SWINGING SLEDGE HAMMER TO TIGHTEN UP CONE CRUSHER ON SATURDAY THEN DID SOME SHOVELING THE NEXT WEEK WHICH IRRITATED IT. THE DOCTOR DID NOT DECIDE TO GIVE TIME OFF FROM WORK UNTIL 11-12-01.

EMPLOYEE'S HAND SLIPPED INTO THE HEATER/AC VENT WHILE BRACING HIMSELF TO PICK UP A LIGHTER FROM THE DASH OF THE FRONT END LOADER.

Fall from machine

THE EE WAS CLIMBING IN STOCKPILE TRUCK HIS RIGHT FOOT WAS ON THE TOP STEP AND THE LEFT FOOT ON THE BOTTOM STEP. HIS RIGHT FOOT SLIPPED OFF THE STEP AND HIT THE GROUND THIS CAUSED A STRAIN ON T HE LEFT KNEE AND ANKLE THUS SPRAINING LEFT LEG.
