EE's rail motor EE was operating had de-railed. EE used a jack and a bar to put the ride back on the track. Upon doing so, the jack turned over causing the bar to strike EE on the right cheek area of EE's face. The ride went back on the track at that time and EE gathered up all EE's tools and went to the outside to be checked out for precaution.

A non-injury roof fall occurred in the primary escapeway on 4 belt, entry 1, just inby crosscut 10. The fall was approximately 25'x15'x5-6'.

EE was at the bottom of the slope attaching the supply car to the 8-ton rail motor. EE reached outside the car to swing the coupling around to align with the motor. EE felt a pop in EE's back. EE had initially filed a file only report and wasn't take off work until 07/11/2025.

a non injury roof fall had at cross cut 24 in entry 3 on the third primary mains.

2 non injury roof falls were discovered at cross cut 20 in #4 and #6 entries on the 3rd Primary Mains. The area of the falls had isolated laminated sandstone layers.

A non injury roof fall occurred in the primary escapeway on the 2nd Primary Mains at break 27. Using measurements from the map, the cross sectional area is 1' larger than the approved plan.

A non injury roof fall occurred in crosscut 17, #2 entry of the 2nd Primary Mains. Deterioration of the immediate roof in this isolated area is the probable cause for the fall.

This is an area of transition between shale and sandstone. No injuries or damage to any equipment occurred.

A non fatal unplanned roof fall occurred on 6/14/2024 on 014 mmu at #4 entry break #20. The fall measured approximately 12' wide x 10' long x 2-3' thick

EE was operating a bolt machine when the canopy unexpectedly dropped 2-3 inches and hit their head.

a roof fall occurred (24"" thick x 20' in length x 12' in width) during retreat mining causing the continuous miner to become lodged.

david was working on a broken conveyor chain in a shuttle car. EE was using a handheld grinder to remove metal clips that keeps the links together. the grinder kicked back striking EE's thumb on EE's left hand.

EE stated that they were pulling a car cable and felt a shock causing them to trip, hitting their head on the shuttle car resulting in 7 sutures. They received a full release to return to work.

Employee states EE was bolting top and a metal roof strap struck EE's right forearm and EE had a laceration that required 6 stitches.

Employee states that EE was operating a shuttle car. EE dumped at the feeder and as EE was returning to the miner, EE hit a ""bump"" in the roadway causing EE to strike EE's head against the canopy of the car, resulting in pain in EE's neck and back area.

Employee was picking up a CO/belt cable and caught their hand against a rock and cut their right 4th digit finger. Sutures were required.

Found an unintentional roof fall at break #10, #1 Left bleeder entry on 2nd Primary Mains.

Employee was installing a T-3 bent rib channel and caught EE's left index finger between the strap and the roof, sutures were required to repair the wound.

A small piece of rock 5"" x 3"" x 1"" thick hit employee on top of EE's right wrist and cut EE's wrist. Sutures were required.

Employee was lifting a piece of rock that fell onto another employee and fell down and injured their left knee. Employee started missing work on 4/19/2021.

An unintentional roof fall was found on the #8 B Belt Line at crosscut #12 in the #4 entry.

The injured employee was standing adjacent to another employee who was pulling a piece of draw rock. As the rock fell the edge caught and struck the injured employees right forearm. Sutures were required to close the wound.

Found an unintentional roof fall in the intersection of No. 7 Entry, Break No. 33, on the 2nd Primary Mains.

Found an unintentional roof fall on the track haulage secondary escapeway at Brk. No. 30, No. 7 Entry, on Energy Mains. The rock fell off from around the bolts and blocked the escapeway. The rock was cleaned up and the area was re-supported.

Found an unintentional roof fall at xcut #53, 6 Heading, Intake side of the 012 MMU.

Found an unintentional roof fall in the #4 entry between breaks numbers 5 and 6 on the #4 Belt Line. Fall is approximately 20' wide by 60' long by 4'-6'thick.

Employee was walking up an entry, leg contacted an energized cable, and when employee stepped back away stepped in a rut and fell. Employee hurt right knee.

Employee was reaching to hang the miner cable and a rock rolled from the rib and struck employee's hand. Employee did not go to the Dr. until later on 9/21/18 and it was determined that the employee had a fractured left 5th carpal bone.

