EE had closed the gate at the mine entrance and was walking back to his truck, he slipped on the metal grate injuring his shoulder. Due to conflicting reports of injury location a medical review was conducted to determine the validity. On 8-24-11 the report indicated the injury was related to the accident. He started missing work on 8-4-11 and surgery was performed on 8-5-11.
Employee was cutting a piece of plastic pipe when knife slipped and cut his hand requiring stitches.
A roof fall occurred on the 1st NW Submains near the mouth of NE-1 panel, that is a continuation of a previous fall that occurred in 2010, that now is approxiamately 120'L X 19'W X 5+'H, the fall is located near spad #34328. A larger area was dangered off and the NE-1 EP was relocated.
A roof fall occurred in a neutral entry at the C-seals set. The fall was approximately 15'L X 19'W X 5.5' in height blocking access to one seal in the C-seals set. MSHA was notified and investigated the fall. All entrances to the fall area have been breakered off.

An unplanned roof fall occurred in the return entry 2400' from the active face, inby survey spad #34863. The fall was approximately 20'x20'x8'. There were no injuries or equipment involved in the fall.
EE was bolting top on #15 belt, break #35 when a piece of draw rock fell, striking him on the back of the head. He was taken outside to an awaiting ambulance but refused transport. EMT place a liquid bandage on the wound, he later transported himself to the ER and received 2 stitches to close the wound.
On Oct. 5, 2010, due to an occupational illness, as substantiated by medical records and reports, employee became unable to work beause of the ailments described in the medical reports. This was not due to one single event at the mine, but repeated trauma over an extended period of time.
An unplanned roof fall occurred approximately 5000' from the active face at survey spad #60381 in the return entry. The approximate size of the fall was 250'x18'x9'. No injuries or equipment was involved in the fall.
EE was bolting in a roof fall area on 1 section when a piece of draw rock appx. 6""x3""x1/2"" fell striking his arm. The rock caused a laceration on his arm. He received stitches to close the wound.
A roof fall occurred on the active 001 MMU section that was approximately 80'x20'x10'.
EE was exiting the A.L. Lee Lo-Trac underground forklift 2 breaks outby #4C Head drive in the #3 entry. He placed his hand on top of the canopy to assist himself out of the deck and a piece of draw fell and struck him on his left thumb, causing a laceration. He was taken outside and transported to the ER where he received stitches to close the wound.
An unplanned roof fall occurred 2,200 feet from the active face in #5 entry SS #46108. The fall was appx. 25'x20'x9'. There were no injuries or equipment involved in the fall.
A roof fall appx. 240'x19'x6' occurred in the #7 entry on 17B. No equipment or persons were involved.
On June 17, 2010 due to an occupational illness, as substantiated by medical records and reports, Jackie Thacker became unable to work because of the ailments described in the medical reports. This was not due to one single event at the mine, but repeated trauma over an extended period of time.
IEE and two other members were removing a 12"" I-beam from the walkway and stopped to rest. When they picked it up again the EE felt a pain in his abdomen and neck. He was fitted with a C-colla r and was transported outside and then to the ER where he was diagnosed with a bi-lateral hernia.
Member was walking along #14 belt and bent down to cross under a water line, slipped and fell on an old piece of conveyor belt, causing a laceration to his knee. He went to Williamson Memorial Hospital ER where he received stitches and a full release to return to work.
Member was cutting the metal band from a new bundle of roof bolts and one of the metal bands snapped and struck his arm, causing a laceration. Member transported himself to hospital where he received stitches to close the wound and a full release to return to work with no restrictions.
Employee was at the roof bolter when he felt pain in shoulder. Employee continued to work until 2-16-2011 when he had surgery on his shoulder and started missing work.
While hanging a miner cable with a rope hanger, the hook of the rope hanger caught his thumb on his left hand. He received 8 stitches and was released to return to work.
While shoveling slope belt employee lost his footing and twisted his left knee. He continued to work until 07/27/2010, when he had surgery to repair a torn meniscus and a tear behind the knee.
Employee was shoveling on the slope belt and felt a pop in his shoulder. He continued to work while receiving physical therapy but did not show any signs of improvement and was scheduled for surgery on 12/01/2010. He continued to work until 11-30-2010.
Member had trammed the roofbolter into #3 left to begin bolting sequence, and as he prepared to get out of the operator's deck a rock , fell onto the ATRS of the bolter, causing the bolter to tilt up in the rear. Member placed his hand on top of the canopy to assist himself out of the deck, and when the bolter tilted up his hand was caught between the canopy and the roof.
Member was scaling roof in the intake when a rock fell knocking him to the mine floor. The rock measured 44"" X 35"" X 6"".

