Flash burns (electric)

An employee electrician was removing a breaker box panel located in the shop building to trace an electrical issue, and as he took breaker panel off a screw driver fell off the top of the box, he grabbed the driver and the metal shank phased to electrical leads causing an arc, burning his left hand & fingers.

Member was walking to return shaft when he fell forward into a small waterhole that was 2"" to 3"" deep he continued to work until 11;30 pm when he stated he might need to go to hospital, he was placed off work for 4 days because of soreness and swelling in his knees

Struck by falling object

Member was knocking a stopping when a piece of draw rock fell out measuring 80""lx15""Wx2.5"" thick fell striking him on the left arm causing a laceration and a compound fracture. Surgery was completed and he was kept in hospital for observation

Conducting on-shift exam going down slope reached right hand out to hold onto top frame of belt structure and accidently stuck his right thumb into top roller causing fracture to tip of thumb and required 1 stitch for laceration.

Fall 1 break inby spad # 11807 on 6th southeast submains at bk-21 on old 3 c belt in the #3 entry fall is 20'w X 20'long and 6' high. It's been dangered off and msha notified. Area won't be cleaned or re-supported.

Fall down stairs

EE was walking up the steps between 1st and 2nd levels on slope tower, when he fell backwards, twisting his right leg behind him and hurt his lower back when he fell. He was given phsical therepy for a couple weeks.

Struck by falling object

EE was removing a set of belt scales off the belt line #3 belt bk-26 he struck the scales with a 4lb hammer and they fell down hitting his ring finger on left hand causing an avulsion which required surgery and placed off work until the wound heals. He was wearing gloves at time of incident.

EE was cable bolting #2 intersection he installed 1 bolt then moved the roof bolter he reset and started back to the front to install another cable bolt when he stepped an a ledge in the floor causing him to fall he tried to catch himself with left hand this caused his back to twist and start to hurt. He was given off 3 days of work.

Employee was walking from #7 to #6 through the crosscut, when he went through the curtain he stepped on a piece of timber that had been sawed off and twisted his right knee. He was taken to hospital and told to see a specialist who placed him off work the following day.

EE was busting rock out of walkway on 2 Belt bk-18 with a sledge hammer and his shoulder started hurting. He was taken to hospital and given off until 11-4-10. He returned back to work on 11-5-10. He was diagnoised with a strained muscle.

Member was spot bolting on 3 A trac operating a single head bolter, he swung the drill pot over mashing his middle finger on his left hand. This required sutures to repair the wound. He returned back to work on his next scheduled shift

EE was walking over to 3 A power center when he caught the toe of his boot on a rock causing him to lose his balance twisting his right knee, back, neck. He was diagnosed with strains. All testing came back negative for broken bones.

EE was hanging miner cable and injuried his back. He was taken to hospital and x-rays were negative, told to follow up with a specialist, he has done that and diagnosed with a muscle strain.

EE stated that he slipped on a lump of coal or rock on the catwalk on the slope belt. He hasn't worked since this date. Please see the attached memo with this form. Based on the MRI report I ask this be classified as an occupational illness or a medical condition as a result of him being away from work.

Rubbed or abraded

Employee went to squat down on his right knee & heard his knee pop. He was seen by ER doctor & referred to a specialist. He had an MRI on 10/19 & is being seen by the specialist on 10/21. He is being kept off work now because of the MRI result & a follow-up with the specialist to determine the nature of his injury. There was no incident other than the squatting down on his knee.

Employee was shoveling on slope belt, stepped on waterline and slipped landing on his back & hip. He continued to work until 10/9/09 at which time he was taken off work by his doctor because of a back strain.

EE was welding on the left c/miner he was down on his right knee he twisted toward his left and when he did this he twisted his back and felt pain in his left leg. He was given 2 days away from work.(After we received a copy of the MRI report we would like to amend this to an occupation illnes.. because we feel this single didnt cause his injury but report movement.

EE's were moving the buggy cable, EE had his foot in a loop whey they pulled the cable it jerked his feet out from under him and he landed on his back. EE was diag. with a sprained ankle and given time off from work.

Employee was bolting in 7 right. He was inserting the 2 piece drill steel into the hole when he pinched his right index finger with the pusher drill steel. He suffered a laceration to this finger.

Rock fall at bk-27 on 6 b belt fall is 16' long by 18' high in the #3 entry it blocked the track entry it was reported to the MSHA hotline it will be cleaned and re-supported. MSHA did investigate the area.

EE was operating the AL Lee forklift, he placed his left hand on the outside of the canopy to pull himself up in the seat, when he did he accidentally hit the reverse tram pedal. This caused the forklift to move backward, smashing his left hand and fingers into a roof bolt plate. Ring finger couldn't be recontructed and was removed, middle finger expected to recover at this time.

Rub rail fell off of the continous miner and as he was putting it back on, his right finger was puntured by the rope hanger that had fallen in there and he didn't see it, which caused a laceration and required stitches to close the wound.