Employee was bent over re-positioning the miner cable and a piece of rock fell from between the bolts and knocked employee face first into the continuous miner. Initial indications were that it was only a couple of cuts that needed sutures and the hospital later determined the injury serious and transferred employee to another hospital. Employee started missing work on 5/29/2018.

Employee was operating walk thru bolter and twisted left knee while bolting in the #5 heading. Employee started missing work on 3/29/2017.

The roof bolter operator dislodged a piece of rock while bolting and it struck thumb. Sutures were required.

The continuous miner was caught by a roof fall in the #4 entry right pillar. After the MSHA 800 number was contacted the existing fall fell more and covered the continuous miner up. The roof fall also extended into the #5 heading. Both falls were in the same area and was a continuation of the first reported fall.

Employee was installing a resin roof bolt, as EE was aligning the drill chuck with the bolt, EE engaged the rotation and caught EE's glove between the bolt and drill chuck. EE was diagnosed with a small fracture to the Ulna bone near EE's right wrist. EE started missing work on 2/9/2016.

Motor malfunctioned on the elevator, motor was replaced.

Employee was using a hammer to cut a rock dust hose and hit hit left hand with hammer. # sutures were required.

Employee strained his back while carrying a 40lb bag of rock dust. He started missing work on 8/31/2015 due to being referred for further medical evaluation and taken off work by the physician.

Employee was moving the miner cable and a rock flipped around and struck him on the right leg and ankle. Diagnosed with a fracture. Started missing work 6/8/2015.

Elevator shut down (Primary Escapeway) and was blowing fuses and was down for most of the day. It was repaired and restored back to service on the same day.

Employee was dragging an oxygen tank down the slope and he tripped and fell onto his right side/shoulder. He has been diagnosed with a fractured rib.

Employee was unloading timbers from a scoop bucket and 2 of them were frozen together and he strained his groin while trying to pull them out of the bucket. He has been diagnosed with an inguinal hernia which will require surgery. He started missing work on Feb. 12, 2014.

He was cleaning rock off of the continuous miner and a rock fell and struck him on the face and knocked him down.

Found an unintentional roof fall in the #6 entry, brk #19, of the 4th left panel off of Left Fork Branch submains. This fall is located in an area where no persons are scheduled to work or travel. It is not in a designated travelway. It is located in a bleeder entry of the 4th left panel.

EE was bolting top and a rock hit his right middle finger and cut his finger. Sutures were required no lost time.

Found an unintentional roof fall in #2 Entry at #11 Drive - Fall was cleaned up - Resupported - and supplemental roof support installed.

Found an unintentional fall at 1st Left off of Left Horn Branch - Break #6 - #9 Heading - Fall was cribbed and dangered off & supplemental support was added to the affected surrounding area.

EE was attempting to unstick a rope on an outby airlock door and cut his finger on the rope.

A piece of rock struck him on the top of his shoulder and knocked him down. Employee tried to continue to work but started missing work on 2-25-2011 due to back pain.

He was drilling a hole in the mine floor with hand drill and caught his finger on the drill steel keeper pin. His right ring finger was cut. 9 sutures required.

EE cut his thumb while cutting a cable. sutures required.

EE was riding to work area on the mantrip, mantrip was going thru an airlock and the door hit the SCSR storage box which then struck ee's right hand. *Restricted duty from 9/2/08 to 9/12/08. Started missing work 9/15/08.

A piece of rock fell from the rib and hit ee on the left ankle. He has a fractured ankle and is working at a restricted duty status.

Employee was bolting top when drill steel hung in the top, fell and hit left middle finger.

Unintentional roof fall on 011 MMU, 5 Right to 6 Right, 60' long x 10' thick x 20' wide.

Unintentional roof fall on 012 MMU, #8 Entry, brk #10. 25' long x 15' thick x 20' wide.

Employee dropped a drill pot onto left hand and cut left 5th fingertip. Sutures required.

Caught finger on boom of bolter. Braked and cut finger.

Employee was drilling hole when top started working loose. He tried to get out of the way by grabbing tray on pinner to pull himself out when a rock fell on hand.

Making a belt splice & Cut his rt hand with knife. Required sutures. No lost time.

