Employee was moving material around shop, lifting idlers and felt pain in lower back.

Employee was standing on the catwalk walkway that had loose coal on it. They attempted to hang chain hoist and twisted they fell down to the walkway on same level twisting left leg, knee and ankle.

Employee was using a chain ratchet to remove bad return idlers, while using excessive force, the chain slipped off of the roller and struck the employee on the right side of face beside of nose.

Employee was sent to get additional tools when EE fell going down steps and landed on buttocks and back.

Employee was splicing #5 belt, when ee went to cut the belt knife slipped and cut right leg just inside of right knee causing a laceration.

Employee was using bobcat and spraying with water hose cleaning under #4 belt. Employee attempted to get off bobcat and slipped hitting their right elbow on boom of bobcat.

Employee was helping to replace a bad step that led up to spiral floor. Employee was hitting step with a hammer when hand slipped and hit a jagged piece of metal that was under the step. Injury required stitches.

Employee was descending ladder to bottom floor, once on the bottom floor ee turned to adjust a valve and stepped on an uneven surface that was under water. Employee started missing 09/28/18.

Employee was knocking a flange off of a pipe when employee caught thumb nail and pulled it off.

Employee had fixed clear well pipe that was 11'-12' on platform. Employee was traveling back to the concrete floor when employee fell.

Driver just drove out of the road and turned the truck upon its side. Driver was loaded driving only 10 miles per hour. They drove into the ditch and turned truck upon its side.

Employee was helping lift a piece of metal out of wash box and thumb got caught under metal.

Employee was helping to pull mechanism across the floor when the 3/4 inch rope came loose causing the employee to fall backwards into the frame of the fine refuge vibrator.

Employee was lowering extension ladder when the ladder slipped and caught right thumb on the handrail.

Hearing loss award date 08/25/15. Last day worked 01/05/14.

Employee misjudged edge of coal pile, dozer nose dived off coal pile. Front and right door glass broke, a piece hit his right leg. Injury required staples.

Grease fitting broke. Injected grease in right forearm.

Employee was installing an idler on a dozer and when he rose up he hit his back on a step. Employee started missing 09-23-13.

Employee was rolling belts on pump when he got his fingers caught between the belts and pulley.

Employee was replacing pipe when a hand rail broke causing him to fall.

Employee was changing oil in raw coal viberator when he inadvertently stepped back off discharge end.

Employee was stepping down from servicing the radiator of the dozer. He possibly slipped and the nozzle for the windshield washer penetrated his skin resulting in a laceration on his left arm.

Employee was repositioning a piece of metal to get punch back out when he got the end of his thumb caught on the guard on the iron worker.

Employee slipped off ladder while working on a loader. Employee started missing 02-06-12.

A piece of key stock had been welded on the end of bend pulley shaft of the 602 clean coal overland belt. Approximately an hour later the fire was discovered.

Employee was replacing dryer screen when his finger got caught between screen and wood block.

Employee was using a grinder when it caught on a boss block and kicked back contacting his left forearm. Injury required 7 stitches.

None. (Employee suffered an apparent heart attack.)

BG-4 loadout building caught on fire. Welding had been done in loadout approximately 2 hours prior to fire being noticed.

Employee was welding on a pipe when his foot slipped off a large pipe he was standing on. He was using 100% fall protection with full-body harness, which stopped his fall, but he bumped his left leg on the pipe & suffered a strained back muscle.

Employee was removing the segments for the dozer using a 10 lb sledge hammer. The hammer hit the segment and a piece of metal broke off and became embedded in his lower left leg.

Employee was helping install a new cable on the 90 ton Linkbelt crane when the cable puller came loose, allowing the cable to fall, striking him on the right shoulder.

Employee was cleaning out the synfuel collector sampler and when he closed it the latch handle snapped down mashing his finger. Injury required stitches.

Employee was working to remove a dryer lid when he lost his balance falling approximately 4-5 feet and fractured his right wrist. Employee later required surgery. Employee started missing 8-9-05.

Employee was helping check empty train cars when he somehow fell under the moving train car and damaged his left leg to the point in which it had to be amputated 6""-8"" below the knee.

THE FRONT END OF WATER TRUCK HAD BEEN JACKED UP AND CRIBBED WITH 6"" CRIB BLOCKS IN ORDER TO REBUILD BRAKES. EE INSPECTED THE AREA BEFORE REMOVING THE WHEELS AND DECIDED TO REPOSITION THE CRIBS . HE JACKED THE TRUCK UP, REPOSITIONED THE CRIBSLIKE HE WANTED AND AS HE REACHED OVER TO PULL THE HANDLE OUT OF THE JACK THE 20 TON JACK SLIPPED OUT AND STRUCK HIM IN THE HEAD (BROW).

EE WAS TAKING PIN OUT OF MECHANISM ON SCREEN AND THE PIPE WRENCH SLIPPED CAUSING SHEAVES TO ROLL OVER CATCHING FINGER BETWEEN PIN AND FRAME.

