Employee was cleaning material under the Cone when employee hit hard material in the center of the pile. The employee felt a ""pop"" between the shoulder blades and had pain in neck.

Fall to the walkway or working surface

EE was attempting to attach a chain from a front end loader bucket to a trailer to lift it up and remove jack stands. The front end loader moved forward and ee stepped back, stumbled and fell on back. EE has a cracked T7 vertebrae.

EE climbed down from his haul truck and stepped on a piece of rock, causing him to fall backwards.When he reached back to break his fall he caused a slight fracture to his right wrist (distal radius).

Employee was using a pry bar. The bar slipped and he hit his hand on the machinery he was prying on, causing a slight fracture to the back of his left hand.

Employee was helping clean trapped material from behind the driven sheave on the primary crusher. He was manually turning the sheave while another employee knocked the material out. He caught his left index finger between the sheave and the crusher frame, fracturing the finger.

EMPLOYEE CLAIMS THAT IN EARLY OCT. HE WAS OPERATING A DOZER PUSHING ROCK, CLEANING UP PIT FLOOR.HE STATES HE WOKE UP THE NEXT MORNING & HIS LEGSWERE NUMB. HE NOTIFIED US ON 11/10/00 AFTER AN MRI SHOWED A HERNIATED DISK. HE'S BEEN OFF WORK SINCE 11/8/00. SURGERY PERFORMED ON 11/13/00.