Inundation of gas after miner cut into an active gas well that was not supposed to be there. All State & Federal agencies were notified immediately.

Employee was struck by a rib roll and suffered a fractured ankle. Started missing work 11-27-06.

Employee was cutting belt with a utility knife & cut his right leg. Had 6 sutures, no lost time.

Walking down belts & tripped over roller, hit forehead on structure.

Had an unintentional fall at #6, setting seals in front of the #3 seal

Employee was removing rock from the continuous miner and cut his right wrist and hand. Required sutures - no lost time.

Employee was using acetylene torches and a piece of hot metal got into his right eye.

Employee was using a slate bar and the bar slipped and hit him on the left side of his chin. Required stitches.

Scattering bolts and rock fell, hitting employee in the right shoulder. *Employee has not called back or contacted anyone on this injury. They have terminated him due to lack of cooperation & poor work.

EE was checking cylinder when he slipped on oil on the floor.

Cutting with a utility knife & knife slipped and cut leg. Sutures required.

Employee was hanging miner cable and strained lower back.

Had unintentional roof fall in the #3 entry of the 011 / 012 MMU supersection. Fall was 60' L x 8' high x 20' wide.

Had an unintentional roof fall on #9 bolt from #11 to #12 breaks. Fall is 60' long x 20' wide x 5' high.

Had an unintentional roof fall on section in #5 entry/break #15. Fall was 20' x 20' x 5' thick.

HAD AN UNINTENTIONAL ROOF FALL IN THE #3 ENTRY 5 BREAKS OUTBY THE FACE OF 011 MMU.

HAD AN UNINTENTIONAL ROOF FALL ON #8 BELT BETWEEN BREAKS #2 AND #3. FALL WAS 85' LONG X 20' WIDE X 8' HIGH.

Bolting top of mine caught fingers on hydraulic hose fitting, cutting fingers.

EMPLOYEE CAUGHT HIS LEFT INDEX FINGER ON A METAL BINDING STRAP. HE RECEIVED 2 SUTURES.

Employee was moving cable and tripped, falling on the right side of his face.

He was going to work on conveyor chain on the miner and rib rolled out on his right foot.

#2 HEADING ON THE #012 MMU. HAD AN UNINTENTIONAL FALL FROM BREAK #21 TO #22. 10'HIGH X 18'WIDE X 60'LONG. FALL WAS TIMBERED OFF & SECTION WAS RE-LOCATED.

Had an unintentional roof fall on the 012 MMU in #2 entry 2 breaks outby the face.

A piece of rock cut a small place on left forearm that required some sutures.

EMPLOYEE CUT HIS HAND AS HE WAS PULLING ROCK OFF OF THE CONTINUOUS MINER. RECEIVED 8 SUTURES, NO LOST TIME

EMPLOYEE WAS BOLTING ROOF AND A PIECE OF ROCK FELL STRIKING HIM ON TOP OF RIGHT FOOT.

Drilling in roof of mine, a piece of rock broke at drill bit & fell, canopy flipped over & struck him on the neck.

EE WAS PICKING UP BOLT SUPPLIES AND CUT HIS HAND ON A STRAP. CUT REQUIRED STITCHES TO CLOSE WOUND.

A CONTROL SWITCH FAILED & ELEVATOR WAS TAKEN OUT OF SERVICE. A REPLACEMENT SWITCH WAS ORDERED & THE ELEVATOR WAS REPAIRED WITHIN 30 MINUTES AFTER RECEIVING THE SWITCH.

A GUIDE WHEEL ON THE ELEVATOR WAS FOUND TO BE DAMAGED AND ELEVATOR WAS DOWN FOR MOST OF THE SHIFT.

Employee informed operator of a cut that received stitches on 5-28-04. Injury occurred 4-22-04 and was not reported to w/c until 5-28-04.

Employee injured his left knee on 2-16-04 when he bumped it against a scoop. He began missing work on 5-27-04.

The elevator was found to have one damaged guide wheel on the bottom right guide assembly. The other 5 wheels were O.K.

Employee was riding in a scoop bucket through a low place when he was brushed against the top.

An unintentional roof fall occurred in section return and it was found on 5-25-04.

