During screen maintenance the sharp edge of a metal panel caused an open fracture to the right index finger of the employee.

EE STEPPED DOWN OFF LADDER OF MOBILE EQUIPMENT TWISTING RIGHT KNEE, INITIALLY DIAGNOSED BY DOCTOR AS A SPRAIN WITH NO RESTRICTIONS. AFTER TOTAL HEALING WAS NOT REACHED WITHIN FOLLOWING WEEKS, EE WAS TAKEN IN FOR FOLLOW UP EXAM WHEN VERY SLIGHT MENISCUS LIGAMENT TEAR WAS FOUND.

EE MASHES RIGHT THUMB BETWEEN FRONT SPRING AND AXLE OF STOCKPILE TRUCK DURING MAINTENANCE CAUSING BROKEN BONE

Rock being transported to dump area was lodged in truck tailgate. When employee was attempting to remove the tailgate he injured his fingers.

EE slipped on ice in office parking lot landing on hip and shoulder. Doctor ordered restricted duty did not occur until 02/02/2015.

Employee was repairing wedag crusher. Right hand was inside wedag attempting to turn rotor when hand was caught between rotor and wall of crusher.

Employee's finger was caught in fan belt and severed 1/3 of right index finger

Exiting fuel truck and knee twisted when foot hit ground.

EE STRAINED HIS SHOULDER WHILE LIFTING/MOVING A HYDRAULIC PUMP AT THE WEDAG CRUSHER OF THE SECONDARY CRUSHING PLANT.

EE RAN OFF RAOD WITH EXCAVATOR.

EE WAS WATCHING A BLAST DETONATION. FLY ROCK STRUCK LEFT CALF & KNEE.