Operating a battery powered four wheeler when the steering knob (suicide knob) on the steering wheel kicked back striking him on the right wrist causing a fracture to his distal bone. EE was traveling the track entry when the accident occured.

Roof fall on 6c beltline at bk 22-23. Roof fall is apprx 70' long x 20 ' wide x 5' high. The roof fall will be re-bolted and the beltline will be ran over top of the rockfall when rebolting is completed in the area.

Roof fall was in the no. 5 crosscut. It was approx. 20' wide by 30' long x 7' high. Fall will be cleaned and resupported. Fall has been investigated by MSHA and OMSI.

At the direction of MSHA's conference & litigation rep, Solid Energy is reporting that EE claims he suffered an injury to his neck on 7/20/07 which he alleges resulted from him running into a rope bolt. This report is being submitted solely to terminate Ciation #6645307. By submitting this report, Solid Energy does not concede or admit that this was a reportable accident or injury.

EE was attempting to load bolts onto the forks of the forklift, he had lowered the forks to the ground. As he started to tram forward, the forklift went in to the rib, pinching his hat between the rib and canopy. The laceration required stitches to close the wound.

Employee riding in mantrip, SCSR hanging outside, SCSR struck 6x6 post at #4 belt track entry. Taken to e.r. for treatment, then went to his family dr, who released him. On 9/18/06 he returned to his dr. for follow-up and learned he had a crack vertabra.

Supply cars were being let down the slope. One came uncoupled allowing it to runaway down the slope striking a support beam which allowed the roof to fall in and take the belt line out of service. Area will be re-supported and cleaned. MSHA and the OMSL have investigated this incident. The fall is app 40' in length x 16' wide x 7' high.

Employee was taking down cable; a hook caught between his right thumb and finger causing a laceration. He was taken to the hospital for treatment. Stitches were used to close the wound. This mine was closed the weekend of 7/4/05 for the holidays.

Employee was assisting getting a man-bus back onto the rail. As he was pushing on the unit he fell onto his hand causing injury. The E.R. doctor used sutures to close his wound.

THE JACK WAS CRADLED IN A CHAIN & WHILE ATTEMPTING TO ALIGN THE TOP OF THE JACK IT FELL ONTO HIS RT. RING FINGER; PINNING IT BETWEEN THE JACK & THE PAN.

Ee was using a 3/4 ton chain ratchet to bind water line on a flat car. Pulling on the ratchet strained his right eye muscle, dislodging a cornea implant. His doctor replaced the implant and inserted stitches to hold it in place.

EE was in the process of loading a step up transformer that was beside the track to a flat car to bring outside. A rock had fallen between the transformer and the flat car. Ee was trying to remove the rock using a slate bar and while doing this the rock rolled off the slate bar onto his hand and arm.

APPROX. 2 HOURS LATER INTO THE SHIFT HE EXPERIENCED INTENSE PAIN IN THE GROINAREA & WAS TRANSPORTED TO THE HOSPITAL WHERE HE WAS DIAGNOSED WITH A MUSCLE STRAIN. 2 DAYS OFF.

AT BREAK #6 ON 002-0 SECTION AN UNMANNED, RUN-A-WAY MANTRIP STRUCK THE 5-TON MANTRIP INJURED WAS RIDING IN. HE SUSTAINED A LACERATION WHEN HE WAS THROWN INTO THE CANOPY POST. STITCHES WERE REQUIRED TO CLOSE THE WOUND.

EMPLOYEE WAS MOUNTING A SCOOP DEFK (FEET FIRST). WHEN HE SAW A PIECE OF DRAW ROCK FALLING FROM THE MINE ROOF. EE TURNED LOOSE OF HTE SCOOP CANOPY AND FELL TO THE GROUND TWISTING HIS LEFT SHO ULDER ON IMPACT. EE WAS TRANSPORTED TO HOSPITAL WERE X-RAYS WERE PERFORMED. X-RAYS WERE NORMAL AND A PRESCRIPTION FOR PAIN.

EMPLOYEE WAS MOVING A FEEDER IN THE REHAB AREA. THE SLACK WAS TAKEN UP ON THE CATS AND SHIMS INSTALLED HOWEVER, THE PRESSURE ON THE JACKS WAS NOT FULLY RELEASED. THE GREASE INSERT USED TO TAKE UP THE SLACK IN THE CATS POPPED OFF AND STRUCK HIM IN THE SHIN AREA OF THE LEFT LEG.

EE WAS OPERATING THE ROOF BOLTER, INSTALLING 8' BRUTER BOLTS. THE BOLT HOLE WAS DRILLED BUT THE DRILL STEELE WAS FOULED IN THE TOP. WHEN THE DRILL POT WAS LOWERED THE STEEL FELL FROM THE TOP S TRIKING EE ON THE MIDDLE FINGER . HE SUSTAINED A FRACTURE TO THE TIP OF HIS FINGER AND A LACERATION THAT REQUIRED STITCHES.
