Fall down raise, shaft or manway

THE EE APPROACHED THE AIRPLANE FAN AREA TO REPOSITION THE END OF THE DISCHARGE HOSE. AS HE WALKED ACROSS THE RAISE ENCLOSURE FOR THE AIRPLANE FAN, A METAL PLATE ALONG THE OUTSIDE EDGE OF THE E NCLOSURE FAILED AND HE FELL INTO THE VENTILATIONSHAFT.

Employee was using the hose from the water truck to spray out the grizzly bars in the primary feeder underground. Employee was stepping down into the crusher mouth to spray underneath the grizzly bars. Employee stated that their foot slipped on the Hazemag Crusher Roll stepping down into the crusher mouth opening and it caught as EE fell.

Employee was cleaning out around the primary area and stepped on wet material and slipped. Employee landed on their lower back, right side on the last step of stairs.

Flash burns (welding)

Employee was seen at the ER 2:00am on 9/21. EE was diagnosed with flash burns from the welding EE was performing 9/20. EE was given prescription eye drops and was released from the ER.

Employee was bolting out of the basket, rock fell, hitting hand rail of basket, rock rolled over onto employee's left leg, causing a break in 2 places

MSHA was called because of an equipment fire on level 3 of the underground mine.

Accident type, without injuries

A high voltage fuse on a power pole controlled by the utility operator blew. The blown fuse continued to spark and burn. The utility provider was contacted but as it took longer than 30 minutes to arrive, notification was made. No injuries or damage resulted from this.

Employee got out of the lube/fuel truck, stepped on a rock, twisted ankle and fell to the ground.

Employee states they were picking up and moving a case of water when the scales went off. Employee went to put the case of water down when they felt a pain in right shoulder. This was not reportable until 10/20/2016.

Employee was putting resin for the bolt on the shelf of the drill when he noticed some discomfort in his right shoulder. He proceeded to bolt and noticed that when he lifted his arms above his head that something was not feeling right

The injured employee was assisting with changing of scalping screen when load shifted smashing his left hand little finger between the plate and frame of the structure.

Injured employee was attempting to shut a chute on a gate using a sledge hammer. Employee missed the gate and hit his right foot causing a contusion.

Injured employee was lifting metal catwalk section when it shifted and smashed hand against catwalk and ground. Causing cuts on two fingers on his right hand.

Material fell from the rib on the level 1 to level 2 decline. The material that fell damaged the conveyor disruping regular mining for more than 1 hour. No injuries or illnessess occurd as result of this accident. There was no immediate danger as no one was in the mine when the event occured. MSHA was notified.

Injured employee was tightening a nut on a hydraulic cylinder and the box end wrench slipped off the head of the bolt. Resulting in a laceration to the left pinky finger

Water run off due to heavy rains in the area caused the water retention areas in the mine to rise. As a precautionary measure the mine was evacuated because water on level 2 of the mine was closed to the base of level 1. two MCC and transformers. There was no immediate danger. MSHA was notified.

Smoke was observed coming from the level 3 motor control center, mine was evaculated and the MSHA Immead. report number was contacted. Prelim. investigation revealed there was no fire. The smoke was the result of a flash arc that had occured. No injuries/illnesses as a result of this incident.

Fall of material from the back on the level 1 to level 2 decline most of the material that fell was caught in the mesh on the back. The loose ground in the mesh above the decline impeded use of the primary escapeway

Employee had a periodic screening chest x-ray read by two ""B"" readers using ILO procedures. The x-ray was characterized as being consistent with pneumoconiosis with reading of 0/1 and 1/0. Damage or impairment cannot be determined by reading chest films only. This employee will be sent for a comprehensive respiratory examination

Struck by flying object

The injured employee was on a jaw crusher when he utilized the hyd. rock breaker to break an oversized rock. A rock about the size of a baseball came off boulder and struck employee on the left side of his head.

Injured EE was cutting 18' drill in half to put in loader bucket to move to the right surface. He threw a 9' cut piece into a pile he had started and this piece bounced back and struck his right foot.

The employee was repairing hoses ont he roof bolter and a rock fell from the back and struck his right shoulder and upper arm resulting in a contusion.

The employee was injured attempting to dislodge a rock from rock box with a scaling bar. While the employee was attempting to dislodge the rock the belt/roller pulled the bar which resulted in his right hand getting caught between the bar & conveyor.

The ee was attempting to raise the bed on a 771 Haul truck. the bed was raised approx. 2 to 3 feet and he stated the bed dropped stricking the frame, resulting in a strained neck.

THE EE WAS PUMPING WATER OUT OF MINE AND RAN AROUND THE PICKUP AND HIT HIS RIGHT ARM ON THE OPENDOOR.

THE EMPLOYEE EXPERIENCED BLOOD IN HIS URINE WHICH REQUIRED TESTING TO DETERMINE CAUSE. IT WAS LATER DETERMINED THAT OPERATION OF A LOADER CAUSED THE BLOOD IN HIS URINE. THIS WAS DETERMINED ON 8/20/02.