Victim was tramming a twin boom roof bolter out of the #1 face. The roof bolter struck the rib pinning his right middle finger between the frame of the machine and the canopy post resulting in loss of the end of his middle finger to the second joint. Due to the mine down for issues with seals he has not missed any scheduled work.
While getting tools out of a tool box, tool box lid fell and caught the last 3 finger on left hand. Was transferred to surface and taken to emergency room and received 7 stitches.
Roof fall occurred on 001-0 MMU in #4 entry 3 crosscuts inby survey spad # 34505 at 1:50 PM. The fall was approximately 20' wide x 50' long and 6 feet in height. No ventilation was affected by the fall. This fall was investigated by MSHA and OMSL.
Walking from Glory Hole area along track. Employee 6'6"" tall, bumped rock with head, rock loose, fell, struck employee on the left forearm and wrist area. Employee taken to ER for treatment. Stitches used to close wound.
Discovered roof fall at 8:50 A.M. in intake, #7 entry, 001 section, 12,000 ft from face. Fall was 20' X 20' X 6' high. No equipment was involved, vent not affected. Area will not be rehabed, both MSHA & OMSL were notified and investigated.
Taking 3rd cut fom #4 entry, squatted down, roof soap stone, rock fell from between rib and bolt striking employee's right shoulder area. Taken to ER for treatment, x-ray negative. Saw orthopedic 3-31-09, diagnosed with contusion to right rotator cuff and right collar bone.
Employee left work due to pain in low back, saw Chiropractor. RTW 3-4-09, work until 3-11-09, return to Chiropractor; taken off work until 3-13-09. 3-13-09 saw Chiropractor, taken off work again, RTW 3-17-09.
Employee was removing rail from old # 18 panel. One rail hooked, had scoop operator turn scoop bucket in order to hook other rail. Bucket hit rib, caused material to fall onto employee's right leg. Taken to PMC ER for treatment X-Ray positive. Was no indication of bad rib.
Employee on the off-side of bolt machine. Operator moved bolter, got on employee's left foot. Treated at the scene, taken to E.R., x-ray indicated hairline break on top left foot.
Helping put wreck motor onto track. Had jack under motor, putting other jack in place with header on top. Jack turned, pinched finger. Taken to ER for treatment, stitches used to close wound.

Tightening belt tensioner, channel lock slipped, hand hit sharp metal on guard, injured fingers. Taken to ER for treatment, stitch used to close wound. RTW 11/22/08.

Fall discovered 10:30 am #7 entry intake outby spad #60648. No passage of persons affected. No vent affected, no equip involved. Fall was 18' x 10' x 5' high. Area breakered off. MSHA and OMSL notified and investigated. Moving stopping & life line around fall & danger off.
Discovered fall in return, #1 entry of 010 MMU 20 ft inby spad 44529. Fall was 19' X 60' X 8' high. No equip involved, no passage of persons affected, no vent. disturbed, entrances were breakered off, will not be rehab, MSHA and OMSL were notified.
Fall discovered in #1 entry, return, 20' outby spad 44529, fall 19' X 60' X 8' high. Vent not affected, passage of people not affected, equipment not affected. Area breakered off, will not be rehabilitated, both MSHA & OMSL notified.
Employee stayed back on mid-night shift to help set air lock doors out-by D-4 hd. A piece of draw rock fell onto his shoulder & back area. Taken to E.R. x-ray negative, RTW 10-15-08. Worked until 10-22-08, saw another Dr., taken off work for 2 wks.
Employee walking down #3 entry, had left hand resting on his SCSR, a piece of draw-rock fell from between bolts onto hand. He completed shift, went to doctor next day, x-ray positive; broken bone on top of hand.
EE was attempting to insert a roof bolt into the top, in order to do so he had to bend the bolt to get it started. When he attempted to bend the bolt he felt a pain in his wrist.
Employee pulling wagon w/tools and parts (200+ lb). Cross over threshold of mine building, pulled hard on wagon, fracture #5 left rib.
EE was starting to drill the inside hole to install a roof bolt and the drill steel bent and struck him on the ritght shoulder.

