Claim filed for pneumoconiosis. Termination date of 8/9/1996. Physician report 9/11/2014.

An employee electrician was removing a breaker box panel located in the shop building to trace an electrical issue, and as he took breaker panel off a screw driver fell off the top of the box, he grabbed the driver and the metal shank phased to electrical leads causing an arc, burning his left hand & fingers.

Member was walking to return shaft when he fell forward into a small waterhole that was 2"" to 3"" deep he continued to work until 11;30 pm when he stated he might need to go to hospital, he was placed off work for 4 days because of soreness and swelling in his knees

Member was knocking a stopping when a piece of draw rock fell out measuring 80""lx15""Wx2.5"" thick fell striking him on the left arm causing a laceration and a compound fracture. Surgery was completed and he was kept in hospital for observation

Conducting on-shift exam going down slope reached right hand out to hold onto top frame of belt structure and accidently stuck his right thumb into top roller causing fracture to tip of thumb and required 1 stitch for laceration.

Fall 1 break inby spad # 11807 on 6th southeast submains at bk-21 on old 3 c belt in the #3 entry fall is 20'w X 20'long and 6' high. It's been dangered off and msha notified. Area won't be cleaned or re-supported.

EE was walking up the steps between 1st and 2nd levels on slope tower, when he fell backwards, twisting his right leg behind him and hurt his lower back when he fell. He was given phsical therepy for a couple weeks.

EE was removing a set of belt scales off the belt line #3 belt bk-26 he struck the scales with a 4lb hammer and they fell down hitting his ring finger on left hand causing an avulsion which required surgery and placed off work until the wound heals. He was wearing gloves at time of incident.

EE was cable bolting #2 intersection he installed 1 bolt then moved the roof bolter he reset and started back to the front to install another cable bolt when he stepped an a ledge in the floor causing him to fall he tried to catch himself with left hand this caused his back to twist and start to hurt. He was given off 3 days of work.

Employee was walking from #7 to #6 through the crosscut, when he went through the curtain he stepped on a piece of timber that had been sawed off and twisted his right knee. He was taken to hospital and told to see a specialist who placed him off work the following day.

EE was busting rock out of walkway on 2 Belt bk-18 with a sledge hammer and his shoulder started hurting. He was taken to hospital and given off until 11-4-10. He returned back to work on 11-5-10. He was diagnoised with a strained muscle.

Member was spot bolting on 3 A trac operating a single head bolter, he swung the drill pot over mashing his middle finger on his left hand. This required sutures to repair the wound. He returned back to work on his next scheduled shift

EE was walking over to 3 A power center when he caught the toe of his boot on a rock causing him to lose his balance twisting his right knee, back, neck. He was diagnosed with strains. All testing came back negative for broken bones.

EE was hanging miner cable and injuried his back. He was taken to hospital and x-rays were negative, told to follow up with a specialist, he has done that and diagnosed with a muscle strain.

EE stated that he slipped on a lump of coal or rock on the catwalk on the slope belt. He hasn't worked since this date. Please see the attached memo with this form. Based on the MRI report I ask this be classified as an occupational illness or a medical condition as a result of him being away from work.

Employee went to squat down on his right knee & heard his knee pop. He was seen by ER doctor & referred to a specialist. He had an MRI on 10/19 & is being seen by the specialist on 10/21. He is being kept off work now because of the MRI result & a follow-up with the specialist to determine the nature of his injury. There was no incident other than the squatting down on his knee.

Employee was shoveling on slope belt, stepped on waterline and slipped landing on his back & hip. He continued to work until 10/9/09 at which time he was taken off work by his doctor because of a back strain.

EE was welding on the left c/miner he was down on his right knee he twisted toward his left and when he did this he twisted his back and felt pain in his left leg. He was given 2 days away from work.(After we received a copy of the MRI report we would like to amend this to an occupation illnes.. because we feel this single didnt cause his injury but report movement.

