Employee was loading belt rollers onto underground fork lift, when his finger got mashed between other rollers, causing a laceration that required stitches.
Employee was recovering track when he stepped into a water hole twisting his left knee.
Employee and rest of crew were in the locker room getting ready for the shift when employee was struck by a small spad shovel resulting in a laceration, requiring stitches.
A roof fall occurred in the #5 return entry of the I Submains that was approximately 10'L X 20'W X 7'H. The fall was discovered by the examiner and the MHSA inspector.
A roof fall occurred in the # 7 return entry of I submains that was approximately 20' X 20' X 5' in height. All entrances to fall have been breakered off. MSHA was notified.
Crew was attempting to move a tailpiece when the 1/2 inch chain broke striking employee in the left hand and ribs resulting in a fracture to the rib area.

An unplanned roof fall occurred in the #1 entry on J-3 panel 1 break outby survey spad #402340, appx. 8000' from the active face. No equipment or injuries were involved in the fall.
An unplanned roof fall occurred in the #4 entry of the K-2 panel, 1 bresk inby 20527, approximately 10,000 feet from the active face. No injuries or equipment were involved.
A roof fall occurred in the #8 entry in return off the #1 belt. No equipment or injuries were involved in the roof fall.
EE was making his weekly examination along the 3rd North panel and found a 6x6 timber in the roadway. While moving the timber out of the roadway, he felt a sharp pain in his lower back.
After finishing the final cut in the #6 entry and cleaning up, an unplanned break through of water came into the section from the adjacent panel and ran into the last open crosscut and remaining 3 entries. All eq. was pulled back and power to each was knocked.
An unplanned roof fall occurred at survey spad #17733 #3 entry outby 3rd North Mains. No injuries or equipment was involved in the fall.
An unplanned roof fall occurred approximately 3200' from the face in the #6 entry at spad #17766 approximately 50'x20'x6'. All entries were breakered off and the area will be rehabilitated.
Employee was walking from #5 to #4 entry and stated he either stepped into a hole / low spot or his leg ""just gave out"", which caused him to fall, hitting his back on a rock that was on the mine floor.
Member was standing behind miner operator in #2 entry when a piece of rock fell from between bolts and struck him on back of legs and twisting his ankle. He completed his shift and went to ER the following morning and was given a full release. He continued to work with some discomfort until his follow up vist and was taken off work due to continued swelling on 08/16/2010.
An unplanned roof fall occurred appx. 3800"" outby face in the #4 entry at spad #17465. The fall was approximately 15'x13'x5'.

