Struck against stationary object

EE was dismounting the haul truck (Euclid R-35)using three points of contact. EE stepped down on a piece of rock and felt a pop in right foot. EE has a metatarsal fracture in right foot.

EE was fueling the loader when she stepped down on the berm around the fuel tank, she lost her balance and fell backward, injuring her left wrist.

EE was using a 3 pound hammer to try & free a stuck troughing roller on a belt conveyor. The conveyor was running at the time. The conveyor belt grabbed the hammer and pulled his arm into the head pulley, causing his right arm to be severed.

EE WAS CLEANING OUT THE PRIMARY IMPACT CRUSHER, WHICH HAD STALLED, USING A PRY BAR. A LARGE ROCK ROLLED UNEXPECTEDLY, SMASHING HIS FINGER AGAINST ANOTHER ROCK. THE FINGER WAS CUT AND REQUIRED STITCHES.

EMPLOYEE WAS FIXING A TWISTED V-BELT ON THE MAIN DRIVE OF THE PRIMARY CRUSHER. WHEN THE BELT FLIPPED OVER, HE CAUGHT HIS LITTLE FINGER BETWEEN THE BELT AND THE DRIVE SHEAVE. THE TIP OF HIS FI NGER WAS FRACTURED AND CUT. HIS FINGER WAS SUTURED AND SPLINTED.

EMPLOYEE WAS ATTEMPTING TO PUT A KEEPER PIN IN TO HOLD UP THE BED OF A 25 TON HAUL TRUCK AND SMASHED HIS FINGER BETWEEN THE BED AND THE HANDLE OF THE KEEPER PIN. THE TIP OF HIS MIDDLE FINGER O N THE RIGHT HAND WAS FRACTURED AND CUT. HIS FINGER WAS SUTURED AND SPLINTED.