EE's were moving the buggy cable, EE had his foot in a loop whey they pulled the cable it jerked his feet out from under him and he landed on his back. EE was diag. with a sprained ankle and given time off from work.

Employee was bolting in 7 right. He was inserting the 2 piece drill steel into the hole when he pinched his right index finger with the pusher drill steel. He suffered a laceration to this finger.

Rock fall at bk-27 on 6 b belt fall is 16' long by 18' high in the #3 entry it blocked the track entry it was reported to the MSHA hotline it will be cleaned and re-supported. MSHA did investigate the area.

EE was operating the AL Lee forklift, he placed his left hand on the outside of the canopy to pull himself up in the seat, when he did he accidentally hit the reverse tram pedal. This caused the forklift to move backward, smashing his left hand and fingers into a roof bolt plate. Ring finger couldn't be recontructed and was removed, middle finger expected to recover at this time.

Rub rail fell off of the continous miner and as he was putting it back on, his right finger was puntured by the rope hanger that had fallen in there and he didn't see it, which caused a laceration and required stitches to close the wound.

Employee was walking bent over when he hit his back on a roof bolt plate that the corner was bent on causing a laceration to his back requiring stitches to close wound.

Roof fall is in No 1 entry and is 8' high x 18' wide x 18' long. Area is dangered off and has been checked by MSHA. Roof fall is on 3 c belt at Bk - 12.

Operating a battery powered four wheeler when the steering knob (suicide knob) on the steering wheel kicked back striking him on the right wrist causing a fracture to his distal bone. EE was traveling the track entry when the accident occured.

Roof fall on 6c beltline at bk 22-23. Roof fall is apprx 70' long x 20 ' wide x 5' high. The roof fall will be re-bolted and the beltline will be ran over top of the rockfall when rebolting is completed in the area.

Roof fall was in the no. 5 crosscut. It was approx. 20' wide by 30' long x 7' high. Fall will be cleaned and resupported. Fall has been investigated by MSHA and OMSI.

At the direction of MSHA's conference & litigation rep, Solid Energy is reporting that EE claims he suffered an injury to his neck on 7/20/07 which he alleges resulted from him running into a rope bolt. This report is being submitted solely to terminate Ciation #6645307. By submitting this report, Solid Energy does not concede or admit that this was a reportable accident or injury.

EE was attempting to load bolts onto the forks of the forklift, he had lowered the forks to the ground. As he started to tram forward, the forklift went in to the rib, pinching his hat between the rib and canopy. The laceration required stitches to close the wound.

Employee riding in mantrip, SCSR hanging outside, SCSR struck 6x6 post at #4 belt track entry. Taken to e.r. for treatment, then went to his family dr, who released him. On 9/18/06 he returned to his dr. for follow-up and learned he had a crack vertabra.

Supply cars were being let down the slope. One came uncoupled allowing it to runaway down the slope striking a support beam which allowed the roof to fall in and take the belt line out of service. Area will be re-supported and cleaned. MSHA and the OMSL have investigated this incident. The fall is app 40' in length x 16' wide x 7' high.

EE was cutting down rope hanger when he cut his left arm. He required stitches to repair the cut.

EE WAS INSTALLING 10' ROPE BOLT. THE BOLT BENT WHILE BEING PUSHED IN. EE HAD HOLD OF BOLT, INTENDING TO LOWER POT. HE HIT WRONG LEVER & BOLT SPUN, HITTING HIS RIGHT WRIST & CAUSING LACERATION.

Employee was taking down cable; a hook caught between his right thumb and finger causing a laceration. He was taken to the hospital for treatment. Stitches were used to close the wound. This mine was closed the weekend of 7/4/05 for the holidays.

POSSIBILITY OF THE WILLIAMSON SEAM BREAKING THRU & LETTING MORE WATER INTO THESE AREAS (H-PANELS AND NORTH WEST MAINS). THE WILLIAMSON SEAM IS OVER TOP OF THE POND CREEK SEAM.