Employee injured 3-28-08; rock fell while installing bolts, struck right side and knee area, went to E.R. x-ray negative, saw specialist 4-2-08 schedule MRI for 7-7-08, after MRI surgery to be scheduled, had surgery and began to miss work.
Preparing to service equipment, was in bucket of scoop, stepped out, level ground-no debri, twisted left ankle. Splint ankle and taken to E.R. diagnosis; Subtalar Dislocation. Employee 6' 4"" wt. 350 lb.
EE had Joy 10 SC at #3 entry intersection taking support rail from reel compartment. When end of rail fell onto mine floor, other end struck top of right foot causing injury. EE was taken to ER, x-ray was positive.
EE was shoveling on B-1 belt break #12 when a piece of draw rock approximatley 15""x18""x1"" fell hitting him on the left shoulder.
Loosening bolts on a scraper, his wrench slipped, striking him in the mouth. The wrench chipped a front upper tooth.EE was helping repairman on a Saturday.
While loading rail onto rail car, section of 60 lb rail fell onto right foot, causing injury. Taken to ER, X-Ray positive.
Cutting belt, pulled knife across belt, which was on his thigh and cut himself. Taken to ER for treatment, stitches used to close wound.

EE was working on power center at B-9 Belt. He was bent over with doors open, piece of rock fell onto his left upper back causing injury. He went to the Williamson Memorial E.R. for treatment.
Cleaned #1 magnet at glory hole, turning to go down steps, heard noise, turned to look, missed top step, fell down stairs, injured right knee.
004 Section, #2 left entry, Member put bolt against roof, rock (1"" x 6"" x 2"" thick) slid down steel, struck left thumb. He completed shift, drove self to E.R. X-ray positive, released and returned to work. 10-26 had follow-up appointment, and taken off work until next visit on 11-06-2007.
Making a final cut for belt-splice, break 4, #6 head. Moved backwards, slipped, cut right leg aboce his knee with a utility knife. ER used stitches to close wound.
Preparing to move RB up for next row of bolts. Rock fell from between rib and outside bolt, striking right side and falling down onto right ankle, causing injury. Taken to ER for treatment. X-ray of right ankle positive.
Installing bolts #5 heading. Draw rock fell when starting steel, struck right wrist, causing injury. Taken to ER for treatment, stitches used to close wound.
Crew had completed belt move. Was hanging tilt switch, stepped down from tailpiece, stepped on a rock and fell to the ground and sprained his ankle. Worked next two days, went to doctor on 8/6/07, took off work until 8/13/07 and RTW at full duty.
Watching tailpiece after belt move. Rock came off feeder, bounced up hit left hand, pinching middle and ring finger against tailpiece. Taken to ER for treatment, stitches used to close wound.
While bolting in the #3 entry on 011-0 MMU, EE states his foot got caught between loose draw rock and caused him to twist his left knee.

Centers was helping to position the canopy on the roof bolter to align the bolts. During this procedure he strained his back. He was transported to Williamson Memorial Hospital by ambulance.
Taking RB plates from scoop to botler. Lifted 2 bundles from scoop, turned, tripped, drop plates onto side of left ankle. Taken to E.R. x-ray positve, fracture to left ankle.
Bringing bolter head down, vibration of machine caused bolt to my on top r.b. as head came down it struck a bolt causing it to flip over onto right leg went to dr monday took off tuesday, rtw work wednesday.
EE turned off belt, turned & stepped on small rock, twisting ankle. Taken to ER for treatment, X-ray positive.
Using railroad jack to lift flat car to put back onto rail. Jack lever slipped, struck right jaw resulting in laceration under jaw & chipped tooth. E.R. doctor used stitches on laceration and going to visit dentist to repair tooth.
CM in #4 cut of pillar block loading SC, rib roll pushed EE against SC causing injury to his left side.
The door switch at the bottom of the elevator malfunctioned, causing the elevator to not operate. Repairman adjusted the switch & the elevator was operational in approx. 30 minutes.
EE WAS HELPING LAY 6""X20' AQUA WATERLINE AT THE BOTTOM OF THE SLOPE. HE DEVELOPED PAIN IN HIS LOW BACK. ON 5/2/06 MID SHIFT, EE LEFT WORK AND VISITED HIS FAMILY DR., WHO TOOK HIM OFF WORK DUE TO BACK STRAIN.
Employee was wedging a crib. On the back-swing he contacted a bolt plate with his right arm causing laceration. He was taken to the ER where stitches were used to close the wound.

