Fall from ladders

Employee was to make repairs to Thermocouple on the feedend of K5 and as employee began to climb caged/fixed ladder employee's hand slipped on rung causing employee to fall backwards. Employee's foot was on the first rung. Laceration to head requiring stitches.

While engaging the pony motor on #1 Kiln it came out of gear and the clutch handle struck EE's right hand bruising the thumb. 1 day of restrictive duty.

While positioning a piece of metal it slipped out of their hand causing an injury to the right forearm. Injury occurred on 3/9/18 doctor visit on 3/14/18. Restricted Duty.

Employee got left foot caught between the first and second step of platform used to shoot kiln ash ring causing employee to fall onto the burner floor causing a laceration to left thumb.

Fall down stairs

Customer truck driver was seen hanging onto handrail at fall protection station near the Hydrate plant by plant employees. Once they arrived the driver was found laying on the ground. Driver experience,birthdate and SS# unkown.

Employee was stepping from barge onto tugboat and caught right hand thumb in U-bolt welded to the pole on toe knee causing a fracture.

Employee was retrieving parts located in the storage building outside the shop. While EE was walking back to the truck EE was observing how the parts fit together and noticed there was a skid lying next to the metal dumpster. Focusing on going around the skid employee came in contact with a piece of metal grating sticking out of the metal dumpster cutting EE's lip.

Employee was walking down small flight of steps six steps total. Employee stated he misjudged the top step causing him to twist his right ankle. Employee stated he did not fall at any time. He was utilizing three point of contact.

The hydrate plant was running product but was experiencing problems with the process flow. The hydrate operator was investigating the problem and found the belt on the weigh feeder had ignited and was on fire. Flame was extinguished after 40 minutes.

Employee stated he was walking up a stairway and felt pain in lower calf. Visited doctor and discovered a tear in knee area. Determined by Workers Compensation after investigation this was work related on 9/21/2015.

The employee was stocking brick on work plate form. Employee stated while placing brick on platform page stick came dislodged causing the brick to fall striking his left forearm. Causing fracture to left humorous bone, Brick weight was 126lbs.

An employee was in a man lift power washing built up lime from a conveyor belt. After taking a break for rest and water, the employee felt faint and ill. The employee was transported to an emergency and treated for dehydration and heat exhaustion.

While lifting tools in a bucket weighing approximately 40lbs from a gangbox the employee felt a pop in his right shoulder.

Employee dropped a piece of metal on his left foot. After follow up visit with physician on 12/8/2011 employee was restricted to sedentary work.

Employee received burns to the right wrist proximal to the thumb. The burn was caused by hot material leaking overhead from Kiln 5 Dust Trap east-side access door. Initial diagnosis was 1st/2nd degree burn. Follow up visit on 11/22/2011 required removal of tissue which designates as Reportable Injury.

While working on K3 stone weigh feed belt,employee bent over to pick up a loose rock & felt something in his left eye. Eye was flushed and prescibed drops.

Employee was moving feeder chute with a cart. The employee had hand on stationary handrail, when the cart shifted, catching left thumb between feeder chute and handrail. This caused a laceration requiring 2 stitches.

EE cleaned top of chute with short scraper, change to long scraper for bottom. Scraper contacted rotary feeder, ee right hand pull against steel grated on chute. EE transported to ER, exam revealed fracture to right hand and lower RT arm. Partial amputation right little finger end.

While entering the cab of CAT 980 loader, employee felt pain in right knee while stepping off top step of ladder. Examination proved a sprain to right knee.

Employee was replacing return rollers on conveyor system. While handing roller to assiting employee strained left lower part of back.

Employee hyper-extended right knee while pulling on vaccuum hose. Employee felt discomfort in knee requiring further diagnosing which revealed a tear requiring surgery on December 2, 2009.

Employee was removing an actuator from compartment on Kiln 1 baghouse. It fell approximately 18"" striking his right hand causing a laceration on his index finger requiring stitches.

Over-exertion in lifting objects

Employee was removing cleaning rod from 02 analyser on kiln #4 and felt a sharp pain just above the left pectoral muscle. Discomfort returned and further evaluation revealed a slight muscle tear which required surgery for repair. Surgery was performed on 1/13/2010.

Struck against a moving object

Mechanic was in a JLG man-basket welding on kiln. Welding lead was secured to the man-basket and as it traveled upwards, the lead caught the toe board of the platform. While observing the snag and bringing the basket back down, the welding lead snapped causing the man-basket to jump from it's present location.