A FIRE OCCURRED ON THE #6 BELT FLIGHT THAT WAS FOUND AT APPROX. 4:OO A.M. ALL PERSONS WERE EVACUATED FROM THE MINE AND FIRE FIGHTERS WERE PULLEDIN BY STATE & FEDERAL AGENCIES.

THE ELEVATOR HAD AN ELECTRICAL PANEL MALFUNCTION THAT CAUSED IT TO BE INOPERATIVE.

EE TRIPPED AND FELL THRU AN OFFICE DOOR. HE DIS-LOCATED A BONE IN HIS FOREARM AND A PLASTER CASTWAS APPLIED.

EMPLOYEE JUMPED FROM A MOVING BATTERY MOTOR AND INJURED HIS LOWER BACK. HE EXPERIENCED EXTREME PAIN AND WAS HOSPITALIZED FOR OBSERVATION.

EMPLOYEE WAS STANDING ON INBY SIDE OF TAKE-UP WHEN HE TRIPPED AND FELL, LANDING ON HIS BACK AND W-65 SELF-RESCURER.

UPON INSPECTION A GUIDE HAD BROKEN ON ELEVATOR DOOR AND THE ELEVATOR WOULD NOT OPEN PROPERLY.

EMPLOYEE WAS MAKING A BELT SPLICE WHEN THE UTLITY KNIFE HE WAS USING SLIPPED CAUSING HIM TO CUT HIS THIGH. EMPLOYEE SUSTAINED A LACERATION THAT NEEDED 5 STITCHES.

UNINTENTIONAL ROOF FALL CAUGHT HEAD OF MINER & WAS STUCK FROM 9:00 PM TIL 3:00 AM NEXT SHIFT. M.M.V. 009-0 WAS ON RETREAT MINING AT THE TIME.

UPON INSPECTION BY WILLIE COLLINS IT WAS DISCOVERED THAT DAMAGE TO ELEVATOR ROPES WAS PRESENT. ELEVATOR WAS SHUT DOWN & ROPES HAVE BEEN CHANGED OUT.

WHILE ROOF BOLTING WRENCH FELL OFF AND STRUCK EE ON LEFT HAND LITTLE FINGER.

EE WAS PRYING ON A SLATE BAR AND FELT PAIN IN LOWER BACK & NECK. EE ON MARCH 10TH HURT HIS BACK AT HOM & BEGAN MISSING WORK. DR. REPORT CANNOT ISOLATE THE INJURIES. REC'D DR.'S REPORT ON 3-19- 02.

EE WAS RERAILING A LOCOMOTIVE WITH A JACK WHEN THE BAR SLIPPED AND HE CUT HIS RIGHT HAND.

EE LIFTING ROOF BOLTS AND FELT PAIN IN LOWER BACK.

A ROOF FALL MEASURING ~18' X 120' BY 5' THICK OCCURRED IN THE #4 ENTRY ON THE 009-0 PANEL. THE FALL IS LOCATED JUST INBY THE 10TH CROSSCUT AND RUNS UP THE ENTRY FOR 120'.

THIS MINE HAS EXPERIENCED A ROOF FALL ON THE 009-0 PANEL. THE AFFECTED AREAS INCLUDE A FULL MESURING APPROX 20' BY 100' BY 5' THICK IN THE #3 ENTRY BEGINNING JUST INBY THE 10TH CROSSCUT ALSO A FALL MEASURING 20' BY 90' X 5' THICK BEGINNING JUST INBY THE 17TH CROSSCUT #3 ENTRY.

A ROOF FALL MEASURING ~90' BY 11' BY 4' OCCURREDON THE 009-0 PANEL IN THE #5 ENTRY. THE AFFECTEDAREA IS LOCATED JUST INBY THE #14TH CROSSCUT.

A ROOF FALL MEASURING ~20' IN LENGTH, 18' WIDE &8' TO 10' THICK WAS FOUND LOCATED IN THE #1 ENTRY 10TH CROSSCUT ON THE NO. 3 BELTLINE MAINS.

THE VERTICAL SHAFT ELEVATOR WAS TAKEN OUT OF SERVICE DUE TO A CONTROLLER PROBLEM CONCERNING THE OVER SPEED, OVERWIND PROTECTION.

LOST CONTROL POWERON THE ELECTRIC BRAKING SYSTEM THRU A TIMER RELAY.

