Struck by flying object

Employee, as a Mechanic, was cleaning out emulsion bin hose when pressure in the hose caused the hose cap to release suddenly striking Mechanic's left thumb and fracturing it.

Employee had laid the master pad for a Caterpillar D-9 dozer in place and turned to get a pry bar and the pad fell and struck his right foot.

Employee was removing a piece of rope from a return roller on the beltline when the rope caught pulling his left hand between the roller and belt resulting in a fracture to the tip of his ring finger.

Employee was scraping roadway with a backhoe and did not see a washed out area, on the side of road, when the backhoe overturned onto it's side.

Employee was walking down a hill and slipped in mud, his left ankle twisted. He said he felt it pop and sharp pain. Employee was transported to HRMC ER at Prestonsburg Ky. X-rays were negative and was told to follow up with family Dr. His Family Dr. Examined him and took him off work till 8/30/11.

Employee had picked up company truck on his way to assigned site. As he went into curve lost control, slid in loose gravel, then across the road into on coming vehicle

EE was helping to install a tilt cylinder on a Komatsu loader. As he was trying to align the cylinder with the mounting bracket it was moved by the boom truck operator and he stated he was jolted, causing pain in his lower back. EE continued to work until 3-22-11.

Fall from ladders

Employee was inspecting bearing on crusher. When getting down from crusher, the ladder fell approx 4 feet, landing against plant wall. Employee saw doctor, but continued to work. M.D. referred him to Neuro. Last day worked 12-30-09.

WHILE CHANGING OIL ON A WHEEL LOADER 25 FEET FROM THE BELT LINE, A PIECE OF COAL THE SIZE OF A SOFT BALL CAME OFF OF BELT LINE, BOUNCED OFF OF COAL PILE STRIKING EMPLOYEE IN LEFT HAND. (DID NOT KNOW HAND WAS BROKEN AT TIME.)

EE WAS IN THE PROCESS OF ADJUSTING STOKER FEEDER. WHILE STEPPING ONTO STEP, EE GRABBED A HOLD OF A MOTOR GUARD. GUARD PULLED LOOSE CAUSING EE TO FALL LANDING ON GROUND, HURTING LT WRIST AND LO WER BACK. EE DID NOT START MISSING WORK UNTIL 9-24-03.

A FIRE WAS DISCOVERED IN THE CLEAN COAL STOCKPILE MSHA INSPECTORS WERE ON SCENE WHEN FIRE WAS DISCOVERED. A PLAN WAS IMPLEMENTED TO EXTINGUISH THE FIRE.

INJURED WAS CLEANING A SIEVE SCREEN HOLDER FRAME. WHILE CLEANING HE HELD ON TO A SUPPORT BRACLED BAR BRAKE LEAVING A SHARP EDGE THAT CUT THORUGH HIS GLOVE AND MADE A SMALL CUT IN THE PALAR OF HIS RIGHT HAND, REQUIRING 3 STITCHES.

CONTRACTOR WAS LEAVING MARTIN COUNTY COAL REFUSE SITE CLEAN-UP. TRUCK WAS ON ROADS OWNED & MAINATAINED BY MARTIN COUNTY COAL. WHEN HE WENT AROUND CRIVE ON SLICK & SNOWY ROAD CONDITIONS & HIT A NOTHER VEHICLE COMING IN OPPOSITE DIRECTION.

A NON-INJURY ACCIDENT OCCURRED AS A RESULT OF AM IMPOUNDMENT BREAKING THROUGH INTO AN UNDERGROUND MINE. FLOODING OCCURRED INSIDE THE UNDERGROUNDMINE AND A DISCHARGE OF SLURRY EXITED THE PORTAL S OF THE 1-C MINE. THE SLURRY ENTERED THE DRAINAGES OF COLDWATER CREEK AND WOLF CREEK. NO INJURIES OCCURRED AS A RESULT OF THIS EVENT.