EMPLOYEE WAS HELPING REMOVE A ROCK FROM THE CM, THE ROCK FLIPPED UP MASHING THE EMPLOYEE'S RT. MIDDLE FINGER AGAINST THE ROOF. EE WAS TAKEN TO THE HOSPITAL FOR STITCHES.

EE was starting first bolt in the fourth row of the entry. They were in the 1st L. X-cut. A rib-roll occurred, falling onto the right ankle, causing a fracture.

MINER OPERATOR REPORTED A LEAK ON OFF SIDE OF MINER & BACKED UP TO NEXT OUT-BY BLOCK. AS FOREMAN WALKED UP TO LEFT SIDE, A RIB ROLL FELL OUT LETTING A ROCK FALL ONTO HIS RT. LEG, PUSHING HIM AGAINST THE MINER.

Employee was carrying two pails of water through a curtain while in the process of mixing mortar to build seals. He stepped on gob, slipped and twisted his left knee.

Employee was assisting getting a man-bus back onto the rail. As he was pushing on the unit he fell onto his hand causing injury. The E.R. doctor used sutures to close his wound.

EMPLOYEE WAS ON 001 SECTION IN #5 RT. HELPING CHANGE THE OPERATOR SIDE OF THE RIPPER JACK. THE BOTTOM OF THE JACK HAD BEEN INSTALLED & THE HOSES ATTACHED. THE JACK WAS CRADLED IN A CHAIN & WHILE ATTEMPTING TO ALIGN THE TOP OF THE JACK IT FELL ONTO HIS RT. RING FINGER; PINNING IT BETWEEN THE JACK & THE PAN.

While assisting with belt move on the 001 section in entry #4, employee was installing bottom stands. Co-worker picked up a stand before employee was ready, and struck him in the mouth with the stand. Employee suffered a chipped front tooth in the process.

EMPLOYEE WAS ACROSS FROM THE FEEDER IN 6 RIGHT REPLACING A HOSE ON A MINER. THE COVER FOR THE PAN JACK FELL ON HIS HAND AND MASHED HIS RING FINGER. HE WAS TAKEN TO THE E.R. FOR TREATMENT. THE E.R. USED STITCHES TO CLOSE THE WOUND.

EE was putting the lid back onto the miner, and let it fall onto his left middle finger. he was taken to the e.r. for stitiches to close the wound.

EMPLOYEE WAS CLEANING THE SCOOP DECK AT THE #4 ENTRY. HE HAD HIS LEFT HAND ON TOP OF THE CANOPY, AND A SMALL PIECE OF DRAW ROCK FELL ON IT CAUSING A FRACTURE TO THE TIP OF THE THUMB.

EMPLOYEE WAS OPERATING THE SUPPLY MOTOR AT 5-A HEAD, IN THE TRACK ENTRY 60 FEET FROM CURVE WHEN THE ENGINE STALLED. ONE ENGINE COVER WAS PROPPED OPEN. WHEN HE RAISED THE OTHER LID, THE FIRST LID FELL AND STRUCK HIS LEFT HAND. HE WAS TAKEN TO THE E.R. WHERE STITCHES WERE USED TO CLOSE THE WOUND.

Ee was using a 3/4 ton chain ratchet to bind water line on a flat car. Pulling on the ratchet strained his right eye muscle, dislodging a cornea implant. His doctor replaced the implant and inserted stitches to hold it in place.

EE WAS TAKING ROPE BOLTS OFF OF THE BOLTING MACHINE IN THE NO. 6 ENTRY. ONE OF THE BOLTS BUTTED AGAINST THE RIB AND BENT. IT SLIPPED FROM HIS HAND AND SPRANG BACK, STRIKING HIS NOSE.

MEMBER WAS WALKING AROUND THE BACK OF THE ROOFBOLTER WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE HEAD, BACK AND LEG CAUSING A LACERATION OVER THE LEFT EYE.

EMPLOYEE WAS ATTEMPTING TO PICK UP A TIMBER USING A PICK HAMMER. HE SWUNG THE HAMMER AT THE END OF THE TIMBER, MISSED IT AND STRUCK HIS RIGHT KNEE WITH THE POINT OF THE HAMMER. HE WAS TAKEN TO THE E.R. WHERE STITCHES WERE USED TO CLOSE THE WOUND.