10/27/05 at about 6:00 pm occured near the #12 head in entry #4 (track entry). The fall was 18'w x 25'l. x 8'h. the entrance to the fall was breakered off, clean up and rehabilitation has begun.
Employee was shoveling heavy wet material onto the #2 belt, and developed pain in the low abdominal area. He was diagnosed with an abdominal hernia.
Operating miner on pillar section, was positioned across intersection at MRS, rock fell onto left foot causing fracture to big toe.
EE WAS HANGING CURTAIN WHILE STANDING ON MACHINE. HE FELL, CAUGHT HIS FIRST FINGER ON THE RIGHT HAND ON A BOLT PLATE AND CUT IT. ER USED STITCHES TO CLOSE THE WOUND
EE WAS ASSISTING THE REPAIRMAN MAKE CABLE SPLICE AND CUT HIS LEFT THUMB. HE WAS TAKEN TO WILLIAMSON MEMORIAL HOSPITAL FOR TREATMENT. STITCHES WERE USED TO CLOSE WOUND.
MEMBER WAS CROSSING BELT CROSS-OVER AND SLIPPED AND FELL. MEMBER CONTINUED TO WORK UNTIL JULY 11, 2005. *NOTE: MEMBER FIRST DAY OFF 7-11-05.
EE WAS HELPNG MOVE THE BOLTER BY PLACING THE CROSS-OVERS FOR THE MINER CABLE. AS THE BOLTER CROSSED THE MINER CABLE, IT CAUGHT THE EE RIGHT KNEE AGAINST THE RIB CAUSING INJURY. X-RAYS WERE NEGATIVE, BUT EE RECEIVE SOFT TISSUE INJURIES TO RIGHT KNEE
EE WAS ROOF BOLTING ON #2 SECTION, IN THE FIRST ENTRY. EE HAD PUT UP THE FIRST BOLT AND WAS SWINGING BOOM OVER TO PUT UP NEXT BOLT WHEN DRAW ROCK FELL, STRIKING EE'S RIGHT KNEE.

EE CHANGING BIT ON STEEL, HEARD POP, TURNED LEFT TO MOVE AWAY. DRAW-ROCK FELL FROM BOLT TO RIB ONTO INSIDE RIGHT LEG. EE WAS PERMANENTLY TRANSFERRED TO OUTSIDE JOB.
EE was helping replace the bumper on a 12-12-Joy C.M. They tack-welded the left side and was heating and hitting the right side with a sledge hammer to get it close to the miner for welding. the tack-weld broke allowing the bumper to fall onto his right foot, fracturing his right big toe
While installing the first bolt at the third row of #1 entry of #2 section, the employee heard a pop. He looked left as he moved away to the right. A portion of the rib (60"" from the floor) popped out onto his lower right leg.
Employee had hands on the steel; steel had penetrated about 2"", hit a hard streak and bent. Before he could let go of it, the drill steel struck his arm.

