Injured employee was attempting to strike a belt clamp with a hand held hammer. IE inadvertently struck right pinky finger. The injury required two sutures to close a wound on the tip of the finger.

At approximately 8:15 am (CST) customer driver climbed over a concrete barricade to retrieve their weigh ticket. When they stepped down, they fell on their left side. Person was treated and released with no restrictions.

EE WAS OPERATING MOBILE VIBRATOR ON THE NEW PARKING LOT BEHIND THE SHOP. THE NEXT DAY HE MADE COMMENTS OF HIS BACK BEING STIFF. ON 08/08/05, HE COMPLAINED ABOUT HIS BACK AND WENT TO HIS DR ABOUT THE BACK PAIN. DR RESTRICTED HIM FROM WORK UNTIL THE 17TH OR UNTIL THE NEXT CHECK-UP.
Employee went to pull the 2x4 board that was attached to the banding strap, which was used for banding the water discharge pipe. The band was stapled to the 2x4 board, snatched the employee's right hand below the thumb, close to the palm.