MEMBER WAS MOVING BUNDLES OF ROOF BOLT PLATES ON BOLTER WHEN HE STRUCK HIS LEFT ELBOW ON A BUNDLE OF PLATES RESULTING IN A LACERATION REQUIRING STITCHES.

MEMBER WAS BACKING SHUTTLE CAR UP AND MASHED LEFT LEG BETWEEN RIB AND SHUTTLE CAR.

MEMBER WAS CUTTING A PIECE OF BELT WITH A UTILITY KNIFE WHEN IT SLIPPED AND CUT HIS LEG REQUIRING STITCHES.

MEMBER WAS CHANGING A HYDRAULIC PUMP ON A ROOF BOLTER. AS HE WAS LIFTING THE PUMP HE CUT HIS RIGHT HAND REQUIRING STITCHES.

THE MINE ROOF IS LAMINATED SHALE WITH A MIX OF SOAP STONE WITH A PILLARED AREA ON THE RIGHT AND MINED OUT OVER TOP.

EE WAS STANDING AGAINST TOOL SHED WHEN FOUR- WHEELER ROLLED FORWARD HITTING HIS LEG CAUSING A LACERATION THAT REQUIRED STITCHES.

AN INUADATION OF WATER OCCURRED IN THE PILLAR WORKS OF THE 002 SECTION. THE WATER BLOCKED THE RETURN AND INTAKE ENTRIES.

A ROOF FALL OCCURRED 1500' OUTBY THE 001-0 SECTION. THE FALL MEASURED 6' HIGH, 20' WIDE AND 20' LONG. THE FALL WAS LOCATED BETWEEN THE #5 AND #6 ENTRY.

MEMBER WAS REMOVING CHAIN FROM AROUND BELT WHEN CHAIN PULLED LOOSE IT HIT HIM IN THE MOUTH FRACTURING TOOTH.

MEMBER WAS OPERATING SHUTTLE CAR AND HIT HOLE IN ROADWAY.

MEMBER WAS MOVING MINER CABLE OUT OF ROADWAY BY DRAGGING IT WITH THE HEEL OF HIS FOOT AND PULLEDMUSCLE.

MEMBER WAS SHOVELING SLOPE BELT WHEN HE SLIPPED AND FELL AND TRIED TO CATCH RAIL AND MISSED LETTING ARM GET CAUGHT BETWEEN BELT AND BOTTOM ROLLER.

MEMEBER WAS INSTALLING STRUCTURE FOR BELT LINE WHEN A ROCK ROLLED OFF RIB AND HIT HIM ON HEAD.

MEMEBER WAS OPERATING CONTINUOUS IN #7 ENTRY WHEN DRAW ROCK FELL STRIKING HIM IN HEAD.

MEMBER STATED HE WAS RIDING IN FRONT OF MANTRIP.MANTRIP HIT RAIL AND STOPPED SUDDENLY INJURING HIS BACK.

MEMBER WAS RIDING IN MANTRIP WHEN IT HIT RAIL & SUDDENLY STOPPED. THIS WAS ORIGINALLY NOT A LOSTTIME ACCIDENT DUE TO THE EE MISSING ONLY ONE DAY(3/8/03), BUT DUE TO THIS BEING A SCHEDULED SAT. IT IT A ONE DAY LOST TIME.

EMPLOYEE STATED WHILE PULLING ON A RING TO OPEN A PLASTER BUCKET HE STRAINED HIS BACK.

EE was in the process of loading a step up transformer that was beside the track to a flat car to bring outside. A rock had fallen between the transformer and the flat car. Ee was trying to remove the rock using a slate bar and while doing this the rock rolled off the slate bar onto his hand and arm.

