While in the process pulling a rock dust hose employee ran head first into a cable bolt, causing him to fall, landing on his right shoulder. Employee continued to work until 7/26/2013 at which time he was seen by a specialist and removed from work pending results from an MRI.

While in the process of installing permanent roof support, employee placed a roof bolt into the drill pot, bolt twisted causing him to strain his left shoulder. ER physician place employee off work.
While in the process of exiting a rail mantrip at the battery station, employee twisted his left knee. He was seen by a local physician whom placed off work pending MRI results.
Employee was walking across the section when he rubbed the mine roof with his hand, knocking a small piece of loose material free, hitting him in the left eye. Employee finished his shift and went for medical evaluation, he was referred to an eye specialist. No scratches, cuts, or embedded objects were found in the eye. Antibiotic was applied to prevent infection.
Employee was using a pry bar to remove loose material from the top of the continuous miner, the bar slipped causing his right ring finger to be mashed between the bar and the miner. Employee finished his shift and went for medical evaluation where x-rays were taken. X-rays showed a fracture to the metacarpal in the tip of the injured finger.
A roof fall was discovered by Mine Examiner and State Inspector while traveling old 2A intake cross cut 15 inby spad 23832. The fall measured 20 foot wide, 20 foot long, and 8 foot high. No injuries were reported as a result of the roof fall.
Employee and co-worker were loading steel beams onto mobile equipment when employee felt sharp pain in lower back. ER diagnosed with a pulled muscle and removed from work.

Employee was advancing the Fletcher roof bolter forward using the inch tram controls when his left foot stepped on some loose material that was on the mine floor causing him to twist his ankle.
A roof fall was discovered by a belt examiner at cross cut 38 of the No.2 conveyor belt. The area was developed sometime in 1982. The roof fall measured approximately 20 feet wide, 25 feet long, and 8 feet high. No injury was reported due to this roof fall.
While in the process of banding roof bolts on a supply car, strap broke, striking employee on his left brow, resulting in a laceration that required glue to close.
A non-injury accident occurred when the elevator became inoperative due to a malfunction on the main control cable at the bottom of the elevator. The main control cable has been repaired and inspected.
While in the process of applying fire extinguishers and rock dust to the high voltage compartment of the 4E power center that was smoking, employee started to hyperventilate and experienced a loss of consciousness. This incident is being reported only to abate a MSHA citation.

Employee was unloading block off a supply car at the end of the track when some blocks fell off the pallet onto the mine floor. As employee reached to pickup the blocks to re stack them one of the block fell from the pallet striking his right hand causing a puncture between the thumb and his index finger that required sutures to close.
Skill saw kicked back while cutting steaks for concrete forms cutting right thumb of employee requiring surgery on 8/10/12.
Employee was cleaning shop when he stepped over a crib block his knee buckled.
Employee was in the process of cleaning a tail piece. As he was shoveling under the tail piece the bottom belt came in contact with the shovel, jerking the shovel causing pain in his right arm. He ceased work and was taken to a local hospital where X-Rays were taken. He was taken off work until a follow up on 8-2-2012.

While in the process of dismounting supply motor employee felt pain in his left knee. He stated that he has a history of knee problems. On 12-23-11 an MRI was performed. On 1-6-12 he was seen by a specialist and was informed it was a new tear. This report was delayed to determine if this was a new or old injury.
While in the process of using a scoop to clean in an outby area, the scoop deck struck rib causing a piece of material to come loose and fall onto employee's back, resulting in contusions. An Xray was performed revealing no broken bones.
Employee completed loading shuttle car from the #6 right, car left to return to feeder and traveled approximately 30 feet up a grade in 5 right. For unknown reason the shuttle car rolled backwards pinning him between shuttle car and the rib, resulting in fracture of the right leg and left ankle.
On 10/05/2011 employee was carrying a guard to install on a continuous miner when he felt pain in his lower back. Employee continued to work, but the pain worsen at which time he was seen by his family doctor on 10/19/11 and removed from work.
On 10/3/2011 employee was installing permanent roof support, while straightening the bolt to place in the church of the pot, employee felt pain in right shoulder and arm. Employee continued to work, but the pain worsened at which time he was seen by his family doctor on 10/24/2011 and removed from work.
While checking the radiator of a bobcat loader employee's right ring and middle fingers came in contact with fan blades resulting laceration and fracture to both fingers.