Employee assisting installing track. Rail bounced when pinched off of block, struck employee on top of left foot & fractured it.
EMPLOYEE WAS 1 BRK INBY THE FEEDER. HE GOT OUT OF HIS CAR TO HELP HANG A MINER CABLE. HE SLIPPED ON A ROCK AND TWISTED HIS RIGHT KNEE. HE WENT TO THE ER, WAS EXAMINED, AND GIVEN A RTW RELEASE. EE CONTINUED TO WORK, PAIN IN HIS KNEE INCREASED AND HE SAW THE DR IN LEXINGTON. HE HAD SURGERY ON 2-14-05 FOR A MENISCUS TEAR, AND RECOVERY TIME IS EXPECTED TO BE ABOUT 2 MONTHS.
EE WAS LETTING THE T-BAR DOWN AND A SMALL PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLTS AND CUT HIS RIGHT INDEX FINGER. HE FINISHED HIS SHIFT, THEN WENT TO THE DOCTOR WHO SUTURED THE WOUND.
Employee had chain on differential of a motor, lifting it with a hoist. The differential got stuck, when he looked to see what stopped it, the chain broke and struck him on mouth.
Empkloyee was on 010-0 section at the feeder checking on a splice. While he was facing the feeder, a rib roll fell striking him and causing injury.
EE WAS IN THE #2 ENTRY WAITING TO DUST THE PLACE. A PIECE OF DRAW ROCK FELL OUT, STRIKING HIM ON THE BACK OF THE HEAD, NECK AND SHOULDER AREA. HE WAS TAKEN TO THE E.R. AT WILLIAMSON MEMORIAL FOR TREATMENT. X-RAYS WERE NEGATIVE AND STITCHES WERE USED TO CLOSE THE WOUND.
MEMBER WAS OPERATING SHUTTLE CAR WHEN HE HIT A HOLE IN ROADWAY CAUSING HIM TO BOUNCE INTO THE CANOPY INJURING HIS NECK AND BACK. THE E.R. GAVE HIM A PRESCRIPTION MEDICATION FOR HIS INJURY.
EMPLOYEE WAS USING A SLEDGE HAMMER TO REMOVE A PIN FROM A SCOOP JACK. WHILE PERFORMING THIS TASK HE FELT PAIN IN HIS BACK. HE REPORTED THE INCIDENT, & A FIRST REPORT OF INJURY WAS GENERATED. THE PAIN SUBSIDED, HE ELECTED NOT TO SEE A DR. EMPLOYEE CONTINUED TO WORK UNTIL 8/9/04 WHEN THE PAIN RETURNED, HE DECIDED TO SEE A DR. WHO TOOK HIM OFF WORK.
EMPLOYEE WAS LOADING 42"" BELT STRUCTURE INTO SCOOP BUCKET IN THE BELT ENTRY. HE SLIPPED IN THE MUD WHILE LIFTING, CAUSING STRAIN TO THE RT. HIP & GROIN AREA.
EE was at the right rear of the bolter. He bent over to move the cable, and was out of sight of the operator. As the machine moved it hit him, causing him to fall. The bolter got onto his left foot and ankle. EE suffered soft tissue damage to the left ankle.
EE was drilling a test hole in #3 entry. A piece of draw rock fell, striking him on the back of the right foot.
EE had spot bolted a corner at cross cut of break #18. as he was swinging the boom in, a bolt which had been stuck in the tray popped out and struck him on the lower lip. He had to be taken to hospital to close the wound.
As ee was setting the roof bolter up, a piece of drawrock fell striking his right shoulder causing injury.
EE WAS SETTING A PUMP ON BELT #15 AT BREAK #16, HE TRIPPED OVER A RAIL. AS HE FELL HE TRIED TO BREAK HIS FALL, INJURING HIS LEFT ARM, LEFT HAND/WRIST, AND HIS BACK.
The member was changing sprays on the miner. his spray wrench slipped, causing him to fall against the miner head, striking a bit lug with a bur. the fall result in a cut to his left wrist. he was taken to the e.r. stitches were used to close the wound.
Member was replacing center drum on 12-12 miner, using a forklift to lift the drum. The drum slid backward on the forks catching the member's finger against the frame. The member was taken to the ER for treatment. the xray was negative. The member's finger was sutured, & he was released.(Location: #2 entry, #4 section, 1 break inby spad #41971)
A ROOF FALL OCCURRED ON THE 004 ACTIVE SECTION WHERE FULL EXTRACTION PILLARING WAS BEING PERFORMED. THE FALL FELL ONTO THE CONTINUOUS MINER AND TWO MOBILE ROOF SUPPORTS (MRS). THE FALL WAS A PPROX. 100' IN LENGTH, 30' HIGH AND 20' WIDE. 1 BREAK OUTBY SS 41578.
MEMBER WAS CHANGING OUT 1000"" OF BELT PULLING ITWITH CHANNEL LOCK WHEN THEY SLIPPED OFF BELT CAUSING HIS HAND TO HIT COAL RIB CAUSING A LACERATION.
MEMBER WAS PUTTING OIL IN THE MINER AND CUT HIS LEFT THUMB ON A SHARP EDGE ON A 5 GALLON OIL CAN HE WAS USING, RESULTING IN STITCHES.

