On 6/26/2025, employee alleged to the plant manager that EE had passed out/lost consciousness while performing clean up in the plant two days prior. This alleged incident was not substantiated during the investigation.

Euclid R40 haul truck caught fire after a hydraulic hose busted and sprayed onto the exhaust pipe causing a fire. It wasn't extinguished within 30 minutes. No one was injured in this fire

Injured employee was hammering a cutting edge back to make room for the tooth to be replaced when the metal chip broke off and went into the injured employees chin.

AFTER GETTING WELDING EQUIPMENT OUT OF TRUCK TOOL BOX, WAS GOING BACK AROUND REAR OF TRUCK AND RAN INTO OPEN TOOL BOX DOOR, CUTTING FACE ON RIGHT CHEEK, REQUIRING 3 INSIDE STITCHES & 11 OUTSIDE STITCHES.

While steam cleaning truck with hot water steam jenny, handle/nozzle got against employee's right leg, burning it above the ankle.

THE EE WAS USING A BOBCAT TO PERFORM CLEAN-UP AROUND THE LIME BIN, WHEN DUST BLEW BEHIND HIS SAFETY GLASSES INTO HIS LEFT EYE. THE EYE WAS FLUSHED BUT UPON RETURNING TO WORK AFTER THE WEEKEND, THE EYE WAS RED & SWOLLEN REQUIRING TREATMENT.

AS THE EE WAS IGNITING A CUTTING TORCH, THE TIP WAS PLUGGED UP & ASIT LIT ON 2ND ATTEMPT, FLAMES CAME OUT AND BURNED THE PALM OF HIS RIGHT HAND. (BURNED THRU GLOVES)

EMPLOYEE WAS CARRYING A 2' LONG 8X8 POST TO THE LOCATION WHERE HE WAS WORKING; THE POST WAS WET DUE TO RECENT RAINFALL IN THE AREA AND SLIPPED FROM HIS HANDS. HE BROKE HIS FINGER WHEN HE TRIE D TO CATCH THE POST BEFORE IT HIT THE GROUND.