Injured employee returned to work and continued to work, but pain worsens. On 6-28-2011 a MRI was performed. On 7-11-11 the specialist removed him from work, to repair the ACL of his knee.

Employee was operating a Joy 10 shuttle car. While in the process of making a left hand turn toward feeder, the operator's compartment rubbed the left rib, causing some coal to be loosened from the rib, striking the employee's left knee. Employee stated that when rib roll started to come inside operator compartment, he twisted his back. This accident is still under investigation.

EE was operating battery powered scoop. While in the process of changing batteries EE placed a crib block under scoop bucket & raised the front tires off mine floor. Scoop bucket slid off the crib block, causing scoop to jar EE. He was seen by local ER physician 3/3/09 & released to return to work. He visited his family doctor 3/9/09 & was removed from work.
A roof fall was discovered during the preshift exam of 3 section, located in #4 entry in the intersection inby spad 25336. The estimated size of the roof fall is 20' wide X 20' wide X 8' high. No injuries were reported as a result of the roof fall.
Injured employee was in the process of installing permanent roof supports in a brow overhanging from the mine roof. He had placed the resin grouted rod to be installed into the hole & was pushing on the rod with his hands to straighten the roof bolt. The brow broke off & struck his right hand, resulting in a laceration that required sutures to close.
Two employees were in the process of changing a broken sheer pin in the sprocket of the conveyor chain of a conveyor belt feeder. The injured employee had placed the new pin in the sprocket & asked the other employee to turn the sheer hub in order to line up the hole. During this process the injured employee's middle finger received a laceration that required sutures to close.
Employee was positioned near a coal rib, while the operator of a dual boom roof bolter was moving the machine. The roof bolting machine moved suddenly to the right resulting in contact with the injured employee. Injured employee is currently waiting to see a specialist.
Injured employee was using a shovel to clean up around 5 D tailpiece. Employee stated that while in the process of raising the shovel to dump the material, he felt pain in his middle part of his back. Employee visited his family doctor on 2/5/09. Doctor recommended physical therapy for two to three weeks.
Employee was operating a 2 man rubber tire johnson ride that was traveling thru a small water hole when the ride stalled. Employee dismounted ride and placed his body in front of the johnson ride to hook a chain to the front axle when the ride started moving forward causing the employee to be struck by the ride resulting in fractured ribs.