EE WAS PULLING ON BELT STRUCTURE AND FELT PAIN IN LOWER BACK. HE BEGAN MISSING ON 11/5/01.

EE TRIPPED AND FELL WITH HIS LEFT KNEE STRIKING A BLOCK OF COAL.

EE LOST GRIP ON BAR USED TO LOAD BELTING AND BARFELL STRIKING HIS RIGHT FOOT.

EE STEPPED ON SIDE OF LO LO STRUCTURE & SLIPPED,INJURING HIS LOWER BACK.

EE STRAINED LOWER BACK WHEN PULLING ON CABLES.

EE WAS BURNED ON BACK OF LEFT HAND FROM A GROUNDED CABLE THAT FLASHED UP NEAR HIM.

AN UNINTENTIONAL FALL WAS FOUND INT HE OLD 010 SECTION AT X CUT 49 IN THE 3 RT X CUT AND GOING TO THE 4 RT X CUT FOR A DISTANCE OF APPRX. 100' LONG BY 20 FT WIDE AND 5' TO 6' HIGH.

AN UNINTENTIONAL ROOF FALL WAS FOUND INT HE #4 TRACK PANEL INT HE #5 ENTRY AT X CUT 18. THE FALL WAS 20' WIDE BY 20' LONG AND 5 TO 6' HIGH.

DRAW ROCK FELL FROM BETWEEN BACK ROW OF BOLTS AND ATRS AND STRUCK EMPLOYEE ON LEFT KNEE & LEG.

AT 7:00 AM ON 9-10-01 AN UNINTINTIONAL ROOF FALL WAS FOUND IN OLD WORKS OLD 010 PANEL. FALL WAS 30 FT LONG 20 FT WIDE AND APPROX. 10 FT HIGH AT #10 X CUT.

EMPLOYEE WAS INSTALLING ROOF BOLTS AND TURNED AROUND, TWISTING HIS RIGHT KNEE. EMPLOYEE BEGAN MINING ON 9/10/01.

UNINTENTIONAL ROOF FALL ON THE #3 BELTLINE BEGINING IN THE #9 X-CUT & EXTENDING TO THE #10 X-CUTFALL WAS 20' WIDE & ~15' HIGH. 5' GLUE BOLTS WERE USED TO SUPPORT AREA.

UNINTENTIONAL ROOF FALL OCCURRED IN THE RETURN OF #9 SECTION IN THE #2 ENTRY AT THE 16TH XCUT. AREA BOLTED WITH 48"" RESIN BOLTS.

A UNINTENTIONAL ROOF FALL OCCURRED IN THE RETURNENTRIES ADJACENT TO NO. 6 BELT. AREA BOLTED WITH48"" TORQUE TENSION BOLTS & 10' ROPE BOLTS INSTALLED IN INTERSECTION.

UNINTENTIONAL ROOF FALL OCCURRED ON 009 SECTION PANEL #3 ENTRY FROM THE 21ST X-CUT TO THE 23RD X-CUT. AREA BOLTED WITH 60"" POINT ANCHOR WITH 10'ROPE BOLTS INSTALLED IN INTERSECTION.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 1ST PANEL OFF #8 SUBMAINS (#4 ENTRY 26TH CROSSCUT).

EMPLOYEE WAS INSERTING A PIN IN HOLE STRUCTURE WHEN A RAIL SLIPPED AND KNOCKED HIM TO THE GROUND. EMPLOYEE BEGAN MISSING WORK ON 7-3-01.

THE ELEVATOR COULD NOT BE USED DUE TO MALFUNCTION.

THE ELEVATOR COULD NOT BE USED DUE TO A FAILURE IN A CENTRAL PANEL BOARD.

EMPLOYEE WAS TRAMMING MINER WHEN DRAW ROCK FELL FROM RIB TO BOLTS AND STRUCK HIM.

EMPLOYEE WAS TAKING SIDE PLATE OFF A CARRIER WHEN THE COVER SLIPPED AND CAUGHT THE TIP OF HIS LITTLE FINGER ON HIS LEFT HAND. RECEIVED MEDICAL REPORT INDICATING THE TIP OF THE FINGER WAS FRAC TURED.