EE HAD AN INJURY AT FREEDOM II MINE. HE IS NOW AT ROCKHOUSE ENERGY MINE #1. HE IS OFF WORK NOW DUE TO THE INJURY HE SUSTAINED AT FREEDOM II MINE. HE STARTED MISSING WORK 11/14/03
MEMBER WAS MANUALLY DISCONECTING THE POWER ONT HE 002 SECTION POWER CENTER. HE WAS LOOKING THU THE PLEXIGLAS WINDOW TO SEE IF THE BLADES WERE DISCONNECTED. HE RECEIVED A FLASH BURN TO HIS EYES REQUIRING USE OF PRESCRIPTION MEDICATION.
AN UNINTENTIONAL ROOF FALL WAS DISCOVERED BY THE 3RD SHIFT MINE EXAMINER. THIS FALL MEASURED 80'LONG X 20'WIDE & APPROX. 6'HIGH. THIS FALL WAS LOCATED ON THE NO.12 BELT 3 CROSSCUTS INBY THE NO. 12 HEAD.

MEMBER WAS HELPING MOVE CONTINUOUS MINER INTO #4 HEADING WHEN DRAW ROCK FELL FROM BETWEEN BOLTS STRIKING HIM ON HEAD AND NECK.
EMPLOYEE WAS OPERATING A MINER WHEN A PIECE OF CAP COAL FELL STRIKING HIM ON THE LEFT HAND CAUSING A LACERATION REQUIRING STITCHES.
MEMBER WAS PICKING TIMBER UP WITH USE OF HAMMER WHEN HAMMER SLIPPED OUT OF TIMBER STRIKING HIM IN HEAD JUST ABOVE EYEBROW CAUSING LACERATION REQUIRING STITCHES.

A ROOF FALL OCCURRED ON THE NUMBER 004-0 MMU AT SURVEY STATION 40388. THE FALL WAS 2880' OUTBY THE ACTIVE FACE AND FELL FROM #1 TO #2 ENTRY. THE FALL MEASURED 30'LX19'WX10'H. 6' RESIN BOLTS WE RE BEING USED.
EE STATED THAT HIS BACK HAS BEEN HURTING SINCE HIS LAST LOST TIME ACCIDENT ON 9-10-02. ON THIS DATE HE STATES THAT WHILE OPERATING HIS SHUTTLE CAR HIS BACK CONTINUED TO HURT AND DECIDED TO SE EK MEDICAL ATTENTION. HIS DR. REPORTS DIAGNOSED HIM AS AN AGGRAVATION OF AN EXISTING CONDITION THAT HAPPENED ON 9-10-02 WHERE EE WAS HANGING A CABLE.
EE WAS HELPING THE ELECTRICIAN REPAIR THE MINER CABLE. HE WAS USING A UTILITY KNIFE TO REMOVE THE OLD TAPE & SPLICE & DURING THE PROCESS HE LAID HIS KNIFE DOWN BESIDE HIM. AFTER REPAIRS WERE M ADE THE ELECTRICIAN NOTICED HE HAD BLOOD ON HIS PANT LEG. FURTHER EXAMINATION REVEALED A LACERATION ON THE BACK OF HIS THIGH. HE WAS TAKEN TO THE E.R. WHERE STITCHES WERE APPLIED.
EE WAS WORKING OUTBY 001-0 SECTION, TRYING TO RESTORE RADIO COMMUNICATION WHEN A PIECE OF DRAW ROCK MEASURING 3'X3'X6"" FELL STRIKING HIM ON THE BACK. EE WAS GIVEN 2 DAYS OFF WORK.
EE WAS OPERATING A ROOF BOLTER INSTALLING 6' BRUTER BOLTS. DURING INSTALLATION ONE OF THE THE BOLTS BENT AND EE TOOK HOLD OF THE BOLT AND SPUN THE DRILL POD IN REVERSE TO RELEASE THE BOLT FROM THE CHUCK. THE BOLT STRUCK EE IN THE RIGTH WRIST AND HAND. EE SUSTAINED A FRACTURE IN THE RIGHT WRIST.
A ROOF FALL OCCURRED ON THE 001-0 SECTION AT 15 HEAD DRIVE IN THE #7 ENTRY 10,000 FEET FROM THE ACTIVE FACE. VENTILATION AND PASSAGE OF PERSONS WAS AFFECTED. THE FALL MEASURED 20'WIDE AND 30' LONG. THIS AREA WILL BE SPOT BOLTED FOR A DISTANCE OF 1000 FEET.