EE WAS WATCHING THE CABLE FOR THE BOLTER OPERATOR AS HE TRAMMED THE BOLTER OUT OF A HEADING INTO THE INTERSECTION. EE WAS REMOVING THE CABLE FROM A HANGER WHEN THE OPERATOR STARTED THE ROOF BO LTER AND WITHOUT WARNING THE CABEL REEL ENGAGED.THE CABLE STRUCK EE UNDER THE CHIN & KNOCKED HIM BACKWARD. HE WAS TAKEN TO THE HOSPITAL WHERE HEWAS DIAGNOSED WITH A MUSCLE STRAIN.

EE WAS REMOVING ROCK FROM ON TOP OF A SHUTTLE CAR WHEN A PIECE OF DRAW ROCK MEASURING 13 INCHES WIDE, 12 INCHES LONG, AND 2 INCHES THICK, FELL STRIKING HIM ON THE LEFT SHOULDER. EE WAS TAKEN T O THE WILLIAMSON MEMORIAL HOSPITAL FOR TREATMENT. HE WAS DIAGNOSED WITH A MUSCLE STRAIN AND GIVEN 2 DAYS OFF.

EMPLOYEE HAD FINISHED BOLTING A HEADING AND WAS WALKING AROUND THE ROOF BOLTING MACHINE TO CLEAN THE DUST BOXES WHEN HE STEPPED ON A LUMP OF COAL AND FELL TO THE MINE FLOOR WITH HIS BODY WEIGH T COMING DOWN ON HIS RIGHT HAND. EMPLOYEE SUSTAINED A FRACTURE TO HIS RIGHT RING AND LITTLE FINGER.

EE WAS BUILDING A PERMANENT STOPPING. THE MINE FLOOR WAS UNEVEN & AS HE WAS POSITIONING A 6 INCH BLOCK, ANOTHER FELL FROM THE STOPPING & STRUCK HIM ON THE LEFT HAND & WRIST. EE CONTINUED TO WO RK UNTIL DECEMBER 30TH, WHEN HE CONTACTED HIS SUPERVISOR TO INFORM HIM THAT HE WOULD BE OFF WORK UNTIL 1/2/2003.

EE WAS MAKING A SPLICE IN THE HIGH VOLTAGE CABLEWHEN THE UTILITY KNIFE HE WAS USING SLIPPED AND CUT HIS RIGHT THUMB. STITCHES WERE NEEDED TO CLOSE THE WOUND.

EE WAS A PASSENGER ON A MANTRIP TRAVELING TOWARD THE SECTION. 2 PIECES OF DRAW ROCK WEIGHING APPROX. 60 LBS EACH HAD FALLEN ON THE TRACK AND EE BEING THE ONLY PASSENGER ON THE MANTRIP GOT OUT & MOVED THE ROCKS. APPROX. 2 HOURS LATER INTO THE SHIFT HE EXPERIENCED INTENSE PAIN IN THE GROINAREA & WAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS DIAGNOSED WITH A MUSCLE STRAIN. 2 DAYS OFF.

EMPLOYEE WAS PUTTING OIL IN A ROOF BOLTER WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE BACK OF THE HEAD. EMPLOYEE RECEIVED TWO STITCHES.

EMPLOYEE WAS OPERATING A CONTINUOUS MINER WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE RIGHT HAND. SEVEN STITCHES WERE REQUIRED TO CLOSE THE WOUND.

WHILE WALKING UP THE BELT ENTRY TO THE SECTION, A PIECE OF DRAW ROCK FELL OUT FROM BETWEEN THE RIB AND THE SECOND ROW OF BOLTS STRIKING EE ON THE HEAD. HE WAS TAKEN TO THE HOSPITAL WHERE HE WA S TREATED AND RELEASED. HE WORKED UNTIL 9-25-02 WHEN HE DEVELOPED A HEADACHE AND NAUSEA. HE WAS THEN DIAGNOSED WITH A CONCUSSION.

WHILE OPERATING A ROOF BOLTER A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS STRIKING EE ON THE LT SIDE OF HIS HEAD. HE WAS KEPT OVERNIGHT IN THE HOSPITAL FOR OBSERVATION AND RELEASED THE NEXT DAY.

