Employee was raising a light bar with a crank handle to illuminate a dump site on the refuge pile when they felt pain in left shoulder. Employee started missing work on 1/15/2026.
Employee was climbing an extension ladder and felt a pain in EE's right ankle.
While cleaning the scalping screens on the 3rd floor there was some gob that broke loose in the chute just above and hit the utility man in the top of the head. Continued to work until 12/11/2025 when surgery was required to repair EE's neck.

While drilling a hole in a piece of metal with a power drill. The drill bit caught, causing the drill to rotate, and hitting their hand near the thumb. the right hand was caught between the metal and the drill. Started missing work on 5/19/2025.
The employee slipped and fell hitting a screen on the chin causing employee to chip a front tooth. The employee did not miss any work.
Miner was straightening a bent roof bolt with miner's hands, as miner was trying to straighten it the bolt slipped in the miner's hands causing a laceration to the side of the right hand requiring 5 stitches. Miner was injured on 05/09/2024.
Belt mechanic was climbing in the bed of the F650 service truck when they felt their left knee pop. The belt mechanic continued to work, first day missed 12/02/2024.
Employee was pulling on a 3 ton chain hoist when EE felt a pop in EE's right shoulder, EE finished EE's shift and went to see a doctor after EE's shift that put EE off of work starting 01/10/2024.

Contractor foreman that was over a crew for Dynamic Plant Services, LLC contractor company I.D. #C5098 was getting on the vertical man elevator to come down to the bottom floor when they missed a hand hold causing them to fall to the bottom floor landing on their feet. They had a fractured wrist, foot and pelvis.
Employee was stepping up on the first step getting in EE's haul truck when EE's back stared hurting EE. EE worked until EE was given an off work slip by the doctor on 2/27/23. EE had an MRI and it is just determined that this is not tied back to a previous accident.

Employee was walking and slipped causing EE to hit head on the handrail. EE continued to work until 12/26/22 when the doctor put EE off of work with restriction. EE came back to work on 01/04/2023 full duty. *EE had surgery for the same injury on 6/26/2023.
Employee was working on a stack sizer when employee slipped and fell landing on left shoulder. Employee continued to work until 06/23/2022 and went off for surgery.

Employee was using a hose to wash down a belt drive motor base. When the pump kicked on that pressurized the hose, it pulled EE backwards injuring EE's right shoulder. EE continued to work but started missing on February 25, 2022 to have surgery on the shoulder.
While using a grinder to remove a weld, a foreign object got on the employees left eye. EE later sought medical attention and had the object removed.

Citation No. 9534972 was issued during a Part 50 Audit regarding this injury. Employee was installing a counter weight fall protection box when EE twisted EE's left knee causing EE to miss work.

While using a compressed air tool to blow dirt away from a belt drive. The tool got away and cut EE's right hand. The cut required sutures.
EE was entering the lift area of the ""Breaker Tower"" at the Prep Plant through a house style steel door. While EE had the door open, the wind began to push the door shut, so the employee tried to stop it, but the door closed onto the tip of EE's right ring finger causing a laceration and a hairline fracture.
Lineman was prepping pole for attachment in bucket truck, got into high voltage Power and was electrocuted. Visible burn damage to right arm/hand and left leg.

While removing a screen side board from a pallet, the employee caught a finger between the side board and the flange of a steel pipe resulting in a fracture to the right index finger.

While attempting to clean the windshield on a Volvo Truck, employee stepped out onto the mat on the fender of the truck and slipped off and fell to the ground. EE landed on right wrist and shoulder. The injury was a fracture to the wrist.
EE was hooking up barge for loading. The winch cable was hung and when it came loose it sprung up and the d ring struck the employee on the mouth fracturing one tooth.
EE was working on the window regulator for a Ford 250 pick-up truck. While EE was taking the window out of the door, the spring on the truck released and caught left thumb cutting it. EE had to have a couple of stitches to close the wound.
Employee cut upper left leg while working on a screen panel. The cut required stitches.

Construction worker assembling a cone using come-along to move steel. He felt a pain in his side, which upon going to the ER, found out it was a broken rib.
Employee was cutting a plastic zip tie off of a new sheave wheel using a utility knife. When the knife cut through the zip tie it continued on cutting his right thigh. The wound to the thigh required sutures for treatment.

The employee was placing a clevis around a pull pin on a dozer. He mashed his finger between the clevis and the pin causing a laceration. The laceration required 3 stitches to close the wound.
Workers comp claim under investigation. Worker claims that a fitting came off a pipe while his feet where planted in the mud and fitting hit his back and he twisted around in the mud. He reported this to us 1 week later.

Employee was loading trash into skid steer. It was stopped with the heel of the bucket on the ground and lip tilted upwards. Employee entered skid steer stepped on the raised lip instead of the skid-proof platform. His boot slipped off of lip causing him to fall, letting his right shin to come into contact with bucket causing a cut below knee.

While the employee was handling a tie rope during a barge move, the rope snapped tight injuring his right wrist. Employee was letting the 1 3/4 inch rope slip around the kevel as the barge was winched downstream. The winch rope pinched against the tie rope and when it freed it jerked suddenly injuring the wrist.
Employee was using a box end wrench to tighten a 1 1/4"" nut on the overland belt flip over. The wrench slipped off and struck him on his abdomen. This resulted in a hernia which will have to be surgically repaired.
EE was nailing divider deck when he fell through the decking landing on his right elbow causing a contusion.
EE was installing wire mesh between arch steel when the wire cut the top of the knuckle on his left 5th finger causing a laceration that required stitches.
EE slipped in the slope and heard a pop in his right wrist when he was catching himself. He was diagnosed with a fracture.
Postive diagnosis for lung irritation.
EE was scaling the top of track entry, when a small piece of rock got in his right eye. EE was wearing safety glasses.
EE was walking across parking lot when he stepped in a water puddle and felt a pain in his back. He continued working and approximately 20 minutes later he bent over to pick up an extension cord causing a back strain.
EE was installing air line on the rib when a piece of coal measuring 6"" X 6"" X 5"" fell from 12"" above his head and rolled down his arms striking him on the forehead causing a concussion/hematoma.
EE was caught between mucker tire and the berm causing a puncture wound and contusion to his right shin.

Medical care was not reported until the medical services bill was recieved.EE failed to notify supervisor of care.