EE WAS IN THE PROCESS OF UNCLOGGING A COAL CHUTEUSING A CHAIN RATCHET TO REMOVE A ROCK WHEN THE COAL BROKE LOOSE AND COVERED HIM FROM THE WAIST DOWN. EE WAS FOULED FOR A SHORT PERIOD OF TIME.
ON 11/23/02 EE 3RD SHIFT ROOF BOLTER WORKED HIS SHIFT & LEFT THE JOB WITHOUT ANY COMPLAINT OF INJURY. THAT NIGHT HE CALLED IN TO SAY HE WAS UNABLE TO WORK DUE TO BACK PAIN. HE VISITED HIS FAMILY PHYSICIAN WHO DIAGNOSED HIM WITH MUSCLE STRAIN DUE TO REPETITIVE MOVEMENT.
EE WAS OPERATING A CONTINUOUS MINER WHEN A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE RIGHT SHOULDER. HE CONTINUED WORKING UNTIL 10-31-02 AT WHICH TIME DUE TO CONTINUED PAIN IN THE CLAVICLE A REA HE SOUGHT MEDICAL ATTENTION EE WAS REMOVED FROM WORK UNTIL CT SCAN CAN BE PERFORMED. THE DR SUSPECTS A FRACTURED CLAVICLE.
EE WAS OPERATING A SCOOP, CLEANING THE #5 ENTRY.HE DID NOT SEE A JOINT OF 4""PVC PIPE THAT HE PICKED UP IN THE SCOOP BUCKET. THE PIPE WAS CAUGHT BETWEEN THE RIB AND THE SCOOP BUCKET. WHEN HE MOVED FORWARD THE PIPE DISLODGED AND WENT INTO THE OPERATORS COMPARTMENT & STRUCK HIM IN THE RTFOREHEAD & EYE AREA. EE SUSTAINED A FRACTURE TO THE ORBIT OF HIS RIGHT EYE.

EE WAS STANDING BY THE RIB IN THE #8 HEADING WHEN A PIECE OF COAL POPPED OFF THE RIB STRIKING HIM ON THE RIGHT EAR. EE SUSTAINED A LACERATION THAT REQUIRED STITCHES.

ROCK STRUCK EE PINNING HIM IN A CROUCHED POSITION MOMENTARILY UNTIL FELLOW WORKERS COULD REMOVE THE ROCK. EE SUSTAINED A FRACTURE TO BOTH THE FIBULA AND TIBIA OF THE RIGHT LOWER LEG.
EE WAS OPERATING A SCOOP THAT WAS LOADED WITH TRACK TIES. THE TRACK TIES BECAME ENTANGLED WITH THE PHONE WIRE AND EE EXITED THE SCOOP AND JERKED ON THE PHONE WITE TO FREE IT FROM THE TRACK TIE S. THE PHONE WIRE CAME LOOSE AND STRUCK HIM IN THE RIGHT EYE CAUSING A LACERATION TO THE RIGHT CORNEA.
AFTER EXPERIENCING A POWER OUTAGE, EE WAS WALKING TO THE FACE OF THE #2 ENTRY TO CHECK ON THE CONTINUOUS MINER. AS EE WAS TRAVELING THRU THE LAST OPEN BREAK, HE STEPPED ON A LUMP OF COAL. HE S TATED THAT HIS ANKLE CREELED TO THE OUTSIDE CAUSING HIM TO FALL TO THE MINE FLOOR. EE SUSTAINED A FRACTURE TO THE FIBULA AS WELL AS A HAIRLINE FRACTURE TO THE ANKLE BONE.
EMPLOYEE AND 3 COWORKERS WERE SPLICING THE 3A BELT WHICH HAD A SPLIT. WHILE INSTALLING A 12 FT PIECE OF BELT, THE WIRE ROPE USED TO SHEAVE THE BELT SLIPPED FROM THE SINGLE CROSBY CLAMP, WHICH CAUSED THE BELT TO LOSE TENSION. THE COME-A-LONGS JERKED TO THE SIDE STRIKING INJURED IN THE STOMACH. WHILE CHECKING THE SCISSORS USED TO HOLD THE BELT CLAMP CAME LOOSE AND STRUCK SHOULDER.
EE WAS STANDING BESIDE THE CONTINUOUS MINER WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLTS STRIKING HIM ON THE HEAD AND NECK. EE WAS TRANSPORTED TO WILLIAMSON MEMORIAL HOSPITAL WHERE HE WAS DIAGNOSED WITH MUSCLE STRAIN. AN APPT. WAS MADE FOR EE AT SCOTT'S ORTHOPEDICS W/DR. HEIG. HE WAS AGAIN DIAGNOSED WITH MUSCLE STRAIN IN THE NECK AND BACK AND PLACED ON RESTRICTED DUTY.

