DRIVER FAILED TO NEGOTIATE CURVE AT FOOT OF HILL STARTING ACROSS ONE LANE AND SENT OVER BRIDGE.

Employee was moving material around shop, lifting idlers and felt pain in lower back.

Employee was standing on the catwalk walkway that had loose coal on it. They attempted to hang chain hoist and twisted they fell down to the walkway on same level twisting left leg, knee and ankle.

Employee was using a chain ratchet to remove bad return idlers, while using excessive force, the chain slipped off of the roller and struck the employee on the right side of face beside of nose.

Employee was sent to get additional tools when EE fell going down steps and landed on buttocks and back.

Employee was helping to replace a bad step that led up to spiral floor. Employee was hitting step with a hammer when hand slipped and hit a jagged piece of metal that was under the step. Injury required stitches.

Employee was descending ladder to bottom floor, once on the bottom floor ee turned to adjust a valve and stepped on an uneven surface that was under water. Employee started missing 09/28/18.

Employee had fixed clear well pipe that was 11'-12' on platform. Employee was traveling back to the concrete floor when employee fell.

Employee was stepping down from servicing the radiator of the dozer. He possibly slipped and the nozzle for the windshield washer penetrated his skin resulting in a laceration on his left arm.

Employee was welding on a pipe when his foot slipped off a large pipe he was standing on. He was using 100% fall protection with full-body harness, which stopped his fall, but he bumped his left leg on the pipe & suffered a strained back muscle.

Employee was removing the segments for the dozer using a 10 lb sledge hammer. The hammer hit the segment and a piece of metal broke off and became embedded in his lower left leg.

THE FRONT END OF WATER TRUCK HAD BEEN JACKED UP AND CRIBBED WITH 6"" CRIB BLOCKS IN ORDER TO REBUILD BRAKES. EE INSPECTED THE AREA BEFORE REMOVING THE WHEELS AND DECIDED TO REPOSITION THE CRIBS . HE JACKED THE TRUCK UP, REPOSITIONED THE CRIBSLIKE HE WANTED AND AS HE REACHED OVER TO PULL THE HANDLE OUT OF THE JACK THE 20 TON JACK SLIPPED OUT AND STRUCK HIM IN THE HEAD (BROW).

Employee bumped his head on a hydraulic line, then slipped and fell catching himself with his right arm. Employee continued to worked for as long as he could with the pain then went to the dr. and it was determined that his pain was due to a torn rotary cuff. Employee then had surgery to repair it. Employee started missing 1-9-04.

INJURED EMPLOYEE HAD BACKED HIS TRUCK UP TO UNLOAD IN BIN. WITH BED UP A PC OF COAL GOT THROWN FROM THE BED HITTING THE EMPLOYEE ON TOP OF HIS HEAD CAUSING CONTUSION LACERATIONS.

EMPLOYEE BENT OVER TO PICK UP A CHAIN FALL TO HAND TO A COWORKER. WHEN HIS BACK GAVE OUT, CAUSING HIM TO DROP CHAIN FALL. EE INDICATED HE THOUGHT HE HAD AGGRAVATED A PREVIOUS INJURY HE HAD IN THE SAME LOCATION OF HIS BACK ON 7-1-00, BUT NOW IDICATES A NEW INJURY AFTER PHYSICIAN VISIT ON 11-25-02.

EE CLAIMS HE RUT ON PULLED SOMETHING IN HIS RIGHT KNEE ON OR ABOUT 10-15-02 WHILE HOLDING THE HOIST DRUM BRAKE DOWN ON A 90 TON FRICTION CRANE. CONTINUED TO WORK UNTIL 12-8-02 WHEN HE WENT TO THE DR WITH IT WHEN HE CLAIMED IT WAS SWELLING AND STILL HURTING HIM.
