EE was cutting plastic glue bucket while doing so the belt knife slipped crossing his left hand cutting it. Gloves were worn at the time of the accident.

Mis judged weight on lift and turned crane over.
While ee was moving the pipe bonder the 12"" Kerco pipe his thumb was pinched between the bonder and pipe. Causing a cut & fracture to his right thumb, 6 stitches.

Injury was work on the vibrator screen when debris in his right eye. Safety glasses were worn at the time of accident. The debris fell on his face going in his eye.
EE and another EE were unloading a pallet with pump hose on it, when injured EE grabbed and pulled on the pallet, it had a nail in it. EE ripped his right thumb on it. Applied first aide to the wound, then he went to the nurses station.

Employee was standing on a piece of metal he was cutting. When metal fell he fell also about 4 feet.
While removing a sump pump from the raw hopper tunnel the pump became wedged on a bolt, EE grabed the pump to move it, it came loose and swung back and mashed his hand on a pipe.
During the dismantling of the dragline, employees were cutting the cab loose from the machine with an oxygen torch. During this process some sparks were going into the cab. At the end of the shift all areas were checked for fire. None were found. After everyone had gone a fire started in the cab. Moderate damage to interior & flooring. No injuries.

EE was cutting a short 2x4 with a skillsaw. When he was almost thru the cut the saw bound up. EE moved his hand to the back of the board and pulled the trigger. The saw kicked bakc cutting his left thumb.

Workman was removing a coal hopper from it's support structure when the walkway he was standing on broke and caused him to fall down approximately 13 feet onto a concrete slab. The fall caused fractures of both ankles.

Employee was getting off the 3500 shovel at the end of his shift. It was dark and he misjudged the distance from the step to the ground causing him to fall and hurt his ankle.

THE FOREMAN WAS TAKING EE INTO THE PIT, WHEN HE HAD HIM MOVE THE HITACHI BACKHOE. AFTER HE DISMOUNTED THE BACKHOE, HE WALKED BACK TO THE TRUCK WIPING THE MUD OFF HIS HANDS. WHEN HE WIPED THE M UD OFF HIS HANDS, HE NOTICED HE HAD A LACERATION TO HIS LEFT HAND. HE DOESN'T KNOW WHEN OR HOW HE CUT HIS HAND. HE WAS TAKEN TO THE EMERGENCY ROOM WHERE THEY PUT 14 SUTURES IN THE HEAL OF HAN

THE DRAGLINE CREW HAD BUILT A METAL CANOPY TO GO OVER THE DRAGLINE BUCKET WHILE THEY WERE REBUILDING THEM. EE & COWORKER WERE IN THE PROCESS OF PLACING THE CANOPY ON TOP OF SOME CRIBBING TIMBE RS SO THEY COULD FINISH BUILDING THE LEGS. AS EE WAS BENDING OVER TO PLACE A CRIBBING TIMBER UNDER THE CANOPY, HE HUNG HIS LEFT EAR ON A WELDING ROD THAT WAS BENT AROUND A PIECE OF ANGLE. THI
THREE EMPLOYEES WERE REBUILDING THE UNDERCARRIAGE ON A HITACHI BACKHOE. TWO WERE UNLOADING THE NEW UNDERCARRIAGE PARTS AND THE THIRD TURNED AND WAS WALKING AWAY FROM THEM. ONE OF THE CARRIAGE ROLLERS ROLLED OFF THE TRUCK AND STRUCK ANOTHER ROLLER THAT WAS LAYING ON THE GROUND. WHEN THE 2 ROLLERS HIT, A SMALL SLIVER OF METAL SHOT OFF & HIT THE THIRD EE WHO WAS ABOUT 20' AWAY.

EE WAS IN THE PROCESS OF TURNING THE MG SETS WITH A SLING AND HOIST. AFTER HOOKING THE SLING TO THE MG SET, HE PUSHED THE UP BUTTON TO START THE HOIST REELING IN. HE REALIZED THE BUTTON WAS ST UCK AND BEFORE HE COULD GET UP AND GET TO THE BREAKER BOX, THE SLING CAME OFF OF THE SETS, CAUSING THE HOIST BLOCK TO POP UPWARDS AND SWING, STRIKING EE INT HE REAR OF THE HEAD. THIS RESULTED
EE WAS WORKING DURING THE FIRST WEEK OF VACATION, HELPING TO CLEAN ON THE PAGE 752 DRAGLINE. EE WAS USINGA PUTTY KNIFE AND A LONG HANDLE SCRAPPER TO SCRAP OLD GREASE OUT FROM UNDER THE DRAG DR UM IN THE HOSE OF THE MACHINE. AS HE TURNED IN ABENT OVER PSITION TO REMOVE GREASE FROM HIS SCRAPPER, HE FELT SHARP PAINS IN HIS LOWER BACK THAT TOOK HIM TO HIS KNEES. HE TRIED TO GET BACK ON

EMPLOYEES WERE REPLACING THE 1/4 "" STEEL HOIST CABLE ON THE PARTS ELEVATOR WENCH AT THE VOLUNTEER SHOP. INJURED WAS USING A SET OF CABLE CUTTERS AND A BALL-PEINE HAMMER TO CUT THE CABLE TO THE RIGHT LENGTH. AS EMPLOYEE STRUCK THE CABLE CUTTER WITH THE HAMMER, THE BALL PART OF THE HAMMER CAME LOOSE AND STRUCK HIM IN THE RIGHT FOREHEAD CAUSING A SMALL LACERATION. THIS REQUIRED TWO SU