An unplanned roof fall was discovered by the mine examiner. The fall location is 5 miles out by the faces on the West Mains. The fall measured 50' long X 18.5' wide X 2-5' high.
On Dec 14, 2009 due to an occupational illness, as substantiated by medical records and reports, employee became unable to work because of the ailments described in the medical reports. This was not due to one single event at the mine, but repeated trauma over an extended period of time. See page 3 of attachment under assessments.
A roof fall was discovered by mine examiner. Fall is located in the #5 entry adjacent to the 8-A belt, 3,500' from the active faces. Fall measured approx. 20'x20'x5'.
A roof fall occurred in the #3 entry 100' inby survey station #18094 between 12:15 AM and 05:00 AM. The area was checked for preshift and when it was checked again for the next preshift, the fall was discovered. The fall is approximately 30' x 10' x 5'. It will not be rehabilitated. The area is timbered and dangered off. The fall was investigated by MSHA.
Roofall occurred on the No. 3 belt, survey station #17298. There were no injuries associated with this roofall. The roof fall was approximately 40' x 20' x 7'. The mine roof strata is laminated with a coal rider seam present. The fall will be cleaned and rebolted. MSHA inspectors Rick Runyon and Randal Thornsbury along with OMSL inspector James Tackett investigated the fall.
A fall appx. 24'x20'x7'10"" occurred in the intake entry (33) on 001-0 MMU. This section is in the process of butting off and the fall will not be rehabiliated. This area was timbered and dangered off.
A roof fall approximately 20' x 19' x 6' occurred at 12:05 AM in the #3 entry (belt entry). The area had been bolted using 5' torque tension bolts. The strata is mixed. The area will be rehabilitated with approval from investigators.
Employee was setting timbers on 001-0 MMU, when a piece of draw rock fell out between the bolts measuring approximately 12"" x 12"" x 11/2"" thick and hit his left hand between middle and ring finger, cutting webbing between fingers.
Roof bolter operator was breaking the bands from a bundle of roofbolts, when one of the bands snapped back and cut his wrist on his right arm. The cut was relatively small and compresses and bandages were applied to the cut to stop the bleeding.
Roof fell approx 1700 ft from active face of #1 section in #3 entry. Fall was approx 30 ft in length, 20 ft in width and 7 ft in height. MSHA Inspector Billy Stiltner investigated the roof fall.
As motor was exiting dip, operator saw man-trip on track ahead. Motor operator braked, but still bumped man-trip (no passengers). Operator of man-trip declined first aid, said he was not injured. Next day man-trip operator went to Dr. due to soreness, E.R. Dr. took him off work.
Helping trace ground in miner cable, listning for thump. Rock fell from roof striking below hard hat causing injury. Taken to E.R., Ct scan negative, stitches used to close wound. Wokring on 001 section, in #3 heading.
Fall discovered 3/24/09 at 1:40 pm. #5 belt, #2 entry, spad 17688, in the return. Fall was 30' x 20' x 8010' high. There was no equipment involved, no ventilation affected & no personal injury, however, passage of people was affected. MSHA & OMSL were notified and MSHA inspected. Area was breakered off and will be rehabilitated.
6:00 AM discored fall main line entry #5 spad # 7321; 60' X 19' X 6' high. Neither vent nor passage of persons were affected. Both MSHA and KY IMSL were notified & investigated. Area was breakered off and will be rehabilated.
0230 discovered fall on #1 belt at break #32. Fall 60' X 20' X 7' high. Ventilation not affectd, passage of people not affected and area breakered off. Both MSHA and KY OMSL notitfied and both agencies investigated.
EE set CM over to right side of #4 right, head hit rib something popped off head and struck right eye, causing injury.

EE bent over to get box glue to put onto RB. Draw rock fell, striking mid back area. He was transported to ER, referred to neurologist. 1/13/09 EE saw neurologist, who took him off work until 3/2/09.
Employee putting up rope bolt, glance down at controls of RB, draw-rock fell, struck him on the left side of forehead, and left chin area. He was taken to the E.R. for treatment stitches were used to close the wound.
Employee throwing bolt from one side RB to the other side. Bolt hit plate, causing plate to come up and strike employee's chin, causing injury. Taken to ER for treatment, stitches used to close the wound.
Discovered fall entry #2, 11:15 A.M. outby spad #20143, J-11 panel in return 20' X 19' X 6'. No equip involved, no vent affected, no personal injury. Area will not be rehabed. Area has been breakered off.
Discovered a fall, entry #9, outby spad #17621 2:30 P.M. 50' X 20' X 7' high. No personal injury, no equipment, in return, no vent affected. Area was breakered off, will not be rehabed; MSHA & OMSL notified.
Employee was on 4-wheeler following the rail, #1 head. 4-wheeler slipped of track, steering wheel spun around striking left hand. He was taken to ER, x-ray positive. Hairline fracture of left thumb.
Roof fall discovered 11:45 A.M., #6 entry, outby #2 belt in the return. Fall was 40' X 18' X 5 > 6' high. No equip involved, passage of people affected. Entrance to fall breakered off, will be rehabilitated. MSHA & OMSL notified.
EE was walking from one roofbolter to the other roofbolter and stepped in a hole and twisted his ankle.

20 Year R.B. experience, lifts approx. 1,000 lb. steel per day. Doing regular job, nothing out of ordinary, at 10:20 P.M. had to quit work due to intense pain in right low back & hip area. Saw Dr. 7-30-08, taken off work & further test ordered. 8-01-08 diagnosis was acute right low pack pain w/radiculopathy, degenerative disc disease & right sciatica (SI) joint dysfunction.
ROOF FALL; Intake at spad #19536, 25' X 20' X 5 1/2 > 7' high. Area affected no ventialation or equipment, was breakered off, will not be rehabed. MSHA & OMSL were notified.
Roof fall measuring 28 ft X 20 ft X 6 ft occurred in #3 entry intake at spad #19535. Area was not breakered off, and will not be rehabilitated. We will crib and danger off the entries to the fall, clean, spot bolt, and re-route the intake. Both MSHA and state agencies were notified.

