The #3 left pillar was being mined and the last run was being taken when a piece of rock measuring approximately 2' thick fell on the head of the miner, causing the miner to get stuck. Then a piece of rock approximately 4' thick x 20' wide fell on the miner causing it to be wedged between the roof and the bottom.
A piece of rock fell in break, one break above 42 switch end. The rock fell to the edge of track. The rock was approximately 12"" sandstone, 24"" slate and measured approximately 7.5' wide x 10' long x 36"" thick. The rock fall pulled 2 roof bolts.
Employee was cutting beltline when knife cut through beltline, resulting in a laceration to his left arm/wrist requiring 4 stitches.
The pin on the truck end of the duster worked out, allowing the duster to set down on the wheels. The duster was being jacked up to see what needed to be done to correct the problem when the jack creeled over and the duster struck the employee on his left foot resulting in a fracture.
Employees were jacking a diesel motor which had de-railed back on to the track. When they slewed the motor over, it pushed a flat car off the track catching the right foot of another employee.
Employee was splicing belt when his knife slipped cutting his right index finger resulting in 3 stitches.
A roof fall occurred in the #6 track entry 10 breaks inby the empty loop switch. Rock fall measured approx. 16'x20'x5'.
A rock fall occurred in the #2 entry of the South Mains located between break #44 & 45 inby the No.4 head drive. The fall measured 20'x20'x5', the area had been bolted with 5' resin bolts and driven in September 1994.
Employee was tramming single head bolter out of #2 heading when a piece of draw rock fell from between roof bolts striking employee on right foot, resulting in a fracture.
Employee was helping make a belt set-up and was operating the front carrier. As the miner was pulling the lolo up to tighten the belt the connecting link being used to connect the chain together broke, and a piece of the link hit employee in the forehead resulting in (8) eight stitches.
Employee was lifting a bag of rockdust when he felt pain in his lower back. Employee started missing 03-08-05.
Employee was tightening a nut on a bolt with a rachet while someone else was holding the head of the bolt with a wrench. The rachet slipped off and caught employees hand between rachet and floor resulting in a fractured middle finger on his right hand.

A roof fall occurred at break 73 on the Big 4 belt. The fall measured 20' wide x 25' long x 8' thick.
The belt stopped and employee found rock fall in the #4 belt line entry, 28 breaks inby the Big#1 headdrive. The rock fall was estimated at 15'x20'x40' by the time top quit working and area was cleaned up the fall measured approx 15'x20'x70'.
Employee was standing beside a co-worker who was jacking up a ride that had wrecked. The jack flew out striking employee in the head resulting in a laceration over his left eye (11 stitches) and a broke tooth.
Employee had packed oil and put on carrier on 10-19-04 and said his back was hurting. He said his back started hurting on 10-18-04 after he packed oil to the carrier and helped put the covers back on the carrier. Employee started missing 10-21-04.
Rock fall occurred 84 breaks inby the Big 4 headdrive on the South Mains. Fall measured 20'wide x 20'long x 6' thick.
Rock fall occurred 15 breaks inby the Big 2 headdrive on the East Mains in the #5 belt entry. Fall measured approximately 18'wide x 40'long x 5' thick.
Rock fall occurred in the #3 entry, between breaks 22 and 23 of the Roll Mains. Fall measured approximately 4' thick x 14' wide x approx. 20' long
Rock fall occurred in the #2 entry, break 15 of the Roll Mains. Fall measured approximately 10' high, 14' wide and 15-20' long.
A rock fall occurred at break 17 in the Roll Mains. The top had been previously bolted with 5' resin bolts, top was resupported with cribs, timber and cross beams. Fall measured 36 inches thick, 7 feet wide and 14 feet long.
A rock fall occurred 3 breaks inby 017-C section headdrive located in the first right of the first right off the Fields Fork Mains. Fall measured 4 ft thick, 40 ft long and 20-25 ft wide.