A ROOF FALL HAS OCCURRED ON THE 001-0 SECTION IN THE #4 ENTRY, AT 5A BELT 2300 FT FROM THE ACTIVE FACE. VENTILATION AND EQUIPMENT WER ENOT AFFECTED. THIS AREA WILL BE REHABILITATED.

EE HAD DRILLED A BOLT HOLE AND WAS INSERTING THE ROOK BOLT WHEN THE PLATE SWUNG AROUND AND STRUCK HIM ON THE RIGHT FOREARM. STITCHES WERE NEEDED TO CLOSE THE WOUND.

EE SAT DOWN ON THE RIB TO EAT LUNCH WHEN A PIECEOF DRAW ROCK FELL STRIKING HIM ON THE HEAD AND BENDING HIM FORWARD. EE SUSTAINED A FRACTURE TO THE T11 & T12 VERTABRE.

EE WAS IN BENT POSITION LIFTING MINER CABLE WHEN HE FELT PAIN IN HIS LOWER BACK. HE CONT'D TO WORK UNTIL 6/17/02 WHEN THE PAIN BEGAN TO RADIATE INTO HIS GROIN AREA. HE WAS TAKEN OFF WORK UNTIL 6/28/02

A ROOF FALL HAS OCCURRED ON THE 002-0 MMU, 4,600 FT FROM THE ACTIVE FACE. THE FALL MEASURED 20' WIDE, 18' LONG AND 7' HEIGHT. VENTILATION, EQUIPMENT, NOR PASSAGE OF PERSONS WAS EFFECTED. THIS AREA WILL BE REHABILITATED.

A ROOF FALL HAS OCCURREDON THE 002-0 MMU, IN THE #1 ENTRY, 4,600 FEET FROM THE ACTIVE FACE THE FALL MEASURED 20' WIDE, 20' LONG AND 6' HIGH. VENTILATION, EQUIPMENT, NOR PASSAGE OF PERSONS WAS EFFECTED. THIS AREA WILL BE REHABILITATED.

A ROOF FALL OCCURRED ON THE 001-0 SECTION IN THE#5 ENTRY, AT 5B TRACK. VENTILATION EQUIPMENT NORPASSAGE OF PERSONS WERE EFFECTED. THIS AREA WILLBE REHABILITATED.

EE UNPLUGGED A SHUTTLE CAR CATHEAD FROM THE SECTION POWER CENTER BUT DID NOT TRIP THE BREAKER. WHEN HE ATTEMPTED TO PLUG ANOTHER CATHEAD INTO THE SAME RECEPTACLE THE CATHEAD EXPLODED. EE SUSTAINED SUPERFICIAL FLASH BURNS TO HIS FACEAND ARMS. SAFETY GLASSED AND GLOVES WERE WORN PREVENTING INJURY TO EYES AND HANDS.

EMPLOYEE WAS MAKING A BELT SPLICE WHEN THE CHAIN RACHET BROKE AND A PIECE OF THE CHAIN STRUCK HIM ON THE RIGHT THUMB CAUSING A LACERATION REQUIRING STITCHES.

EE WAS OPERATING A ROOF BOLTER WHEN A PIECE OF DRAWROCK FELL STRIKING HIM ON THE LEFT HAND AND FRACTURING THE LITTLE FINGER.

EE WAS OPERATING A CONTINUOUS MINER WHEN A PIECEOF DRAW ROCK FELL STRIKING HIS RIGHT FOREARM. HESUSTAINED A LACERATION REQUIRING STITCHES.

CARRYING A METAL PANEL FOR AN ELEVATOR INSTALLATION. AS HE WAS SETTING IT DOWN HE EXPERIENCED PAIN IN RIGHT SHOULDER.

EE WAS HELPING CHANGE BATTERIES IN A SCOOP. HE WAS PULLING JUMPER CABLES APART, WHEN THE CAME APART, THE BELL END OF THE JUMPER CABLES STRUCK HIMIN THE CHIN. THE INJURY REQUIRED 5 STITCHES.

