to try to dislodge the buggy from off of the rails. When they pushed down on the bar that was inserted in the jack, they felt a pain in their lower back.

Struck by falling object

EE was bolting in 3 left. EE had already installed the outside bolt and swung in to install the inside bolt when a piece of draw rock fell from the top striking EE on the right hand around the pinky finger.

A roof fall occurred in the #4 entry on the Narrow Branch mains between c/cuts 65 & 66. The fall is approximately 5' high by 19.5' wide by 40' long.

While retreat mining was being performed in the #4 coal pillar, x-cut 32, on the F Panel off the Mudlick submains, a large rock from the mine roof dislodged and caused the continuous miner to become stuck.

EE picked their tool bag up out of the tool box to put in a scoop bucket and felt a pull in their shoulder as EE swung the bag into the bucket.

Employee was attempting to unload a speed reducer at the track to take to the section. The speed reducer slipped while EE had a hand on it catching EE's fingers on the right hand between the shaft of the reducer and the lip of the ride. This resulted in a broken index finger and 7 stitches on the middle finger. Employee received a full release to return to work.

Struck by falling object

A roof bolter operator was getting drill steel out of the hole when it fell, knocked off EE's glasses, and hit and scratched EE's left eye. A prescription medication was prescribed and used.

Employee stepped down off of belt and tailpiece and felt a pop in left leg/hip. Diagnosed with a tear in hamstring. Started missing work on 8/2/2022

Found an unintentional roof fall on the #11 Beltline (Stratton Fork Mains) at break no. 21. Roof fall was approx. 4-5' high by 20' wide by 30' long.

Employee was handling and loading belt structure stands into the scoop bucket. One on the stands was stuck. As EE pushed and another employee pulled on the stand, the stand broke free pinched EE's finger between the stand and scoop bucket. 6 sutures were required.

Employee was using a lifting jack to align the low structure to insert a pin. The jack slipped causing the pin to pinch employees' thumb between the pin and low structure. Sutures were required to repair the wound.

Employee was troubleshooting a stuck bridge conveyor chain and caught foot under the bridge conveyor as it was being let down. EE was diagnosed with a fracture to a metatarsal bone in the foot.

Employee was handling a belt structure rail when a sharp edge penetrated EE's glove and cut EE's right ring finger. 3 sutures were required.

Employee knelt down on right lower leg pushed down against a roof bolt plate and it cut the employee's leg. 4 sutures were required to repair the wound.

Employee stated was tossing roof bolts across the roof bolter and EE's knee started hurting. Employee did not notify mine management until 2/22/2021 and EE's immediate foreman was unaware of the injury. EE started missing work on 2/10/2021.

Employee was bending a 5' 3/4"" rebar roof bolt and the bolt slipped and employee felt a pain in left arm and shoulder. Employee was diagnosed with a tear in the rotator cuff on 1/22/2021 and taken off work.

A piece of rock fell and struck employee on the left forearm and sutures were required to close the wound.

Employee was helping load high voltage cable into a supply car and felt a pain in the groin when EE pulled on cable. Started missing work 6/8/2020.

Found an unintentional roof fall at Break #41 of the #11 Belt Line. Fall was approximately 50' long by 20' wide and 3' to 5' thick.

Employee lifted and carried a rail motor coupler and felt a pain in shoulder. EE injured this same shoulder in January 2019 that resulted in loosing time from work. It was thought that this was a re-occurrence but diagnostic tests results received by the operator on 6/2/2020 indicated that a new tear in the bicep was found in a different location than before.

Employee was prying up on an MBC rub rail with a slate bar, the bar slipped and they felt a pain in left shoulder. Employee continued to work and was then referred to a specialist on 3/10/2020. The orthopedic specialist diagnosed employee on 4/28/2020 with a tear on rotator cuff and took employee off work.

An employee fell and struck a belt tailpiece and cut their arm. Sutures were required and they returned back to work full duty.

Employee was removing an underground mandoor and the door inadvertently shut onto EE's left middle and ring fingers. A medical procedure and stitches were required on the finger tips.

Found an unintentional roof fall at cross-cut #44 on the #2 track and connecting crosscut to the belt line. Dimensions of fall were approximately 18' wide by 55' long by 4' thick.

Employee was shoveling and strained left arm, left shoulder, and neck. Employee returned back to their job as a belt man but started missing work on 7/17/2019 due to further complications.

Employee was riding a rubber tire mantrip and right knee was sticking out from the perimeter of the vehicle and struck the rib causing a dislocated femur and pelvis fracture.

Employee turned around to walk and slipped and fell. Employee has continued to work since DOI of 11-7-18. They started to miss work on 6/13/2019 due to having surgery to their wrist.

Employee was walking beside belt line and slipped and fell on the wet and slick bottom. As EE caught self, EE struck left elbow on the bottom. EE was diagnosed with a chipped bone on the elbow.

Employee had their hand on the drill pot of the roof bolter checking the suction and a rock fell and struck their hand. Employee fractured their right middle finger.

Employee was sitting down and starting to eat lunch when a rock fell from between the rib and bolts and struck EE's right foot. Employee went to an Orthopedic Specialist on 9/11/18 and was diagnosed with a fractured bone in EE's right foot. EE started missing work on 9/12/18.

Employee was tightening bolts on the stacker speed reducer. EE socket slipped off the bolt causing right elbow to strike against a guard bracket. EE received 3 sutures to right elbow and returned back to work during the same shift.

Employee changing roller on head drive. Taking bull gear off, hitting bull gear with a sledge hammer. Hammer handle hit employee's safety glasses knocking them off employee's face. The head of the hammer hit the edge of the bull gear and a piece of metal went into employee's left eyeball. Employee had surgery on eye the next day

Employee was found unconscious and unresponsive underground, employee was transported to the surface. Death Certificate was received 11/7/17 and it states the manner of death as Natural and cause of death as a Myocardial Infarction. Investigations revealed no impending circumstances at the mine contributing to employee's death.

Riding to section in a Mac 12 rubber tire mantrip. While making a turn toward section the Mac 12 hit rib catching SCSR storage box shoving it onto a fire extinguisher and the fire extinguisher hit employee's hand. The right hand palm was cut requiring 6 stitches

Employee was striking a roof bolter tram chain with a hammer when a small fragment of metal came off and imbedded into left arm. A Dr. removed the metal fragment and applied 3 sutures to repair the incision on 2/15/16.

A surface pond overflowed and the excess water flowed into the mine portal blocking the primary and secondary escapeway with 1 1/2' to 2' of water. The blocked area was from break # 1 to # 4 from the surface portal and no persons were in the mine at the time.

Trying to put 4"" waterline together and while pulling on waterline, he felt a pain in his lower back. He returned back to work his next shift but had to start missing on 10/3/14 due to his back not getting any better.