A piece of rock was hung up in the 12-A headdrive, while removing rock employee strained/sprained right shoulder.
Employee was making belt set-up and was rehanging cables at head drive. Employee got little finger caught between backboard and cable cutting finger on backboard.
Employee was putting scoop on charge. When he plugged charger into batteries the male end of the plug shorted out causing 1st to 3rd degree burns on right hand.
Rock fall occurred 56 breaks inby the Big #4 Head-drive, #5 entry (track entry) of the South Mains. Rock fall measured eighteen feet by seventy feet by one and a half feet to three feet thick. Ventilation was not impaired.
Ride was blocked up where rear end had been replaced, employee was letting ride down when jack slipped out and bar caught his right thumb against bottom. (Required 4 stitches)
Employee attempted to remove a bag of rock dust from scoop while scoop was in motion and was caught between batteries on scoop and mine roof. Accident resulted in 5 stitches behind right ear and a fractured collar bone.
Rock fall occurred in number 4 entry of the South Mains 73 breaks inby the Big Number 4 head drive, fall was on the return side. The fall measured 18 feet wide, 25-30 feet long and approximately 4 feet thick. (No ventilation was obstructed.)
Mine examiner and state inspector were making the BMP in #4 right off the #1 right of the Fields Fork Mains and found several falls in which had fell above the anchorage points in the travelway.
Supply car de-railed in a waterhole, car had been jacked up and pushed over on rail. Employee couldn't see rail to make sure car was on track because of water. Employee stuck hand under car to feel the rail and wheel to see if it was on the track, the car slid over catching his hand. (Fractured index finger on right hand and 15 stitches)
Employee was shoveling belt near Big #4 Head drive and struck/bumped right foot against belt roller. Employee stated he had had a history of bone spurs in both feet.
Section had to pull off panel because of bad top, state inspector was making returns and abandoned areas on 017 section and found roof fall at the face of abandoned panel located 3rd right of the 1st right of the Fields Fork Mains. The fall measured 20'x20'x9'.
Employee was cutting belt to make a splice and knife slipped cutting right leg (5 stitches)
Employee was bolting top, after he put the last bolt up he went to get up off knee's to move roofbolter up and when he stood up his right knee gave away. Employee had surgery and started missing on 06-16-04.
Scoop operator was attempting to move large loop of belt, scoop moved through under belt too far causing end of belt loop to come into operator compartment with operator. Employee started missing 05-03-04.
3RD SHIFT TRIED TO START THE BELTS AT APPROX. 5:30 AM AND THEY WOULDNT START. THEY BHOUGHT A BELT SWITCH MAY HAVE BEEN LEFT OFF SO THEY STARTED WALKING THE BELTS. AT APPROX. 7 AM THEY FOUND A ROCK FALL OCCURRED 57 BREAKS INBY THE #4 HEAD-DRIVE IN THE BELT ENTRY. THE FALL MEASURED APPROX. 80'X20'X10'.

EE WAS PUTTING A RACE IN A COAL CAR WHEEL WITH A SLEDGE HAMMER, THE HAMMER HANDLE HIT HIS LEG CAUSING THE HAMMER TO GLANCE OFF TO ONE SIDE AND HIT HIS FINGER ON HIS RIGHT HAND.
EMPLOYEE WAS PULLING BELT WITH VISE GRIPS AND SLIPPED, HE CAUGHT HIMSELF ON THE LOLO CUTING HIS RIGHT HAND ON A PIECE OF METAL ON LOLO. 8 STITCHES.

EE DEVELOPED NECK/SHOULDER PAIN AFTER PICKING UPA ROLL OF BELT AND PLACING IT ON THE TOP BELT OFTHE 14-C BELTLINE.