EE was picking up pizza pans when a rock fell out of the top hitting him on the side of his hard hat and left wrist. The rock was approximately 25""x16""x1.5""

Employee was on his knees with his back to the rib, wiring a tram motor on CM, the rib broke off and fell onto the back of the right foot. He was taken to the WMH E.R. for treatment, X-ray positive [chipped bone in right foot].
Between 2:00 P.M. & 10:00 P.M. a roof fall occurred at #7 belt, entry #4 spad # 18683. Fall was 30' X 19' X 5'6"" high. All entrances were breakered off, MSHA-D. Fannin & OMSL-W. Taylor were notified. Ventiltaion and passage of persons was not affected. Area will be rehabilitated, cleaned, additonal support installed.
Roof fall on #2 belt discovered 1:00 A.M. entry #4, spad #16700, neutral entry; 22' X 19' X 8' high. Entrances breakered off, will be rehabilitated; extra support will be added. Both MSHA & OMSL investigated.

EE had performed maintenance on CM, had broken the chain, and was walking around the miner. He stepped on a slick rock, his left leg slipped, causing injury to the left knee.

1045 fall occurred 001 section #6 entry spad #20693. Entries to fall have been dangered off; fall will not be rehabilitated. Fall was 70' x 6' x 7'; both MSHA and OMSL were notified.
On 001 section at intersection of #4 heading preparing to put up corner bolt. Draw-rock (20"" X 8"" X 0-1"") fell onto extended left arm, causing laceration. EE taken to ER for treatment.
EE tossed a chain across the scoop bucket, it sprang back at him, letting the hook strike him in the mouth chipping a bottom tooth.
Fall occurred in belt neutral entry, #8 belt, break #10; 001 section #4 entry, spad #18864. Measured 20' x 20' x 7'. Ventilation and travel were not affected. Both OMSL & MSHA were notified. Area was breakered off; will be rehabilitated.
Drilled hole for bolt, walking back to get rope bolt, piece of draw rock fell onto left arm. Cleaned and bandaged wound at scene, taken to ER, 3 stitches used to close wound and returned to work next shift.
10-28-07 a fall was discovered outby in track entry adjacent to spad #18483. Fall measured 35' x 19' x 6-8'. Area was breakered off, will be cleaned and additional roof support installed. There was a rider seam 6"" thick, present 8' above the roof. Ventilation was not affected.
EE on 001 section, #9 Belt, Break #40 setting head drive; adjusting chains on belt line, heard roof crack, rock fell striking him on left back area. Treated at scene, taken to ER, X-ray negative, released and schedule for follow-up 10/31/07.
10-23-07 a fall occurred outby in track entry 1 break outby spad #17609. Fall measured 40' x 19' x 8'. Area was breakered off, will be cleaned and additional roof support installed. Ventilation was not affected.
Between 12-15 & 10-22 a fall occurred water seal area 2 breaks from EP-W-2A. Fall measured 70' x 19' x 6'. Area was breakered off, will be cleaned and additional roof support installed. Ventilation was not affected.
Fall discovered which occurred between 9/17/07 and 9/19/07 on the 001 section in entry #7 spad #19508. Fall was 25 ft x 19 ft x 10 ft high. Both OMSL and MSHA were notified. Area breakered off, will be cleaned and rehabilitated. Ventilation was not obstructed.
Trimming corner from block, Continuous Miner head caught RB cable, cable fell onto and pulled operator against CM.
Sometime between 3:00 PM 9/1/07 and 9:50 PM 9/3/07, a fall occurred 1 break outby spad # 18097 in #4 entry. The fall measured 35' x 19' x 8' - 10' thick. The area was breakered off and will be cleaned with additional roof support installed. MSHA and OMSL were notified upon discovery.
Outby in #4 entry, #3 belt, break #2 a fall 40'L x 19'W x 4 to 5'H occurred at about 9:00 p.m. Both MSHA and KY OMSL were notified. All entrances to the fall were breakered off. Area to be cleaned, additional support and rehabilitated.
Fall 20 ft x 30 ft x 8 ft thick occurred at #4 belt entry on 001 section. Area was cleaned and additional support added.
Installing 8 ft. rope bolts, bolt bent, attempted to straighten by spinning chuck. Did not notice head out of chuck. When the bolt began to spin, it came out of the chuck. The plate and bolt head struck his left shoulder area. He completed the shift, went home and later sought treatment.
5:00 am fall measuring 30'L x 20'W x 10'H occurred in the neutral entry at spad #20167 (1 bk. outby). The area will be rehabilitated with the addition of additional roof support. Both MSHA and OMSL inspectors viewed the fall.
Shoveling mud out of take up at #4 head-drive, injured left shoulder. Return to work 2/21/2007. Worked until 8-14-07, had surgery on his left shoulder 8-15-2007.
Employee had completed setting spads, was walking toward transit, stepped in hole & twisted right knee.

