EE's rail motor EE was operating had de-railed. EE used a jack and a bar to put the ride back on the track. Upon doing so, the jack turned over causing the bar to strike EE on the right cheek area of EE's face. The ride went back on the track at that time and EE gathered up all EE's tools and went to the outside to be checked out for precaution.

A non-injury roof fall occurred in the primary escapeway on 4 belt, entry 1, just inby crosscut 10. The fall was approximately 25'x15'x5-6'.
EE was at the bottom of the slope attaching the supply car to the 8-ton rail motor. EE reached outside the car to swing the coupling around to align with the motor. EE felt a pop in EE's back. EE had initially filed a file only report and wasn't take off work until 07/11/2025.

A non injury roof fall occurred in the primary escapeway on the 2nd Primary Mains at break 27. Using measurements from the map, the cross sectional area is 1' larger than the approved plan.
A non injury roof fall occurred in crosscut 17, #2 entry of the 2nd Primary Mains. Deterioration of the immediate roof in this isolated area is the probable cause for the fall.
This is an area of transition between shale and sandstone. No injuries or damage to any equipment occurred.

david was working on a broken conveyor chain in a shuttle car. EE was using a handheld grinder to remove metal clips that keeps the links together. the grinder kicked back striking EE's thumb on EE's left hand.
EE stated that they were pulling a car cable and felt a shock causing them to trip, hitting their head on the shuttle car resulting in 7 sutures. They received a full release to return to work.
Employee states EE was bolting top and a metal roof strap struck EE's right forearm and EE had a laceration that required 6 stitches.
Employee states that EE was operating a shuttle car. EE dumped at the feeder and as EE was returning to the miner, EE hit a ""bump"" in the roadway causing EE to strike EE's head against the canopy of the car, resulting in pain in EE's neck and back area.
Employee was picking up a CO/belt cable and caught their hand against a rock and cut their right 4th digit finger. Sutures were required.

Employee was lifting a piece of rock that fell onto another employee and fell down and injured their left knee. Employee started missing work on 4/19/2021.
An unintentional roof fall was found on the #8 B Belt Line at crosscut #12 in the #4 entry.
The injured employee was standing adjacent to another employee who was pulling a piece of draw rock. As the rock fell the edge caught and struck the injured employees right forearm. Sutures were required to close the wound.

Found an unintentional roof fall on the track haulage secondary escapeway at Brk. No. 30, No. 7 Entry, on Energy Mains. The rock fell off from around the bolts and blocked the escapeway. The rock was cleaned up and the area was re-supported.

Employee was walking up an entry, leg contacted an energized cable, and when employee stepped back away stepped in a rut and fell. Employee hurt right knee.
Employee was reaching to hang the miner cable and a rock rolled from the rib and struck employee's hand. Employee did not go to the Dr. until later on 9/21/18 and it was determined that the employee had a fractured left 5th carpal bone.
Employee was bent over re-positioning the miner cable and a piece of rock fell from between the bolts and knocked employee face first into the continuous miner. Initial indications were that it was only a couple of cuts that needed sutures and the hospital later determined the injury serious and transferred employee to another hospital. Employee started missing work on 5/29/2018.

The roof bolter operator dislodged a piece of rock while bolting and it struck thumb. Sutures were required.
The continuous miner was caught by a roof fall in the #4 entry right pillar. After the MSHA 800 number was contacted the existing fall fell more and covered the continuous miner up. The roof fall also extended into the #5 heading. Both falls were in the same area and was a continuation of the first reported fall.
Employee was installing a resin roof bolt, as EE was aligning the drill chuck with the bolt, EE engaged the rotation and caught EE's glove between the bolt and drill chuck. EE was diagnosed with a small fracture to the Ulna bone near EE's right wrist. EE started missing work on 2/9/2016.

Employee was using a hammer to cut a rock dust hose and hit hit left hand with hammer. # sutures were required.
Employee strained his back while carrying a 40lb bag of rock dust. He started missing work on 8/31/2015 due to being referred for further medical evaluation and taken off work by the physician.
Employee was moving the miner cable and a rock flipped around and struck him on the right leg and ankle. Diagnosed with a fracture. Started missing work 6/8/2015.
Elevator shut down (Primary Escapeway) and was blowing fuses and was down for most of the day. It was repaired and restored back to service on the same day.

Employee was unloading timbers from a scoop bucket and 2 of them were frozen together and he strained his groin while trying to pull them out of the bucket. He has been diagnosed with an inguinal hernia which will require surgery. He started missing work on Feb. 12, 2014.

INJURED EE WAS ATTEMPTING TO COUPLE TOGETHER A TRACK MOUNTED ROCK DUSTING MACHINE TO A PERSONNEL CARRIER. HE HAS HIS HAND AROUND COUPLING PIN WHEN THE PERSONNEL CARRIER WAS ENGAGED HIS FINGER WAS CAUGHT BEHIND PIN RESULTING IN MESHED AND LACERATED FINGER.
TIED PUMP UP IN AIR SO THAT EMPLOYEES COULD SHOVEL COAL UP SO THAT PUMP WOULDN'T STOP UP, ROPE BROKE HOLDING PUMP STRIKING EMPLOYEE IN HEAD AND NECK.
