Employee suddenly became nauseous, dizziness, and lightheaded. EE was taken to the shop and brought out of the elements and treated for over exertion. EE was given fluids and cooled down when EE started cramping. 911 was called while our first responders were treating for heat exhaustion. EE was then transported to the hospital and was diagnosed with dehydration.

EE was removing the roof over the gas motor on the high frequency screen plant. The edge of the roof cut EE's tip of left index finger. EE went to urgent care and got six stitches in the finger. No work restrictions were issued.
Hit the tip of thumb on left hand around the tip of the nail while hammering a socket on a final drive plug and had caused a cut approx 1/4 inch

Mechanic was installing brakes on a water truck inside the shop area with garage doors opened. A rain storm developed causing wind to blow dust and the garage doors acted as a funnel to allow dust to enter shop area. Mechanic did have on appropriate PPE (eye protection).

EE had pulled his truck off the scales to the right side out of the way in order to change a tire. As he was finishing he became unconscious. Employees on the site commenced CPR until the ambulance arrived.
Employee had just gotten off work and was in the parking preparing to go home. The employee then briefly lost consciousness and slid to the ground. A fellow employee proceeded to help the affected employee and took him down to the office. An ambulance was called and the employee went to the Whitesburg ARH Hospital.

EE was checking coolant level in radiator. He stepped up onto tire to see coolant. His foot slipped off tire causing him to fall down straddling tire. Groin injured. EE came to ofc. and reported incident. He then went to his family Dr. Dr. examined him prescribed medication and released him to light duty until 10/14/13. EE RTW on 10/9/13 on light duty.

EE was cutting free a section of suspended catwalk, the welders hand was struck by the falling catwalk's handrail, severing 2 fingers. One finger was totally severed the other was partial. The other finger was re-attached.

On 6/16/10 EE was lifting a bearing into a shelf, when he experienced back pain. The ER doctor said it was a pulled muscle & told him to take Ibuprofen. He worked every scheduled shift until 8/16/10 when he went to his personal doctor. At that time she ordered tests & placed him on restricted duty. He did not return to work until 8/19/10, at which time this event became reportable.
Worker was loosening a bolt and wrench slipped resulting in worker catching himself during fall and spraining wrist.

EE was cleaning dust build up from a chute, when the wall of the bin - hopper directly above him gave way, then a large amount of dust was allowed to fall onto him pushing him downward into the chute & railing where he was standing.

Employee was loading a roll of conveyor belt onto the boom truck. After unhooking the hoist from the roll of belt, it began to roll, rolled off the truck, striking ee and landing on his foot. He completed his shift that day, but went to his family doctor the next day. The doctor ordered x-rays, finding a fracture in his foot bone.
The EE was using a shop hammer to drive a bearing off a shaft. When he struck the bearing, it shattered and a fragment of the bearing struck him in the mouth, cracking a tooth.

Employee was using a ratchet and socket to remove a bolt from a v-belt guard to change v-belts. While pulling on the ratchet, the socket slipped off the bolt head. When this happened, his little finger (pinky) on his right hand struck the angle iron on the edge of the v-belt guard cutting it to the bont.

EE was attempting to clean buildup out of a return roller guard on a beltline when the belt pulled the bar into the pinchpoint also pulling his right hand into the pinchpoint. He was able to radio for the plant operator to shut off the belt, and freed himself. He had minor cuts and abrasions, but stated that he was ok.