EE WAS USING A CUTTING TORCH TO BLOW MATERIAL OFF OF A PIECE OF METAL WHEN A FOREIGN OBJECT WENT UNDER HIS GLASSES INTO HIS ELFT EYE. A SMALL PIECE OF SANDSTONE WAS FLUSHED FROM HIS LEFT EYE. EE WENT TO THE EYE DR. & WAS DIAGNOSED WITH A SCRATCHED CORNEA. HE WAS GIVEN PRESCRIPTION EYE DROPS, NO DAYS WERE LOST. THIS IS TO REPORT MEDICAL TREATMENT ONLY.
EE SHUTTLE CAR OPERATOR WAS DRIVING A SPAD NAIL IN THE RIB ONT HE 016 SECTION WHEN IT CAME OUT OF THE RIB AS HE STRUCK IT WITH A HAMMER AND STRUCK HIM ON HIS NO.10 TOOTH BREAKING IT IN HALF. T HIS IS TO REPORT MEDICAL TREATMENT ONLY.
FOREMAN WAS LIFTING ON A 30 HP PUMP WHEN HE FELT A SUDDEN PAIN IN HIS LOWER BACK. EE WENT TO THE DR. ON 9-4-01 & WAS DIAGNOSED WITH A MUSCLE STRAIN.
WHILE LOADING CINDER BLOCK INTO THE SCOOP BUCKETAT THE END OF THE TRACK ON THE 012-0 MMU EE FELTA SUDDEN PAIN IN HIS LOWER BACK. HE WORKED UNTIL8/27; HE RETURNED 8/30 MISSING 3 DAYS WORK.
EE UTILITYMAN WAS INSTALLING A TOP BELT ROLLER OAND RECEIVED A SMALL CUT ON HIS RIGHT INDEX FINGER. HIS FINGER STARTED SWELLING AND ON 8-13-01 HE WAS DIAGNOISED WITH BLOOD POSITIONING AND STAR TED OFF WORK.
SHUTTLE CAR OPERATOR WAS PULLING HIS SHUTTLE CARCABLE ACROSS THE 014 MMU SECTION BELT WHILE IT WAS OFF TO ANCHOR IT IN ANOTHER LOCATION. AS HE CROSSED THE BELT IT SUDDENLY STARTED AND HIS RIGH T ANTERIOR THIGH WAS ABRAIDED BY A TOP ROLLER ASHE GOT OFF THE BELT. HE HAS DEVELOPED AN INFECTION, SAW HIS PHYSICIAN ON 8/27/01 AND STARTED OFFWORK ON THE SAME DATE.
THE ABOVE MINE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL ONT HE EAS MINE RAIL HAULAGE TRACK NEAR THE JUNCTION OF 25 RIGHT. THE FALL IS APP. 80 FEET LONG, 6 FEET THICK AND 18 TO 20 FEET WIDE. ROOF MATERIAL IS SLATE AND SANDSTONE.SIXTY INCH FULLY GROUTED AND CRIBS WERE INITIALLY INSTALLED INT HE AFFECTED AREA. THE ROOF FALL WILL BE CLEANED UP AND RESUPPORTED.
THE MINE EXPERIENCED A NON-INJURY, UNINTENTIONALROOF FALL OUTBY THE ACTIVE 14 SECTION ON 14-E PANEL IN #6 ENTRY (NEUTRAL). FALL WAS ~200' LONG, 20' WIDE & 4'-6' THICK. LAMINATED SHALE AND SAND STONE. NO DAMAGE TO PROPERTY. AREA WAS INITIALLYSUPPORTED WITH 60"" FULLY GROUTED RODS WITH STRAPS AND CRIBS.
THE INJURED EE TWISTED HIS RIGHT KNEE WHILE LIFTING AND TURNING WITH A BAG OF ROCKDUST.
THE ABOVE EE STRAINED HIS BACK WHILE ATTEMPTING TO LIFT A ROLL OF CONVEYOR BELT. THE EE WAS RELEASED TO RETURN TO WORK ON 6-30-01, BUT DUE TO VACATION AND HOLIDAYS HE RETURNED ON 7/9/01 WITH 3 LOST WORK DAYS DUE TO HIS INJURY.