Injured EE was seen by the local ER physician and received medical attention. The physician gave him a full release to return to work.
Injured was a passenger on a johnson rubber tire vehicle that was in the process of parking next to a power center when his right knee struck an electrical plug. This resulted in a laceration which required medical treatment. He was given a full release from the ER physician to return to work on 7/1/08. He in return visited his family doctor and was removed from work.
Employee claims that while carrying his lunch bucket and a bucket of miner bits from the elevator doors to the 6 ton battery station. Employee stated that he felt pain in his back, employee visited the local hospital and was seen by the ER and was removed from work at this time. 6/24/08.
Empoyee was in the process of making a permanent splice in a trailing cable, the utitily knife he was using in the process came into contact with his finger, resulting in a laceration that required sutures to close.
Injured employee was in the process of installing permanent roof support, he had put together the roof plate and resin rod to be installed, pitched them toward the drill boom, as he was walking toward the drill boom, a piece of draw rock fell from the outside roof bolt toward the rib, striking his right shoulder and right foot, resulting in fracture to his foot.
Injured employee was standing on top of a parked scoop hanging a ventilation curtain. While attaching the ventilation curtain to a roof plate, he lost his balance and reached for another roof plate to keep from falling. The roof plate he grabbed had a sharp edge causing employee to receive a laceration to his left thumb that required several sutures to close.
EE was loading supplies with a front-end loader. As he was moving forward the front wheel came off of the machine. He continued to work the reainder of his shift. Later that day he visited a local hospital's emergency room where the attending dr diagnosed his condition as degenerative disc diease and contusions. Mgts feels due to dr' opiona. this is best classified as occ illness.
EE was walking around a parked scoop, when his right ear struck a roof plate, resulting in a laceration that required 6 sutures.
EE was riding a rubber-tire mantrip, when the man trip hit a hole in the roadway. This resulted in the EE's left foot to come in contact with the front tire of the machine, causing a contusion to his left ankle.
Employee was opening an airlock door at the bottom of the elevator when he felt a pain in his neck. He continued to work approximately 4 hours until his neck got stiff.
EE was walking to his shuttle car. While reaching down to remove a piece of curtain out of the roadway, he felt pain in his left knee. The EE was taken to a local emergency room where his left knee was examined and x-rays taken. No abnomalities were noted. Employee was told to follow up with his family Dr. on Monday, March 31, 2008. At his time he was taken off work.
EE was helping the continuous miner operator turn a break. When the ripper head was lowered, the bumper of the miner set down on EE's left foot. X-rays taken indicated EE received fractures to 3 of his left toes. EE returned to work on his next scheduled shift.
At or around 3:00pm a roof fall occurred on the 004 section bleeder entry, just outby Spad No 23014. The fall measured 18 feet wide x 18 feet long x 8 feet high. No injuries were reported as a result of this fall of roof rock. The fall will not be cleaned and all entries leading to the roof fall have been timbered and danger signs installed.
Injured was working on the cable reel of a shuttle car. When the cable reel freed up the cable caught his right leg. This caused him to fall and injured his right knee.
Employee was using a 10 ton lifting jack to lift a derailed track motor. While in the process of applying pressure to the jack bar, employee's hands slipped, resulting in the jack bar striking injured employee's lower jaw.
While taking down a miner cable that was supported from the mine roof by a metal cable hanger, EE was struck by the hook end of the hanger between the right index finger and middle finger resulting in a laceration requiring three stitches to close.
While loading belt rollers into a scoop bucket, EE got his right middle finger caught between two rollers. He worked until 2/11/08. At this time he made a visit to family physician where x-rays show he has a fracture to the tip of his right middle finger.
While installing a roof bolt, a piece of draw rock fell, striking EE on the right hand, resulting in a fracture to his right little finger.
EE was operating a tired mantrip. EE drove over a piece of draw rock causing the steering wheel to be jerked. This jerking of the wheel caused it to spin around striking EE on the right hand resulting in a fracture to his right hand.
While installing a roof bolt, the bolt bent. The EE placed his hand on the bolt and engaged the rotation level on the roof bolt machine. The roof bolt spun around, striking the EE on the right thumb.
EE was installing perminant roof support, when a piece of draw rock fell, striking him on the left foot. EE finished the shift, however later on that evening, he went to a local ER where X-rays revieled he had a fractur to the bone directly behind his big toe.
A roof fall was discovered Tuesday morning 01/01/2008, 1 break outby spad 22161 on number 001 section. No injuries were reported as a result of this roof fall. The fall measured 18' x 18' x 6'. This area has been dangered off and will not be cleaned up.