EMPLOYEE WAS STRIKING PIPE WRENCH WITH A BAR AND THE BAR GLANCED OFF THE WALL OF THE PLANT STRIKING EE ON RIGHT HAND CAUSING A HAIRLINE FRACTURE.

EE WAS PLACING A HYDRAULIC CYLINDER ON A PALLET FOR STORAGE WHEN HE FELT PAIN IN LOWER RIGHT BACK.

THIS IS AN ACCUMULATIVE EFFECT OF MANY YEARS OF REPETITIVE MOTION. EE STARTED MISSING WORK 06-06-03.

EE WAS HOLDING BOLT HEAD FROM A CUTTING EDGE OF DOZER, ANOTHER EE WAS USING AN AIR WRENCH AND THE BOLT HEAD TURNED CAUSING LACERATIONS TO INDEX FINGER AND RING FINGER ON LEFT HAND. (9 STITCHES

Employee bumped his head on a hydraulic line, then slipped and fell catching himself with his right arm. Employee continued to worked for as long as he could with the pain then went to the dr. and it was determined that his pain was due to a torn rotary cuff. Employee then had surgery to repair it. Employee started missing 1-9-04.

INJURED EMPLOYEE HAD BACKED HIS TRUCK UP TO UNLOAD IN BIN. WITH BED UP A PC OF COAL GOT THROWN FROM THE BED HITTING THE EMPLOYEE ON TOP OF HIS HEAD CAUSING CONTUSION LACERATIONS.

EMPLOYEE BENT OVER TO PICK UP A CHAIN FALL TO HAND TO A COWORKER. WHEN HIS BACK GAVE OUT, CAUSING HIM TO DROP CHAIN FALL. EE INDICATED HE THOUGHT HE HAD AGGRAVATED A PREVIOUS INJURY HE HAD IN THE SAME LOCATION OF HIS BACK ON 7-1-00, BUT NOW IDICATES A NEW INJURY AFTER PHYSICIAN VISIT ON 11-25-02.

EE CLAIMS HE RUT ON PULLED SOMETHING IN HIS RIGHT KNEE ON OR ABOUT 10-15-02 WHILE HOLDING THE HOIST DRUM BRAKE DOWN ON A 90 TON FRICTION CRANE. CONTINUED TO WORK UNTIL 12-8-02 WHEN HE WENT TO THE DR WITH IT WHEN HE CLAIMED IT WAS SWELLING AND STILL HURTING HIM.

EE WAS WHEELING CONCRETE IN A WHEELBARROW INSIDE THE PREP PLANT. HE PICKED UP THE WHEELBARROW AND COMPLAINED OF PAIN IN GROIN AREA BUT DID NOT SEEK MEDICAL ATTENTION UNTIL 10/7/02. REPORTED TO PCC SAFETY DEPT 10/7/02.

EMPLOYEE WAS SHOVELING COAL FROM AROUND TAILPIECE IN BG#4 TUNNEL WHEN HE FELT PAIN IN HIS BACK. HE STARTED MISSING WORK ON 9/16/02.

EE WAS HELPING WITH A CONCRETE POUR AND TRIPPING OVER THE REINFORCING WIRE, BUMPING TWISTING HIS KNEE.

FEEDER WAS BEING PULLED WITH A CHAIN HOIST. FEEDER MOVED TO SIDE, BUMPED PIPE. EE HAD HAND ON PIPE, MASHED TIP OF FINGER RIGHT HAND BETWEEN PIPE AND SKIRT BOARD. CUT AND FRACTURED TIP OF RING FINGER RIGHT HAND.

INJURED HAD WENT TO EXAMINE A DISTURBANCE OF THE HILLSIDE IN HAPPY HOLLOW. UPON WALKING ABOUT 200 YDS. FROM HIS VEHICLE UPON WHAT APPEARED TO BE A SMOOTH DIRT ROAD HE STEPPED IN A DEPRESSION A ND FELL BREAKING HIS SMALL LEG BONE 1"" ABOVE THEANKLE.

EE WAS WALKING DOWN A HILL WHEN HE FELL STRIKINGHIS BACK ON A ROCK. INITIALLY, HE WASN'T LOSING WORK BUT HIS BACK KEPT HURTING AND HE WAS SUFFERING FROM NUMBNESS IN HIS FOOT. LOST TIME BEGAN ON NOVEMBER 6, 2000.

EMPLOYEE WAS REMOVING A METAL COVER FROM A PLANTDRYER WHEN HE KNOCKED OFF A WELDING STINGER AND MASHED AND CUT HIS RIGHT HAND LITTLE FINGER.

WHILE CUTTING SMALL TREES AND BUSHES WITH A LARGE KNIFE, THE EE FELL ON THE KNIFE AND CUT HIS LEFT HAND. (LACERATION)

EMPLOYEE WAS LOWERING A HYDRAULIC JACK FROM A LOADER BUCKET; THE STEM CAME OUT OF THE JACK AND HE TRIED TO HOLD TO THE JACK AND STRAINED HIS BACK.