THE ABOVE MINE HAS EXPERIENCED A NONINJURY UNINTENTIONAL ROOF FALL IN #2 ENTRY(NEUTRAL), OUTBY MMU 013-0 SECTION AT 25 FEET OUTBY SPAD #6257. THE FALL WAS APP. 110 FEET LONG, 19 FEET WIDE AND 5 TO 7 FEET THICK. THE AREA WAS SUPPORTED WITH 60 INCH FULLY GROUTED RODS, THIN STRAPS AND CRIBS INSTALLED. SOME DAMAGE TO HIGH VOLTAGE CABLE.
THIS MINE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL OFF SECTION FOR MMU 014-0 IN #5 NEUTRAL ENTRY AT SS NO. 160 & EXTENDING INBY TO SURVEY SPAD NO. 161 APPROX. 120'. THE EE WAS USING A BELT KNIFE TO CUT A PIECE OF CONVEYOR BELT, THE KNIFE SLIPPED AND CUT HIS LEFT WRIST.
THE EE JAMMED HIS NECK WHEN A SCOOP HE WAS OPERATING SLIPPED OFF BLOCKING AND BOUNCED HIS HEAD AGAINST THE ROOF OF THE SCOOP CANOPY.
THE ABOVE MINE HAS EXPERIENCED A NON-INJURY UNINTENTIONAL ROOF FALL ON THE MAIN LINE BELT APP 5 CROSS CUTS OUT BY THE #1 BELT TAIL PIECE. THE FALL OCCURRED IN #4 ENTRY (BELT ENTRY). FROM 5 FT TO 6 FT THICK WIDE AND 20 FT LONG. SOME DAMAGE TO BELT STRUCTURE AND WATER LINE.
THE EMPLOYEE WAS LIFTING A ROLL OF BELT WHEN HE STRAINED HIS BACK.
INITIALLY THE EMPLOYEE WAS INJURED ON 2/16/01. STRAINED HIS SHOULDER WHILE PACKING A PUMP IN OLD WORKINGS. HE WORKED UNTIL 4/24 WHEN LOST TIME BEGAN.
WHILW WALKING TO THE MAN TRIP NEAR THE END OF THE TRACK ON 12 SECTION, THE EMPLOYEE TRIPPED OVER SOME SUPPLIES AND SPRAINED HIS ANKLE.
WHILE HAULING SUPPLIES ON A SUPPLY CAR, THE CAR WRECKED. WHEN HE ATTEMPTED TO PULL THE CAR BACK ONTO THE MINE TRACK, A CHAIN BROKE AND STRUCK HIS RIGHT ELBOW. (CHIPPED BONE).
THE EE INITIALLY INJURED HIS LEFT SHOULDER ON 2-5-01. THE EE WORKED AT FULL CAPACITY UNTIL 2-13-01. HIS LOST TIME BEGAN 2-13-01. THE OPERATOR BECAME AWARE OF HIS INJURY ON 2-21-01 WHEN THE INI TAL REPORT WAS COMPLETED FOR THE CO. THE EE WAS HELPING HIS PARTNER ON A ROOF BOLTER. THE INJURED EE WAS HOLDING A BOLTING MACHINE WRENCH AND THE WRENCH SLIPPED & STRAINED HIS LEFT SHOULDER.
THE EE STRAINED HIS BACK WHILE LIFTING AN AUTOMATIC COUPLER, DRAW HEAD.
THE EMPLOYEE WAS BEGINNING TO ASSEMBLE ROOF BOLTS TO BE USED WHEN THE ROOF BOLTER OPERATOR TRAMMED THE MACHINE A SHORT DISTANCE TO RELOCATE AND TRAMMED OVER THE RIGHT FOOT OF THE OTHER BOLTER MAN WHO WAS ASSEMBLING ROOF BOLTS.