12/11/06 discovered fall 5 breaks outby spad #3723, in return. area will be rehabilitated, 12 ft. rope bolts will be installed at 8 ft intervals 200 ft inby and 200 ft outby. both omsl and msha were notified.
EE was setting a timber and as he was reaching for a wedge, the timber fell striking his hammer which flew up and hit him in the mouth. This shattered his left bicuspid and split a small place in his lower lip.
On 12-01-2006 at approximately 7:45 PM on #1 section between no. 4 and 5 entries a roof fall occurred. The fall measured approximately 80' long, 19' wide and 8' high. Both MSHA and State officials were notified and the fall was investigated. All entrances to the fall have been timbered off.
Employee was setting timbers in the #5 entry approx. 70'inby spad station 19891, while carrying timber he twisted his right knee. He started missing work on 11/15/2006.
Roof fall (60' x 19' x 6' thick) occurred on #1 section in #4 entry, 1 break out-by spad #18898 in belt entry. MSHA notified by phone. No ventilation was affected.
Cutting 1st cut in 2L, standing at cross cut between 2 & 3. Rib rolled from top onto right foot (14"" X 17"" X 4'). Taken to ER, X-ray indicated bone on top of foot broke.
Rebuilding air-lock doors. Installing wedge, holding wedge with left hand, struck left thumb with hammer. Taken to ER for treatment. X-ray showed chipped end bone in thumb.
Sometime between 8/30/06 & 8/31/06 a fall occured in the return airway entry #2, section #3, 80' inby spad 17909. Fall was 60' x 19' x 8'. Both MSHA and OMSL were contacted and investigated the area. Area is to be rehabilitated, will install additional support as needed. Both MSHA and OMSL were notified immediately.
8:00 AM a fall occured on 001 section in #4 entry out-by at spad #18921 in the neutral. No equipment was involved, air was not affected, fall was 10' long, 20' wide, & 4' high. Area will be cleaned and additional support installed. MSHA and OMSL investigated the accident.
Discovered a fall at 2:30 pm on #8 belt #2 entry. Fall was 60' x 20' x 10' high. The roof is shale with no rider. No people were affected and no equipment was involved. Fall was in the intake. Area will be rehabilitated for future use.
#8 Belt break 16. a fall occurred 30' L x 20' W x 6' H, no injuries, no equipment involved, and no restriction to the ventilation. The area will be rehabilitated by additional support. The roof strata is shale with no rider seam. Incident was investigated by Massey, OMSL and MSHA.
At 2:00 pm a fall was discovered in the #1 entry, outby at spad #17319. The fall measured 25' x 18' x 4' thick. It is being cleaned up and additional support will be added as needed. Both MSHA and OMSL were notified immediately.
Employee is not assigned to Clean Energy. He part of Sidney Coal Re-hab crew. He was at this mine to deliver a feeder. While delivering the feeder, the lowboy derailed at a switch. EE injured his lower back putting wreck back on track. Next day worked at another site, 6/20/06 saw a dr, taken off work waiting for MRI. Please classify this as a degree 9 accident.
EE carrying bucket of roof bits from end of track to section. Shoulder popped, causing pain, he bolted two places, then came outside to see doctor. ER exam revealed strain trapezius muscle.
Employee was on operator side of bolter, installing a 5 ft. bolt in 6 ft. ht. A piece of drawrock fell onto his left forearm. He was treated at the scene, transported outside, driven to the hospital. Examination revealed a fracture to the left forearm.
Helping employee lower miner cable, hanger came loose, fell, struck injured's right eye. X-ray revealed blow-out fracture in orbit socket.
Employee was dragging curtain through #2 Hd intersection. Drawrock fell causing injury to left ankle. The fall caused laceration over right eye and nose. Stitches were used over the eye and x-ray of ankle was positive.
During inspection. Hoist at brushy was foung inoperative. Escape way was re routed & hoist repaired. Hoist was reinstated 2/14/06.