Employee was removing screw jacks from the lifting basket taking them to the brick rig. A 2 ft section of pipe that covers the threaded section slipped off and fell to the floor. His toe caught on one end of the pipe and the opposite end caught on the floor grating causing employee to lose his balance and fall.

Employee was removing oxygen bottle from storage building. He removed bar that keeps them secured. He pulled bottle out and when he went to put bar back in place, he saw oxygen bottle tipping over and he reached with right arm to catch it and felt pain in his right shoulder.

Employee was raking coating from kiln when a piece hit rake pole knocking employee into handrail. Abrasion to upper right arm. Returned to physician 12/1/08 shoulder pain, restricted activities of no lifting over 10 lbs., unable to use power tools and limited use of Rt upper arm. Procedure for raking coating not followed. Note: Employee retired.

While performing inspection of the Wash Plant the employee stepped around the oil pump filtering unit located between the classifiers and struck a handle of an electrical disconnect chipping a tooth.

Employee was removing roller line from the kiln. He was walking on newly installed refractory bricks that had a buildup of lime/brick dust. He slipped on dust and struck his right leg ""shin area"" on the edge of a 2x10 wood plank.

Employee was using fire hose to wash off spilled material, when fire hose busted and knocked employee off balance and fell, striking left elbow on ground. Medical services did not discover a fracture until 9-6-08, on his return visit for further treatment.

Employee was walking through the plant area, when he stepped into an area that looked dry by his testimony, but in fact was mud. When he stepped out of the mud area, he felt a sprain in his left knee.

Employee was entering the Lime Kiln hood area to check for coating on cooler grates inside Kiln. The door to the Kiln was open, due to Kiln Rake sticking out of it. When he entered area the Kiln puffed causing causing dust or grit to enter eye.

Employee was cleaning or washing away spilled material (lime) from a conveyor, when the water and material splashed back into his face area. The material was able to get under his face shield and into his eyes. He was wearing safety glasses as well.

Employee was cleaning the housing of a pre-mixer at the Hydrate Plant. Lime dust was on the pre-mixer, and while cleaning this off got into the employees eye. First aid was given to the employee at the site as well as an off site medical facility. He did receive prescription medication.

Employee was assigned to the surface plant area and alleges he sustained a lime burn to his foot, however he did not; nor has yet reported this incident to plant management. The employee notified plant management on 3-29-08 he was going to seek medical treatment for this alleged incident through his family.

Plant employee was attempting to unplug a chute below a dust bin feeder, using a sledge hammer. While striking the chute with the sledge hammer, the employee felt discomfort in his left shoulder down to his elbow.

The employee was assigned to install metal siding on a building in the plant. While working with the material the employee alleged the following day he needed to go to the doctor something was in his eye. After visiting the eye doctor, a small piece of metal was removed from the left eye.

A small fire was observed on the bull gear of the # 5 Rotary Kiln. The fire was immediately extinguished by 8:15 am. The fire was due to a build up of grease around the gear. Directions received from the local MSHA office is to report all fires.

A contracted employee, after receiving Site Hazard Training, was assigned to service heaters on the property, had a JLG Vertical Man-lift fall on his leg while unloading the equipment from their trailer. The employee with another person from the company was taking the machine down the ramp on their trailer when the machine broke free and tipped over on said employee's left leg.

Employee, Plant Utility Operator, was performing part of essential job functions in the area of #1 Kiln Dust Bin, when an air line broke loose, allowing LKD (Lime Kiln Dust) to spray in his eyes and face. Employee is currently still hospitilized.

Enerfab ID# G941 was replacing inpact rollers in product silo using torch to remove existing rollers. Torch was last used at 4:15pm with a walk through inspection conducted at 4:40pm another inspectionat 4:55pm. Hot Work permit was in effect. During shift change eereported a belt fire. Due to dense smoke the fire department was called. Call was made to Arlington office.

Employee stepped off curb and believes he rolled his right ankle on a rock, causing a severe sprain. Due to the weakness of his ligaments, they put a short-term walking cast on his ankle. He was clocked out and heading home from his shift at the time.

High temp alerts were going off for Kiln 3 cooler tunnel. It was during shift change and the 7 pm conveyor man was called to go check it out. They saw smoke from the building and called fire dept for help at 7:02 pm. MSHA was notified at 8:15pm. Belts 57-811 and 57-812 were burned. Fire dept was cleaning up at 8:30pm. Unsure exact extiguish time.

EE was working in product screen, in which he needed to lay on his back while performing the work. It was a hot area. He wore a tyvek suit to prevent lime exposure, but he sweat enough to draw the lime through to his back. Over time, the EE didn't remove the lime in a timely and therefore got a 2nd degree burn.
