The S-2 screen had stopped while crushing. The injured employee was removing the screen doors using a 16 inch spud bar. As the employee unhinged a door, the door sprung outward and struck the employee in left forearm.

A plant Mechanic was attempting to remove a rear center pin block on a Euclid 324TD Haul Truck. EE was using hand tools such as a hammer, chisel and wrenches. As ee worked ee noticed a pain in lower back that radiated through pelvic area.

Second-hand info: Five minor children approx. 15 y/o were trespassing on mine property, bypassed several No Trespassing signs, a locked gate, and a barricade in front of an old underground portal. Emergency services contacted the mine site to report people stuck in the underground. Children were off property before mine personnel arrived at the old portal.

Injured employee was using a come-a-long attached to a sheave on a 4 foot cone crusher. While he was pulling the come-a-long handle back and forth, the chain slipped and he lost his balance and fell back approximately 57"" to the ground.