While putting suction hose onto roof bolter, a draw rock fell onto his right little finger. Stitches were used in treatment due to nail being removed.
A roof fall occurred in #2 intake entry spad #17327 which was 60'L x 20'W x 8'H; 5' torque tension bolts were in use in mixed shale and sand stone roof. The area was breakered off. MSHA & OMSL officials were notified. The area will be cleaned & additional support added.
10-19-2005 ON THE #1 SECTION IN THE #5 NEUTRAL ENTRY A ROOF FALL MEASURING 60' X 19' X 5' HIGH OCCURRED AT 10:45 PM. THE ROOF SUPPORT WAS 5' TORQUE TENSION BOLTS & 8' ROPE BOLTS. THE AREA WILL BE CLEANED AND ADDITIONAL SUPPORT ADDED.
(BELTS WERE OFF) EMPLOYEE WAS HELPING CHANGE ROLLERS. OTHER EMPLOYEE WAS FREEING MIDDLE ROLLLER WITH SLATE BAR. HE STUMBLED, FELL TOWARD EMPLOYEE AND STRUCK HIM IN THE MOUTH WITH THE BAR, CAUSING DAMAGE TO HIS TEETH.
EMPLOYEE WAS GETTING HIGH LINE READY FOR SPLICE. HE HAD CABLE IN BETWEEN LEGS WHILE CUTTING OUTER JACKET. KNIFE SLIPPED FROM CABLE CUTTING HIS RT. LEG. HE WAS TAKEN TO THE HOSPITAL & RECEIVED STITCHES TO CLOSE THE WOUND.
1 BREAK OUT-BY SPAD #5460 IN #2 ENTRY, A FALL 18' X 20' X 8' OCCURRED. VENTILATION WAS NOT AFFECTED, THERE WAS A 3"" RIDER SEAM ABOUVE THE COAL SEAM
7-22-05 6:45 AM A ROOF FALL (40'L X19'W X 5'H) OCCURRED IN THE NEUTRAL TRACK ENTRY 200' INBY THE ELEVATOR. THE ROOF STRATA IS LAMINATED MIXED SHALE WITH NO RIDER
EE WAS CARRYING A BOX OF GLUE FROM THE SCOOP TO THE BOLTER. HE SLIPPED ON A ROCK AND INJURED HIS BACK. EE SAW ER DOCTOR 4/9/05. RTW 4/11/05, WORKED 4/11 & 4/12. RETURNED TO DOCTOR ON 4/12/05 & WAS TAKEN OFF WORK. EE HAD MRI 4/14/05 AND IS WAITING FOR RESULTS AND RE-EVALUATION.
AT 1715, A FALL OCCURED IN #6 ENTRY 1 BREAK OUTBY SPAD #17610 IN THE NEUTRAL. FALL WAS 75' X 19' X 5'. ENTRANCES TO FALL HAVE BEEN BREAKERED OFF. MSHA AND KOSL NOTIFIED.
At 7:15 pm a fall occurred in #6 entry out by spad #17610, in the neutral. Fall was 75 ft l by 19 ft w by 5 ft thick. Entrances to fall have been breakered off. MSHA and KOSL notified. MSHA investigated.
EE WAS CARRYING A BELT ROLLER, TRIPPED OVER LOOSE ROCK, FELL AGAINST THE MAN-BUS AND INJURED THE SIDE OF FACE. EE WAS TAKEN TO THE ER FOR TREATMENT REQUIRING STITCHES. THE ROLLER WEIGHED APPROX 10 LBS, EE WAS 4 BREAKS INBY #2 HEAD
EE ARRIVED ON SECTION, SET BUCKET OF BITS AND JUG OF WATER ON THE BOLTER. A PIECE OF DRAW ROCK FELL ONTO LEFT HAND AND RIGHT FOREARM
Employee was helping load 48"" rigid top structure onto a gator for a move. While lifting the 18th piece onto the machine he strained his lower back muscles.
EE WAS SUPERVISING INSTALLATION OF A 125 H.P. PUMP AT THE FAN SHAFT. AS THE SCOOP WAS PULLING THE PUMP, EE SLIPPED IN THE MUD. TO KEEP FROM FALLING INTO THE FREEZING WATER EE GRABBED THE ROPE WICH PULLED THE PUMP, AND HIS RIGHT LITTLE AND RING FINGERS WERE PULLED INTO THE PULLEY. (AMPUTATION OF LITTLE FINGER: DISTAL PHALANGE.)
EE WAS MOVING SHOT DOWN MATERIAL WHERE CONSTRUCTION OF OVER-CAST WAS TAKING PLACE, AREA HAD BEEN BOLTED. EE BACKED SCOOP INTO THE LOW AREA, BUMPED A LOOSE ROCK WITH THE LIGHT GUARD CAUSING ROCK TO FALL ON SCOOP, A PORTION OF THE ROCK STRUCK LEFT LEG.
AT 2:00 PM A FALL WAS DISCOVERED ON THE #6 TRACK ENTRY OF THE 001 SECTION, 80' OUTBY SPAD #17314. THE FALL WAS 80' X 18' X 5' HIGH. THE ROOF IS MIXED AND SUPPORTED WITH 5' TORQUE TENSION BOLTS.