A ROOF FALL HAS OCCURRED ON THE 001-0 SECTION, AT BREAK NUMBER FIVE AT 7-A BELT-LINE. THE FALL MEASURED 18 FT. WIDE 40 FT. LONG AND 5 FT THICK. VENTILITATION, EQUIPMENT, NOR PASSAGE OF PERSONS WERE EFFECTED. NO INJURIES WERE REPORTED. THIS AREA WILL BE REHABILIATED.

A ROOF FALL HAS OCCURRED 001-0 SECTION, INT HE NUMBER 4 ENTRY AT SPAD 6705. VENTILATION, PASSAGE OF PERSONS, NOR EQUIPMENT WAS AFFECTED. THE ROOF FALL MEASURED 20 FT. WIDE, 25 FT, LONG AND 10 FT. THICK. THIS AREA WILL BE REHABILITATED.

A ROOF FALL HAS OCCURRED IN THE 002-0 SECTION IN THE #5 ENTRY

A ROOF FALL HAS OCCURRED ON THE 003-0 SECTION IN THE #4 ENTRY, 6000 FT FROM THE ACTIVE FACE. THE FALL MEASURED 25 FT WIDE 18 FT LONG AND 6 FT THICK. VENTILATION, PASSAGE OF PERSONS, NOR EQUIPM ENT WAS AFFECTED. AREA WILL NOT BE REHABILITATED. TIME OF FALL IS UNKNOWN.

EMPLOYEE WAS USING A UTILITY KNIFE TO CUT A FAULK COUPLING IN ORDER TO INSTALL THE COUPLING ON THE UNDERGROUND FEEDER. THE KNIFE SLIPPED OFF THECOUPLING AND WENT INTO HIS RIGHT THIGH. HE SUSTA INED A LACERATION REQUIRING STITCHES.

A ROOF FALL HAS OCCURRED ON THE #002-0 SECTION, 2 BREAKS OUTBY THE PILLAR LINE, BETWEEN ENTRIES 3 & 4 AT SPAD #10214. THE FALL MEASURED 18' WIDE X 40' LONG X 4' THICK. PASSAGE OF PERSONS NOR V ENTILATION WAS AFFECTED AND NO INJURIES WERE REPORTED. THIS AREA WILL NOT BE REHABILITATED.

HE WAS OPERATING THE CONTINUOUS MINER LOADING A SHUTTLE CAR WHEN A PIECE OF DRAW ROCK CAME OFF THE BOOM OF THE MINER AND STRUCK HIM ON THE RIGHT ANKLE. HE SUSTAINED A FRACTURE TO THE ANKLE AS A RESULT.

EMPLOYEE WAS OPERATING THE CONTINUOUS MINER WHENA PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLTSSTRIKING HIM ON THE BACK AND RIGHT ARM. HE WAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS DIAGNOS ED WITH SEVERE MUSCLE STRAIN TO THE BACK AND WASADVISED TO REMAIN OFF WORK.

A ROOF FALL OCCURRED ON THE 001-0 SECTION IN THE #4 ENTRY AT #5 BREAK. THE FALL MEASURED 20' WIDE X 25' LONG X 6' THICK. THE FALL WAS APPROXIMATELY 1400' FROM THE ACTIVE FACE. NO EQUIPMENT WAS INVOLVED, NO INJURIES WERE REPORTED AND VENTILATION OR PASSAGE OF PERSONS WAS AFFECTED. THIS AREA WILL BE REHABILITATED.

EE WAS A PASSENGER IN THE REAR COMPARTMENT OF A 5-TON MANTRIP. DRIVER & A FEDERAL INSPECTOR WEREIN THE FRONT COMPARTMENT. AT BREAK #6 ON 002-0 SECTION AN UNMANNED, RUN-A-WAY MANTRIP STRUCK THE 5-TON MANTRIP INJURED WAS RIDING IN. HE SUSTAINED A LACERATION WHEN HE WAS THROWN INTO THE CANOPY POST. STITCHES WERE REQUIRED TO CLOSE THE WOUND.