Employee was helping to unload parts to repair a shuttle car when a roller shaft slipped out of the packing tube and struck him on the right hand resulting in a fracture to his pointer finger.
Employee was changing the brake pads on a rubber tired 4 wheel mantrip when the wrench he was using slipped causing his right hand to be cut by the brake rotor. Seven stitches were required to close the laceration.
Employee stepped into a hole in the mine floor and twisted his right ankle as he was getting off a rubber tired 4 wheeler.
Employee was in the process of supporting the continuous miner cable from the mine roof when he felt his left shoulder pop. He was transported to a local emergency room where he was diagnosed with a torn rotator cuff. The amount of time he will be away from work has not been determined at this time.
While attempting to get a supply motor back onto the rail after it had de-railed, employee was struck on the left foot by the track motor resulting in 4 broken bones.
on 11-16-07, around 6:30 am a roof fall was discoved at number 9 break on number 1 belt line. The fall measure 4'x18'x18'. no injuries were reported as a result of this roof fall.
EE was operation a shuttle car in an area where the continuous miner cable was supported from the mine roof. He turned a curve the mine cable was jerked loose from the mine roof, falling onto the operation deck of the shuttle car. The cable struck him on the neck and shoulders. He was transported to a local E.R. where he was told not to return to work.
Employee was sitting against the coal rib watching the roof bolter operator tram the machine backward when the machine jerked toward him grazing his left knee. A sharp piece of steel on the machine made a laceration on the top of his knee that required numerous stitches.
Employee was building a brattice. When he put a concrete block against the rib completing a course a concrete block from the course above fell striking his right pointer finger resulting in a fracture and a laceration that required several stitches to close.
On 11-21-07 employee brought to the attention of the Safety Dept. he had missed a day of work due to a work-related injury he received 10-27-07. He was struck by a piece of draw rock while bolting top. He finished his shift, went to a local E.R. the next day (Sunday) and was instructed by the E.R. physician to take the next day off work.
EE was holding an air lock door open until a scoop trammed past. When the rear tire of the scoop rolled past the door it struck the metal door pushing it upward. This upward movement of the door pinned his hand against the top causing 3 of his fingers to be fractured & 2 received lacerations. He was taken to a local hospital, examined & transferred to another hospital for surgery.
While hauling seal supplies the deck of the scoop struck a loose coal rib causing the rib to fall striking ee on the left side of his head (jaw). He was transported to a local hospital where he was diagnosed with bruising to the left side of the head. EE was placed off work over the concern of infection. He has a follow up appointment on October 31, 2007.
A roof fall was discovered on Break 27 on Number 1 belt, in the neutral between the track and belt entries. The area has been cribbed, timbered and dangered off. This fall will not be cleaned up. The fall measured 6' high x 20' wide and 30' long. No injuries were reported as a result of this roof fall. This fall was discovered on 10/15/07.
EE was installing roof support when a piece of draw rock fell striking the rotating drill pot causing a small piece of rock to be thrown striking him over the right eye resulting in a laceration that required two stitches to close.
While riding to the section, Employee was struck by a piece of draw rock. He was transported to a local hospital where he was examined, released and returned to work but was scheduled to see a specialist. On Tues. 9/17/2007 he was diagnosed with a broken collar bone and is scheduled to be off work for four to six weeks.
On August 29, 2007 Employee was hanging a continuous miner cable when he felt a pain in his lower back. On 9/17/2007 he informed the mine superintendent he will be off until 10/25/07.
EE slipped and fall, striking his right on an open utility knife that was he had laid on the tail piece where he was working. The laceration required two stitches to close. NOTE: the knowledge that he had received two stitches was not brought to the attention of the Safety dept. until 11-19-07
While installing roof support, EE activted the rotation lever when a roof bolt bent as he was pushing it into the top, resulting in a laceration to his wrist and palm that requires several stitches to close.
A roof fall was discovered on Break 47 on #3 belt, right return. Time and date roof fell is unknown. Fall measured 8' high X 18' wide X 30' long. All sides have been timbered off. The fall will not be cleaned up and no injuries were reported as a result of this fall. Ventilation and travel in this mine has not been affected.
While removing a rub rail from a continuous miner, employee got his left hand caught between the rub rail and the tram controller resulting in a fracture to the left middle finger.
Employee was struck by a piece of draw rock while checking supplies on a roof bolter. He was transported to a local ER where x-rays and a CAT scan indicated he has a compression fracture of a vertebrae in the lumbar area of his back. He was released and is scheduled to begin physical therapy. He is expected to return to work in 4 to 6 weeks.
This form is being submitted so citation 6644419 can be terminated. Management has conducted an investigation into this alleged event, conferred with medical professionals, and determined no reportable injury occurred.
While installing roof support employee activated the lever by mistake causing the roof bolt to bend striking him on the right thumb. X-rays indicated no broken bones. Employee released to return to work. Pain persisted and employee went to another ER where a 2nd x-ray indicated a broke bone in right thumb. EE scheduled to have a cast put on and began missing work July 16, 2007.
Two employees were using pry bars installing a rail onto a series of track ties. When force was applied to the rail, the pry bar slipped out of one employee's hands causing the weight of the rail to shift onto the bar the other was using, causing the bar to slip out of his hand striking him on the right foot. X-ray showed no broken bones. He began missing work 7/9/07.
Employee was unloading rail from a supply car when the bar he was using slipped causing him to fall, twisting his left knee in the process. He began missing work on July 9, 2007.
Employee was installing a ventilation curtain when he felt a burning pain in his right groin. EE was examined by a physician and diagnosed with a hernia. Employee worked until August 7, 2007 when he under went surgery to repair hernia.
A roof fall was discovered at the #2 seal of the third south set of mine seals. The fall measured approx 15'h x 20'by 30'. No injuries resulted from this fall. The time of accident is listed as the start of the shift.
Employee was operating a supply motor attempting to pull a derailed supply car back onto the tracks. The chain attached to the supply motor and supply car broke, flew back into the operator's compartment striking his forehead causing a laceration requiring several stiches to close.