Employee was operating a shuttle car on the 003 pillar section. As he returned to the miner he ran over a piece of rock causing him to bounce up and hit his head on the inside of the canopy injuring his neck.
Employee was assisting with a 1 break pull back on 42"" belt. He picked up a top roller, walked about 10 feet, dropped the roller, went to his knees, and complained with a pain in his low back.
NON-EVENT: At the beginning of the shift at 7:00 A.M. the employee told his foreman that his back was hurting too bad to allow him to work. He was sent to the E.R. and is being treated for back strain. There was no event to cause the pain.
SOMETIME BETWEEN THE END OF THE 12/05/2004 EVENING SHIFT AT 2400, AND THE NEXT SCHEDULED PRE-SHIFT EXAM A FALL OCCURED ON THE 003 SECTION IN THE #5 ENTRY AT BREAK #9 2B BELT. THE FALL MEASURED 30' X 15' X 5'
EE was in #6 entry, 3 rows in 6R cross cut putting up torque tension bolts. A bolt head broke off in the pot. while removing the bolt head a piece of draw rock fell onto his back. x ray were negative.

EE was injured on 08/04/2004, and worked without missing until 11/23/2004 when he had orthoscopic surgery on his right knee. On 8/4/04 he was on 001 sect., in #4 hd. putting up bolts. EE dragged a bolt from the machine, and turned. His r. foot was between two rocks and twisted his right knee. He continued to work until 11/23/2004.

HE WAS TRANSPORTED TO HOSPI TAL WHERE X-RAYS SHOWED NO ABNORMALITIES. HIS FAMILY DOCTOR ADVISED HIM TO REMAIN OFF WORK UNTIL AN MRI WAS PERFORMED.
EE WAS USING A UTILITY KNIFE TO CUT A PIECE OF CONDUIT. EE HAD THE CONDUIT LAYING ACROSS HIS LEGS. THE KNIFE SLIPPED OFF THE CONDUIT AS HE WAS CUTTING AND WENT INTO HIS RIGHT THIGH. HE SUSTAIN ED A LACERATION REQUIRING STITCHES. EE WAS SUSPENDED FOR 3 DAYS AT WHICH TIME HE WILL ATTEND A 24 HOUR ACCIDENT PREVENTION CLASS.
EE WAS INSTALLING A ROOF BOLT WHEN THE WRENCH CAME OFF STRIKING HIM ON THE LEFT HAND. EE SUSTAINED A FRACTURE TO THE LITTLE FINGER.

EMPLOYEE WAS REMOVING OVERLY TIGHT LUG NUTS FROMA ROOF BOLTER TIRE USING A 3/4 RATCHET, A 4' CHEATER BAR & A 4' SLATE BAR. AFTER SEVERAL ATTEMPTTO LOOSEN THE BOLT, IT SLIPPED SUDDENLY AND MCNE ELY FELL TO THE MINE FLOOR LANDING ON HIS RIGHT WRIST & TAILBONE. MCNEELY WAS ATTENDED BY AN ORTHOPEDIC SURGEON AND ADVISED TO STAY OFF WORK.

EMPLOYEE WAS PULLING ON MRS CABLE THROUGH WATER AND MUD WHEN HE SLIPPED AND FELL. EE ATTEMPTED TO CATCH HIMSELF AND STRETCHED HIS ARM OUT LANDING ON HIS RIGHT WRIST. THIS RESULTED IN FRACTUR E TO THE RIGHT WRIST.