EMPLOYEE WAS USING A SLEDGE HAMMER TO BREAK UP A PIECE OF SANDSTONE THAT HAD FALLEN IN THE HAULAGE ROAD. AS HE WAS STRIKING THE ROCK THE HAMMRE BOUNCED OFF THE ROCK AND STRUCK HIM ON THE LEFT SIDE OF THE HEAD. EE SUSTAINED A LACERATION THAT RQUIRED STITCHES.

EE WAS OPERATING THE MINE IN #3 ENTRY OF 001-0 SECTION EE BACKED THE MINOR OUT OF THE RIGHT CUT& WAS PREPARING TO CUT THE LEFT SIDE WHEN THE MINE ROOF BGAN WORKING AT THE FACE. HE WAS WAITING FOR THE DRAW ROCK AT THE FACE TO QUIT FALLING AND WAS 24' OUTBY THE UNSOPPORTED AREA WHEN WITHOUT WARNING A PIECE OF OBLONG SHAPED SLATE MEASRING 42""LONGX73""WX1/16/"" TO 2"" THICK

EMPLOYEE WAS MOUNTING A SCOOP DEFK (FEET FIRST). WHEN HE SAW A PIECE OF DRAW ROCK FALLING FROM THE MINE ROOF. EE TURNED LOOSE OF HTE SCOOP CANOPY AND FELL TO THE GROUND TWISTING HIS LEFT SHO ULDER ON IMPACT. EE WAS TRANSPORTED TO HOSPITAL WERE X-RAYS WERE PERFORMED. X-RAYS WERE NORMAL AND A PRESCRIPTION FOR PAIN.

EMPLOYEE HAD THE COVER ON THE SUPPLY CAR RAISED IN ORDER TO CHECK THE OIL AND WATER LEVELS. WHEN HE LET GO OF THE COVER TO LET IT SLAM SHUT IT STRUCK HIS RIGHT INDEX FINGER CAUSING A LACER ATION THAT REQUIRED STITCHES.

EMPLOYEE WAS MOVING A FEEDER IN THE REHAB AREA. THE SLACK WAS TAKEN UP ON THE CATS AND SHIMS INSTALLED HOWEVER, THE PRESSURE ON THE JACKS WAS NOT FULLY RELEASED. THE GREASE INSERT USED TO TAKE UP THE SLACK IN THE CATS POPPED OFF AND STRUCK HIM IN THE SHIN AREA OF THE LEFT LEG.

EE WAS OPERATING THE ROOF BOLTER, INSTALLING 8' BRUTER BOLTS. THE BOLT HOLE WAS DRILLED BUT THE DRILL STEELE WAS FOULED IN THE TOP. WHEN THE DRILL POT WAS LOWERED THE STEEL FELL FROM THE TOP S TRIKING EE ON THE MIDDLE FINGER . HE SUSTAINED A FRACTURE TO THE TIP OF HIS FINGER AND A LACERATION THAT REQUIRED STITCHES.

A ROOF FALL HAS OCCURRED ON THE 001-0 SECTION IN THE #4 BREAK AT 5-A BELT. THE FALL MEASURED 19' WIDE, 60' LONG, AND 5' THICK. VENTILATION NOR PASSAGE OF PERSONS WAS AFFECTED AND NO INJURIES W ERE REPORTED. THE AREA WILL BE CLEANED UP AND RESUPPORTED.

A ROOF FALL HAS OCCURRED IN THE #3 ENTRY ON THE 001-0 SECTION, 270' FROM THE ACTIVE FACE. THE ROOF FALL MEASURED 18' WIDE, 30' LONG, AND 5' THICK. VENTILATION NOR PASSAGE OF PERSONS WAS AFFECT ED AND NO INJURIES WERE REPORTED. THE FALL WAS CLEANED UP AND RESUPPORTED.