THE EE FELL ON AN ICEY SURFACE AND BROKE HIS LEFT ARM.

THE AREA HAD BEEN SUPPORTED WITH 4 AND 5' FULLY GROUTED RODS. NO DAMAGE

THE FALL WAS APPROX 8 TO 10 THICK 20 FEET WIDE AND 30 TO 35 FEET LONG. ROOF WAS LAMINATED SLATE AND SANDSTONE. THE AREA WAS INITIALLY SUPPORTED WITH 48 INCH FULLY GROUTED RODS. NO EQUIPMENT WAS INVOLVED, NO PROPERTY DAMAGE.
THE ABOVE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL ON BOX CUT MAINS FOR MMU 012-0 INTAKE AIRWAYS APPROXIMATELY 12 CROSS-CUTS INBY THE BOX CUT PORTALS IN #8 ENTRY. THE ROOF FALL WA S 30 FEET LONG , APPROX 6 FEET THICK AND 20 FEETWIDE IN AN INTERSECTION. IT IS UNKNOWN OF THE EXACT DATE OF THE FALL, BUT IT WAS DISCOVERED 4-19-00.

LAMINATED SLATE AND SANDSTONE GAVE WAY. THE AREA WAS INITIALLY SUPPORTED WITH 60 INCH FULLY GROUTED RODS. NO DAMAGE TO PROPERTY. ROOF FALL WAS DISCOVERED DURING A WEEKLY EXAMINATION.
THE ABOVE MENTIONED MINE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL IN #3 ENTRY ON MMU 013-0 2 CROSSCUTS INBY SPAD # 4140 DURING RETREAT MINING. THE ROOF FALL TRAPPED THE CONTINUOUS MINER AND 15 FEET OF THE FRONT BRIDGE. IT IS UNDETERMINED OF THE DAMAGE TO PROPERTY AT THIS TIME.
THE EE WAS REPAIRING ON A SCOOP CHARGER WHEN HE MOVED SUDDENLY AND STRAINED HIS BACK.
THE EE WAS ROCK DUSTING WHEN HIS HOSE GOT TANGLED IN SOME BELT STRUCTURE, HE JERKED THE HOSE AND INJURED HIS SHOULDER. THE EE WORKED UNTIL 3/8/00 WHEN HE FOUND OUT HIS SHOULDER WAS MORE SERIOU SLY INJURED. LOST TIME BEGAN ON 3/8/00.
THE EE WAS HANDLING CONCRETE BLOCKS WHEN HE HAD PAIN IN HIS STOMACH. THE EE WORKED UNTIL 3/8/00 WHEN HE FOUND OUT HE HAD A HERNIA. LOST TIME BEGAN ON 3/8/00.
DURING RETREAT MINING ON MMU 013-0 WORKING SECTION, THE MINE ROOF SUDDENLY GAVE WAY TRAPPING THE SHEAR HEAD OF THE CONTINUOUS MINER. THE MINER SUSTAINED NO DAMAGE. NO INJURIES.

THE ABOVE MINE HAS EXPERIENCED A NON-INJURY, UNINTENTIONAL ROOF FALL OUTBY THE SECTIONS BETWEEN 10 AND 11 CROSS-CUTS INBY THE OLD 42 TRACK SWITCH IN #3 TRACK ENTRY. THE FALL WAS APP. 5 FT THIC K, 14-16 FT WIDE AND 30 FEET LONG. LAMINATED SLATE AND SANDSTONE WITH A SMALL COAL SEAM WAS THE MINE STRATA. THE EE GOT HIS RIGHT KNEE CAUGHT BETWEEN THE COAL RIB AND A SHUTTLE CAR CABLE. THE EE WORKED UNTIL 1-31-00 WITHOUT ANY PROBLEMS WITH HIS KNEE. LOST TIME BEGAN ON 1-31-00. WE WERE NOTIFIED OF T HE INJURY ON 2-1-00.