EMPLOYEE WAS USING A SLATE BAR TO PRY A PLANETARY OUT OF A CONTINUOUS MINER. THE PLANETARY DROPPED OUT OF THE MINER CATCHING THE SLATE BAR AS IT FELL. THE SLATE BAR FLIPPED UP AND STRUCK EMPLO YEE IN THE EAR. STITCHES WERE REQUIRED TO CLOSE THE WOUND.
A ROOF FALL HAS OCCURRED IN THE #3 ENTRY ON THE #2 SECTION AT SPAD #19138. THE FALL MEASURED 20' WIDE BY 30' LONG BY 4' THICK. PASSAGE OF PERSONS NOR VENTILATION WAS AFFECTED. THIS AREA WILL BE REHABILITATED.
EMPLOYEE WAS REPLACING A BLOWER MOTOR ON A ROOF BOLTER WHEN HE GOT HIS FINGER CAUGHT BETWEEN THE MOTOR AND THE FRAME OF THE ROOF BOLTER CAUSING A LACERATION TO HIS RIGHT THUMB THAT REQUIRED STITCHES.
EE WAS TAKING DOWN CABLES AND THE HYDRAULIC HOSE TO 78 HAND DRIVE WHEN THE CABLE FELL OUT OF THE HANGER STRIKING EE ON THE NOSE CAUSING A FRACTURE.
EMPLOYEE WAS TAKING DOWN CABLES AND THE HYDRAULIC HOSE TO 7B HEAD DRIVE WHEN THE CABLE FELL OUT OF THE HANGER STRIKING EMPLOYEE ON THE NOSE CAUSING A FRACTURE.
EE AND 2 CO-WORKERS WERE RAISING THE SLOPE TRACK IN ORDER TO MAKE IT HIGHER THAN THE WATER LEVEL. A 10 TON MECHANICAL JACK WAS BEING USED TO RAISE TRACK. EE HAD PLACED ON HALF HEADER UNDER TRA CK AND WAS IN THE PROCESS OF PLACING ANOTHER HALF HEADER WHEN THE JACK KICKED OUT CAUSING THE TRACK TO FALL. EE'S RIGHT THUMB WAS CAUGHT BETWEEN TRACK & HEADER. SUSTAINED AVULSION TO THUMB.
EE WAS OPERATING A MANTRIP. HE WAS IN A CLOSE CLEARANCE AREA. A PIECE OF SLATE THAT HAD FALLEN OUT FROM BETWEEN THE LAST ROW OF BOLTS AND THE RIB WAS ANGLED OUTWARD INTO THE TRAVELWAY. THE ACC IDENT INVESTIGATOR AND THE INJURIES SUSTAINED INDICATE THAT HE WAS RESTING HIS ARM ON THE OUTER FRAME OF THE MANTRIP & WHEN HE PASSED THIS PIECE OF SLATE IT STRUCK HIS ARM CAUSING EVULSION
ON 1-1-2000, DUE TO AN OCCUPATIONAL ILLNESS, (DEGENERATIVE DISC DISEASE), EE BECAME UNABLE TO PERFORM HIS NORMAL WORK DUTIES. THIS AILMENT IS DUE TO REPEATED TRAUMA OVER AN EXTENDED PERIOD OF TIME AND NOT A SINGLE EVENT AT THE MINE SITE.
