Contracted tugboat employee threw hose on upper level of tug above Carmeuse employee and it missed the landing, striking Carmeuse employees left hand resulting in a fractured finger.

Employee was raking #1 kiln grates and felt a sharp pain on the right side of back. Restricted duty.
Employee reported a fire on the outside of cone on #3 coal mill. After the fire was noticed there was an explosion on the coal grinding circuit. At 0300 it was noticed that there was cable trays arcing above the coal mill. The power was then shut off to the circuits. Investigation continues of the root cause.

While working on thermo couple on #1 cooler, lime dust entered employee's eyes. Eyes were flushed having 1 piece of dust having to be removed by physician.
At approximately 10:50am an explosion occurred in the #1 Kiln coal system.

Employee was emptying pail of pulverized lime in the lab when the side of the pail broke causing it to fall on the floor sending lime dust into the air and in the employees eyes.
Employee got a piece of lime in his eye while replacing plow in #3 cooler. Was not wearing proper PPE (goggles)
While raking coating from kiln employee felt a pop in his right knee resulting in restricted duty work. Employee has had prior non work related issues with same knee.
Employee slipped on ice while exiting work truck causing fracture to leg.
While relocating piping to another jobsite an employee was walking along side a all terrain forklift when his foot got under the tire. Employee was taken to emergency room and x ray showed fracture in big toe and a laceration between the toes.

Employee had moved vacuum hose out of the walk way. As he turned to walk away he tripped on his own foot and fell forward. He extended his arms to catch himself and when his left hand hit the floor he incurred a fracture to his wrist.
Employee cut left index finger with pocket knife while cutting light strap requiring stitches.
Employee was using a utility knife to cut a bad end off a 4"" plastic corrugated pipe. The knife blade slipped out of the pipe striking the right forearm causing a laceration that required stitches.
EMPLOYEE WAS ALIGNING #3 KILN DRIVE MOTOR USING PRY BAR TO LIFT MOTOR. THE PRY BAR SLIPPED OUT STRIKING EMPLOYEES FINGER RESULTING IN LACERATION REQUIRING STITCHES.
Lime dust entered employee's left eye while removing safety glasses. Employee flushed eye on day of incident removing the irritant. Reported to work on 3/20/14 stating eye still felt different and requested to see doctor. Prescription eye drops were prescribed.

Employee was pressure washing I-beams in an 80 ft boom lift. His hand became wedged between lift basket and steel structure.
Employee picked up two buckets of material (approximately 20 lbs.) when he felt a pop in his right bicep. Employee did not seek medical attention until 12/13/2012. He was put on restrictions at this time.
Struck head on medal cage while taking out the trash.
Employee cut his left forearm while attempting to cut a hose.
Employee was raking coating from Kiln #4 and felt a twist in his left knee while stepping backwards.
Employee was lifting bucket when he felt pain in the middle of his back.
Employee was un-spooling cable with a utility knife in his right hand. His hand slipped and he punctured his left middle finger.

Employee was stepping over a spray bar at the M&M wash plant when he felt pain in his right knee.
Employee was walking down LS-4 catwalk, when the catwalk broke through causing the employee to fall approximately 20 ft. to the ground below.
Employee was attempting to clear a chute plugged with lime. Using a rod to clear the plug he struck a pocket of hot lime which blew back into the employees face entering his eyes behind the safety glasses he was wearing resulting in the use of antibiotic medication.
While attempting to retrieve a tie-off line on a barge, a seperate haul line came free and smashed his left thumb between the line and a permanent structure. This incident resulted in a fractured right thumb along with several stitches.

Injured was attempting to remove mud build-up from a return idler by striking it with a bar. While striking the idler, injured's left index finger was pinched between the bar and the idler's frame causing a laceration.

Employee was working on load cell for LS-5 belt scales. While operating manlift employee mashed his righthand ring finger by catching finger between basket and belt frame.
Employee & another person were lifting a 4x8 piece of wood when employee's left leg slipped on gravel creating a slip & causing pain in lower back. He continued to work until job was completed. On 3-31-04 his back still bothered him so he went to see doctor. The diagnosis was strained back. Doctor took employee off work to rest from 4-1 to 4-13-04.

EMPLOYEE WAS RAKING CLINKER GRATE ON #3 KILN ON THE UNDERSIDE WHEN A CLINKER FELL STRIKING THE RAKE POLE CAUSING THE POLE TO MASH HIS RIGHT HAND BETWEEN THE POLE AND HANDRAIL.

EE WAS ATTEMPTING TO ADJUST A PROXIMITY SWITCH (ZERO SPEED SWITCH) WHEN HE INJURED HIS LEFT INDEX FINGER WHEN IT CAME INTO CONTACT WITH PIECE OF METAL, WHICH REQUIRED STITCHES.
LANDING A LOAD OF METAL ROOFING ONTO PREHEATER DECK, WHILE SWINGING LOAD IN, GOT HAND CAUGHT BETWEEN LOAD AND HANDRAIL. STITCHES.
CONTRACTOR EE WAS PULLING CABLE WHEN DECIDED TO GO TO BREAK, STEPPED ON GRATING FALLING THROUGH. GRATING WAS LOCATED ON PLATFORM WHICH WAS BARRICADED OFF AT BOTTOM OF LADDER TO RESTRICT ENTRY. EE CLIMBED OVER RAIL TO PLATFORM.

HE WAS USING A JACK HAMMER TO TEAR OUT BRICK. THE BRICK WAS STUCK TO THE SHELL AND TO EACH OTHERBUT CAME LOOSE AND SLID DOWN THE SHELL AND SEVERAL FELL FROM THE TOP HITTING HIM.

EE WAS REMOVING FAN FROM BAGHOUSE WHEN HIS RIGHT MIDDLE FINGER BECAME CAUGHT BETWEEN FAN AND THE DOOR. HE RECEIVED A SMASHED END OF THE FINGER. THE INJURY REQUIRED STITCHES, MAKING THIS REPORT ABLE.