EE was installing a set of air lock doors. While walking over to check a brace he walked past a rubber tired four wheel personnel carrier that was sitting on a slight incline. While in front of the four wheeler, the operator released the brake which allowed the four wheeler to roll forward, striking ee on the right leg.
Employee was injured while opening a set of steel air lock doors. He opened one door & went on the back side of the other door to open it. While he was pushing on the 2nd door the 1st door blew shut against the 2nd door. His fingers were caught between the two doors during the impact of the doors. The impact caused fractures of the index and middle finger on the left hand.
EE was installing 5' fully grouted resin bolts at time of injury. After inserting the resin & roof bolt into the drilled hole he pushed the bolt toward the roof using the boom of the bolting machine. The head of the bolt wasn't fully inserted into the drill chuck when he engaged the rotation of the pot, the bolt was suddenly thrown to one side striking & fracturing his right thumb.
Employee was cleaning around an underground conveyor belt drive when he slipped and fell, twisting his right knee. He did not receive medical attention until 3/4/07 when he visited the local E.R. where x-rays were performed but did not show any damage. He was advised to follow up with his family physician and was taken off work for further testing.

Employee finished installing a mechanical belt splice on an underground conveyor belt. During the clean-up process of all hand tools he used his right hand to pull the splicing board out from underneath the belt line, mashing his index finger between the board and a rock lying near his work area. Force of impact caused a fracture of the distal joint. No lost time.
Employee was walking across the section when a small piece of draw rock fell measuring approx 6"" x 6"" x 0 to 1/2"" in thickness striking him on the back of the neck. The impact caused a laceration that required 4 sutures to close. Employee returned to work on his next scheduled shift.
Employee was injured while assisting with the repair of an electrical cable. Using a utility knife to remove the outer jacket of the cable the knife slipped causing a laceration to the left hand which required five sutures to close.
EE was injured during roof bolting procedures. He inserted the drill steel into the drill pot of a dual head roof bolter, raised the boom to start the drilling process. When the drill steel came in contact with the roof a small piece of draw rock fell measuring approx 1' x 1' & 0 to 1"" thick striking him on the forearm causing a laceration that required four sutures to close.
Employee was working in the shop using a sledge hammer to knock out a steel pin. During the hammering process a small piece of metal broke off the pin impelling into his left leg, knee area. The small piece of metal had to be removed and approx. 4 stitches to close the wound. He returned to work on his next scheduled shift.
Employee was working on a section belt line tailpiece when suddenly water started spraying from the end of the 4"" water line lying next to the belt line. While moving suddenly out of the way to keep from getting wet, employee stumbled and struck his right hand against the side of the feeder frame causing a fracture to the ring finger.
Employee was in the process of roof bolting procedures, inserting a section of drill steel into the drill pot of a double head bolting machine. Suddenly, a piece of draw rock fell striking him on the left hand causing a fracture of the ring finger between first and second joint and tip of the little finger distal joint.
Employee stated he was moving a piece of draw rock off the rail near company #4-b Belt drive when suddenly another piece of draw rock fell measuring approx 8""x 8""x 0-3"" thick striking him in the lumbar region of the back while he was bent over, causing a bruise in this area.
Employee was in the process of drilling a hole in the roof during roof bolting procedures. Ee placed his hand against the boom of the bolting machine to balance himself preparing to remove the drill steel from drilled hole. Suddenly a piece of draw rock fell measuring approx 1.5'x2'x1""-3"" thick striking him on side of index finger fracturing & lacerating his finger.
EE was searching by hand through some coal and gob material for a greasing device that was accidently covered up. While moving material his right hand came in contact with a sharp object which caused a laceration in the palm area. The wound was approx. 1.5 inches in length and re- quired 3 stitches to close.
A roof fall was found during a weekly exam on aug. 4th, 2006. The fall is located in the 1st intersection out-by the 1st north a seals, #4 entry. Measuring appro.x 7 to 8 ft. high, 20 ft wide, 40 to 50 ft in length. There was no equipment trapped, or injuries reportefd in connection with the fall.
EE was driving a rubber tired, battery powered 4-wheeler. It was being operated in the track entry when the accident occurred. During the operation ee veered off the track to one side of the entry when the steering wheel suddenly started spinning. During the spinning process he struck his right hand against the steering knob causing a fracture of the ring finger.
Employee was helping other employees secure a 4"" poly discharge line to the roof. During the lifting process, employee stated he experienced sharp pain in his lower back, and radiating down his right leg. He visited his family Dr. two days later who diagnosed him having lumbar strain.