THE ELEVATOR MALFUNCTIONED IN THAT THE DOOR WILL NOT OPEN WHEN DROPPED TO BOTTOM OF SHAFT. ELECTRIC CONTROL PANEL FAILED.

EE WAS HELPING ALIGN LO LO STRUCTURE WHEN A RAIL SLIPPED AND STRUCK HIM ON THE LEFT HAND.

FAULT IN TOP. ROOF FALL MEASURING 20' W BY 90' L BY 6' H.

THE MAIN POWER FROM AEP WAS KNOCKED OUT DUE TO A STORM. 2 MEN WERE ON THE ELEVATOR AT THE TIME OF THE POWER OUTAGE. THE POWER WAS RESTORED IN APP. 2 1/2 HOURS.

A RIB ROLL OCCURRED AND STRUCK EE.

A SECTION OF DRAW ROCK MEASURING APPROX. 113"" X 20"" X 4' FELL FROM THE MINE ROOF AND STRUCK THE INJURED EE IN THE BACK AND HIP RT. SIDE.

A ROOF FALL MEASURING 20' WIDE BY 70' LONG BY 8' THICK WAS FOUND BY MINE PERSONNEL AT APPROX 2:30 AM ON THE 010 MMU. THE FALL IS LOCATED BETWEEN THE NO. 22 AND 24 IN THE #1 ENTRY.

A ROOF FALL MEASURING 20' WIDE BY 35' LONG BY 4 1/2"" THICK WAS FOUND BY MINE PERSONNEL AT APPROX 2:00 AM ON THE 010 MMU. THE FALL IS LOCATED IN THE NO. 2 ENTRY AT THE 21ST X-CUT.

INJURED EE WAS ATTEMPTING TO COUPLE TOGETHER A TRACK MOUNTED ROCK DUSTING MACHINE TO A PERSONNEL CARRIER. HE HAS HIS HAND AROUND COUPLING PIN WHEN THE PERSONNEL CARRIER WAS ENGAGED HIS FINGER WAS CAUGHT BEHIND PIN RESULTING IN MESHED AND LACERATED FINGER.

TIED PUMP UP IN AIR SO THAT EMPLOYEES COULD SHOVEL COAL UP SO THAT PUMP WOULDN'T STOP UP, ROPE BROKE HOLDING PUMP STRIKING EMPLOYEE IN HEAD AND NECK.

A ROOF FALL OCCURRED ON THE 009 SECTION BELT LINE APPROX. 1 1/2 HOURS AFTER THE 2ND SHIFT STARTED. FALL MEASURED 6'THICK BY 20'WIDE BY 30'LONG AT THE #10 X-CUT ON THE 6-B BELTLINE. APPROX. 25' OF CONVEYOR BELT & STRUCTURE WAS AFFECTED. NO PERSONNEL WAS INVOLVED.

EE WAS BOLTING TOP AND DRILL STEEL WRENCH CAME OUT AND HIT EE ON LEFT HAND AND BROKE HIS FINGER.

EMPLOYEE CUT HIS RIGHT LEG WITH A UTILITY KNIFE. REQUIRED SUTURES.

EE MASHED A FINGER ON HIS ELFT HAND THAT REQUIRED STITCHES.

STRUCK BY A PIECE OF DRAW ROCK THAT FELL FROM BETWEEN ROOF BOLTS.

EE TWISTED HIS BACK WHILE PUTTING IN BELT ROLLERS ON 2-21-01. HE BEGAN MISSING WORK ON 5-9-01 BECAUSE OF PAIN. DR. CANNOT FIND ANY PHYSICAL DAMAGE OR INJURY. EE SCHEDULED TO SEE ANOTHER DR. AN D FINAL DESCISION TO BE MADE THEN.

EE WAS WALKING AND STRUCK A METAL STAMP AND INJURED AN EYE THAT REQUIRED MEDICATION FOR THE EYE.

DRAWROCK FELL AND HIT EMPLOYEE ON LEFT ARM (SUTURES).

EMPLOYEE WAS CUTTING PIECE OF BELT; KNIFE SLIPPED AND CUT HIS LEG.

EMPLOYEE CUT HAND AS HE WAS REMOVING THE METAL BANDS OFF OF A BUNDLE OF CABLE BOLTS.
